Abstract:Since 1990s, the prevention and control of tuberculosis has been an important public health issue in the world and China. In order to keep down the tuberculosis development, the control model improves from the specialized management with Center for Disease Control to the participant of all types of medical institution and Center for Disease Control. This paper reviews the development of tuberculosis prevention and control in China, sums up achievements and finds potential problems for providing a direction for continuous improvement of the tuberculosis control. Analysis on knowledge of tuberculosis prevention and control in middle school students in 3 provinces of western China

Abstract:Objective To analyze status quo of tuberculosis(TB)related knowledge,attitude and practice in middle school students and provide evidences for TB health education intervention.Methods Three epidemiological survey sites where lots of active TB cases were found and two adjacent counties were selected in Sichuan,Guizhou and Yunnan province.One senior high school was purposively sampled in each county,and two classes in grade 2of each school were randomly sampled.A self-designed questionnaire investigation concerning TB core information,TB related attitude,practice and health education was conducted among all students in the 10 classes selected.443 questionnaires were sent out while 437(98.6%)recovered.General awareness rate,awareness rate of each message,attitude distribution and score were calculated.Results General awareness rate of the 10TB related messages was 54.3%(2375/4370).Awareness rates of TB healthcare strategy and hazard were low,in which was 23.9%(209/874)and 23.8%(104/437)respectively.The median score was 6(P25=4,P75=7).7.0%(30/430)of the students would conceal the facts if getting TB,21.2%(91/430)would keep away from TB patients,and 0.5%(2/430)would not contact with TB patients.65.7%(287/437)of the students were never proactive in knowing TB knowledge,whereas 96.3%(420/436)thought it necessary to carry out TB health education in school.Conclusion Students in the western region of China had poor TB knowledge and negative attitudes.Collaboration of health and education departments and comprehensive methods of information spread were needed to improve the situation.

Abstract:Despite the achievements obtained worldwide in the control of tuberculosis in recent years, many countries and regions including China still face challenges such as low diagnosis rate, high missed diagnosis rate, and delayed diagnosis of the disease. The discovery strategy of tuberculosis in China has changed from "active discovery by X-ray examination" to "passive discovery by self-referral due to symptoms", and currently the approach is integrated involving self-referral due to symptoms, active screening, and physical examination. Active screening could help to identify early asymptomatic and untreated cases. With the development of molecular biology and artificial intelligence-assisted diagnosis technology, there are more options for active screening among the large-scale populations. Although the implementation cost of a population-based active screening strategy is high, it has great value in social benefits, and active screening in special populations can obtain better benefits. Active screening of tuberculosis is an important component of the disease control. It is suggested that active screening strategies should be optimized according to the specific conditions of the regions to ultimately ensure the benefit of the tuberculosis control.

Abstract:China has the world's second largest tuberculosis epidemic, but progress in tuberculosis control was slow during the 1990s. Detection of tuberculosis had stagnated at around 30% of the estimated total of new cases, and multidrug-resistant tuberculosis was a major problem. These signs of inadequate tuberculosis control can be linked to a malfunctioning health system. The spread of severe acute respiratory syndrome (SARS) in 2003, brought to light substantial weaknesses in the country's public-health system. After the SARS epidemic was brought under control, the government increased its commitment and leadership to tackle public-health problems and, among other efforts, increased public-health funding, revised laws that concerned the control of infectious diseases, implemented the world's largest internet-based disease reporting system, and started a programme to rebuild local public-health facilities. These measures contributed to acceleration in efforts to control tuberculosis. By 2005, the detection of cases of tuberculosis had increased to 80% of the estimated total new cases, permitting China to achieve the 2005 global tuberculosis control targets. At the same time, specific efforts to improve tuberculosis control also contributed to strengthening of the public-health system. We examine how the strengthening of a disease control programme and the public-health system worked together to achieve a desired health outcome.

Abstract:Purpose: Tuberculosis remains a major public health problem globally, especially in undeveloped countries. This study aimed to evaluate and review the long-term epidemic trends of tuberculosis in China. Methods: Data were extracted from the Global Health Data Exchange. Metrics (prevalence, incidence and mortality) and Joinpoint regression were used to identify the epidemic trends. Results: From 1990 to 2017, decreasing trends in prevalence (average annual percent change, AAPC: -0.5%, 95% CI: -0.6% to -0.5%), incidence (-3.2%, 95% CI: -3.5% to -2.9%), and mortality (-5.7%, 95% CI: -6.2% to -5.3%) of tuberculosis were observed. The incidence and mortality of multidrug-resistant tuberculosis (MDR-TB) decreased with AAPC of -2.3% (-3.1% to -1.4%) and -4.9% (-5.4% to -4.5%), respectively, while the prevalence increased with an AAPC of 1.2% (0.3% to 2.0%). The burden of extensively drug-resistant tuberculosis (XDR-TB) increased with an AAPC of 12.5% (11.9% to 13.2%) in prevalence, 7.6% (6.5% to 8.7%) in incidence, and 4.5% (3.6% to 5.4%) in mortality. The disease burden of tuberculosis increased with age and peaked among those aged over 70. Conclusion: The epidemic of tuberculosis decreased in China, while the disease burden was still challenging to control. MDR-TB and XDR-TB should be emphasized along with the epidemic. It will certainly be a difficult task to achieve the post-2015 global targets by 2025 and 2035.

Abstract:To understand the barriers to tuberculosis (TB) control and prevention in undergraduate students and to propose the strategies to improve this work, we conducted in-depth interviews with three groups of individuals in infirmaries of universities and colleges, TB departments of district Centers for Disease Control and Prevention (CDCs), and undergraduates with TB. The study identified four major themes: inadequate workforce and less than optimal functioning of infirmaries and district TB control agencies; poor cooperation in the process of discovery, treatment, and surveillance of undergraduates with TB; poor acceptance of the national TB policy; and inadequate attention to TB prevention. Failure to carry out the national TB policy was the major barrier to TB control and prevention in undergraduates. TB control agencies should strengthen the implementation of the national TB policy.

Abstract:Abstract Background China has made progress in tuberculosis control, but this disease remains a burden in many regions of China. We performed time-series analysis to examine changes in the rates of newly notified and newly smear-positive cases of tuberculosis in different regions of China from 1997 to 2017 and assessed the effect of the current control program. Methods National and provincial notification data on tuberculosis from 1997 to 2017, which covers 31 provinces in mainland China, were extracted from the Chinese public health science data center. The annual percentage changes in newly notified and smear-positive cases were analyzed using a joinpoint regression method. Results There were 17,823,330 newly notified tuberculosis cases from 1997 to 2017, with the greatest number in 2005. A total of 6,378,785 of these cases (35.79%) were smear-positive cases. Most provinces had significant declines in newly notified and smear-positive cases during the most recent years. The declines in newly notified cases were faster in the eastern and central provinces than in the western provinces. Conclusions The burden of tuberculosis has been declining throughout China during recent years, but tuberculosis in western China continues to be a public health emergency that needs to be urgently addressed. Effective prevention and control strategies are needed for regions with high disease burdens and those with increasing or unchanging numbers of newly notified and smear-positive cases of tuberculosis.
Framework for Establishment of a Comprehensive and Standardized Administration System for Prevention and Control of Tuberculosis in College Student Community in China.

Abstract:BACKGROUND:College student community is the one with high risk of tuberculosis (TB). A systemic and standardized administration model for prevention and control of TB is significance in controlling TB spread in universities. Currently, the universities in China have not established the comprehensive and standardized administration system for TB prevention and control in college student community.METHODS:Firstly, the literature research and brainstorming method (n=13) were used to construct the clause and sub-clause pool for the administration of TB prevention and control within college student community in 2014. Secondly, a total of twenty experts in the field of TB prevention and control who are representatives of the east, west, south and north parts of China were selected and invited to participate the Delphi letter-inquiry. After two rounds of letter-inquiry, the opinions of the experts reached a consensus and the framework for the administration system was constructed.RESULTS:A framework for the administration system was constructed, which included 8 first class indexes, 26 second class indexes and 104 third class indexes.CONCLUSION:The results are highly scientific and reliable, which can be helpful for improving the systemic and standardized levels for the administration of TB prevention and control in universities in China and perhaps in other developing counties with high TB burden as well.

Abstract:Tuberculosis is a chronic respiratory infectious disease that seriously endangers human health. It can cause work incapacity of patients and bring serious economic losses to society and families. China is one of the countries with the most severe tuberculosis burden, with the number of tuberculosis patients ranking 3rd in the world. [ 1 ] In recent years, preferential policies for tuberculosis prevention and treatment have been continuously enhanced and improved in China. Local governments have actively implemented policies to reduce tuberculosis treatment costs and to effectively reduce the burden of tuberculosis patients . [ 2 ] The awareness of patients on preferential policies for tuberculosis prevention and treatment will directly affect their compliance with standard management of tuberculosis.

Abstract:Background: Tuberculosis (TB) is a major infectious disease globally. Adequate and proper use of anti-TB drugs is essential for TB control. This study aims to study China's production capacity and sales situation of anti-TB drugs, and to further discuss the potential for China to contribute to global TB control.Methods: The production data of anti-TB drugs in China from 2011 to 2013 and the sales data from 2010 to 2014 were extracted from Ministry of Industry and Information Technology database of China and IMS Health database, respectively. The number of drugs was standardized to the molecular level of the key components before calculating. All data were described and analyzed by Microsoft Excel.Results: First-line drugs were the majority in both sales (89.5 %) and production (92.3 %) of anti-TB drugs in China. The production of rifampicin held the majority share in active pharmaceutical ingredients (APIs) and finished products, whilst ethambutol and pyrazinamide were the top two sales in finished products. Fixed-dose combinations only held small percentages in total production and sales weight, though a slight increase was observed. The production and sales of streptomycin showed a tendency of decrease after 2012. The trends and proportion of different anti-TB drugs were similar in production and sales, however, the production weight was much larger than that of sales, especially for rifampicin and isoniazid.Conclusions: First-line drugs were the predominant medicine produced and used in China. While the low production and sales of the second-line TB drugs and FDCs rose concerns for the treatment of multiple drug resistant TB. The redundant production amount, as well as the prompt influence of national policy on drug production and sales, indicated the potential for China to better contribute to global TB control.

Abstract:9870 thousand people develop tuberculosis (TB) in 2020. Despite being a preventable as well as curable communicable disease, tuberculosis still is the leading communicable disease killer globally. The WHO’s End TB Strategy envisions a world free of TB, zero deaths, disease, and suffering due to tuberculosis by 2035. China ranks second for TB cases globally. Over the past 20 years, China has made remarkable achievements in tuberculosis control, with the prevalence and mortality rate of tuberculosis reduced by half compared with 1990. China has achieved the United Nations Millennium Development Goals on tuberculosis control 5 years ahead of schedule. However, in spite of concerted international, regional, and national efforts to address tuberculosis in China, the tuberculosis burden in China is still high. At the same time, China also faces problems such as the high burden of latent TB infection, rifampicin-resistant/multidrug-resistant tuberculosis, and low detection rate of tuberculosis. China needs to further improve the quality of tuberculosis control work to reduce the incidence of tuberculosis and ultimately achieve the goal of ending tuberculosis.
Effect of Different Interventions for Latent Tuberculosis Infections in China: a Model-Based Study

Abstract:Background Tuberculosis (TB) has a serious impact on people’s health. China is one of 30 countries that has a high TB burden. As the currently decreasing speed of the incidence of TB, the WHO’s goal of “End TB Strategy” is hard to achieve by 2035. As a result, a SEIR model that determines the impact of different tuberculosis preventive treatments (TPTs) in different age groups, and the effect of different interventions on latent TB infections (LTBIs) in China is developed. Methods A Susceptible-Exposed-Infectious-Recovered (SEIR) model was established. Goodness-of-fit tests were used to assess model performance. Predictive analysis was used to assess the effect of different interventions on LTBIs and achieving the goals of the “End TB Strategy”. Results The Chi-square test indicated the model provided a good statistical fit to previous data on the incidence of TB (χ 2 = 0.3085, p > 0.999). The 1HP treatment regimen (daily rifapentine + isoniazid for 4 weeks) was most effective in reducing the number of TB cases by 2035. The model indicated that several strategies could achieve the 2035 target of the “End TB Strategy”: completion of active case finding (ACF) for LTBI and TPT nation-wide within 5 years; completion of ACF for LTBIs and TPT within 2 years in high-incidence areas; completion of TPT in the elderly within 2 years; or introduction of a new vaccine in which the product of annual doses and vaccine efficiency in the three age groups above 14 years old reached 10.5 million. Conclusion The incidence of TB in China declined gradually from 2005 to 2019. Implementation of ACF for LTBIs and TPT nation-wide or in areas with high incidence, in the elderly, or administration of a new and effective vaccine could greatly reduce the number of TB cases and achieve the 2035 target of the “End TB Strategy” in China.
Status quo of tuberculosis health education in primary and middle schools

Abstract:Objective To analyze status quo of tuberculosis(TB) health education in primary and middle schools and provide evidences for the policy-making.Methods 676 teachers and 3127 students in three counties of two provinces selected by stratified purposeful sampling and random sampling were investigated by questionnaires.8 principals and 64 teachers in two counties were interviewed.Results The core TB knowledge awareness of teachers,pupils and middle school students were respectively 47.9%,36.1% and 38.3%.The proportion of professional health education teachers was only 44.8%.92.6% of all the teachers had never been trained on TB and 39.8% had never given TB health-education activities for the students in class.The main constraints of tuberculosis education in schools at present were short of training for the teachers,teaching materials and facilities.And teaching ability and attitude of the teachers,the recognition of the government and schools and the interests of the students had a great impact on the implementation of TB health-education activities.Conclusion Now TB health education in schools is very important and need to be strengthened urgently.An effective collaboration mechanism should be established between health and education sectors.More teaching materials and guidelines should be developed and the teaching ability needs to be improved.A stable and continued financing mechanism also should be established.

Abstract:What is already known about this topic?: Tuberculosis (TB) is a multisystem disease that might affect any organ. Currently, the National TB Program (NTP) issued by the State Council of China, only covers pulmonary tuberculosis (PTB), and the status of extrapulmonary tuberculosis (EPTB) remains unclear nationwide.What is added by this report?: The survey conducted by China CDC reported that there were no specific health facilities responsible for diagnosis, treatment and management of EPTB in China, while more than half of counties thought it should be included into NTP.What are the implications for public health practice?: China should include EPTB into NTP to achieve the target of End-TB strategy, a world free of TB. Zero deaths, disease and suffering due to TB.

[Current Status of Tuberculosis Burden in China].

Abstract:Tuberculosis, caused by Mycobacterium tuberculosis, is an infectious disease that most often affects the lungs. China is still among the high-burden tuberculosis countries in the world. Although the estimated incidence of tuberculosis in China has declined in recent years, the declining rate is slow. It still faces major issues such as a slower rate of decline, a widening gap between estimated and notified incidence, higher risk among middle-aged and older adults, a high number of cases among agriculture and related workers, and a heavier disease burden in the country's western regions. In addition, latent tuberculosis infection, drug-resistant tuberculosis, tuberculosis coinfection with HIV, and extrapulmonary tuberculosis have also exacerbated the disease burden of tuberculosis to some extent. This paper reviewed the epidemic characteristics of tuberculosis, the epidemiological triad, three links and two factors in the transmission process, the disease burden, and other aspects to provide a reference for formulating prevention and control strategies on tuberculosis.
Comprehensive Understanding of Health-Seeking Behaviour among Pulmonary Tuberculosis Patients in China

Abstract:In China, there were 918 000 tuberculosis (TB) cases in 2015 alone. The primary challenge facing TB control is the allocation of limited health care resources.To gain a comprehensive understanding of the first choice of health care facility among Chinese patients with suspected pulmonary TB (PTB) and the number of visits required to make the diagnosis.Relevant full-text articles in three Chinese and one English literature databases up to November 2016 were reviewed. Meta-analyses were performed using Stata v12.0.Among 1257 potentially relevant selected articles, 27 cross-sectional studies involving 9891 patients were included in the final analyses. Most PTB patients chose county-level hospitals (40%, 95%CI 33-46) and village clinics (34%, 95%CI 27-42); only 13% (95%CI 10-16) of patients chose to visit PTB dispensaries first. Before obtaining the correct diagnosis, 28% (95%CI 11-44) of patients had to visit health facilities more than three times.Patients with suspicion of PTB were more likely to visit low-level facilities than dispensaries. Repeated visits resulted in both overall delay and high risk of PTB transmission. These findings suggest that a shift in government policy for PTB is required.

Abstract:Tuberculosis (TB) is one of the top ten causes of death worldwide, taking the lives of over a million people annually. In addition to being a serious health issue, TB is also closely linked to eradicating poverty according to the Sustainable Development Goals (SDGs) of the United Nations (UN). All UN members have committed to ending the TB epidemic by 2030. China has one of the highest TB loads worldwide, ranking third in the world on many TB burden indices. The national strategy for TB control is aimed at creating a collaborative network and integrating TB treatment into the medical system. According to the WHO's global TB report, China is expected to have 748,000 new cases of TB in 2022 and an incidence of 52 cases per 100,000 people. Ending TB remains a huge challenge and requires comprehensive control strategies in China. In this work, we have discussed the challenges of TB prevention and control in China and proposed specific measures to end TB.

Abstract:[Objective]To evaluate the effect of the implementation of TB control strategies in Tai'an city from 1998 to 2007,provide scientific basis for further development of tuberculosis prevention and control strategies.[Methods]Data of TB control and prevention from 1998 to 2007 in Tai'an city were analyzed.[Results]107 141 suspected TB patients visited the Center from 1998 to 2007,with an average annual examination rate of suspected persons of 1.98 ‰.The sputum examination rate was 29.91% during their first visit with smear-positive rate of 41.32%,showing an upward trend year by year(P0.01).17 729 active TB patients were registered from 1998-2007,with an average annual registration rate of 32.72/100 000,of which an annual average registration rate was 24.45/100 000 and 19.81/100 000 in smear-positive patients and new smear positive patients.Of the registration rate of new smear positive patients,28.70/100 000 of them were male and 10.50/100 000 of them were female(P0.01).It was.1.07/100 000,17.78/100 000,19.78/100 000,14.21/100 000,24.16/100 000,33.50/100 000 and 47.65/100 000 in the people aged from 0 to 14,15 to 24,25 to 34,35 to 44,45 to 54,55 to 64 and over 65(P0.01).10 735 new smear positive TB cases were treated in 10 years.The cure rate was 95.59%,the mortality rate was 0.82%.[Conclusion]The registration rate of new active tuberculosis patients was going up year after year from 1998-2007 in Tai'an city,but the attendance rate is well below national requirements.

Abstract:Objective To explore public sentiment on TB among newspapers and periodicals in 2012,and provide evidence for improving public awareness and enhancing government advocacy on TB.Methods 487 newspapers and periodicals were searched and analyzed with tuberculosis as keyword every month in an electronic database of Beijing University library.Results A total of 592 media reports were found with 27 reports(4.6%)in national level media,152 reports(25.7%)in provincial level and 413 reports(69.8%)in prefecture level.Out of the 592 media reports,there were 195 reports with the theme of TB control actions(32.9%),389 short news(65.7%),and 310 reports published in March(52.4%).In regard of theme-wise analysis,TB control actions was the top theme in all different media levels.The maximum reports in all themes were short news except education and other.The maximum reports in all themes were published in Marchexcept Policy and Regulation and Scientific Research.The maximum reports of all themes were unknown information source except Policy and Regulation and Other.Conclusion Health communication professionals in TB control need to understand the characteristics and demands of all kinds of media and their audiences,cultivate news sensitivity,and rely on trusted information source such as government officials and experts to provide interested source materials for media for in-depth reports.
