Effect of case management model on the quality of life in elderly depression in the community

Abstract:Objective:To explore the effect of case management model on the quality of life in elderly depression in the community.Methods:120 elderly depressions were randomly divided into intervention group and control group.Intervention group patients had access to comprehensive care which was provided by psychiatrists,primary care doctors and case managers with health education,psychotherapy and antidepressants,etc.,while control group patients were treated in usual medical model.The period of observation was 12 weeks.Efficacy and safety were assessed with HAMD17 and TESS at baseline,week 2,4,6,8 and 12 after treatment.And the quality of life was evaluated with SF-12 at baseline,week 4,8 and 12 after treatment.Results:At 12th week of treatment,the HAMD17 total score(14.19±7.66)in intervention group was significantly lower than those(24.93±5.86)in control group(P =0.000).The rates of response(83.1%)and remission(27.1%)in intervention group were much higher than those of control group(39.3%,5.4%),and the differences were statistically significant(P =0.000 and 0.002).By the end of period,the MCS(41.04±9.57)and the PCS(39.34±9.23)of SF-12 in intervention group were significantly higher than those(30.30±9.90,30.45±6.77 respectively)in control group(both P =0.000).All adverse effects were mild in both groups.Conclusion:The case management model appears to be effective to relief depressive symptoms and significantly improve the quality of life.
