Epidemiologic Investigation and Clinical Manifestation in Patients with Different Subtypes of IgA Nephropathy

Abstract:Objective:To retrospectively investigate the epidemiology and clinical characteristics in patients with different subtypes of IgA nephropathy. Methodology:Seven hundred and fifty nine IgAN patients from November 2003 to June 2006 were enrolled in this study. They were divided into seven subtypes: isolated microscopic hematuria (I-H), asymptomatic abnormal urinalysis (U-ab), recurrent macroscopic hematuria (R-H), crescentic IgAN (cres. IgAN), mass proteinuria (MP), hypertension (HT) and end stage renal disease (ESRD). Their epidemiological features, clinical manifestations were investigated and compared. Results:They were 371 males and 388 females with mean age (30.0±10.0) years old. Among the 759 patients, there were 2.8% I-H, 13.2% R-GH, 42.9% U-ab, 7.2% Cres.IgAN, 10.9% MP and 18.9% HT. Different subtypes had different clinical characteristics: isolated microscopic hematuria was the only clinical feature of I-H. U-ab was the most common subtype in IgA nephropathy, 47% patients without clinical findings. 69% patients with R-GH had the upper respiratory infection or diarrhea before the onset of the macroscopic hematuria. Severe hematuria, proteinuria and renal dysfunction were the most noticeable clinical manifestations in Cres.IgAN. The amount of urinary protein excretion in MP was markedly more than that in other subtypes. And there were a number of clinical features for HT: these include the male predominance, the peak incidence in the third decades of life, and 62.5% patients with renal dysfunction at the onset of the disease. Conclusion:The epidemiologic investigation and clinical manifestation were distinct among the 7 subtypes, these might indicate the different pathogenesis of IgA nephropathy, and might be helpful for the treatment of IgAN.
