Management of penetrating aortic ulcer

Abstract:Objective To summarize the experience of patients with penetrating aortic ulcer(PAU) treated by open surgery or endovascular repair.Methods Ten patients with PAU were reviewed.Six of their lesions located in descending aorta,while the other four were in abdominal aorta,with complications consisting of abdominal aortic aneurysm(AAA)(1/10),intramural hematoma(IMH)(2/10),ruptured aortic ulcer(2/10),and aortic dissection(6/10).Results Four cases were treated by open surgery,and one of them suffered from pleural effusion postoperatively and recovered after 17 days' management.The other six were treated by endovascular repair,and one of them presented a mild endoleak,which disappeared spontaneously within the first month.All patients were followed up by computed tomography angiography,and the mean time of follow-up were(17.8±11.53)months.One patient was lost to follow-up at 12 months.No procedure-related complications occurred in the patients treated by open surgery.And no stent graft migration was found in patients treated by endovascular repair.Conclusion Open surgery or endovascular repair in the management of properly selected patients with PAU is feasible and safe,with favorable short-and mid-term outcomes.
