Lower extremity nerve blockade directed by percutaneous electrode guidance

Abstract:Objective To evaluate the clinical application of percutaneous electrode guidance(PEG)in peripheral nerve blockade for lower extremities surgery.Methods Forty-one patients underwent the unilateral leg(below knee level)surgeries were conducted both sciatic nerve and femoral nerve block guided by nerve stimulator.Gastrocnomius twitch represented the PEG reaching the sciatic nerve successfully while quadriceps jerk stands for femoral nerve being close to the pad.Data collected included the success rates of nerve blocks and PEG reaching nerves,percutaneous current needed for detecting the nerves and the distance between the injection points detected by PEG and the classical Labat's point for sciatic nerve block.Results The success rate for PEG on sciatic nerve and femoral nerve block were 39% and 100%,respectively.The mean percutaneous current was(50.00±17.00)mA for detecting the sciatic nerve and(7.20±2.70)mA for the femoral nerve.The mean distance between the actual injection point of sciatic nerve block detected by PEG and the injection point using classical posterior approach of Labat was(0.90±0.56)cm.Conclusion The success rate for PEG detecting the sciatic nerve in posterior approach is lower than that for the femoral nerve.Nevertheless,applying PEG can significantly increase the success rate of nerve block.
