Abstract:To assess the influence of cyclosporin A (CsA) and tacrolimus (FK506) on mycophenolic acid (MPA) and correlation analysis of the pharmacokinetic parameters and patient characteristics, clinical outcome in Chinese kidney transplant recipients, the pharmacokinetics of 1000 mg mycophenolate mofetil (MMF) twice daily was measured by high-performance liquid chromatography (HPLC). PKS (Pharmaceutical Kinctics Software) 1.0.2 software package was used for the calculation of pharmacokinetic parameters. The meanCmax,tmax, and AUC(0–12) were (21.88±10.52) μg/ml, (1.20±0.95) h, and (52.546±13.215) μg·h/ml, respectively. The level of AUC(0–12) in the FK506 group was significantly higher than that in the CsA group. MPA appeared not to be affected by renal function. MPA AUC(0–12) showed statistically significant difference according to the patient's gender.

Abstract:Objectives: The interaction between the components of traditional Chinese medicine (TCM) is an important basis for their synergy. Rhein and curcumin exert various pharmacological activities, including anti-tumour, anti-inflammatory, antioxidant, anti-fibrosis and renoprotective effects. However, no investigation has reported the synergistic anti-fibrosis effect yet. This study aims at determine the pharmacokinetics and pharmacodynamics of the combination of rhein and curcumin in the treatment for chronic kidney disease in rats. Design: Fifty two male Sprague-Dawley (SD) rats were randomly divided into rhein group, curcumin group and their combination group for pharmacodynamics studies. HE and Masson staining was conducted to observe the changes of renal morphology. Kits were used to detect the level of urea nitrogen (BUN) and creatinine (Scr). For pharmacokinetic study, 36 SD rats were randomly divided into rhein group, curcumin group and a combination group, the content of rhein and curcumin in plasma and renal tissue was determined by ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS). In additon, molecular docking method and cell experiments was used to disclose the interaction mechanism between curcumin and rhein. Results: The pharmacodynamic results showed that the degree of renal fibrosis was improved obviously by co-administration rhein and curcumin. Meanwhile, compared to single administration, the Cmax and AUC of rhein and curcumin in plasma and renal tissue were enhanced significantly after co-administration. Moreover, the result of molecular docking and cell experiments showed that both two compounds could interact with P-gp, CYP2C9 and CYP2C19. Conclusion: Together, these findings demonstrated that rhein and curcumin had a synergistic effect in ameliorateing chonic kidney disease, providing an important explanation on the synergistic mechanism of curcumin and rhein from a pharmacokinetic viewpoint.

Abstract:OBJECTIVE To study the interindividual variability of cyclosporin blood concentration in children to promote rational medication in pediatrics.METHODS The CsA plasma concentration data in the past 4 years(2002.7~2006.8)were collected in Children Hospital of Zhejiang University school of medicine,Hangzhou,China.The data of 74 patients(female 39,male 35,age from 2 months to 15.7 years)were analyzed.RESULTS The daily doses to reach the target trough level was significant different among different age groups(P=0.008).The results were as follows 0-1 year,5.04 mg·kg-1·d-1;1.1-2 year,6.98 mg·kg-1·d-1;2.1-3 year,12.85 mg·kg-1·d-1;3.1-6 year,7.26 mg·kg-1·d-1;6.1-8 year,5.69 mg·kg-1·d-1;8.1-12 year,5.44 mg·kg-1·d-1;12.1-16 years,5.36 mg·kg-1·d-1.CONCLUSION The different developmental age are of great importance for the initial rational drug therapy in children.This study offers some information towards individualized therapy in children.
The Pharmacokinetic Study of Tacrolimus and Wuzhi Capsule in Chinese Liver Transplant Patients

Abstract:Objectives: Wuzhi Capsule (WZC) is often administrated with tacrolimus in liver transplant patients to reduce the toxicity of tacrolimus and relieve the financial burden of patients. We aimed to investigate the interaction between Wuzhi Capsule (WZC) and tacrolimus in liver transplant patients.Methods: We applied the LC-MS/MS analytical method previously established to study the pharmacokinetic characteristics of the analytes in 15 liver transplant patients. CYP3A5 genotypes were determined in 15 donors and recipients, and they were categorized into CYP3A5 expressers and non-expressers respectively.Results: The influences of CYP3A5 in donors and recipients on the pharmacokinetics of tacrolimus with or without WZC were also studied. We found that 1) WZC could influence the metabolism of tacrolimus, which shortened the Tmax of tacrolimus and decreased V/F and CL/F. 2) Moreover, our results showed that, in donors, the CL/F of tacrolimus were significantly lower in CYP3A5 (CYP3A5*1) expressers (decreased from 24.421 to 12.864) and non-expressers (decreased from 23.532 to 11.822) when co-administration with WZC. For recipients, the decreased trend of CL/F of tacrolimus was seen when co-administrated with WZC by 15.376 and 12.243 in CYP3A5 expressers and non-expressers, respectively.Conclusion: In this study, the pharmacokinetics effects of WZC on tacrolimus were identified. The co-administration of WZC can increase the tacrolimus blood concentration in Chinese liver transplant patients in clinical practice.

Abstract:Background: Intestinal microbiota has been confirmed to influencing the pharmacokinetic processes of a variety of oral drugs. However, the pharmacokinetic effects of the gut microbiota on cyclosporine A, a drug with a narrow therapeutic window, remain to be studied. Method: Twenty-one rats were randomly divided into three groups: (a) control group (CON), (b) antibiotic treatment group (ABT) and (c) fecal microbe transplantation group (FMT). The ABT group was administrated with water containing multiple antibiotics to deplete microorganisms. FMT was with the same treatment, followed by oral administration of conventional rat fecal microorganisms for normalization. Result: The bioavailability of CSA increased by 155.6% after intestinal microbes were consumed by antibiotics. After intestinal microbiota reconstruction by fecal transplantation, the increased bioavailability was significantly reduced and basically returned to the control group level. Changes in gut microbiota alter the protein expression of CYP3A1, UGT1A1 and P-gp in liver. The expressions of these three proteins in ABT group were significantly lower than those in CON and FMT groups. The relative abundance of Alloprevolleta and Oscillospiraceae UCG 005 was negatively correlated with CSA bioavailability while the relative abundance of Parasutterella and Eubacterium xylanophilum group was negatively correlated with CSA bioavailability. Conclusion: Intestinal microbiota affects the pharmacokinetics of CSA by regulating the expression of CYP3A1, UGT1A1 and P-GP.

Abstract:Objectives: The aim of this study is to investigate the population pharmacokinetics (PopPK) of cyclosporine (CsA) in the Chinese hematopoietic stem cell transplantation (HSCT) recipients for promoting the individualization of CsA administration. Methods: A total of 887 retrospective drug monitoring data points were collected from 58 HSCT recipients. Whole blood samples were collected at predose (C-0) and 2 hours (C-2) post dose. The administration of CsA was intermittent intravenous infusion, continuous intravenous infusion and oral. Population modeling was performed using the NONMEM (nonlinear mixed-effect modeling) program. A one compartment pharmacokinetic model was used to fit the data. Results: Body surface area (BSA), administration route and postoperative days were identified as significant covariates for clearance (CL) according to the final model: CL = 31.0 x (BsA/1.59)(0.761) x (ROUT) x (POD), where ROUT was 1.91 if the administration route was intravenous infusion, otherwise it is equal to 1. The POD was 0.818, 0.753, 0.539, and 0.509 for post-transplant Days 0 - 10, 11 - 20, 21 - 30 and more than 30 days, respectively. Administration route was a significant covariate for volume (V) according to the final model: V = 192 x (ROUT), where ROUT was 4.10, 3.63 and 1 when the administration route was continuous intravenous infusion, intermittent intravenous infusion and oral. The other covariates were not identified as a significant effect on CsA pharmacokinetic parameters. Conclusion: Body surface area, administration route and postoperative days should be considered in individual pharmacotherapy of cyclosporine for HSCT patient to achieve the desired therapeutic target.
No Effect of Genetic Polymorphisms on Cyclosporine Pharmacokinetics in Renal Transplant Recipients Based on Nonlinear Mixed Effects Modeling

Abstract:Cyclosporine is a preferred calcineurin inhibitor that is considered to be the backbone of current immunosuppressive drug therapy. The aim of this study was to build a population pharmacokinetics (Pop-Pk) cyclosporine model in renal transplant recipients and identify the influence of a range of potential covariates, including genetic polymorphisms that affect cyclosporine pharmacokinetic parameters. The analysis was performed with 1051 retrospective drug monitoring data points in 80 renal recipients receiving cyclosporine-based immunosuppressive therapy. Clinical characteristics were collected for up to 28 postoperative days. CYP3A4* 18B, CYP3A5*3, ABCB1 C1236T, G2677T/A, and C3435T genotyping was performed. A Pop-Pk model was developed using a nonlinear mixed effects modeling (NONMEM) program. After building the final model, 1000 bootstraps were performed to validate the final model. A one-compartment open model with first-order absorption and elimination was chosen to describe the pharmacokinetics of cyclosporine. The Pop-Pk analysis showed that postoperative days, weight, direct bilirubin and hematocrit, significantly affected the pharmacokinetics of cyclosporine. The final model with the clearance covariates was presented as: Cl/F = 60.7 x POD-(0.41) x(DBIL/4.6)(-0.11) x (WT/60)(0.31) x (HCT/33)(-0.24) (Lxh(-1)), and the final model with the volume covariates was presented as: Vd/F=348xPOD(-0.39) (L). The CYP3A4* 18B, CYP3A5*3, ABCB1 C1236T, G2677T/A, and C3435T genotypes were not identified as having significant effect on the cyclosporine pharmacokinetic parameters. The model will help to calculate cyclosporine dose requirements in renal transplant recipients according to specific clinical factors affecting the cyclosporine clearance. It will also be useful in individual cyclosporine pharmacotherapy for renal transplantation patients, to achieve the desired therapeutic target.

Abstract:Purpose To establish the population pharmacokinetic (PPK) model of cyclosporine (CsA) in Chinese renal transplant recipients and evaluate the influence of various indexes including CYP3A5 and MDR1 genetic polymorphism on pharmacokinetic parameters. Methods Trough (C 0 ) and peak (C 2 ) CsA concentration were monitored conventionally after renal transplantation. C 0 and C 2 were collected for 5 months in 146 patients. The CYP3A5*3 genotype and MDR1 haplotype were determined by methods based on amplification refractory mutant PCR. The data were analyzed by nonlinear mixed-effect modeling (NONMEM). The model was evaluated using goodness of fit plots and relative error measurements. Physiological and pathological factors including CYP3A5 and MDR1 genotypes were evaluated as covariates of CsA pharmacokinetic parameters. Results Pharmacokinetics of CsA was best described by a one-compartment disposition model followed a first-order absorption process. The estimated clearance (CL/F) was 49.5 l·h −1 , the volume of distribution (Vd/F) was 226 l. K a was fixed as 1.25 h −1 . Post-transplant data, body weight, total bilirubin, and MDR1 genotype were covariates of CL/F ( P < 0.005). Gender and MDR1 haplotype were covariates of Vd/F ( P < 0.005). The AUC estimated based on the Bayesian method was 7,465 ± 1,708 ng·h·ml −1 (2,946 ∼13,926 ng·h·ml -1 ). Conclusion The PPK model developed in this study could be used to optimize CsA dose for Chinese renal transplant recipients by using conventional therapeutic drug monitoring (TDM) data.

Abstract:Objective To observe the factors influencing the serum concentration of cyclosporin A(CsA) and to offer reference to the rational use of drugs.Methods The present domestic and international materials were analyzed and summed up.Results These influencing factors included transplanted organs,functions of liver and gastrointestinal tract,postoperative time,food and beverage,methods of administration,dosage forms and quality of drug and combining use of drugs.Conclusion According to the patient's actual conditions,doctors should regularly carry on therapeutic drug monitoring,adjust the scheme of using medicine in time and ensure the patients in their safe and effective use of drug.

Abstract:Cyclosporine is an immunosuppressive drug largely used in organ transplantation.It is characterized by a narrow therapeutic index and wide interindividual variability in its pharmacokinetics. Cyclosporine is metabolized primarily by CYP3A4 and CYP3A5 in the liver and small intestine and also substrate for P-gp, encoded by MDR1. The interindividual heterogeneity in enzymatic activity of small intestine and liver CYP3A4 and CYP3A5, and intestine P-gp have contributed to oral cyclosporine pharmacokinetics in kidney, heart,liver and lung transplant patients. The differences in enzymatic activity of CYP3A and P-gp are believed to be due to CYP3A4, CYP3A5 and MDR1 genetic mutation. Therefore, a investigation which the effect of sequence variants in gene encoding drug-metabolizing enzymes and drug transporter to cyclosporine disposition may lead to individualized drug dosing and improved therapeutics.

Abstract:Traditional Chinese medicines (TCMs) have a long history for safely treating human diseases. Unlike western medicine, TCMs usually contain multiple components synergistically and holistically acting on the diseases. It remains a big challenge to represent rationally the in vivo process of multiple components of TCMs for understanding the relationship between administration and therapeutic effects. For years, efforts were always made to face the challenge, and the achievements were obvious. Here, we give an comprehensive overview of the recent investigation progress (from 2015 to 2017, except the part of 'integrated pharmacokinetics of TCMs' from 2014 to 2017 and the part of 'reverse pharmacokinetics in drug discovery from natural medicines' in 2014) on pharmacokinetics of TCMs, mainly referring to the following six aspects: (1) classical pharmacokinetic studies on TCMs; (2) absorbed components and metabolites identification of TCMs; (3) pharmacokinetic herb-drug interactions and herb-herb interactions with TCMs; (4) integrated pharmacokinetics of TCMs; (5) pharmacokinetic and pharmacodynamic combination studies to dissect the action mechanisms of TCMs; and (6) reverse pharmacokinetics in drug discovery from natural medicines. Finally, based on the insights from the recent progress and our latest efforts, we propose new perspectives on the integrated pharmacokinetics of TCMs.

Systematic review of influence of MDR1C1236T genetic polymorphism on cyclosporine pharmacokinetics

Abstract:Objective To systematic review the influence of MDR1 C1236T genetic polymorphisms on cyclosporine pharmacokinetics. Methods Relevant databases were searched by computer for investigating the influence of MDR1 C1236T genetic polymorphism on cyclosporine pharmacokinetic and meta-analysis was performed by Revman 5.0 software. Results Seven literatures (n=605) were included. The CC genotype had lower dose-adjusted concentration at the time 2 h after administration(C2) level than other genotypes, and statistical difference was observed between CC genotypes and TT genotype (P0.05). No statistical difference was observed between CC genotype and other genotypes in mean dose-adjusted C0 level and daily dose.Conclusion MDR1 C1236T genetic polymorphism has a little effect on the cyclosporine dose-adjusted C2 level.

Effect of CYP3A5*3 and CYP3A4*18B Genetic Polymorphisms on Cyclosporine Pharmacokinetics in Renal Transplant Patients

Abstract:Aim To evaluate retrospectively the effect of genetic polymorphisms in CYP3A4 and CYP3A5 on cyclosporine pharmacokinetics during the early period in renal transplant patients.Methods Polymerase chain reaction-restriction fragment length polymorphism was performed to genotype for CYP3A4*18B and CYP3A5*3.Cyclosporine whole blood levels were measured by fluorescence polarization immunoassay.Results The frequencies of CYP3A4*18B and CYP3A5*3 variant alleles were 0.235(95 CI:0.235~ 0.241)and 0.770(95 CI:0.767~0.773),respectively in 63 of renal transplantation patients, and these alleles exhibited incomplete linkage disequilibrium. The median (range) value of cyclosporine dose adjusted blood trough concentrations (C0/D) and C0 in CYP3A4*1/*1 genotype patients was higher than that in CYP3A4*1/*18B and CYP3A*18B/*18B patients, the same result was found in CYP3A5 expression (CYP3A5*3/*1 and CYP3A5*1/*3) (P0.05, Mann-Whitney U test); cyclosporine dose in CYP3A5*1/*1 genotype patients were higher than that in CYP3A5*1/*3 or CYP3A5*3/*3 patients (P=0.0040.01,Kruakal-Wallis Test) in the first month after renal transplantation. In addition, no other statistically significant association was found between cyclosporine and CYP3A4*18B as well as CYP3A5*3 SNPs.Conclusions Genetic polymorphism of CYP3A5*3 and CYP3A4*18B may be partly responsible in large interindividual variability of cyclosporine pharmacokinetics after transplantation, but the test for CYP3A5*3 genotype before transplantation remains to be explained.

Abstract:In the research of pharmacokinetics of traditional Chinese medicine it is helpful, if pharmacodynamic indexes are timely introduced into observation, to improving the characterization of the dynamic process in vivo of the tested drugs even though the characterization represents limited components that can be tested from the drugs. Nevertheless, such problems as how to conduct the experiment of pharmacodynamics, how to choose testing indexes and what is the characteristics of pharmacodynamics of traditional Chinese medicine should be taken into consideration in the research. Based on the researches already done and in comparison with literatures abroad this article discusses these problems as mentioned above and holds that in the experiment of pharmacodynamics end indexes should be taken as the main choice so as to be able to reflect and grasp the dynamic process of curative effects in vivo as a whole.

The observed concentrations were matched well by ρ_0 prediction and were better than those calculated by ρ_2 (concentration at 2 h after dosing) and the union of ρ_0 and ρ_2. CONCLUSION It is feasible to perform individual administration with only one concentration using EXCEL, based on the results of population pharmacokinetics.
Influence of CYP3A and ABCB1 Polymorphisms on Cyclosporine Concentrations in Renal Transplant Recipients.

Abstract:Aim: Cyclosporine is a substrate of CYP3A and ABCB1. This study examined the role of CYP3A and ABCB1 polymorphisms on cyclosporine pharmacokinetics in renal transplant recipients. Patients & methods: CYP3A and ABCB1 SNPs were detected in 521 recipients. The relationships of dose-adjusted concentrations with corresponding genotypes were investigated at the different terms. Results: CYP3A5 rs776746 and CYP3A7 rs10211 genotype affect C0/D at the short-term, medium-term and long-term after transplantation (p < 0.05). CYP3A7 rs2257401 genotype affects C2/D at the medium-term (p < 0.05). CYP3A4 rs4646437, CYP3A5 rs776746 and CYP3A7 rs2257401 genotype affect C2/D at the long-term (p < 0.05). There are no relationships between ABCB1 polymorphism and cyclosporine C/D. Conclusion: CYP3A genetic factors (rs776746, rs4646437, rs2257401 and rs10211) were varied in different stages after transplantation. The role of CYP3A7 in cyclosporine metabolism requires further study.
