Abstract:This article analyzes health manpower addition and losing in Zhejiang province from 1978 to 1998. The result show that: 1. The number of health manpower addition and losing in sampling area in zhejiang province increases in different degrees in twenty-one years. 2. Health manpower with middle-high ranking lost seriously, which was 35.0% in all losing numbers. 3.The proportion of losing manpower who moving to another health unit was fairly high, the proportion was about 41.7%. In addition, this article forecasts the number of addition and losing in 2000 year using index curve method.

Abstract:In line with the hospital development practice in recent years,this paper discussed the early-warning system for hospital talents drainage.First of all,it points out that early-warning indicators,early-warning analysis and early-control measures are key elements of such a system.On this basis,it pinpoints key points in building this system,including organizational assurance,by-level management,dynamic tracking,fairness,bettering of HR mechanism,and reservation of potential HR resources.Therefore,this will be highly useful and practical for studying how to minimize talents drainage and maintain competitive advantages of hospitals.
Analysis of Doctors' Resignation in Public Hospital and its Influential Factors

Abstract:Objective To explore the characteristics and related factors of doctors' resignation in a public hospital and to provide evidences for the management of human resources in hospitals.Methods In total of 72 doctors who resigned from our hospital from 2009 to 2014 were included in our study, and there basic information was collected to conduct retrospective analysis. In-depth interview and questionnaire were adopted to find out each doctor's resignation reason and new job after resignation so as to conduct descriptive analysis.Results Doctors' resignation was influenced by many different factors including personal factors, family factors, job satisfaction, and external career opportunities. There were differences of resignation reasons among doctors at different professional ranks and levels.Conclusion It is necessary to improve the performance salary distribution system in order to ensure doctors maintain a fair and reasonable income level, to increase the job security by improving the job environment, to improve the training and welfare for young doctors, and to establish a beneficial and reasonable recruiting and promotion system to ensure the healthy career and skill development as well as fair opportunity for doctors.

Abstract:Objective To explore the reasons for new employees to resign , and put forward the counter-measures to reduce the rate of resignation.Methods Questionnaire and fish bone method were used to analyze the influencing factors of new employees to resign .Results The number of new workers resignation was increasing , and most of the resignation was focused on people with the low level educational degree , but doctors’resignation was rising.The main influencing factors leading to the high resignation rate were job factors , interpersonal factors, fam-ily, personal factors and social factors .Conclusion The hospital should improve the quality of recruitment , strengthen the new staff training , set up platforms for a variety of talent training , improve the fair and reasonable compensation distribution system so as to reduce the resignation rate of the new employees .

Abstract:Abstract Background The serious shortage of human resources for primary health care (PHC) is a common issue in health reforms worldwide. China has proposed that it is an effective way to encourage and guide qualified medical personnel to work in primary health care institutions (PHCIs). However, few studies have been conducted on the willingness and influencing factors of medical personnel to seek employment at PHCIs. Methods Based on implicit theory and lexical approach, pre-investigation was conducted to collect the items that influence the medical personnel to seek employment at PHCIs from the perspective of guided objects. Through a three-phase investigation of 1160 doctors in 29 public hospitals in 9 cities, the items were categorized, and a structural equation model was established and verified to explore the interrelationship of influencing factors. Results A total of 6 factors were rotated, including Sense of Gain (SG), Internal Organization Development (IOD), Remuneration and Development (RD), Condition of the City Where the PHCI Is Located (CCPL), Job Responsibilities (JR) and Family Support (FS). The results of the model showed that IOD, RD, JR and FS had a significantly positive effect on the SG. In addition, the FS, RD and JR significantly mediated the relationship between the internal and external environment of PHCIs and SG. The values of the fit index indicated an acceptable-fitting model. Conclusion Family, remuneration, individual development, and job responsibility are closely related to the willingness of medical personnel to seek employment at PHCIs, and the internal and external environment of PHCIs is also an important factor. Therefore, the development of PHC providers can be promoted by paying attention to the family members of medical personnel, establishing a reasonable remuneration system, providing suitable development opportunities, arranging work rationally and improving the internal construction of PHCIs.

Abstract:Objective To provide evidences of the workforce management out of organization in the public hospital.Methods The literature review and questionnaire survey and expert consult method were used to analyze the phenomenon,courses,and main problems of workforce out of organization in the public hospitals of Shanghai.Results The phenomenon of using workforce out of organization is common in public hospital.The satisfaction degree of them was associated with the their post character,the scale of the hospital,the type of the work,the salary system,and promote of the post system.Conclusions Government,employers and employees are all needed to be improved to make this special personnel system more harmonious.
Health-personnel recruitment and retention target policy for health care providers in the rural communities: A retrospective investigation at Pudong New Area of Shanghai in China

Abstract:To tackle the shortage of health personnel in the rural areas of Pudong New Area of Shanghai, the local government issued an incentive policy as one of the medical reforms. The current investigation focused on the relevant incentive measures and their corresponding effects and problems with a view to providing referential and useful experiences for those who are engaged in addressing the same problem at home and abroad. The details of the incentive policy were derived from the government document, and the related data about the flow of the rural community health care providers, from the institutional investigation. As indicated by the current investigation, the incentive policy produced some positive effect in attracting health care providers to work in the rural community health centers, especially general practitioners, nurses, MS/MD degree holders, and intermediate professional title holders to be employed in the farther ones. However, it was turned out that the population of high quality health care providers was still not sufficient enough to cover the whole rural areas, which suggested that it was still hard to draw such qualified medical individuals. To conclude in the current investigation, we made three recommendations for the policymakers to take into account in terms of policy maintenance, benefits for health personnel, and guarantee of their lawful rights and interests.

Abstract:The principal problems of healthcare services in China are “difficulty in seeing a doctor”and “high expense of getting medical service” (commonly known in Chinese as “kan bing nan, kan bing gui”). The central Chinese government has already launched the bottom-up cascading medical system and two-way referral system recently in order to solve these problems (1). Only when patients go to medical institutions in an orderly fashion, can we see the hope of breaking the kan bing nan, kan bing gui (2). However, we face a number of obstacles when implementing the referral policies. The biggest obstacle is the lack of Human Resource (HR) for primary care both in capacity and volume (3). The central Chinese government has launched a series of policies to deal with the shortage of HRs in rural areas. Profound measurements involve postgraduate training for General Practitioner (GP) (a three-year plan beginning in 2010 for producing health professionals for rural areas and improving rural retention), “3 2” medical education model (3-year diploma education and 2-year postgraduate GP training), and in-service training for physicians in rural areas (4). It is not the time to assess their effectiveness, however, these measurements are certain to improve the capacity of Community Health Service (CHS) institutions.

Abstract:Introduction: With the deepening of the healthcare reform in China, the competition in the sector is becoming fiercer leading to several unintended consequences such as talent turnover, which highly affect hospital development. Taking the case of a specialized university-affiliated stomatology hospital in Guangzhou, this thesis aims to analyze the main factors that contribute to talent turnover in this type of hospitals. The loss of talents in the field of stomatology is not unique to China and represents a significant problem in both developed and developing countries. Methods: The data for the study were collected from post facto live interviews conducted with 21 resigners to understand their feelings and motivations after the passage of time. The data were coded and concepts refined using MAXQDA software to form 18 sub-categories and six aggregate dimensions leading to the construction of two theoretical models. Results: The balance of power embedded in the dual leadership system characteristic of Chinese organizations and corresponding leadership style and behavior, deeply affect the turnover of talents. The more unbalanced the power, the greater the impact on turnover. Conclusion: Based on the situation studied, a power imbalance allowed for individual behaviors that determined the development of the hospital and directly affected institutional fairness, culture, working atmosphere and ultimately led to turnover.

Analysis of Qualification of Nurses Hired by a Tertiary Hospital in Beijing

Abstract:Objective To evaluate the qualification of nurses and the effect ofrecruitment so as to improve therecruitment and training of nurses. Methods The qualification of nurses recruited from 2011 to 2013 were evaluated by head nurses and nursing teachers of each department through questionnaires. Results The comprehensive qualification of nurses recruited in recent years was high and overall satisfaction reached 80%. The score of basic individual quality was the highest (average score of each item>3.50). Scores of theoretical knowledge and working ability were also high, whereas scores of research ability (3.17±0.70) and nursing management (3.14±0.56) were lowest. Differences of scores of theoretical knowledge, working ability and developing potential between undergraduatesand junior college students were statistically significant (P<0.05). Conclusion A comprehensive assessment of candidates should be made in recruitment and more attention should be attached to career management and training of nurses. A good assessment mechanism should be established to improve nurses' comprehensive quality and to ensure the sustainable development of nursing talents.
Human Resources Sinus and Analysis among the Second Grade Hospitals in China

Abstract:Objective:To describe the current status of the human resources management among the Second Grade hospitals. Method:Totally 948 head nurses from different Second Grade hospitals in 30 provinces were investigated by a self-designed questionnaire. Results:Nurse allocation:bed/nurse ratio was 1:0.32 in clinical department and 1:1.99 in ICU; most nurses were 20 to 30 years old and graduated mainly from junior college; about 70.99% departments adopted responsibility nursing system and one nurse took care of 10.35 patients on average, and only 24.65% departments had care workers; nurse turnover rate in the past year was 4.00%, and the main reason was transferring to other hospitals or other business; only 32.17% departments had performance appraisal system. Conclusions:Bed/nurse ratio is far less than the announced standards and nurses are in the status of being young, low educational background and job titles. The performance appraisal system is imperfect. We should increase nursing manpower, improve the level of nurses' educational background and titles and strengthen the construction of the logistical support system.

Abstract:Objective To investigate the reasons for leaving of contract nurses in a tertiary oncology hospital in Beijing , propose related countermeasures and improve the ways of human resource management to stabilize the nursing team .Methods Retrospective analysis combined with deep interview was used to study the reasons of nurses'leaving by reviewing their basic information .Results Leaving nurses were mostly not the household register of Beijing, younger than 26 years old, college degree, with low title and short working years. The reasons were mainly family related , such as marriage , taking care of parents or kids , high work pressure and low treatment , etc.Conclusions Hospital managers should improve human-based management and nurse's payment, as well as relieve their work pressure in order to stabilize the nursing team .

Abstract:Objective To investigate the current staffing of nurses and need of hospital nursing human resources in China.Methods Data were collected from 181 secondary and tertiary hospitals and 9774 nurses in mainland China by questionnaires.Results The average doctor-nurse ratio was 1 ∶ 1.39 in tertiary hospitals and 1 ∶ 1.31 in secondary hospitals.The doctor-nurse ratios in 164 hospitals (97.6％) and the proportion of nurses in health care staff in 105 hospitals (61.8％) had not reached the standard set by the Ministry of Health of China.62.3％ nurses held secondary diploma for their initial nursing education.The constituent ratio of nurses held secondary diploma decreased,while the ratio of nurses held advanced diploma and bachelor degree increased in the last 5 years from 2003 to 2007.Sixty percent of newly employed nurses were contract nurses in 2003.The proportion increased to 78％ in 2007,and in some regions it accounted for more than 90％ of new nurses.The needs and constituent ratio of nurses with.secondary diploma and advanced diploma would decrease while nurses with bachelor degree and master degree would increase in the next 5 years (from 2009 to 2013).Conclusions The nursing shortage is still severe in China,and nursing staff mainly held secondary diploma for their initial nursing education.Most of new nurses were employed as conwact nurses.The need of hospitals for nurses has increased,especially for nurses with higher educational level such as bachelor degree and master degree,while the need for secondary diploma hold ers have decreased.The need for advanced diploma holders has increased in the last 5 years and would decrease in the next 5 years,but they would still be the majority of employed nurses.The authors suggested that the staffing of nursing manpower,the work environments and career development of contract nurses should be improved,and the initial nursing education should be upgraded to meet the needs of hospitals.

Abstract:The nurse turnover and job transfer rate among nurses is a major concern for global health organizations, [ 1 ] with a nurse turnover rate of 15.1%, 19.9%, 26.8%, and 44.3%, in Australia, Canada, the United States, New Zealand, and other countries, respectively. [ 2 ] High nurse turnover and job transfer rates result in a shortage of clinical front-line nurses, which is detrimental to the stability of the nursing team and the enhancement of nursing quality. [ 3 ] In particular, during the novel coronavirus pandemic in 2020, the shortage of nurses and the overburdened workload became more apparent, which has become the focus of medical management in several countries. Currently, most studies are focusing on the factors that influence nurse turnover or job transfer. Different working hours, professional titles, work pressure, occupational fatigue, job satisfaction, and other factors influence nurse turnover and job transfer, and hospital managers are continuously looking for solutions. [ 4–6 ] Studies have shown that refined performance management produces positive outcomes in the management of intensive care units (ICU), pediatric wards, and operating rooms in hospitals. [ 7 , 8 ] A retrospective analysis of nurses in the Tangshan Workers' Hospital showed that clinical nurses transferring to nonclinical departments had become a serious problem with 72 nurses transferred from 2014 to 2018, it is urgent to take effective intervention to reverse the tendency. The study received significant support and policy support from hospital executives, it intended to evaluate the influencing factors, build a new model of a performance-based nursing salary system, and assess its effect on inhibiting job transfer tendency among clinical nurses. Abstract:Objective:Analyze on the allocation of human resources and equipment in public medical institutions from 2008 to 2012 in order to provide the information for the government to optimize resource allocation. Methods Secondary data and data from the survey of public general hospitals and community health centers in the district were used in the analyses. Results The number of health care professionals in the public general hospitals and community health centers increased, but the number of physicians and the proportion of physicians with high professional level declined in community health centers. In addition, talent drain existed in the above institutions and insufficient allocation of large equipment existed in the district. Conclusion In order to improve medical competence of public medical institutions in the district, human resource allocation, compensation and incentive mechanism should be improved, and large equipment in the public medical institutions should be invested by the government.

Abstract:This research conducted a quantitative and qualitative analysis which based on 93 private hospital ' s data.It had identified that the status of private hospital' s efficiency,health care quality,and the(?)r operating models.The study found that private hospitals were in a small scale,lower quality,and higher price of health care. It may be affected by low standard of market entry policy,regulation status,tax policy and contracting rate to public health insurance programs.The alternative policies were proposed which adequate local situation.
