Clinical analysis on glucose metabolism in liver cirrhosis patients combined with primary hepatocellular carcinoma

Abstract:Objective To discuss the clinical features of glucose metabolism among the patients suffered from liver cirrhosis combined with primary hepatocellular carcinoma during the perioperative period.Methods Levels of blood glucose,serum insulin and serum C peptide were estimated in fasting state,and 0.5,1,2,3 h respectively after an oral ingestion of glucose with a dose of 75 g in 40 cases of liver cirrhosis combined with primary hepatocellular carcinoma in one group and other 10 normal persons as control. Thus,the insulin resistant index and β cell function index for each patient were recorded and calculated.Results The oral glucose tolerance in hepatocellular carcinoma group was significantly lower than that of control group. The serum insulin in the control group was (10.3±3.7)mIu/L for fasting state and 35.2±12.8,65.1±10.2,19.5±11.2,12.2±10.1 mIu/L for 0.5,1,2,3 h respectively after oral glucose ingestion,while that in the hepatocellular carcinoma group are (20.4±5.7)mIuU/L for fasting state and 55.1±12.6, 95.1±14.2, 75.3±21.5, 36.4±16.5 mIuU/L for 0.5, 1, 2, 3 h after glucose ingestion (P<0.05). The serum C peptide in the control group was (3.2±0.1)ng/ml for fasting state and 6.3±0.3, 10.1±0.9, 8.3±1.2, 4.8±1.1 ng/ml for 0.5, 1, 2, 3 h after glucose ingestion, while that in hepatocellular carcinoma group was 1.5±0.3,4.1±0.1,6.1±1.4,1.9±0.6,2.1±0.9 ng/ml,respectively(P<0.05). The insulin resistant index and β cell function index in control group were 2.42 and 93.6 respectively, while that in hepatocellular carcinoma group was 5.26 and 177.4. There was an significant difference between two groups. Among hepatocellular carcinoma group, the blood glucose, insulin and C-peptide level after ingestion of glucose are significant higher in Child B liver function patients than that in Child A patients.Conclusion Patients of liver cirrhosis combined with hepatocellular carcinoma are prone to the occurrence of glucose metabolism abnormality, which are demonstrated mainly by the impaired glucose tolerance, resistance of insulin and abnormal function of pancreatic islet beta cell. Furthermore, the extent of glucose metabolism abnormality is correlated with the level of liver function.
