Abstract:BACKGROUND: Post-stroke shoulder-hand syndrome (SHS) is a common complication after stroke. To date, there is still a lack of consistently effective and good patient compliance methods for SHS.METHODS: Fifty patients with SHS were included in this study. Patients in the control group received rehabilitation training (RT) for 2 weeks; each session was 30 min, 1 time per day, and 5 times per week. Patients in the observation group were additionally given Bo's abdominal acupuncture (BAA) with the same frequency and duration. The primary outcome was the change value in the VAS score from baseline to 2 weeks. The secondary outcomes measured were motor function of the upper limb, shoulder range of motion, ADL, and swelling volume.RESULTS: Compared with baseline values, the mean VAS score at 2 weeks was reduced by 3.68 in the observation group and by 1.92 in the control group, with a difference between the two groups of 1.84 (P < 0.001); the mean MBI score at 2 weeks increased by 10.44 in the observation group and by 4.79 in the control group, with a difference between the two groups of 5.84 (P = 0.032); the mean swelling volume at 2 weeks decreased by 9.64 in the observation group and by 3.29 in the control group, with a difference between the two groups of 6.48 (P < 0.001). BAA-related adverse events were not found during the study.CONCLUSIONS: BAA combined with RT is superior to RT alone in improving shoulder pain, swelling, and ADL in post-stroke SHS patients.TRIAL REGISTRATION: ChiCTR2100045464 (www.chictr.org.cn).

Abstract:The clinical effect of acupuncture in combination with rehabilitation therapy for post-stroke shoulder-hand syndrome (SHS) was explored. Patients (178) with post-stroke SHS who received treatment in the Dalian Second Hospital from March 2012 to March 2016 were included in this study. The patients were divided into experimental group (89 cases) and control group (89 cases). Patients in the control group received rehabilitation therapy, while those in the treatment group received acupuncture treatment in addition to rehabilitation therapy. Visual analogue scale (VAS) was applied to assess the pain degree of patients. Fugl-Meyer assessment (FMA), functional comprehensive assessment (FCA) and assessment of quality of life (QoL) were used to evaluate rehabilitation condition of the patients. Early pain relief, rehabilitation of upper extremity motor function and improvement of QoL after treatment were compared between the two groups. The scores of VAS, FMA, FCA and QoL showed obvious differences between the two groups after treatment (P<0.05). The scores of the experimental group were significantly better than those of the control group, and the improvement in upper extremity motor function of the patients in the experimental group was better than that of the patients in the control group. The total effective rate of the patients in the experimental group was higher than that of control group (P<0.05). The effect in improving the upper extremity motor function of the patients in the experimental group was better than that of control group. The scores of QoL of the patients in the experimental group were better than that of the patients in the control group (P<0.05). In conclusion, acupuncture in combination with rehabilitation therapy can improve early pain and rehabilitation significantly and enhance QoL for patients with post-stroke SHS, which is worthy of being widely used in clinical practice.

[Effect of early acupuncture intervention on post-stroke depression: a randomized controlled trial].

Abstract:OBJECTIVE:To observe the effects of conventional drug therapy combined with acupuncture and conventional drug therapy on the morbidity of post-stroke depression and recovery of neurological function in patients with acute stroke, and provide new ideas for prevention and treatment of post-stroke depression. METHODS:Sixty patients were randomly divided into an observation group and a control group, 30 cases in each one.Conventional drug therapy was given in the control group;on the basis of conventional drug therapy,acupuncture was applied at Baihui (GV 20), Sishencong (EX-HN1), Neiguan (PC 6), Hegu (LI 4), Taixi (KI 3), etc. The treatment was given once every day, 6 times a week for 4 weeks in the two groups. The 24 Hamilton depression rating scale (HAMD), modified Edinburgh-Scandinavian stroke scale (MESSS) scores were recored before treatment, after 1 week of treatment, and after 4 weeks of treatment in the two groups,and the morbidity of post-stroke depression was compared between the two groups. RESULTS:After 1 week of treatment, the HAMD scale scores of the two groups were higher than those before treatment (both P<0.05), and the MESSS scores were lower than those before treatment in the two groups (both P<0.05). After 4 weeks of treatment, the HAMD scale scores in the observation group were lower than that before treatment (P<0.05), and significantly better than that in the control group (P<0.01), but there was no significant difference in the control group before and after treatment (P>0.05). The MESSS scores were lower than those after 1 week of treatment in the two groups (both P<0.05), and the score in the observation group was significantly lower than that in the control group (P<0.01). After 4 weeks of treatment, the morbidity of post-stroke depression in the observation group was 23.3% (7/30), which was lower than 66.7% (20/30) in the control group (P<0.05). CONCLUSION:Early acupuncture intervention on stroke patients can not only promote the recovery of neurological function, but also reduce the morbidity of post-stroke depression effectively.

Abstract:Background: Shoulder-hand syndrome (SHS) is prevalent in hemiplegic patients after stroke. Potential benefits of acupuncture were shown in recent clinical trials. This systematic review aimed to comprehensively evaluate the safety and efficacy of acupuncture for SHS in stroke patients.Methods: Five English databases (PubMed, Embase, CINAHL, CENTRAL, and AMED) and four Chinese databases (CBM, CNKI, CQVIP, and Wanfang) were searched from their inceptions to January 2019. Randomized, controlled trials that evaluated the add-on effects of acupuncture to rehabilitation for post-stroke SHS were identified.Results: Thirty-eight studies involving 3,184 participants fulfilled the eligible criteria and were included in the review. The overall meta-analysis showed that acupuncture combined with rehabilitation significantly improved motor function (upper-limb Fugl-Meyer Assessment (FMA): 34 studies, mean difference (MD) 8.01, 95% confidence interval (CI) [6.69,9.33]), and reduced pain (visual analog scale (VAS): 25 studies, MD −1.59, 95%CI [−1.86,−1.32]). It also improved activities of daily living (ADL) when compared with rehabilitation alone (ADL: 11 studies, MD 9.99, 95%CI [5.91,14.06]). However, the certainty of evidence of all these outcomes was assessed as "low." Subgroup analyses of acupuncture stimulation types and treatment duration all showed significant add-on effects comparing with rehabilitation alone. The safety of acupuncture was unclear because there is a lack of detailed reporting of adverse events in most of the included studies.Conclusions: Acupuncture therapy seems effective for motor function, pain relief and activities of daily living in stroke patients with mild SHS, when it is used in combination with rehabilitation. The low certainty of evidence downgrades our confidence in making recommendations to clinical practice.

Abstract:OBJECTIVE: To investigate the effectiveness of traditional manual acupuncture combined with rehabilitation therapy versus rehabilitation therapy alone for shoulder hand syndrome after stroke.DATA SOURCES: PubMed, EMBASE, the Cochrane Library, Chinese Biomedicine Database, China National Knowledge Infrastructure, VIP Information Database, Wan Fang Database and reference lists of the eligible studies were searched up to July 2017 for relevant studies.METHODS: Randomized controlled trials that compared the combined effects of traditional manual acupuncture and rehabilitation therapy to rehabilitation therapy alone for shoulder hand syndrome after stroke were included. Two reviewers independently screened the searched records, extracted the data and assessed risk of bias of the included studies. The treatment effect sizes were pooled in a meta-analysis using RevMan 5.3 software.RESULTS: A total of 20 studies involving 1918 participants were included in this study. Compared to rehabilitation therapy alone, the combined therapy significantly reduced pain on the visual analogue scale and improved limb movement on the Fugl-Meyer Assessment scale and the performance of activities of daily living (ADL) on the Barthel Index scale or Modified Barthel Index scale. Of these, the visual analogue scale score changes were significantly higher (mean difference = 1.49, 95% confidence interval = 1.15-1.82, P < 0.00001) favoring the combined therapy after treatment, with severe heterogeneity ( I2 = 71%, P = 0.0005).CONCLUSION: Current evidence suggests that traditional manual acupuncture integrated with rehabilitation therapy is more effective in alleviating pain, improving limb movement and ADL. However, considering the relatively low quality of available evidence, further rigorously designed and large-scale randomized controlled trials are needed to confirm the results.

Clinical Observation of Floating Needling Combined with Rehabilitation Training for Treatment of Post-stroke Shoulder-hand Syndrome

Abstract:Objective To observe the curative effect of floating needling combined with rehabilitation training for patients with post-stroke shoulder-hand syndrome. Methods Eighty shoulder-hand syndrome patients after stroke were randomly divided into 2 groups. Treatment group had 42 cases and were given rehabilitation training combined with floating needling. Control group had 38 cases and was treated with routine acupuncture combined with rehabilitation training. After 14 days of treatment, the shoulder-hand syndrome assessment scale(SHSS), pain visual analogue scale(VAS), neural function defect scale(NDS) and the upper limb motor function of upper limb Fugl-Meyer simplified scale(FMA) were used to evaluate the clinical curative effect in the two groups. Results(1) After treatment, the degrees of upper limb injury, pain, neurological deficits, and motor function were improved in the two groups, the difference being significant( P ﹤ 0.01 compared with those before treatment). And the improvement in the treatment group was superior to that in the control group(P﹤0.05).(2) The total effective rate was 92.86% in the treatment group and was 86.84% in the control group, and the difference was significant(P ﹤0.05). Conclusion The treatment of floating needling combined with rehabilitation is effective for the treatment of shoulder-hand syndrome, which is simple to operate and has a high clinical value.

The observation of effects to treat stroke and shoulder-hand syndrome by laser acupuncture irradiation joint acupoint sticking and rehabilitation training

Abstract:Objective:To observe the laser acupuncture point according to joint rehabilitation training and acupoints were applied to the curative effect of shoulder hand syndrome after stroke. Methods: 108 patients with shoulder hand syndrome after stroke were randomly divided into observation group and control group, 54 cases each. The control group take the rehabilitation training; Observation group on the basis of rehabilitation training and laser acupuncture point according to the acupoint sticking therapy, treatment time is 4 weeks. Respectively on two groups before and after therapy by using visual analog score(VAS), Barthel index(BI), improved Fulg-Meyer(FMA) method to assess degree of lateral upper limb edema, finger pain, joint activities, active ability of daily life(ADL) and upper limb movement function and assess its clinical curative effect. Results: Two groups after treatment of hemiplegia side, edema, shoulder pain, joint mobility Barthel index and FMA-Meyer scores were significantly improved than before treatment(P 0.05), the observation group is much better than the control group(P 0.05), and the clinical curative effect of observation group total effective rate(96.30%) were significantly better than the control group(74.07%), P 0.05). Conclusion: Laser acupuncture point and meridians to stick apply joint rehabilitation training has a good effect on shoulder hand syndrome after stroke.

Abstract:BACKGROUND: Acupuncture, traditional Chinese herb hot compress and rehabilitation training is useful in post-stroke shoulder–hand syndrome (SHS), but it is less commonly used in combination. OBJECTIVE: To analyse the therapeutic efficacy of a combination of acupuncture, traditional Chinese herb hot compress and rehabilitation training in SHS patients. METHOD: Patients diagnosed with post-stroke SHS and treated from January 2022 to March 2023 at Beijing Friendship Hospital and Beijing Pinggu Hospital were selected and randomly divided into three groups (A, B and C). Group A received conventional rehabilitative treatment (35 cases), Group B underwent conventional rehabilitative treatment + acupuncture (35 cases), and Group C received conventional rehabilitative treatment + acupuncture + traditional Chinese herb hot compress treatment (35 cases). The post-treatment effectiveness rate, visual analogue scale (VAS) score, activities of daily living (ADL) score, Fugel–Meyer assessment (FMA) score, SHS assessment scale (SHSS) score and shoulder joint mobility were compared among the three groups. RESULTS: The post-treatment VAS score (FVAS score= 232.512, P< 0.001), FMA score (FFMA score= 239.412, P< 0.001), ADL score (FADL score= 412.634, P< 0.001), SHSS score (FSHSS score= 212.542, P< 0.001) and shoulder joint mobility (FShoulder Joint Mobility= 197.531, P Group B > Group C, while the FMA, ADL and shoulder joint mobility scores increased in the order of Group C > Group B > Group A. The post-treatment effectiveness rates (χ2= 48.510, P Group B (82.86%) > Group A (71.43%). CONCLUSION: The treatment efficacy of the combined acupuncture, traditional Chinese herb hot compress and rehabilitation training for SHS patients is significant, warranting its clinical promotion.

Abstract:ObjectiveTo explore the effect of acupuncture combined with training of the modified rehabilitation apparatus on upper limb motor function of patients with ischemic stroke.Methods86 patients with ischemic stroke were divided randomly into the treatment group ( treated by acupuncture combined with training of the rehabilitation apparatus) and the control group (treated by simple acupuncture). The upper limb motor function of two groups was evaluated by Fugl-Meyer Assessment (FMA) before and after treatment.ResultsAfter 28 days treatment, scores of FMA of two groups were all raised significantly, but it was significantly greater for the treatment group than the control group ( P0.01).ConclusionIt is shown that acupuncture combined with training of the rehabilitation apparatus can improve the upper limb motor function and activity of daily living of patients with ischemic stroke.
Effectiveness of acupuncture for pain relief in shoulder-hand syndrome after stroke: a systematic evaluation and Bayesian network meta-analysis

Abstract:Background: Shoulder-hand syndrome (SHS) is a common complication after stroke, and SHS-induced pain significantly hampers patients' overall recovery. As an alternative therapy for pain relief, acupuncture has certain advantages in alleviating pain caused by SHS after stroke. However, choosing the best treatment plan from a variety of acupuncture options is still a serious challenge in clinical practice. Therefore, we conducted this Bayesian network meta-analysis to comprehensively compare the effectiveness of various acupuncture treatment methods. Methods: We systematically searched for randomized controlled trials (RCTs) of acupuncture treatment in patients with post-stroke SHS published in PubMed, Embase, Cochrane, and Web of Science until 9 March 2023. We used the Cochrane bias risk assessment tool to assess the bias risk in the included original studies. Results: A total of 50 RCTs involving 3,999 subjects were included, comprising 19 types of effective acupuncture interventions. Compared to single rehabilitation training, the top three interventions for VAS improvement were floating needle [VAS = -2.54 (95% CI: -4.37 to -0.69)], rehabilitation + catgut embedding [VAS = -2.51 (95% CI: -4.33 to -0.68)], and other multi-needle acupuncture combinations [VAS = -2.32 (95% CI: -3.68 to -0.94)]. The top three interventions for improving the Fugl-Meyer score were eye acupuncture [Meyer = 15.73 (95% CI: 3.4627.95)], other multi-needle acupuncture combinations [Meyer = 12.22 (95% CI: 5.1919.34)], and traditional western medicine + acupuncture + traditional Chinese medicine [Meyer = 11.96 (95% CI: -0.59 to 24.63)]. Conclusion: Multiple acupuncture methods are significantly effective in improving pain and upper limb motor function in post-stroke SHS, with relatively few adverse events; thus, acupuncture can be promoted. Abstract:OBJECTIVE To compare the clinical efficacy between wrist-ankle acupuncture and conventional acupuncture on shoulder-hand syndrome (SHS) phaseⅠafter stroke. METHODS A total of 64 patients with SHS phaseⅠafter stroke were randomized into a wrist-ankle acupuncture group and a conventional acupuncture group, 32 cases in each group. On the basis treatment of internal medicine and conventional rehabilitation, wrist-ankle acupuncture was applied at upper 4 area, upper 5 area and upper 6 area on the affected side in the wrist-ankle acupuncture group, while acupuncture was applied at Jianyu (LI 15), Quchi (LI 11), Shousanli (LI 10), etc. on the affected side in the conventional acupuncture group. The treatment was given 30 min each time, once a day, 5 days a week for 3 weeks in both groups. Before and after treatment, the visual analogue scale (VAS) score, degree of hand swelling, shoulder-hand syndrome scale (SHSS) score, Fugl-Meyer assessment for upper extremity (FMA-UE) score and modified Barthel index (MBI) score were observed, and the clinical therapeutic effect was evaluated in both groups. RESULTS After treatment, the VAS scores, degree of hand swelling and SHSS scores were decreased (P<0.05), and the FMA-UE scores and MBI scores were increased (P<0.05) compared before treatment in both groups; in the wrist-ankle acupuncture group, the VAS score, degree of hand swelling and SHSS score were lower (P<0.05), and the FMA-UE score and MBI score were higher (P<0.05) than those in the conventional acupuncture group. The total effective rate was 96.9% (31/32) in the wrist-ankle acupuncture group, which was superior to 90.6% (29/32) in the conventional acupuncture group (P<0.05). CONCLUSION Wrist-ankle acupuncture can effectively relieve pain and hand swelling, improve motor function of upper extremity and self-care ability of daily life in patients with shoulder-hand syndrome phaseⅠafter stroke, the therapeutic effect is superior to conventional acupuncture.

Abstract:Objective In order to explore the effects of rehabilitation training and acupuncture on the neuralfunction deficit and motor function in patients with ischemic stroke. Methods Eighty patients with ischemic strokewere randomly divided into rehabilitation and acupuncture groups. According to different recovery stages,the rehabilitation group received purposeful rehabilitation training for 28 days while the acupuncture group received scalpneedling combined with body acupuncture. The neural deficit scores (NDS) and motor function comprehensive assessmen(tFCA)were evaluated at the recruitmen(tM0),the 28t(hM1)and 56t(hM2)days after treatment. Results Nosignificant differences were found in the NDS and motor FCA between the rehabilitation and the acupuncture groupsat the recruitment. The significant differences appeared at the 28th and the 56th days comparing with baseline assessments in each group. There was no significant difference in the changes of NDS and motor FCA between thetwo groups at 28th day,however,the NDS and motor FCA in rehabilitation group revealed better outcome than the acupuncture group at the 56th day. Conclusion The study indicated that both rehabilitation training and acupuncture could improve the neural function and enhance the motor function in patients with ischemic stroke.
The integrated treatment on rehabilitation for cerebral apoplexy patients complicated with shoulder-hand syndrome

Abstract:Objective:To observe the effect of hemiplegic shoulder-hand syndrone by integrated rehabilitation in stroke patients.Methods:185 stroke patients with hemiplegia were randomly divided into integrated rehabilitation group(observe group) and control group.Two groups all received routine rehabilitation.The observe group were subjected to many rehabilitation disciplinary methods such as the corrective positioning of limbs and trunk,compressive centripetal wrapping , cold-warm water marinated alternant, etc. Four weeks after treatment,the efficacy was evaluated.Results:The efficacy on the observe group was significantly higher than in control group(P0.01).Compared with that before treatment, the VAS score on shoulder pain,ROM of joint,edema of affected limb were obviously improved on the observation group after treatment,and the VAS score also had a significant difference between the two groups after treatment(P0.05).The FMA and BI scores in the two groups was markedly improved after treatment(P0.05 ),especially in observation group(P0.05).Conclusion:the integrated rehabilitation has significant effectiveness on rehabilitation for stroke patients complicated with shoulder-hand syndrome,and is helpful to recover the function of shoulder joint.

Abstract:Objective To study the therapeutic effect of acupuncture plus rehabilitation training in the treatment of post-stroke upper limb spasticity.Method Thirty patients with post-stroke upper limb spasticity were randomized into a treatment group and a control group,15 in each.The treatment group was given acupuncture to Jing-well points as well as upper anti-spasm points(original),and the rehabilitation training would begin afterwards when the upper limb achieved real-time relief.The control group only received rehabilitation training.Both of the two groups were evaluated by using the Modified Ashworth Scale(MAS) and Fugl-Meyer Assessment(FMA) for the upper limb before intervention and a month after intervention.Results Compared to the control group,the treatment group had advantage in regulating muscular tension and improving the motor function of upper limb,and the differences were statistically significant(P<0.01).Conclusion Puncturing Jing-well points and the upper anti-spasm points has a real-time relieving effect for post-stroke upper limb spasticity.It can also enhance the effect of the rehabilitation training.

Abstract:Objective To summarize and evaluate the effects of acupuncture combined with rehabilitation training for post‐stroke shoulder pain(PSSP) .Method Randomized controlled trials (RCTs) involving the effects of acupunc‐ture combined with rehabilitation training for PSSP ,published from the beginning of databases to February 2015 , were obtained from the Cochrane Library ,JBI(Joanna Briggs Institute) ,evidence‐based nursing database ,PubMed , EMbase ,CBM ,Wanfang Data and CNKI Databases .Result 13 RCTs involving 869 patients met the inclusion crite‐ria .Meta‐analysis showed that acupuncture combined with rehabilitation training made significant differences on pain improvement[95% CI(0 .92 ,2 .09) ,P<0 .00001] ,upper extremity motor function[95% CI(1 .16 ,11 .60) ,P=0 .02] and the activity of daily life[95% CI (3 .00 ,19 .49) ,P=0 .007] .Conclusion Acupuncture combined with reha‐bilitation training is more effective than rehabilitation training alone for PSSP .Since the quantity of inclusive studies is limited ,it is recommended that more future trials be conducted on this topic to verify the conclusion .

Abstract:Stroke is a disease with a high disability rate, having a serious impact on that patient’s working and daily survival quality and bringing economic burden to the family and society. Patients with stroke hemiplegia are mostly tetraplegic and have difficulty regulating their balance, and their long-term symmetry has been destroyed. The application in the rehabilitation process of acupuncture in patients with hemorrhagic stroke may produce unexpected effects. It is very effective to study the effect of acupuncture combined with rehabilitation training on limb movement and patient survival. It is very helpful in improving normal motor function and normal life, increasing joint mobility and muscle strength, and reducing muscle tension. In this paper, it is found that the observational group has a complication rate of 2.13%, in contrast to 17.02% as in the group of control, and the pin-prick combined with a rehabilitative training makes a significant improvement to the patients. This study provides suggestions for the study to investigate acupuncture combined with recovery exercise on limb movement and living capacities of people with stroke paraparesis.

Abstract:BACKGROUND Eighty percent of stroke patients develop upper limb dysfunction, especially hand dysfunction, which has a very slow recovery, resulting in economic burden to families and society. AIM To investigate the impact of task-oriented training based on acupuncture therapy on upper extremity function in patients with early stroke. METHODS Patients with early stroke hemiplegia who visited our hospital between January 2021 and October 2022 were divided into a control group and an observation group, each with 50 cases. The control group underwent head acupuncture plus routine upper limb rehabilitation training (acupuncture therapy). In addition to acupuncture and rehabilitation, the observation group underwent upper limb task-oriented training (30 min). Each group underwent treatment 5 d/wk for 4 wk. Upper extremity function was assessed in both groups using the Fugl-Meyer Assessment-Upper Extremity (FMA-UE), Wolf Motor Function Rating Scale (WMFT), modified Barthel Index (MBI), and Canadian Occupational Performance Measure (COPM). Quality of life was evaluated using the Short-Form 36-Item Health Survey (SF-36). Clinical efficacy of the interventions was also evaluated. RESULTS Before intervention, no significant differences were observed in the FMA-UE, MBI, and WMFT scores between the two groups (P > 0.05). After intervention, the FMA-UE, WMFT, MBI, COPM-Functional Mobility and Satisfaction, and SF-36 scores increased in both groups (P < 0.05), with even higher scores in the observation group (P < 0.05). The observation group also obtained a higher total effective rate than the control group (P < 0.05). CONCLUSION Task-oriented training based on acupuncture rehabilitation significantly enhanced upper extremity mobility, quality of life, and clinical efficacy in patients with early stroke.

Abstract:Background: Acupuncture is a well-known treatment option for ischemic stroke recovery, but evidence of its effectiveness remains limited. This is a randomized controlled trial to evaluate the effectiveness of acupuncture treatment for ischemic stroke rehabilitation. Methods: Rehabilitation training was provided to the control group. In acupuncture arm 1, these acupoints were derived from the ancient books, including GV20 ( baihui ), GV26 ( shuigou ), PC9 ( zhongchong ), ST6 ( jiache ), ST4 ( dicang ), LI15 ( jianyu ), LI11 ( quchi ), LI4 ( hegu ), GB30 ( huantiao ), GB31 ( fengshi ), GB34 ( yanglingquan ), and GB39 ( xuanzhong ). In acupuncture arm 2, the acupoints used were GV20 ( baihui ), PC6 ( neiguan ), LI11 ( quchi ), LI10 ( shousanli ), SJ5 ( waiguan ), LI4 ( hegu ), GB30 ( huantiao ), ST36 ( zusanli ), GB34 ( yanglingquan ), SP6 ( sanyinjiao ), ST41 ( jiexi ), and LR3 ( taichong ), which were extracted from Acupuncture and Moxibustion Science . After acupuncture, the needles were left in for 30 min and manually manipulated every 10 min. The three groups received treatment once a day, 5 times a week for 2 weeks. The primary outcome was the National Institutes of Health Stroke Scale (NIHSS), and the secondary outcomes were the Barthel Index (BI) and the Modified Ashworth Scale (MAS). Outcomes were measured in patients both before and after treatment. Results: A total of 497 patients with ischemic stroke were randomized into either arm 1 (159 cases), arm 2 (173 cases), or the control group (165 cases). After 2 weeks of treatment, the NIHSS scores for arm 1 were lower than those of the control group ( P = 0.017); the BI scores were higher in arm two than that in the control group at T2 ( P = 0.016) and follow-up ( P = 0.020). Additionally, there was no significant difference between arm one and the control group for either the BI scores or the MAS scores ( P > 0.05) and no significant difference between arm two and the control group for the MAS scores or the NIHSS scores ( P > 0.05). Conclusion: The clinical efficacy of arm 1 and arm 2 (acupuncture groups) was superior to that of the control group, but there was no difference between the effects of the two acupuncture groups. Effectiveness of Huangqi Guizhi Wuwu decoction combined with rehabilitation training for shoulder hand syndrome after stroke: A systematic review and meta-analysis

Abstract:Background: Shoulder hand syndrome (SHS) is a common complication of stroke. This meta-analysis aimed to evaluate the effectiveness of Huangqi Guizhi Wuwu decoction (HGWD) combined with rehabilitation training in managing it, as its efficacy remains inconclusive. Methods: Seven databases, including PubMed, EMBASE, Cochrane Library, SinoMed, Chinese National Knowledge Infrastructure, Wanfang Data, and VIP database were searched in this study. The search deadline was April 30, 2023. Randomized controlled trials that included either standalone rehabilitation training or HGWD combined with rehabilitation training were included, and data were independently extracted by 2 reviewers who assessed the risk of bias. Results: Thirteen studies involving 1270 patients were included in this study. Meta-analysis showed that the combined treatment was significantly more effective than standalone rehabilitation therapy (odds ratio = 4.49; 95%CI: 2.98-6.76; Z = 7.17; P < .00001). Compared with the control group, the intervention group had a lower visual analog scale score (mean difference [MD] = -2.80, 95%CI (-3.15, -2.45), Z = 15.84, P < .00001). In addition, the Fugl-Meyer assessment scale score improved (MD = 9.69, 95%CI (7.60, 11.78), Z = 9.08, P < .00001). The SHS score in the intervention group decreased more compared to the control group (standard mean difference = -2.27, 95%CI (-3.19, -1.34), Z = 4.79, P < .00001). Serum biomarkers related to SHS decreased, including serum substance P (MD = -7.52, 95%CI (-8.55, -6.48), Z = 14, P < .00001) and bradykinin (MD = -1.81, 95%CI (-2.68, -0.95), Z = 4.1, P < .00001). Although there was no statistical difference in joint mobility score (MD = -4.19, 95%CI (-8.16, -0.22), Z = 4.79, P = .28), sensitivity analysis after excluding one study still suggested that the joint mobility score of the combined treatment group was higher than that of the standalone rehabilitation treatment group. Conclusion: The results of this study indicate that HGWD combined with rehabilitation training may be more effective in treating SHS after stroke compared to standalone rehabilitation therapy.
Effect of Electroacupuncture on Shoulder Subluxation in Poststroke Patients with Hemiplegic Shoulder Pain: A Sham-Controlled Study Using Multidimensional Musculoskeletal Ultrasound Assessment

Abstract:Objective: This study aimed to use multidimensional musculoskeletal ultrasound imaging technique to investigate the effect of electroacupuncture (EA) on shoulder subluxation in poststroke patients with hemiplegic shoulder pain. Methods: In this prospective single-blind, randomized, sham-controlled study, thirty-four patients with shoulder subluxation and hemiplegic shoulder pain were recruited and randomly assigned into the EA group or the sham EA (SEA) group. In the EA group, EA was applied to the Jian yu (LI15), Bi nao (LI14), Jian zhen (SI9), and Jian liao (TE14) acupoints. In the SEA group, the EA was applied 15 mm away from the Lou gu (SP7), Di ji (SP8), Jiao xin (KI8), and Zhu bin (KI9) acupoints. Both groups underwent treatment 30 minutes/day, five days a week, for two weeks using dense waves with a frequency of 2/100 Hz. A Visual Analogue Scale (VAS) was used to evaluate the effectiveness of treatment in reducing shoulder pain. Musculoskeletal ultrasound was used to evaluate the changes of measures of shoulder subluxation in multidimensions (i.e., the acromiohumeral distance, AHD; acromion-greater tuberosity, AGT; and acromion-lesser tuberosity, ALT). Both the within- and between-groups treatment effects were assessed. Results: The pain intensity measured by VAS and shoulder subluxation measured by musculoskeletal ultrasound (i.e., AHD, AGT, and ALT) showed significant (p < 0.05) within-group difference in both groups. The between-group difference appeared in the pain intensity (p < 0.05), while it disappeared in the three measures of shoulder subluxation (p > 0.05). Conclusions: Using VAS for measuring pain intensity and multidimensional musculoskeletal ultrasound imaging technique for measuring shoulder subluxation, this study finds that the hemiplegic shoulder pain can be improved significantly by the EA while the shoulder subluxation cannot be. Our findings further reveal the analgesic mechanism of EA on hemiplegic shoulder pain following stroke.
