Impact of ABO incompatibility on allogeneic peripheral blood stem cell transplantation in leukemia patients

Abstract:Objective To analyze the impact of ABO incompatibility on allogeneic peripheral blood stem cell transplantation(allo-PBSCT) in leukemia Patients. Method 38 cases of ABO-incompatible allo-PBSCT patients and 30 cases of ABOcompatible patients were analyzed. Results Compared with ABO-compatible recipients, time of erythrocyte recovery(hemoglobin lever>100 g/L) after allo-PBSCT was delayed in ABO-incompatible recipients [(73.47±43.04)d vs(49.83±38.30)d,P=0.024].Within three months after transplantation,RBC requirements increased in ABO-incompatible recipients compared with ABO-compatible recipients [(10.08±10.10)u vs(4.00±3.56)u,P=0.001]. Cox proportional hazard model analysis demonstrated that ABO types in donor/recipient pair and mononuclear cell numbers were correlated with the time of erythrocyte recovery(P=0.030 & P=0.044). Pure red cell aplasia was observed in two patients with blood group O/A in recipient/donor pair. Hemolysis was observed in three major ABO-mismatched patients. The incidence of graft versus host disease(GVHD),cytomegalovirus(CMV) infection,relapse and mortality had no significant differences between two groups(P>0.05). Conclusions Compared with ABO-compatible recipients, delayed erythrocyte recovery and increased RBC requirements were observed in ABO-incompatible recipients. ABO blood group of O/A in recipient/donor pair was high risk factor for PRCA. Hemolysis was more frequently observed in major ABO-mismatched patients. ABO incompatibility did not affect clinical outcomes.
