Abstract:Leukoaraiosis is a common imaging marker of cerebral small vessel disease. In recent years, with the continuous advances in brain imaging technology, the detection rate of leukoaraiosis is higher and its clinical subtypes are gradually gaining attention. Although leukoaraiosis has long been considered an incidental finding with no therapeutic necessity, there is now growing evidence linking it to, among other things, cognitive impairment and a high risk of death after stroke. Due to different research methods, some of the findings are inconsistent and even contradictory. Therefore, a comprehensive and in-depth study of risk factors for leukoaraiosis is of great clinical significance. In this review, we summarize the literature on leukoaraiosis in recent years with the aim of elucidating the disease in terms of various aspects (including pathogenesis, imaging features, and clinical features, etc.).

Better Correlation of Cognitive Function to White Matter Integrity Than to Blood Supply in Subjects with Leukoaraiosis

Abstract:Leukoaraiosis is associated with increased risk of cognitive impairment, but its pathophysiological pathway is unclear. The aim of the present study was to determine whether brain structural damage or cerebral blood supply better correlated with the global cognitive outcome in subjects with leukoaraiosis. Seventy-five subjects with leukoaraiosis were included in present study, with age ranged from 43 to 85 years, with mean white matter hyperintensities (WMH) volume 30.69 ± 24.35 mL. Among them, 19(25.33%) subjects presented with cerebral microbleeds (CMB) and 40 (53.33%) subjects presented with lacunes. These participants received arterial spin labeling perfusion MRI, diffusion-tensor imaging (DTI) and diffusion Kurtosis imaging. We analyzed the cerebral blood flow (CBF) by dividing the brain tissue into three regions: WMH, normal appearing white matter (NAWM) and cortex. After adjusting for age and gender, the CBF of NAWM was significantly correlated with fractional anisotropy (FA) (r = 0.336, p = 0.004) and mean diffusion (MD) (r = -0.271, p = 0.020) of NAWM, while there lacked of association between CBF of cortex and mean kurtosis (MK) of cortex (r = -0.015, p = 0.912). Meanwhile, both NAWM-FA (r = -0.443, p < 0.001) and NAWM-MD (r = 0.293, p = 0.012), as well as cortex-MK (r = -0.341, p = 0.012) was significantly correlated with WMH volume. Univariate regression analysis demonstrated that global cognitive function was significantly associated with mean FA or MD of both WMH and NAWM, and cortex-CBF, but neither with the cortex-MK, nor the presences of CMB or lacunes. Finally, multiple linear regression analysis revealed that global cognitive function was independently associated with NAWM-FA (standardized β = 0.403, p < 0.001) and WMH-FA (Standardized β = 0.211, p = 0.017), but not with the cortex-CBF. A model that contained NAWM-FA, WMH-FA and years of education explained 49% of the variance of global cognitive function. Cerebral perfusion status might have a significant impact on the maintenance of white matter integrity in subjects with leukoaraiosis. Global cognitive function was more strongly associated with white matter integrity than with blood supply. DTI parameters, especially FA could serve as a potent imaging indicator for detecting the invisible alteration of white matter integrity and implying its potential cognitive relevance.

Abstract:Concerning that the lesion areas of leukoaraiosis in Magnetic Resonance(MR) image present hyper intense signal on T2 flair sequence,a level set segmentation method based on C-V model was proposed.First,the C-V model was improved to avoid the re-initialization;second,the Otsu threshold method was used for image's pre-segmentation,and then the image's pre-segmentation result was directly used as the initial contour for the improved C-V model;finally,the segmentation result was obtained by curve evolution.The results show that the proposed segmentation method can get better separation effects,and realize fast auto-segmentation.It has certain application value for clinical diagnosis and prognosis on leukoaraiosis.

Abstract:The term Leukoaraiosis is used to describe diffuse white matter abnormalities on computer tomography(CT)or magnetic resonance imaging(MRI)brain scans.It is one type of small vessel disease and defined as diffuse,confluent white matter abnormality(low density on CT,hyperintensity on T_2-weighted or fluid-attenuated inversion-recovery MR scans).This article reviews the research advance in mechanisms of Leukoaraiosis in five theories as hypoperfusion, brain-blood barrier dysfunction,endothelium,β-amyloid deposit and gene polymorphism.
Relationship Between Cerebral Atherosclerosis and Leukoaraiosis in Aged Patients: Results from DSA

Abstract:BACKGROUND AND PURPOSE:There are some controversial results on the relationship between cerebral atherosclerosis and leukoaraiosis in the published papers, where cerebral atherosclerosis was often evaluated by ultrasonography, CTA or MRA. We analyzed data in which patients underwent both MRI and DSA to explore the above relationship in the aged people.METHODS:Three hundred and thirty-three patients were enrolled in the study from the Nanjing Stroke Registry. They underwent both cerebral DSA and MRI. Age, sex, and vascular risk factors were collected. Atherosclerosis was scored from grade 0 to 4. Leukoaraiosis was scored from grade 0 to 3.RESULTS:Cerebral artery stenosis was not correlated with the presence of leukoaraiosis. There were no correlations between the number of cerebral moderately or more severely stenotic arteries and the severity of leukoaraiosis in periventricular, deep, or whole white matter (P = .747, .268, and .608, respectively). Old age (odds ratio = 1.103, P = .027) and hypertension (odds ratio = 2.748, P = .003) were correlated with leukoaraiosis in the periventricular white matter. Old age (odds ratio = 1.073, P = .031) and prior stroke (odds ratio = 2.678, P = .002) were correlated with leukoaraiosis in the deep white matter.CONCLUSIONS:No apparent correlation exists between cerebral artery stenosis and the presence and severity of leukoaraiosis.
Relationship Between the Prothrombotic State and Leukoaraiosis in Elderly Patients with Metabolic Syndrome

Abstract:Objective To investigate the relationship between the prothrombotic state and leukoaraiosis(LA)in elderly patients with metabolic syndrome.Methods A total of 350 metabolic syndrome patients in the affiliated hospital of Zhejiang University who received treatment from January 2013 to January 2015.According to the results of head MRI,patients were divided into LA group(n=203)and non-LA group(n=147).The severity of LA was graded by its MRI appearance and the Blennow scale.The plasma levels of fibrinogen (Fib),prothrombin time (PT),thrombin time (TT),activated partial blood coagulation time(APTT),D-dimer (D-D) and yon willebrand factor (vWF) were measured and compared between the two groups.Results The levels of Fib,vWF and D-D were higher in LA group than in non-LA group(all P＜0.05).Spearman correlation analysis showed that the severity of leukoaraiosis was positively correlated with Fib,vWF and D-D(r=0.014,0.089,0.215,respectively,all P ＜ 0.05).Multivariate linear regression analysis showed that Fib,course of hypertension,vWF and HbA1c were the main influencing factors with the standardized regression coefficient of 2.516,0.312,0.206 and 0.167 respectively for leukoaraiosis in elderly patients with metabolic syndrome.Conclusions The course of hypertension and HbAlc level are positively related with LA.The prothrombotic state markers including Fib and vWF can be considered as the predictors for LA severity in elderly patients with metabolic syndrome.

Abstract:Ischemic stroke is characterized by high morbidity, high disability rate, high mortality, and high recurrence rate, which can have a grave impact on the quality of life of the patients and consequently becomes an economic burden on their families and society. With the developments in imaging technology in recent years, patients with acute cerebral infarction are predominantly more likely to be diagnosed with leukoaraiosis (LA). LA is a common degenerative disease of the nervous system, which is related to cognitive decline, depression, abnormal gait, ischemic stroke, and atherosclerosis. The etiology of LA is not clear and there is no gold standard for imaging assessment. Related studies have shown that LA has an adverse effect on the prognosis of cerebral infarction, but some experts have contrary beliefs. Hence, we undertook the present review of the literature on the mechanism and the effect of LA on the prognosis of patients with acute ischemic stroke.

Abstract:Background and Purpose: Leukoaraiosis, also called white matter hyperintensities (WMH), is frequently encountered in the brain of older adults. During aging, gray matter structure is also highly affected. WMH or gray matter defects are commonly associated with a higher prevalence of mild cognitive impairment. However, little is known about the relationship between WMH and gray matter. Our aim was thus to explore the relationship between leukoaraiosis severity and gray matter volume in a cohort of healthy older adults. Methods: Leukoaraiosis was rated in participants from the PROOF cohort using the Fazekas scale. Voxel-based morphometry was performed on brain scans to examine the potential link between WMH and changes of local brain volume. A neuropsychological evaluation including attentional, executive, and memory tests was also performed to explore cognition. Results: Out of 315 75-year-old subjects, 228 had punctuate foci of leukoaraiosis and 62 had begun the confluence of foci. Leukoaraiosis was associated with a decrease of gray matter in the middle temporal gyrus, in the right medial frontal gyrus, and in the left parahippocampal gyrus. It was also associated with decreased performances in memory recall, executive functioning, and depression. Conclusion: In a population of healthy older adults, leukoaraiosis was associated with gray matter defects and reduced cognitive performance. Controlling vascular risk factors and detecting early cerebrovascular disease may prevent, at least in part, dementia onset and progression.

Corelation Study of Cystatin C and Leukoaraiosis

Abstract:Aim To explore the relationship between the level of cystatin C (CYSC) and leukoaraiosis,as well as the level of CYSC in different degrees ofleukoaraiosis.Methods The clinical data were studied between 213 patients with leukoaraiosis and 201 normal control to estimate the risks of leukoaraiosis.Fazekas grading of leukoaraiosis were evaluated to analyze the CYSC level in different degree of leukoaraiosis.Results There were significant difference between the normal control and patients with leukoaraiosis in hypertension,glycosylated hemoglobin,renal function,plasma homocysteine and CYSC (P＜0.05),which indicated that they were related to the occurrence of cerebral white matter lesions.Conclusion The patients with leukoaraiosis were classified by image of MRI,and the results indicated that CYSC level were raised with the increased scores in leukoaraiosis.
Magnetic resonance spectroscopy study of leukoaraiosis in patients with cerebral infarction

Abstract:Objective:To study the feature of MR spectroscopy in patients with cerebrovascular disease-related leukoaraiosis.Methods:51 patients with leukoaraiosis and 21 patients with non-leukoaraiosis were selected to be examed by magnetic resonance spectroscopy(MRS).The concentrations of N-acetylaspartate(NAA),Choline(Cho),Creatine(Cr)were measured in bilateral frontal lobe white matter and the ratio of NAA/Cr and Cho/Cr were calculated.Results:There is no significant difference in NAA/Cr and Cho/Cr between leukoaraiosis group and non-leukoaraiosis group,P>0.05;Compared with the patients with non-leukoaraiosis,mild leukoaraiosis,midrange leukoaraiosis,Cho/Cr value in patients with serious leukoaraiosis increased significantly.Cho/Cr value was positively correlated with the degree of leukoaraiosis.Conclusion:Increasement of Cho/Cr is the main feature of MRS in patients with leukoaraiosis of bilateral frontal lobe white matter,and the value of Cho/Cr increases gradually with exacerbation of white matter impairment.The fundings show that leukoaraiosis is nerve fiber damage,and not nerve cell loss.
Voxel-based Morphometric Study of Brain Structure in Patients of Leukoaraiosis

Abstract:Objective:To explore the abnormalities in structure and distribution of gray matter (GM) in leukoaraiosis (LA) patients by voxel-based morphometric (VBM).Methods:This study involved 34 LA patients with cognitive impairment and 23 normal individuals (control group);subjects were matched for age,sex,and education level.All participants underwent neurocognitive tests and 3.0T magnetic resonance imaging (MRI).Based on cognitive scale ratings,the LA group was further divided into mild cognitive impairment (LA-MCI) and vascular dementia (LA-VaD) groups.VBM was employed to assess characteristics of GM density and volume in all participants,and location and distribution of reduced GM density in LA patients were analyzed for clinical significance.Results:Compared with the control group,the LA-VaD group showed decreased GM density of the left prefrontal cortex and Heschl's gyrus (P＜0.05);the LA-MCI group displayed decreased GM density of the right frontal cortex,left parietal gyrus,and left caudate nucleus (P＜0.05);compared with the control group,the LA-MCI group showed decreased GM density in the left middle frontal gyrus and right middle frontal gyrus (P＜0.05).Executive function scores of LA patients with cognitive impairment were significantly correlated with left middle frontal lobe GM volume (P＜0.05).Conclusion:There are broad areas of gray matter loss in LA patients.Decline of executive function is significantly correlated with loss of gray matter.
Resting-state Network of Brain in Leukoaraiosis Patients:A Magnetic Resonance Imaging Study

Abstract:Objective To explore the diversity of resting-state network of brain between the patients with leukoaraiosis and the healthy people. Methods 31 patients with leukoaraiosis (patients) and 27 healthy persons (controls) were checked with resting-state functional mag-netic resonance imaging (rs-fMRI), and analyzed with the independent component analysis (ICA) to explore the resting-state functional brain network. Results The resting-state brain network was found in both the patients and the controls, which was coincident with the previ-ous studies. The active areas were the same in both groups, and the activation was weaken in the patients than in the controls, especially in quadrate gyri, posterior cingulate cortex, superior temporal gyrus, superior parietal gyrus, anterior central gyrus, post central gyrus, insula and prefrontal cortex. Conclusion There is a significant diversity of resting-state network of brain between the patients with leukoaraiosis and healthy people in the activation of active areas.
Increased Visibility of Deep Medullary Veins in Leukoaraiosis: A 3-T MRI Study

Abstract:Cerebral venous collagenosis has been implicated in leading to white matter hyperintensities (WMHs) via venous ischemia. We sought to determine whether cerebral venous dilation or ischemia correlate with the severity of WMHs by quantitative in vivo imaging techniques. This was an investigator-initiated prospective single-center study. We reviewed clinical, laboratory data from 158 consecutive WMHs patients and 50 controls, and measured the number of voxels of deep medullary veins (DMVs) on susceptibility-weighted image and assessed the WMH volume (as a marker of the severity of WMHs) on a 3-T magnetic resonance system. We then performed the logistic-regression analysis and partial Pearson's correlation analysis to examine the association between the venous voxel count and WMH volume. The number of voxels of DMVs was significantly higher in WMHs than in controls. Increased number of voxels of DMVs was independently associated with both WMH volume of the whole brain and coregistered regional WMH volume after adjusting for age and number of lacunes. Our study indicates that cerebral deep venous insufficiency or ischemia play a role in the pathogenesis of WMHs, which may provide prognostic information on patients with WMHs and may have implications for therapeutic interventions.
Leukoaraiosis is Associated with Clinical Symptom Severity, Poor Neurological Function Prognosis and Stroke Recurrence in Mild Intracerebral Hemorrhage: a Prospective Multi-Center Cohort Study.

Abstract:Leukoaraiosis (LA) results from ischemic injury in small cerebral vessels, which may be attributable to decreased vascular density, reduced cerebrovascular angiogenesis, decreased cerebral blood flow, or microcirculatory dysfunction in the brain. In this study, we enrolled 357 patients with mild intracerebral hemorrhage (ICH) from five hospitals in China and analyzed the relationships between LA and clinical symptom severity at admission, neurological function prognosis at 3 months, and 1-year stroke recurrence. Patients were divided into groups based on Fazekas scale scores: no LA (n = 83), mild LA (n = 64), moderate LA (n = 98) and severe LA (n = 112). More severe LA, larger hematoma volume, and higher blood glucose level at admission were associated with more severe neurological deficit. More severe LA, older age and larger hematoma volume were associated with worse neurological function prognosis at 3 months. In addition, moderate-to-severe LA, admission glucose and symptom-free cerebral infarction were associated with 1-year stroke recurrence. These findings suggest that LA severity may be a potential marker of individual ICH vulnerability, which can be characterized by poor tolerance to intracerebral attack or poor recovery ability after ICH. Evaluating LA severity in patients with mild ICH may help neurologists to optimize treatment protocols. This study was approved by the Ethics Committee of Ruijin Hospital Affiliated to Shanghai Jiao Tong University (approval No. 12) on March 10, 2011.
[Characteristics of Cognitive Impairment in Patients with Leukoaraiosis].

Abstract:OBJECTIVE:To explore the characteristics of cognitive impairment in patients with leukoaraiosis (LA). METHODS:Forty-six LA patients and 38 age and gender-matched healthy subjects were recruited from the Department of Neurology, Beijing Chaoyang Hospital, Capital Medical University between September 2010 and March 2011. All participants underwent the neuropsychological tests recommended by the National Institute of Neurological Disorders and Stroke-Canadian Stroke Network Vascular Cognitive Impairment Harmonization Standards (NINDS/CSN). The were divided into 3 different groups (mild, moderate and severe) according to the Fazekas scale. The differences of neuropsychological performances were compared among 3 groups. RESULTS:The LA patients were associated with comprehensive cognitive function deficits, including MMSE (24.4 ± 3.2 vs 28.3 ± 1.2), MoCA (20.4 ± 3.0 vs 26.2 ± 0.8), digital span forward (5.7 ± 0.9 vs 6.8 ± 1.0), digital span backward (3.5 ± 0.7 vs 4.1 ± 0.7), Stroop-B (69 ± 13 vs 43 ± 5), Stroop-C (141 ± 42 vs 65 ± 10), trail making test-A (73 ± 15 vs 31 ± 7), trail making test-B (126 ± 18 vs 82 ± 6) and digit symbol test (25 ± 6 vs 37 ± 5, P < 0.05). However, there was no difference in the performance of verbal fluency (12.7 ± 2.5 vs 13.4 ± 2.5, P > 0.05). Correlation analysis showed that the severity of LA had a negative correlation with the performance of MoCA (r = -0.601, P = 0.002). CONCLUSIONS:The LA patients are closely correlated with cognitive impairments of attention, memory, executive function and information processing speed. It may be attributed to the frontal-subcortical circuitry dysfunction.
Leukoaraiosis severity and post-reperfusion outcomes in acute ischaemic stroke: A meta-analysis

Abstract:Objectives: Severity of leukoaraiosis may mediate outcomes after reperfusion therapy in acute ischaemic stroke (AIS) patients. However, the level of the association remains poorly understood. We performed a meta-analysis to investigate the impact of leukoaraiosis severity on functional outcome, survival, haemorrhagic complications, and procedural success in AIS patients treated with intravenous thrombolysis and/or endovascular thrombectomy. Materials and methods: PubMed, EMBASE and the Cochrane library were searched for studies on leukoaraiosis in AIS receiving reperfusion therapy. A random-effects meta-analysis was conducted for post-reperfusion outcomes in AIS patients with absent-to-mild leukoaraiosis and moderate-to-severe leukoaraiosis. The strength of association between moderate-to-severe leukoaraiosis and poor outcomes was quantified using odds ratios (OR). Results: A total of 15 eligible studies involving 6460 patients (1451 with moderate-to-severe leukoaraiosis and 5009 with absent-to-mild leukoaraiosis) were included in the meta-analysis. Moderate-to-severe leukoaraiosis was significantly associated with poor 90-day functional outcome (OR 3.16; 95% confidence interval (CI) 2.69-3.72; p < .0001), 90-day mortality (OR 3.11; 95% CI 2.27-4.26; p < .0001) and increased risk of symptomatic intracerebral haemorrhage (OR 1.69; 95% CI 1.24-2.32; p = .001) after reperfusion therapy. Overall, no significant association of leukoaraiosis severity with haemorrhagic transformation (HT) and angiographic recanalization status were observed. However, subgroup analysis revealed a significant association of WML severity with HT in patients receiving EVT. Conclusion: Leukoaraiosis is a useful prognostic biomarker in AIS. Patients with moderate-to-severe leukoaraiosis on baseline imaging are likely to have worse clinical and safety outcomes after reperfusion therapy.

Abstract:Leukoaraiosis (LA) is a group of clinical syndrome used to describe diffuse white matter abnormalities on computer tomography or magnetic resonance imaging brain scans,which increases the risk of stroke,cognitive and mobility impairment.Hypertension tends to be the most important risk factor of LA.However,with the development of LA pathophysiological mechanism study,hypotension and blood pressure variability have been found to be related to LA.The relationships between LA and blood pressure and its variability were summarized.

Abstract:Objective By researching magnetic resonance spectroscopy of cerebral infarction patients with leukoaraiosis,and relationship between magnetic resonance spectroscopy and cognitive function ,to provide evidences for earlier identification,prevention and trentment of patients with vascular cognitive impairment.Methods 51 patients with leukoaraiosis and 21 patients with non-leukoaraiosis were selected to be examed their cognitive function,including mini mental state examination(MMSE),clock drawing test (CDT),fuld object memorye valuation(FOM),rapid verbal retrieve(RVR) test and WAIS digitspan test;at the same time,magnetic resonance imaging(MRI) and magnetic resonance spectroscopy(MRS) examination were performed for them. The concentrations of N-acetylaspartate(NAA),Choline (Cho),Creatine (Cr) were measured in bilateral frontal lobe white matter. Then we analyzed the features of leukoaraiosis in MRS and its relationship with cognitive impairment.Results Cho/Cr value was positively correlated with the degree of leukoaraiosis. NAA/Cr value of left white matter of frontal lobe was positively correlated with the scores of MMSE and CDT. Cho/Cr value of right frontal lobe white matter was negatively correlated with WAIS score. Conclusion Increasement of Cho/Cr is the main feature of MRS in patients with leukoaraiosis of bilateral frontal lobe white matter,and the value of Cho/Cr increases gradually with exacerbation of white matter impairment. It shows that leukoaraiosis is nerve fiber damage,and not nerve cell loss. Vascular Cognitive impairment is not at equal pace with nerve fiber impairment of bilateral frontal lobe white matter.
The Effect of Ischemia-Related Leukoaraiosis on the Conscious Disturbance after Stroke

Abstract:OBJECTIVE:To explore the effect of leukoaraiosis on conscious disturbance in patients with acute cerebral infarction.METHODS:A follow-up study including 138 patients with acute cerebral infarction matched with the diagnostic criteria of the Forth Cerebrovascular Disease Conference, were carried out. Patients were divided into two groups, using MRI to estimate the white substance process around cerebral ventricle, including 78 of them with leukoaraiosis and 60 without leukoaraiosis were followed up using Glasgow coma scale scores and England OCSP classification in 1 month, 3 month and 6 month after onset.RESULTS:The independent factors of conscious disturbance included leukoaraiosis (OR = 5.294, 95% CI: 1.451-19.318), and OCSP classification (TACI and POCI especially) (OR = 14.489, 95% CI: 4.121-50.934). At the initial, the first month and the third month of the stroke episodes, significant difference (P < 0.05) was noticed when using Glasgow coma scales, and the scales in leukoaraiosis group was lower than the control.CONCLUSION:TACI and POCI in OCSP classification were independent risk factors of conscious disturbance, and leukoaraiosis was also the independent factor. The incidence of conscious disturbance after stroke in patients with leukoaraiosis were lower than in that without leukoaraiosis. On the other hand, the degree of conscious disturbance was more serious and slower than those without leukoaraiosis, suggesting that the effect of leukoaraiosis was duplicate for conscious disturbance. Because patients with leukoraiosis had tolerance of chronic cerebral ischemia. The number of patients with conscious disturbance after stroke was fewer relatively. Leukoaraiosis had inactive effect for amelioration of conscious disturbance after three months of the episode. The grouping of OCSP played a primary while leukoaraiosis playing a secondary role, despite the patients with or without conscious disturbance after stroke.
