MRI-Based volumetry of medial temporal lobe possibly predicts the cognitive decline in Alzheimer's disease

Abstract:Objective To explore whether medial temporal lobe (MTL) atrophy would possibly predict the cognitive decline in the progression of Alzheimer's disease(AD).Methods Eighteen patients with probable AD(NINCDS-ADRDA criteria) were studied longitudinally at 2-year interval.At baseline and the end-point,assessment with Mini-Mental Status Examination(MMSE),Weschler Memory Scale(WMS-R) and Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog),and quantitative MRI-based volumetry of medial temporal lobe were performed,including hippocampus,amygdala,and parahippocampal gyrus(PHG).(Results)The total scores of MMSE and WMS-R were significantly lower at end-point(2 years) than baseline(P<(0.001)).The total score of ADAS-Cog was significantly higher at end-point than baseline(P<0.001).Standardized hippocampal volume was significantly reduced at end-point compared with baseline(P<0.05).There was a sine curve relationship between the change rate of PHG volume and that of total score of ADAS-Cog.Multiple regression analyses found that baseline hippocampal volume was associated with the change rate of MMSE score(β=-0.759,P<0.05),and baseline PHG volume with the change rate of ADAS-Cog total score(β=0.906,P<0.05).Conclusion Medial temporal lobe atrophy is associated with cognitive impairment in AD,and possibly predicts the progression of AD.
