Evidence-Based Therapy for A Patient with First-episode Schizophrenia

Abstract:Objective To develop an individualized treatment plan by evidence-based medicine methods for a patient with first-episode schizophrenia.Methods After a complete assessment of the patient′s condition,clinical questions were put forward according to PICO principle.Treatment plan was made based on evidences collected from Cochrane Library(1991-2011),Medline(1991-2011),Guideline,and patient′s preference.Results A total of 12 systematic reviews/meta-analyses,1 randomized controlled trial and 1 clinical guideline were included.The evidence showed that the conventional and atypical antipsychotics are comparable in efficacy.The atypical antipsychotics are considered as first-line treatments because of the lower risk of side effects comparing to conventional antipsychotics.Olanzapine has superior efficacy and lower risk of side effects comparing to other atypical antipsychotics but carries greater risk of weight gain.Family intervention and cognitive behavior therapy are suggested to be effective.We made a treatment plan based on the evidence and patient′s preference: Olanzapine was given at the initial dose of 5 mg/d,the dosage increased to 15 mg/d in first week,in combinatioin with family intervention and cognitive behavior therapy.The patient′s positive symptoms improved significantly after one week treatment.The patient received olanzapine 15 mg daily for maintenance therapy after discharge.After 6 months of follow-up,the patient didn′t present with signs of recurrence or significant side effects.Conclusion Evidence-based treatment can determine a rational therapy for first-episode schizophrenia,thus can improve therapeutic effect.
