Feasibility of Human Gamma-nterferon Assay(HGA) Using rCFP-10/ESAT-6 Fusion Protein

Abstract:The objective of this study is to evaluate the sensitivity and specificity of a whole blood human gamma-interferon assay(HGA) using Mycobacterium tuberculosis-specific antigens rCFP-10/ESAT-6 fusionprotein(antigens from the Mycobacterium tuberculosis RD-1 region) and tuberculin skin testing(TST) for the detection of Mycobacterium tuberculosis infection.n this double-blind,randomized,controlled,parallel group trial,prospective testing,229 patients with suspicious TB infection were tested.t is proved lately 129 of the 229 with tuberculosis disease(TBCP) and 100 with non-tuberculosis disease(NTBD) who had no TB history.Using a defined level of specific FN-γ production and TST≧10mm as positive cut-offs,the sensitivity of HGA was 96% compared to 89% for TST,(0.025<p<0.05),and the specificity of HGA was 99% compared to 65% for TST(p<0.005).Prior immunization with M.bovis(Bacilli Calmette-Guerin)(BCG) or the type of non-TB mycobacterium had no effect on the sensitivity or specificity of the HGA pulsed by rCFP-10/ESAT-6 fusion protein.The distribution of TST results in TB patients showed a broad peak between 10 and 25mm,while the results in the HGA were bimodal in both TB and NTBLD patients.HGA with rCFP-10/ESAT-6 fusion protein may prove an alternative to skin testing for detecting M.tuculosis infection in certain settings.
