Osteotomy correction of fixed kyphosis in ankylosing spondylitis

Abstract:Purpose of study: To investigate the selection of osteotomy methods, resection zone and fixation area for correction of kyphosis secondary to ankylosing spondylitis. Methods used: Fifty-four patients (49 males, 5 females; age range, 28 to 56 years) were divided into two groups. Group 1 (23 cases) underwent polysegmental V-shape osteotomy. Group 2 (31 cases) was corrected by monolevel transpedicular wedge osteotomy. of findings: No deaths or infection occurred. Two cases had dural tear, one had pedicle fracture, incomplete osteotomy closure was found in six cases during operation, transient lower extremity numbness occurred in two patients and superior mesentery artery syndrome was found in one case. The mean correction in Group 1 was 44 degrees, in Group 2, 36 degrees. After a follow-up of 6 to 39 months, Group 1 lost 4 degrees. Group 2 had no loss, and patient satisfaction was 94%. Relationship between findings and existing knowledge: Polysegmental V-shape osteotomy and monolevel transpedicular wedge osteotomy are the standard correction methods for fixed kyphosis in ankylosing spondylitis, and have been applied widely in recent years. Compared with existing reports, both groups in our study have good clinical results, and the complications are low. Overall significance of findings: Two kinds of osteotomy techniques have similar outcome, so the main decision to make is the degree of anterior column ossification.
