Changes of levels of tumor necrosis factor in human chronic wound during the vacuum assisted closure

Abstract:AIM: To study whether vacuum-assisted closure (VAC) promotes chronic wound healing induced by the decrease of inflammatory reaction by the inhibition of synthesis of tumor necrosis factor (TNF) or not. METHODS: Twenty-one patients with acute or chronic wound, who were treated at the Tangdu Hospital, Fourth Military Medical University from April 2001 to April 2002 as experimental objects, in which the acute wound patients were breast cancer patients from common surgery with 10 cases, and the chronic wound patients were patients from Department of Plastics and Burns with 11 cases. All the patients were without other severe diseases. Diagnosis criteria of chronic wound: The patients with wound forming for over 1 month, but without healing trend, and were treated with traditional method, but inefficiency. The wound drainage liquor at 1, 2 and 3 days after breast cancer operation in 10 patients with acute wound were collected, meanwhile the wound exudates at the 1, 3, 5 and 7 days before and after VAC operation in 11 patients with chronic wound (5 cases with pressure ulcer and 6 cases with venous ulcer). The content of TNF of acute and chronic wound were detected with sandwich enzyme-linked immunosorbent assay (Sandwich-ELISA), respectively, and dynamic observation and comparison were performed. RESULTS: Totally 10 included patients with acute wound and 11 patients with chronic wound were involved in the result analysis. ① Comparison of content of TNF at different time after acute wound operation: The TNF value at the 1st day after the breeding of drainage liquor of acute wound was low at (0.35±0.07) μg/L, and decreased at the 2nd day at (0.12±0.04) μg/L, and increased at the 3rd day at (0.24±0.08)μg/L. The difference was significant between the 1st day and the 2nd day (t=3.042,P=0.008). ② Comparison of TNF content of before and after chronic wound treatment: The level of TNF before treatment in venous ulcer and pressure ulcer of chronic wound exudates was higher, and the former was significant (0.82 ±0.14),(0.45±0.05)μg/L. The level of TNF decreased with the time prolongation of VAC operation. The difference was significant at the 1, 3 and 5 days after VAC in the former (t=2.909, P=0.008;t=3.223, P=0.005), while the difference was significant at the 5 and 7 days after VAC in the latter (t=2.059,P=0.037). CONCLUSION: The level of TNF is low in the exudates of acute wound, and appears slight fluctuation with the prolongation of time While the level of TNF is high in chronic wound, and the venous ulcer is more obvious, and the TNF decreases with the time prolongation of VAC operation. It is indicated that the chronic wound has severe inflammatory reaction before the VAC operation, and the inflammatory reaction is controlled after treatment. It is verified that the increase of TNF is an important factor affecting the formation of chronic wound.
