Abstract:BACKGROUND: Carboxymethyl chitosan (CMC) film is characterized by rapid degradation, so the degradation rate is hard to be measured. In addition, there is still difficulty of the correlation between simulation in vitro and animal experiment. OBJECTIVE: To find a suitable model for degradation in vivo and study the degradation and biocompatibility of three CMC films. DESIGN,TIME AND SETTING: A controlled observational experiment was performed at the Testing Center for Medical Device of National Institute for the Control of Pharmaceutical and Biological Product from September 2007 to July 2008. MATERIALS: There were three anti-adhesion films: single CMC film, CMC: cellulose: polyvinyl alcohol (PVA) = 80: 10: 10, and CMC: cellulose: PVA = 90: 5: 5. All anti-adhesion films were prepared by adopting the method of forming extension of water. METHODS: In vitro simulation: The three samples were maintained in simulated body fluid (SBF) and the lysozyme phosphate buffer solution (L-PBS) (mass : volume < 1:30).Thereafter, the samples were obtained to calculate degradation rate. In vivo simulation: ① The three samples were subcutaneously implanted into back of 15 SD rats, with the interval wider than 1 cm. The degradation was observed at day 7, 15, and 33 post-implantation. ② Thirty SD rats were randomly divided into three groups, with 10 rats in each group. The samples were put in tubes containing polymethylmethacrylate (PMMA), and then they were subcutaneously implanted into back of rats in each group. Thereafter, rats were sacrificed at day 7, 15, 22, 33, and 77, every 2 rats at each time point. The tube was obtained to calculate degradation rate. ③ 24 SD rats were randomly divided into three groups, with 8 rats in each group. The three samples were subcutaneously implanted; thereafter, the rats were sacrificed at day 7, 15, 30, and 100 post-implantation, every 2 rats at each time points. The tissue in the implanted region was obtained for histopathological changes using hematoxylin-eosin staining. MAIN OUTCOME MEASURES: ① Studies on degradation based on gross observation and mass loss rate; ② histopathological changes post-implantation. RESULTS: ① To a certain extent, two models of in vitro degradation could reflect the degradation of carboxymethyl chitosan films, but the model of L-PBS was faster than the model of SBF in the speed of degradation.In fact, the model of L-PBS was coincidence with in vivo degradation. ② Carboxymethyl chitosan films which were added a small amount of cellulose and PVA could not improve their degradation very well, but could improve the integrity of film after degradation. ③ Due to the rapid degradation of carboxymethyl chitosan, anti-adhesion films could lead to a large number of phagocytes and fibroblasts, resulting in a certain inflammatory reaction. Addition of cellulose and PVA could not reduce inflammation, by contraries, it could extend the period of healing, especially on the sample of carboxymethyl chitosan/cellulose/polyvinyl alcohol. CONCLUSION: ① The model of L-PBS can simulate in vivo degradation and improve the efficiency of detection. ② Carboxymethyl chitosan anti-adhesion film has a short degradation cycle and a fast healing processing.
