Abstract:It is because of the distinctive anatomical complexity of the brain, surgery on the neoplasm of the brain is different from surgery of neoplasm of the other system of the body. Firstly, due to the limitation of skull, it is more difficult and risky to obtain sample of cerebral neoplasm. That is why diagnostic neuroimaging is very important for non-invasive diagnosis and defining anatomical location. Secondly, the dilemma of surgery on neoplasm of the brain is to maximize the extent of tumor resection while minimize damages on the normal brain with function. Therefore, it is essential to obtain as much information from diagnostic neuroimaging before surgery. Diffusion tensor imaging (DTI) is a new method to enable unprecedented, in vivo visualization of individual white matter (WM) tracts and their relationships with cerebral neoplasm, which is vital for treatment planning. In despite of some limitation, DTI has revealed tremendous value in neurosurgery. This new technology will enhance to development of neurosurgery. With a brief introduction of the basic principles underlying DTI, this article has reviewed current clinical applications relevant to treatment of cerebral neoplasm.
