Relationship between Emergency Onset of Cerebral Infarction and Preceding Infection

Abstract:Objective To examine impact of preceding infection on acute onset of cerebral infarction(ACI) in emergency setting.Methods The retrospective analysis was used for the data of consecutive 2 253 patients from the Department of Emergency from January 1,2010 to December 31,2012,with the data of concurrent 4 348 patients with hypertension as controls from the same department.Results The infection rate within one week before onset of the ACI group was 13.3%(299 /2 253),being higher than 2.6%(114 /4 348) of the hypertension group(P < 0.001).Logistic regression analysis of ACI onset showed the odds ratio(OR) of recent infection was 2.735 〔95% CI(1.611,5.423),P = 0.004〕and that of infection within 1 week before onset was 6.589 〔95% CI(3.754,24.257),P < 0.001〕.The infection rate of ACI group was 10.8%(243 /2 253) within 3 days before onset,and 2.5%(56 /2 253) 4 to 7 days before onset,being respectively higher than 1.2%(51 /4 348) and 1.4%(63 /4 348) of the hypertension group(P < 0.01).The number of infection cases 7 days before onset in the ACI group accounted for 67.8%(299 /441) of the cases 28 days before onset,and the number 3 days before accounted for 81.3%(243/299) 7 days before,as compared respectively with 26.1%(114/437) and 44.7%(51/114) of the hypertension group the differences were significant(P < 0.001).As for the infectious foci 28 day before onset,in the ACI group the case number of infection accounted for 10.3%(233 /2 253) for respiratory tract,4.3%(98 /2 253) for gastrointestinal tract or 2.8%(64 / 2 253) for the gum,as compared with the hypertension group the differences were statistically significant(P < 0.01).In the pathogenic microorganism,the cases of recent infection accounted for 9.8%(220 /2 253) for bacteria,for 4.2%(95 /2 253) for viruses,for 5.6%(126 /2 253) for the mixed microorganisms,or un-certain microorganism,or uncertain causes,as compared with the hypertension group the differences were statistically significant(P < 0.01).Conclusion Infection in the preceding period can increase the risk of onset of ACI,and it is an inducing factor of ACI.
