The value of mucosal pit and micro vascular structure in the diagnosis of benign and malignant gastric ulcer

Abstract:Objective To study on the value of magnifying endoscopy in diagnosing benign and malignant gastric ulcer from mucosal pits and micro vascular structure. Methods One hundred and fifty eight patients with gastric ulcer were studied using Pentax EG-3430Z magnifying endoscope, after routine observations. Histopathological examination was taken with biopsy samples from the magnified sites of stomach, after carefully observing the changes of mucosal pits and mucosal micro vascular structure. Results The pit patterns of benign gastric ulcer were regular, 34. 4% patients appearing type C and 67.7% patients type D. The pit patterns of 93. 8% malignant ulcer were irregular and different in sizes. There were significant difference between them（ X^2 = 103. 888, P 〈 0. 001 ）. Regular micro vascular network can be seen in 69. 9% benign gastric ulcer patients, otherwise 80% malignant gastric ulcer subjects micro vessel were winding, deforming, irregular and reticular. Tumorous vessels can be seen in 29. 2% malignant gastric ulcer patients. There were significant difference between them（ X^2 = 130. 963, P 〈 0. 001 ） ; mucosal pits and mucosal micro vascular structures have high sensitivity and specificity in distinction malignant gastric ulcer. Conclusion The changes of mucosal pits and micro vascular structure in gastric ulcer by magnifying endoscopy are crucial in the diagnosis and identification of benign and malignant gastric ulcer and helpful for early detection of gastric cancers.\r\n\r\nKey words: \r\nGastric ulcer;  Microvascular;  Diagnosis ;  Magnifying endoscopes
