Abstract:Aims: To compare the efficacy and safety of daily lipoic acid (300-600 mg i.v.) plus methylcobalamin (500-1000 mg i.v. or im.) (LA-MC) with that of methylcobalamin alone (MC) on diabetic peripheral neuropathy (DPN).Methods: Electronic database were searched for studies published up to November 1, 2012 and study quality was assessed in duplicate. A random or a fixed effect model was used to analyse outcomes which were expressed as risk ratios (RRs) or mean difference (MD). I-2 statistic was used to assess heterogeneity.Results: Seventeen studies were included. Combined data from all studies showed that the LA-MC combination therapy was significantly superior to MC monotherapy (RR = 1.47; 95% CI: 1.37-1.58). Superiority of the LA-MC combination was shown in nerve conduction velocity (NCV) with WMDs of 6.89 (95% CI: 4.24-9.73) for median motor nerve conduction velocity (MNCV), 5.24 (4.14-6.34) for median sensory nerve conduction velocity (SNCV), 4.34 (3.03-5.64) for peroneal MNCV, and 4.53 (3.2-5.85) for peroneal SNCV. There were no serious adverse events associated with treatment.Conclusions: The results of the meta-analysis show that treatment with LA-MC for 2-4 weeks is associated with better outcomes in NCV and neuropathic symptoms relative to MC treatment. However larger well-designed studies are required to confirm this conclusion. (C) 2013 Elsevier Ireland Ltd. All rights reserved.

Abstract:Objective To evaluate the efficacy and safety ofLingyi Capsuleon patients with diabetic peripheral neuropathy.Methods 206 patients were included in a randomized controlled multi-center double-blind clinical trial.These participants were randomized into two groups;treatment group in which 107 cases were treated withLinyi Capsuleand control group in which 99 cases were treated with Methylcobalamin;both groups accepted the treatments to control the blood glucose and blood pressure as much as possible.The short-form pain questionnaire and nerve conduction velocity were assessed.Results The total effective rate was 82.24%in treatment group,76.77%in control group(P0.05).The effect in alleviating the pain was better in treatment group than in the control group(P0.05).No severe side-effect was observed during clinical trial.ConclusionLinyi Capsuleis effective and safe in improving diabetic peripheral neuropathy.

Abstract:Chemotherapy-induced peripheral neuropathy (CIPN) seriously affects the quality of life of patients with multiple myeloma (MM) as well as the response rate to chemotherapy. Acupuncture has a potential role in the treatment of CIPN, but at present there have been no randomized clinical research studies to analyze the effectiveness of acupuncture for the treatment of CIPN, particularly in MM patients.

Abstract:Objective:To explore the clinical effect of different dose of methyl cobalamin combined ganglioside GM1 in treatment of diabetic peripheral neuropathy patients.Methods:116 patients with diabetic peripheral neuropathy patients accepted in our hospital from March 2013 to March 2016 were selected and randomly divided into group A and B with 58 cases in each group.The patients in group A were given high dose methyl cobalamin combined GM1,and the patients in group B were given regular dose methyl cobalamin combined GM1.The clinical effect,neuroelectrophysiology,adverse reactions and life quality of two groups were observed and compared.Results:The remarkable effect rate and total efficiency of group A were 63.79 ％ and 96.55 ％ respectively,obviously higher than 39.66 ％ and 84.48 ％ of group B with statistical significance (P＜0.05).motor nerve conduction velocity (MCV) and sensory nerve conduction velocity (SCV) of sural nerves and median nerve of two groups after treatment were obviously better than before,and the indexes were improved much better in group A than those of group B with statistical significance (P＜0.05).The adverse reaction rate of group A was 6.70 ％,of group B was 3.45 ％,and the difference was not statistically significant (P＞0.05).The life quality score of group A after treatment was much higher than that of group B (P＜0.05).Conclusions:Using high dose methyl cobalamin combined GM1 has good effect in treatment of diabetic peripheral neuropathy patients,which is good at life quality improvement and worth of clinical applications.
Five Traditional Chinese Medicine External Treatment Methods Combined with Mecobalamin for Diabetic Peripheral Neuropathy: A Network Meta-Analysis

Abstract:Background. Diabetic peripheral neuropathy (DPN) is one of the most common chronic complications of diabetes. Traditional Chinese medicine (TCM) external treatment has been widely used in China as adjunctive treatment, and some small sample clinical studies have proved its effectiveness. However, due to the limited number of studies, we used network meta-analysis to compare the effectiveness of 5 commonly used external treatment methods of traditional Chinese medicine in the treatment of diabetic peripheral neuropathy. Methods. We searched PubMed, EMBASE, The Cochrane Library, Web of Science, CNKI, CBM, WanFang Knowledge Service Platform, and VIP databases and collected and screened randomised controlled trials on the external treatment of traditional Chinese medicine combined with mecobalamin in the treatment of DPN according to the inclusion and exclusion criteria. The search period was from 2011 to May 2021. The quality of included studies was assessed using the revised Cochrane risk-of-bias tool for randomized trials. The outcome indicators are Toronto score, median nerve sensory conduction velocity, and median nerve motor conduction velocity. Results. A total of 22 publications were included in the study. The results of the network meta-analysis showed that acupuncture combined with mecobalamin was superior to other TCM external treatments combined with mecobalamin in terms of decreasing the Toronto score (MD = −2.8, 95% CI: −5.2∼−0.49), improving median nerve sensory conduction velocity (MD = 3.6, 95% CI: 2.4∼4.9), and median nerve motor conduction velocity (MD = 4.5, 95% CI: 2.6∼6.5). The SUCRA value and probability ranking chart showed that among the three outcome indicators, acupuncture combined with mecobalamin was the best, followed by acupoint injection combined with mecobalamin. Conclusion. In this network meta-analysis, acupuncture combined with mecobalamin shows the best results in the treatment of DPN, followed by acupoint injection combined with mecobalamin.

Oral Α-Lipoic Acid Combined with Methylcobalamin in Treatment of Diabetic Peripheral Neuropathy

Abstract:Objective To observe the clinical efficacy and safety of oral α-lipoic acid combined with methylcobalamin in treatment of diabetic peripheral neuropathy(DPN).MethodsSixty-four DPN patients were selected and randomly divided into experimental group(n=32) and control group(n=32).All the patients were given diet control,insulin or oral hypoglycaemic agent and the blood glucose was satisfactorily controlled;the original antihypertensive and antilipemic therapies were maintained.Meanwhile,patients in the control group were also given oral methylcobalamin(three times per day and each for 0.5 mg),and those in the experimental group were given oral α-lipoic acid(once a day and each for 600 mg) and oral methylcobalamin(three times per day and each for 0.5 mg).The total symptom scores(TSS) and the nerve conduction velocity(NCV) were evaluated in the two groups before and 12 weeks after treatment;and the adverse drug reactions were also observed.ResultsThe TSS and NCV were significantly improved in both groups after treatment compared with before treatment(P0.05). After treatment,TSS was significantly lower in the experimental group than in the control group(P0.05),and there were significant differences in the transmission velocities of the median motor nerve,nervus peroneus communis motor nerve and its sensory nerve between two groups(P0.05). No serious side effects were found in the two groups during the treatment.ConclusionOral α-lipoic acid combined with methylcobalamin on DPN is superior to methylcobalamin used alone for treatment of DPN,and the combined treatment is simple and has better patient compliance,making it worth popularizing.

Abstract:Aim To investigate the curative effect and the mechanism of methylcobalamin,gliclazide and combined application of both on diabetic peripheral neuropathy(DPN)in streptozotocin-induced diabetic(STZ-D)rats.Methods STZ-D rats were administered with drugs orally for 8 weeks after streptozotocin treatment.Morphologies of sciatic nerve were observed with histological method.Nerve growth factor(NGF)contents in plasma or in sciatic nerve tissues were determined by ELISA kit or immunohistochemistry.Results Morphological degeneration in sciatic nerve,decrease of NGF concertration in plasma,as well as in nerve axis cylinder,in diabetic rats was all ameliorated by methylcobalamin and/or gliclazide.However,no significant differences in morphology and NGF contents were found between single drug treated groups and combined application group.Conclusions The observations suggest that methylcobalamin,gliclazide and combined application are effective in treating DPN in STZ-D rats.No significantly enhanced effect is found in combined application group as compared with methylcobalamin or gliclazide group.

Abstract:AIMS:This study investigated the efficacies of gliclazide (GLZ), methylcobalamin (MCA), and GLZ+MCA combination therapy on DPN by evaluating the treatment-related changes in peripheral nerve function, the polyol pathway, and oxidative stress in the sciatic nerve of streptozotocin-induced diabetic rats.MATERIALS AND METHODS:The rat model of streptozotocin-induced diabetes was orally given GLZ (25mg/kg/day), MCA (175μg/kg/day), and GLZ+MCA (25mg/kg/day+175μg/kg/day) combination therapy for 8weeks, in order to observe its effects on the motor nerve conduction velocity (MNCV), on the activities of Na(+), K(+)-ATPase, aldose reductase(AR), AR mRNA expression, on the polyol contents, antioxidative enzyme activities and peroxidation products in the sciatic never tissue.KEY FINDINGS:Most of the indicators of DPN, such as delayed MNCV, altered/damaged nerve structure, inhibited Na(+),K(+)-ATPase activity, enhanced AR activity and AR mRNA expression, increased polyol contents, altered Cu,Zn-superoxide dismutase, catalase, and glutathione-peroxidase activities, and elevated malondialdehyde level in the sciatic nerves of the diabetic rats, were significantly ameliorated by treatment with either GLZ or MCA. Moreover, the combination of GLZ and MCA was found to enhance the curative effect on DPN in parts of above-mentioned parameters as compared to monotherapy.SIGNIFICANCE:Monotherapy with GLZ or MCA, and especially the combined application of GLZ and MCA, could be efficient therapeutic strategies for combating experimental DPN in diabetic rats.

Abstract:Objective To evaluate the effect of the therapy of a-lipoic acid combined with mecobalamin on diabetic peripheral neuropathy(DPN). Methods 80 type 2 diabetic patients complicated with DPN were randomly divided into 2 groups: treatment group(n=40),receiving ALA combined with mecobalamin therapy for 2 weeks;and control group(n=40),receiving mecobalamin therapy for 2 weeks.Total symptom score(TSS) were used to evaluate ,CRP was measured before and after the therapy. Results Compaired with the control group ,the TSS of the treatment group were improved better, the level of CRP was reduced significantly(both P<0.05).Conclusions ALA combined with mecobalamin effectively improve the symptom of DPN patients.

Abstract:Abstract Background: Diabetic neuropathic pain (DNP) is a common complication of diabetes mellitus, it severely affects the quality of life of Diabetic patients. Acupuncture is proofed to have favorable effects in treating DNP, however, evidence needs to be gathered and interpreted. We will make a comprehensive review of clinical trials concerning acupuncture in treating DNP and do meta-analysis if possible. Method: The following databases will be searched from the inception to September 2020: PubMed, Embase, Web of Science, China National Knowledge Infrastructure, Wan-Fang Database, and Chinese Scientific Journal Database. RCTs that evaluated acupuncture for patients with DNP will be included. The primary outcome will be patient-reported pain intensity using validated scales or verbal reporting. The secondary outcomes including the Toronto clinical scoring system, Sensory Nerve Conduction Velocity, Motor Nerve Conduction Velocity, and quality of life. The study selection, data extraction, and study quality evaluation will be performed independently by 2 researchers. A meta-analysis will be performed using RevMan V5.3 statistical software if possible; otherwise, descriptive analysis or subgroup analysis will be conducted. The quality of evidence for outcomes will be assessed with the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Results: This study will evaluate the effect and safety of acupuncture in treating DNP. Conclusions: The evidence we generated from the present study will provide more options for DNP management in clinical practice. Systematic review registration: INPLASY202090043.

Abstract:OBJECTIVE:To observe the effect of acupuncture in treating diabetic peripheral neuropathy (DPN). METHODS:Sixty patients with DPN were randomly assigned to the acupuncture treated group and the control group, 30 in each group. Besides basic treatment, patients were treated additionally with acupuncture and orally administration of Methycbal (Mecolbalamin) for 8 weeks respectively. Changes of symptoms, blood glucose, HbA1c, whole blood and plasma viscosity, the nerve conduction velocity (NCV) of sensory and motor nerves before and after treatment were observed. RESULTS:Acupuncture can ameliorate symptoms and signs of the nervous system, decrease whole blood and plasma viscosity, and improve the NCV of sensory and motor nerves in patients with DPN. CONCLUSION:The effect of acupuncture in treating DPN is definite.
Is acupoint injection the optimal way to administer mecobalamin for diabetic peripheral neuropathy? A meta-analysis and trial sequential analysis

Abstract:Objective Mecobalamin is a commonly used drug in the treatment of diabetic peripheral neuropathy (DPN). This study aimed to systematically evaluate the efficacy and safety of acupoint injection of mecobalamin for DPN. Methods Relevant clinical trials on acupoint injection of mecobalamin for DPN published before 31 January 2023 were searched in eight commonly used databases. After screening and confirming the included studies, meta-analysis and trial sequential analysis were performed. Results A total of 10 relevant studies were confirmed, and the total sample size was 927 cases. On the efficacy endpoints, meta-analysis showed that compared with other administration methods, acupoint injection of mecobalamin significantly increased the clinical effective rate by 27% [RR = 1.27, 95% CI = (1.19, 1.36), P < 0.00001], motor nerve conduction velocity (median nerve) by 5.93 m/s [MD = 5.93, 95% CI = (4.79, 7.07), P < 0.00001], motor nerve conduction velocity (common peroneal nerve) by 5.66 m/s [MD = 5.66, 95% CI = (2.89, 8.43), P < 0.0001], sensory nerve conduction velocity (median nerve) by 4.83 m/s [MD = 4.83, 95% CI = (3.75, 5.90), P < 0.00001], and sensory nerve conduction velocity (common peroneal nerve) by 3.60 m/s [MD = 3.60, 95% CI = (2.49, 4.71), P < 0.00001], and trial sequential analysis showed these benefits were conclusive. In terms of safety endpoints, meta-analysis indicated that the total adverse events for acupoint injection were comparable to other methods of administration, and trial sequential analysis suggested that the results needed to be validated by more studies. Subgroup analysis demonstrated that the benefits of acupoint injections of mecobalamin were not limited by the dose, duration of treatment, or number of acupoints reported in the included studies. Harbord's test showed no significant publication bias ( P = 0.106). Conclusion The efficacy of acupoint injection of mecobalamin for DPN was significantly better than other administrations, and its safety was comparable to other administrations. Therefore, acupoint injection may be the optimal method of mecobalamin for DPN. Abstract:Abstract Background: Many clinical trials and systematic reviews have suggested that acupuncture (include moxibustion) could be effective in the treatment of diabetic peripheral neuropathy (DPN). However, clinical practices vary greatly leads to different choices which are mainly based on personal experience. The aim of this Bayesian network meta-analysis is to compare the efficacy of different acupuncture methods for DPN. Methods: Randomized controlled trials on acupuncture treatment of DPN published before January of 2021 will be searched in 9 databases including Medline, Web of Science, PubMed, Cochrane Library, Excerpta Medica Database, Sinomed, China National Knowledge Infrastructure, WanFang, and China Science and Technology Journal Database. The methodological assessment performed using the risk of bias assessment tool of Cochrane, and the level of evidence quality for the main results will be evaluated by a recommended grading, evaluation, formulation, and evaluation system approach. Bayesian network meta-analysis will be conducted using STATA V.14.0 and WinBUGS V.1.4.3. Results: The primary outcome involves: clinical efficacy. The secondary outcomes include: motor nerve conduction velocity, sensory nerve conduction velocity, Toronto clinical scoring system, Michigan neuropathy screening instrument, the modified Toronto Clinical Neuropathy Scale, the Utah early neuropathy scale, or the neuropathy disability score, and adverse reactions. Conclusion: To find the most effective acupuncture therapy for the treatment of DPN supported by evidence-based medicine.
Effects of Combination of Mecobalamin and Alprostadil on Diabetic Peripheral Neuropathy

Abstract:Objective:To observe the effects of combination of mecobalamin and alprostadil(PGE1),mecobalamin only and vitamin 12 only on the patients with diabetic peripheral neuropathy(DPN).Methods:A total of 60 patients with DPN were divided into three groups(Group1,Group 2,and control group).Twenty patients in group1 were given mecobalamin(0.5 mg IM.qd.) and alprostadil(10 μg IV.qd.) for consecutive 2 weeks.Twenty patients in Group2 were only given mecobalamin(0.5 mg IM.qd.) for consecutive 2 weeks.Twenty patients in control group were only given vitamin(0.5 mg IM.qd.) for consecutive 2 weeks.All patients were received adjuvant therapy with salvia injection.Results:Compared with control group,complains and physical signs of patients and every nerve conduction velocity(NCV) improved.The effects of Group1 were better than that of Group2.No side effect was found in three groups.Conclusions:The combination of mecobalamin and alprostadil could obtain better effects in patients with DPN.

Abstract:AbstractThis study aimed to compare the efficacy and safety of prostaglandin E1 plus methylcobalamin (PGE1–MC) with that of methylcobalamin alone (MC) on diabetic peripheral neuropathy (DPN). We searched published randomized controlled trials (RCTs) of PGE1 combined with MC for DPN up to June 1, 2013. Data were extracted to evaluate methodological quality and describe characteristics of studies in duplicate. A random or a fixed effect model was used to analyze outcomes which were expressed as relative risk (RR) or mean difference with a 95 % confidence interval (CI). All data were analyzed using Review Manager 5.2 software. Twenty-six RCTs involving 2,107 individuals were included. Meta-analysis showed that PGE1–MC combination therapy was significantly better than MC monotherapy (RR = 1.40; 95 % CI 1.33–1.48) on efficacy. The weighted mean differences in nerve conduction velocities (NCVs) were 6.72 (95 % CI: 5.42–8.02) for median motor nerve conduction velocity (MNCV), 5.13 (CI 4.13–6.13) for median sensory nerve conduction velocity (SNCV), 5.74 (CI 4.87–6.61) for peroneal MNCV and 4.62 (CI 3.89–5.34) for peroneal SNCV in favor of the PGE1 + MC combination group. Moreover, there were no serious adverse events in both groups during the treatment period. The results of the meta-analysis show that treatment with PGE1–MC is safe and can gain better outcomes in neuropathic symptoms and NCVs compared with MC alone. However, the conclusion may not be strong because most of the studies included in this meta-analysis have poor methodological quality.

Abstract:AIM: To investigate the effects of methylcobalamin, gliclazide and combined application of both on diabetic peripheral neuropathy in streptozotocin diabetic rats. METHOD: Drugs were administrated orally for 8 weeks. Effects of drugs on pain threshold to mechanical stimulation, sciatic never conduction velocity, glycated hemoglobin and glycated serum protein content, rheologic parameters, aldose reductase activity and Na+,K+-ATPase activity of streptozotocin diabetic rats were determined. RESULT: Compared with diabetic group, pain threshold to stimulation was improved, sciatic nerve conduction velocity was increased, aldose reductase activity was depressed and Na+,K+-ATPase activity was increased in each administrated group. Glycated hemoglobin and glycated serum protein contents were decreased in gliclazide and combination group. In addition, erythrocyte maximal aggregation index was decreased in gliclazide group. Compared with methylcobalamin or gliclazide group, remarkable difference was not observed in combination group. CONCLUSION: Methylcobalamin, gliclazide and combined application of both could prevent peripheral neuropathy in streptozotocin diabetic rats. More effective action was not observed in drug combination group.

Abstract:Objective: To observe the clinical effect on diabetic peripheral neuropathy (DPN) treated with acupuncture combined with medication and explore its effect mechanism. Methods: Sixty-two patients of DPN were randomly divided into a combined therapy group (31 cases) and a medication group (31 cases, 2 cases dropped out); besides, 20 healthy subjects were recruited as a normal group. On the base of routine intervention, in the medication group, thioctic acid capsules were administrated orally, 0.2 g each time, 3 times a day. In the combined therapy group, besides the medication as the medication group, acupuncture was performed on bilateral Quchi (LI 11), Waiguan (TE 5), Hegu (LI 4), Tianshu (ST 25), Zusanli (ST 36), Sanyinjiao (SP 6) and Taichong (LR 3) and the needles were retained for 30 min, acupuncture was delivered once daily, 6 times a week. The duration of treatment was 4 weeks in the two groups. The score of Toronto clinical scoring system (TCSS), the nerve conduction velocity of median nerve (MN) and common peroneal nerve (CPN) were observed before and after treatment in the two intervention groups; and the serum lipid metabolism was detected before and after treatment in the two intervention groups and the normal group. Results: Compared with that before treatment, the scores of TCSS were reduced in the combined therapy group and the medication group (P<0.05) after treatment, and the score decrease in the combined therapy group was larger than that of the medication group (P<0.001). The motor nerve conduction velocity and the sensory nerve conductive velocity of MN and CPN after treatment all increased in the combined therapy group and the medication group compared with those before treatment (P<0.05), and the improvements in the combined therapy group were larger than those of the medication group (P<0.001). Before treatment DPN patients had 365 differential lipid metabolites, including sphingosine (SPH, d18:0), involved in the inositol phosphate metabolism, compared with the subjects of the normal group. There were 103 differential lipid metabolites in the medication group before and after treatment, including lysophosphatidyl ethanolamine (LPE, 18:1/0:0), participated in glycerophospholipid metabolism. In the combined therapy group, before and after treatment, there were 99 differential lipid metabolites, including lysophosphatidylcholine (LPC, 18:0/0:0), participated in the neuroactive ligand-receptor interaction. Acupuncture greatly affected 50 lipid metabolites such as lysophosphatidic acid (LPA, 0:0/22:6), LPA(0:0/18:2) and LPC(O-18:0), which was mainly involved in glycerophospholipid metabolism. Conclusion: Acupuncture combined with medication ameliorates the symptoms and the nerve conduction velocity in DPN patients, which may be related to the regulation of serum lipid metabolism.

Abstract:Diabetic peripheral neuropathy (DPN) is one of the neurological diseases with high morbidity, and its course is migratory and progressive. Currently, the effectiveness of acupuncture therapy in the prevention and treatment of diabetes and its complications has been confirmed by clinical and basic research. Acupuncture treatment modality has the advantages of low cost, high safety, no toxic side effects, and good efficacy, and is easily accepted by patients in treatment. The authors have documented the inflammatory response, glycolipid metabolism disorders, oxidative stress, blood rheology and nerve conduction in function. Abstract:Objective To observe the therapeutic effects of Tangluoning combined with point massage on diabetic peripheral neuropathy(DPN).Methods 60 DPN patients who were confirmed to the diagnostic criteria were randomly divided into the treatment group(30 patients) and control group (30 patients).On the basis of routine treatment,the patients in treatment group were given Tangluoning combined with points massage. The patients in control group were given mecobalamin.The changes of symptoms, nerve conductive velocity and amplitude were observed in both groups.Results The total effective rate of treatment group was 93.3%,which was obviously superior to that in control group (63.3%)(P0.05).The symptom score and MDNS were decreased obviously,the effect of treatment group was superior to that of control group,the difference was significant (P0.05 or P0.01).There were significant differences in symptoms of the limbs, pain,weakened ankle reflex, improvement of nerve conductive velocity and amplitude between two groups (P0.05).There were no obvious side effects during the treatment.Conclusion Tangluoning capsule combined with points massage could improve therapeutic effect on DNP.

Abstract:Abstract This randomized controlled study used corneal confocal microscopy (CCM) to compare the efficacy of Mecobalamin intramuscular injections vs oral tablets in treating mild to moderate diabetic peripheral neuropathy (DPN) by detecting early nerve fiber repair. Enrolled patients were randomized approximately 1:1 to receive Mecobalamin intramuscular injections (0.5 mg/day, 3 times/week) or Mecobalamin oral tablets (1.5 mg/day) for 8 weeks. Primary outcome was change of inferior whorl length (IWL) from baseline. Secondary outcomes included changes of corneal nerve fibre length (CNFL), corneal nerve fibre density (CNFD), corneal nerve branch density (CNBD) and the Survey of Autonomic Symptoms (SAS). 15 (93.75%) patients in the injection group and 17 (89.47%) patients in the tablet group completed the study. The injection treatment significantly improved patients’ IWL from baseline (21.64 ± 3.00 mm/mm 2 vs 17.64 ± 4.83 mm/mm 2 , P < 0.01) while the tablet treatment didn’t. Additionally, the injection treatment led to significantly improved CNFL, CNBD and SAS from baseline (all P < 0.05) while the tablet treatment did not. No patient experienced any adverse events. In conclusion, CCM is sensitive enough to detect the superior efficacy of 8-week Mecobalamin intramuscular injection treatment for DPN compared to the oral tablet treatment.
