Abstract:By investigating 18 Chinese Medicine hospitals' financial revenue in one city of Sichuan Province, this article presented Chinese Medicine hospitals' developments in counties, analyzed the problems they faced, and provided suggestions for their sustainable developments.

Abstract:Objectives:To analyze the revenue and expenditure of the Chinese Traditional Medicine (CTM) hospitals from 2002 to 2007 in Shanghai. Data and Methods: We extracted the financial management data of the four tertiary and sixteen secondary CTM hospitals,and information from key person interview. Results: The Chinese Traditional Medicine hospitals are more likely to be affected by controlling drug markup policies in comparison with other hospitals .Due to the human resources costs escalation and the drug markup decreasing,the CTM hospitals felt deficit by 188 million,which accounts for a large proportion of total deficit of hospitals in Shanghai in 2007. Recommendations: We should set price and reimbursement appropriately and modify the reimbursement mechanism in TCM hospitals guarantee the medicine quality and encourage the use of TCM medicines which are of high quality and cost effective.

Abstract:Objectives:This study is to describe and analyze the finance status of health institutions at Shenmu County.Methods:Descriptive statistics was used to analyze the data from 2008 to 2009 provided by Shenmu Health Bureau.Results:(1) The medical service operation expenses were 65.67 million in 2008 and 233.80 million in 2009.(2) In 2008,health institutions’ assets were 63.73% in fixed assets and 35.32% of current assets,while the liabilities and net assets of county hospitals were 7.63% and 94.70% of the total.(3) In 2009,business income was 77.66% of the total,business expenditure accounted for 98.98% of the total;the net balance and net rate of balance in health institutions had a negative growth,which was-46.27% and-28.23% respectively;the net balance of county hospitals was 99.84% over the total.Conclusions:(1) Health expenditure was with rapid growth.(2) The distribution of health institutions’ assets was with imbalance,and the proportion in county hospitals was too large.(3) Balance of health institutions was reducing which led to rural hospitals facing the survival dilemma.

Abstract:Objective: To investigate the status and trends of economic operation of public traditional Chinese medicine hospitals in Shanghai.Methods: Data of economic indicators of 2 tertiary traditional Chinese medicine hospitals and 5 secondary traditional Chinese medicine hospitals were collected and a descriptive statistical analysis was conducted.Results: An analysis of the overall balance and structure,the allocation of resources,the operation efficiency and benefits of above hospitals was conducted.Conclusion: Tertiary traditional Chinese medicine hospitals have a rapid growth in their sizes and service capabilities,but secondary hospitals developed relatively slowly.Financial assistance subsidized by government has increased,but the proportion in the total income of the hospital is still low.Traditional Chinese medicine hospitals have different income constitutions with western medicine hospitals,which have been influenced greatly by drug policies.Therefore,a suitable compensation mechanism for Chinese medicine hospital should be established.

Abstract:Analyzed in the paper is the current subsidy mechanism for public hospitals in Zhejiang province, with analysis of problems found. The authors recommended to build a mass fraction subsidy mechanism to cover up the insufficiency of government financial subsidy; to fully leverage the price compensation of medical services; to subsidize in view of the functional positioning of various medical institutions;and explore more channels for public hospitals′subsidies.

Abstract:Objectives:To analyze the status of revenue and expenditure of 20 centers for disease control and prevention,and 20 health supervision facilities,from the year 2002 to 2007. Data and Methods: Make full use of the data of Shanghai total expenditure on health (STEH),especially on pulic health facilities. This article provides a descriptive analysis. Results: The expenditure of public health facilities accounted for around 5.2% of STEH until SARS,then going up to 6.1% in the year 2004,and then declining significantly. The phenonmenon that attaching more importance to medical treatment than to prevention still exists. 24% revenue of the centers for disease control and prevention still has to rely on charges. There is still great difference on government public health input among different districts. And the fiscal input has only accounted for about 88% personnel expenditures. Recommendations: Increase the government health input gradually,adjust the existing public health service system,and elevate the financing level to guarantee the accessibility and fairity of basic public health services to all the people. Meanwhile,strengthen the performance evaluation and supervision to improve the efficient utilization of fiscal input.

Abstract:Objective: To analyze the economic operation status of 8 tertiary hospitals in Shanghai from 1999 through 2009 and to understand existing problems in their development.Methods: Comprehensive descriptive analysis of 11 years of balance sheet,revenue and spending sheet,and other indicators was conducted.Results: The service capacity increased,but there are still unreasonable compensation mechanism,the overload of the medical work and other problems.Conclusion: It is suggested to establish a reasonable medical pricing system and payment system,improve the compensation mechanism,and strengthen cost accounting.

Abstract:Objectives:To analyze the economics status and government input of public hospitals at different levels from the year 2002 to 2007. Data and Methods: Make full use of the data of Shanghai total expenditure on health (STEH),especially on pulic hospitals,and give a descriptive analysis. Results: The revenue of hospitals accounts for around 78% of STEH,mainly in secondary and tertiary hospitals. The surplus turns into deficit in 2007 in public hospitals for the first time. Drug retailing remains the major source of public hospitals revenue. The government input accounted for about 1/5,1/3 and 1/2 personnel expenditure of the tertiary,secondary and primary hospitals respectively. And the drug mark-up rate is restricted to 17.8%,21.0% and 23.5% in 2007 in tertiary,secondary and primary hospitals respectively. Recommendations: A clear increase of government health input is needed so as to adjust the policy of financial subsidies. Meanwhile the medical care price system should be modulated. Cost accounting on medical care and health information system should be strengthened. In addition,hospitals of diverse investment should be introduced to promote public hospitals reform.

Abstract:BACKGROUND:Compare changes in types of hospital service revenues between traditional Chinese medicine (TCM) hospitals and Western-medicine based general hospitals.METHODS:97 TCM hospitals and 103 general hospitals were surveyed in years of 2000 and 2004. Six types of medical service revenue between the two types of hospitals were compared overtime. The national statistics from 1999 to 2008 were also used as complementary evidence.RESULTS:For TCM hospitals, the percentage of service revenue from Western medicine increased from 44.3% to 47.4% while the percentage of service revenue from TCM declined from 26.4% to 18.8% from 1999 to 2004. Percentages of revenue from laboratory tests and surgical procedures for both types of hospitals increased and the discrepancy between the two types of hospitals was narrowed from 1999 to 2004. For TCM hospitals, revenues from laboratory tests increased from 3.64% to 5.06% and revenues from surgical procedures increased from 3.44% to 7.02%. General hospitals' TCM drug revenue in outpatient care declined insignificantly from 5.26% to 3.87%, while the decline for the TCM hospitals was significant from 19.73% to 13.77%. The national statistics from 1999 to 2008 showed similar trends that the percentage of revenue from Western medicine for TCM hospitals increased from 59.6% in 1999 to 62.2% in 2003 and 66.1% in 2008 while the percentage of revenue from TCM for TCM hospitals decreased from 18.0% in 1999, 15.4% in 2003, and 13.7% in 2008.CONCLUSION:Western medicine has become a vital revenue source for TCM hospitals in the current Chinese health care environment where government subsidies to health care facilities have significantly declined. Policies need to encourage TCM hospitals to identify their own special and effective services, improve public perception, increase demand, strengthen financial sources, and ultimately make contributions to preserving one of the national treasures.

Abstract:Objective To know the drug income of 340 public hospitals ranking country-level and above in Sichuan province from the year 2003~2007,and then provide information for the reform and reimbursement mechanism. Methods We probed into the proportion of drugs in hospital total income by reviewing literatures,consulting experts,collecting questionnaires,and the results were analyzed by statistic description and two-level regression model. Results The proportion of drugs in hospital total income was decreasing year by year; in the hospitals at the level of Grade Ⅲ,the proportion was lower than those of Grade Ⅱ and below; the percentage of traditional Chinese drugs in the overall drugs income was only 10.30%.Conclusion The proportion of drugs 39.1% in hospital total income in Sichuan province is lower than that of national average 41.83%; the government should increase input to hospitals at the level of Grade II and below; the deficit can be maken up by improving efficiency,shortening average length of stay and charging extra fee for pharmacy service; we suggest that the percentage of TCM and folk medicine prescribed from essential drug list be enlarged.

Abstract:Objective:To understand the change of curative and preventative functions in township hospitals.Methods: Medical and preventative expenditure of 49 township hospitals in Eastern China were surveyed in 2000 and 2008 respectively by using maximum-difference sampling method.Results: Medical expenditure was far more than preventative expenditure,but the growth rate of preventative expenditure was much faster in recent years.The township hospitals with better viability could allocate more resources to support preventative care and the capacity of prevention could be improved.Conclusion: The government and hospital should strengthen the importance of the preventative work and provide more input to preventative work,especially in poor areas.The township hospitals should balance the development of medical and preventive functions.

Abstract:Objective The aim of this study was to have a view of the income and expenses of community health service centers(CHSC) in Shanghai suburb from 2003 to 2005.Methods A cluster sampling survey was conducted to investigate the data of Shanghai Suburban CHSC.Results In 2003～2005,the average annual gross income,the average annual gross expense,the average annual medicament income and the average financial subsidies income showed a tendency to increase.The average annual progressive increases were 23.42%,20.84%,29.41%,39.22%,respectively.Conclusion The overall financial condition of Shanghai Suburban CHSCs is good,but they still need the government to support strongly when carrying out CHSC functions and continues to develop soundly.

Abstract:Overall operational status,services in medical care and public health,education,research and specialty development and purchase of large equipment in public hospitals affiliated to one Shanghai district government were analyzed. It is suggested that government should subsidize different types of public hospitals differently,use financial lever to facilitate the provision of Chinese medicine and pediatric care in public hospitals,enhance the specialty development in public specialty hospitals,and fully compensate the purchase of large equipment in public hospitals.

Abstract:Objective:To learn about the current development and the key problems in private hospitals of Chengdu City after the practice of new medical reform,and put forward countermeasures and suggestions. Method:Referring to the relevant data of private hospitals in Chengdu City provided by Chengdu Medical Information Institute,analyze by intervie-wing the institution number,bed number,human resources,service volume,regional distribution and the coming up difficul-ties in Chengdu private hospitals. Result:Considerable development has been made in Chengdu private hospitals since the new medical reform. But the development is far from even for private hospitals are facing great difficulties in land,medical insurance,talent,equipment,and so on. Conclusion:The authors suggest that the report and guide of private hospital devel-opment in Chengdu City be issued to lead the development of private hospitals in suburbs and in an internally balanced di-rection. The intensified policy implementation in private hospitals should be encouraged, multi-sited medical practice be promoted to alleviate talent pressure in private hospitals,and reimbursement policy of medical insurance in private hospitals adjusted to “equal reimbursement of same region and same degree”.

Abstract:Objective For establishing policy of Chinese medicine community service,to compar with working and service in different community health service center in Shanghai city.Methods In 2008,to investigate working and service in the different health service center with the Chinese medicine and west medicine.Results Public finance subsidy being not good in characteristic community health service center,and hospitalization expenses being not high,but service offering and Making use of service being good.From 2005 to 2007,the year average increasing rate was 5.39% in the characteristic Chinese medicine community health service center in each thousand population inpatients,but-6.10% in non-characteristic Chinese medicine community health service center.Conclusion To enhance the public finance inputs,and to increase the service function.In same time,we hope the different community health service center enhance the community health management,and increase the resident health level,control medical treatment total expenses.

Abstract:This article used a combination of quantitative and qualitative research to collect data of Pudong New Area Hospital of Traditional Chinese Medicine and other relevant institutions. It considered the experts’ opinion and drew up a plan and tracked the reform effect. After two years,the data showed that in two traditional Chinese medicine hospitals,58 general hospitals and other non-traditional Chinese medicine institutes in Pudong New Area,traditional Chinese medicine services rose by 72.01%in 2012,compared to that in 2010. Early-stage exploration has formed a scientific and reasonable Chinese medicine service project compensation mechanism,and this exploration has played a positive promotion role of Chinese medical service development.

Abstract:Objective: To analyze the economic operation status of urban community health service centers in Shanghai from 1999 through 2009 and understand the problems in their development.Methods: Comprehensive descriptive analysis of 11 years of balance sheet and revenue and spending sheet was conducted.Results: The economic operation of urban community health service centers in Shanghai is stable and the residents' disease burden is controllable,but the operation efficiency and service efficiency of these centers are low.Conclusion: It is suggested to increase investment from the government continuously and improve the community health service pricing system and the referral system.

Abstract:Objectives:To analyze the revenue and expenditure of the community health facilities. Data and Methods: The financial data of community health facilities are from Shanghai Municipal Health Bureau. Results: Although the government has increased its fiscal input to community health facilities,it is hard for community health facilities to use drug benefits to cover medical service deficit in 2007 due to the combination effect of policies to control the drug markup rate,lower pricing,and the escalation of human resources cost. The community health providers still rely heavily much on drug in terms of income,and the government has not fully accomplished its commitment on input. Recommendations: Considering the consistency of policies,we propose that first the content of service of community health facilities should be regulated and cost should be accounted,second the level of government input should be set,third the policy of dividing the revenue and expenditure should be further regulated,and finally the price of service that contains knowledge and skills be progressively raised.
