Significance of evaluation on brain frontal lobe development in patients with perinatal brain injury using three-dimensional ultrasonography

Abstract:Objectives To investigate the significance of the three- dimensional (3D) ultrasound evaluation on development of the frontal lobe volume in infants with perinatal brain injuries. Methods The frontal lobe volumes in 226 neonates of different gestational ages as well as 86 term infants with various perinatal brain injuries were measured by means of 3D ultrasonography. The effect of perinatal brain injury on frontal lobe development and relationship between retarded frontal lobe volume increase and the developmental neurological abnormalities were evaluated in the first 3 months and 6 months of life,respectively. Results Neonatal frontal lobe volume demonstrated proportional growth to the gestational age within the first seven days after birth( r = 0.676,P < 0.01). Among 33 cases of neonates with severe brain injury, their frontal lobe volumes were less than those of the controls at one month of age, and displayed significant diffe- rence at 3 months and 6 months of age (P < 0.01). At age of six months, frontal lobe volume of the severe and mild brain injury groups and the control group were(107.82 ± 28.76)cm3,(156.42 ± 12.61)cm3 and (174.0 ± 13.27)cm3,respectively. Long term development of neurological abnormalities correlated with slow increment of frontal lobe volumes. Among infants with mild and severe neurological developmental abnormalities, the rates of retarded increment of frontal lobe volumes were 22 % and 77 %, respectively. Conclusions Frontal lobe volumes increased proportionately with the gestational ages. Prenatal brain injury affected the development of the frontal lobe and was correlated to the abnormalities of neurological dev- elopment. It is helpful to use 3D ultrasound to evaluate normal and abnormal brain development.
