Abstract:Objective To evaluate the effect of diltiazem on improving tacrolimus blood concentration in liver transplantation patients.Methods One hundred and sixteen cases with long-term administration of FK506 after liver transplantation were research subjects.FK506 and diltiazem simultaneously administration were the test group(58 cases),simple oral administration of FK506 for the control group(58 cases).FK506 blood concentrations,FK506 dosage,liver and kidney function of the two groups were evaluated.Results After taking Diltiazem,FK506 blood concentrations of test group increased more(4.5±0.5)μg/L than pre-administration,representing an increase of 45.9%,that is more significant(P0.01);compared with the control group there was significant difference too(P0.01).Test group significantly reduced the dosage of FK506.There was no significant side-effect about the hepatic function,renal function and other data.Conclusion Diltiazem can increase tacrolimus blood concentration after liver transplantation.No significant side effect was found.

Abstract:Background: Co-administration of diltiazem and cyclosporine A (CsA) in kidney transplant recipients shows improvement of renal transplantation outcomes. Methods: We respectively analyzed 1531 kidney transplant recipients treated by different immunosuppressive therapy schemes from 1986 to 2003. They were divided into three groups depending on their immunosuppressive therapy schemes: control group with a standard triple therapy without use of diltiazem; study group I with the combination of diltiazem and the standard triple therapy but slightly low CsA; study group II with combination of diltiazem and a modified standard triple therapy but lower CsA. The CsA blood concentrations, posttransplant complications, and long-term survival in the three groups were compared. Results: The results showed that the patient and allograft survival in the study group II was 69.9 and 65.1%, respectively, significantly higher than that in the control group (50.7 and 47.6%). Occurrence of hepatotoxicity and nephrotoxicity episodes was higher in the control group than those in the study group I and the study group II. The incidence of acute rejection in the control group was 30.3% (23/76), similar to 28.0% (184/657) in study group I, but statistically significantly higher than 7.6% (61/798) in the study group II. Conclusion: Combination of diltiazem and CsA in the kidney allograft recipients tends to reduce the CsA oral dosage, improve patient survival, and decrease the occurrence of hepatotoxicity and nephrotoxicity.
Diltiazem Combined with Nifedipine in Treatment of Twenty Patients with Kidney-Transplanted Hypertension

Abstract:Objective:To study the therapeutic effect of diltiazem with nifedipine after kidney-transplantation with ciclosporin(CsA). Methods:Fourty patients were randomly divided into 2 group evenly. The treatment group took diltiazem 30 mg, po, bid, and nifedipine 10mg, po, tid. The control group received nifedipine 10 mg, po, tid. The drug dosages were adjusted according to the level of blood pressure. The artery pressure and renal function were checked for more than 2 years. Results:Diltiazem changed the metabolism of CsA, resulting in a forty percent reduction in CsA dose. But the risk of CsA toxicity did not increase. Conclusion:Diltiazem with nifedipine can safely be used in the treatment of hypertension after kidney transplantation, and can lower the dose of CsA.

Abstract:Diltiazem is a benzothiazepine calcium blocker and widely used in renal transplant patients since it improves the level of tacrolimus or cyclosporine A concentration. Several population pharmacokinetic (PopPK) models had been established for cyclosporine A and tacrolimus but no specific PopPK model was established for diltiazem. The aim of the study is to develop a PopPK model for diltiazem in renal transplant recipients and provide relevant pharmacokinetic parameters of diltiazem for further pharmacokinetic interaction study.

Abstract:ObjectiveThe effects of diltiazem on 1692 kidney transplant recipients under the immunosuppressive regimen of cyclosporine A (CsA) in combination with either mycophenolate mofetil or azothioprine were assessed. The two treatment groups were compared for blood concentrations of CsA, the extent of acceptable dosage reduction for the maintenance of immunotherapy, potential effects of kidney protection, and promotion of graft function.MethodWe monitored changes of blood concentrations of CsA in the two different patient treatment groups for post-transplant graft function, episodes of acute rejection, and hepatic and renal toxicity in 1640 renal transplant recipients after treatment with diltiazem.ResultsIn patients treated with the triple immunosuppressive regimen consisting of CsA, azothioprine, and prednisolone (Pred), the sub-group of patients receiving the diltiazem treatment saw a significantly reduced CsA dosage in comparison to the non-diltiazem group (control group 1) (P < 0.05), but the blood concentrations of CsA of the diltiazem group were higher than those of control group 1 (P < 0.01). Of the patients treated with CsA, mycophenolate mofetil, and Pred, the sub-group of patients also treated with diltiazem showed similar effects: CsA dosage was reduced (P < 0.01) and the blood concentrations of CsA significantly increased (P < 0.01) in comparison with those of control group 2. In addition, recovery time of graft function decreased to 4.7 ± 1.8 days and 3.9 ± 1.4 days in the two diltiazem treatment groups, respectively (P < 0.05), and the rate of acute rejection decreased to 21 (p < 0.05) and 7.9% (P < 0.01), respectively.ConclusionIn our cohort of renal transplantation patients, co-administration of CsA and diltiazem increased CsA blood concentration, thereby resulting in a reduction in its required dosage treatment, which lightened the patients’ economic burden while improving primary and long-term kidney function by promoting the recovery of graft function and decreasing hepatic and renal toxicity. The co-administration of diltiazem may also reduce the rate of acute rejection, especially in patients who also receive the triple immunosuppressive regimen consisting of CsA, mycophenolate mofetil, and Pred.

Abstract:The impact of CYP3A5*3 , a CYP3A5 nonexpresser genotype, on inhibitory effects of diltiazem on tacrolimus metabolism has not been assessed. In retrospective study, when coadministered with diltiazem, mean increments in dose-adjusted C 0D7 , C max and AUC 0–12 h for tacrolimus were larger in CYP3A5 expressers than in CYP3A5 nonexpressers (48.7 vs 3.7%, 31.7 vs 17.2% and 38.2 vs 18.5%, respectively). Subsequently, a prospective study was carried out, patients were randomized to algorithm-predicted dosing or standard dosing. For CYP3A5 expressers, an algorithm guided by CYP3A5 and diltiazem significantly reduced tacrolimus maintenance dosage ( P =0.009) and improved the accuracy of tacrolimus initial dose, resulting in reduction in out-of-range C 0 after initial dose ( P =0.002) and dose adjustments ( P =0.004). However, for CYP3A5 nonexpressers, primary end points were not achieved, and tacrolimus-sparing effect of diltiazem was not remarkable. Our study results show that CYP3A5 genotype-guided tacrolimus–diltiazem combination is a promising therapy in renal transplant recipients in the early postoperative stage.
Diltiazem Augments the Influence of MDR1 Genotype Status on Cyclosporine Concentration in Chinese Patients with Renal Transplantation

Abstract:Co-administration of diltiazem can reduce the dosage of cyclosporine (CsA) in patients with renal transplantation. In this study, we investigated how diltiazem altered the relationship between MDR1 genetic polymorphisms and CsA concentration in Chinese patients with renal transplantation. A total of 126 renal transplant patients were enrolled. All the patients received CsA (2–4 mg·kg−1·d−1), and diltiazem (90 mg/d) was co-administered to 76 patients. MDR1-C1236T, G2677T/A, and C3435T polymorphisms were genotyped. The whole blood concentration was measured using the FPIA method, and the adjusted trough concentrations were compared among the groups with different genotypes. In all patients, MDR1-C1236T did not influence the adjusted CsA trough concentration. With regard to MDR1-3435, the adjusted CsA trough concentration was significantly higher in TT carriers than in CC and CT carriers when diltiazam was co-administered (58.83±13.95 versus 46.14±7.55 and 45.18±12.35 ng/mL per mg/kg, P=0.011), and the differences were not observed in patients without diltiazam co-administered. With regard to MDR1-2677, the adjusted CsA trough concentration was significantly higher in TT carriers than in GG and GT carriers when diltiazam was co-administered (61.31±12.93 versus 52.25±7.83 and 39.70±7.26 ng/mL per mg/kg, P=0.0001). The differences were also observed in patients without diltiazam co-administered (43.27±5.95 versus 35.22±7.55 and 29.54±5.35 ng/mL per mg/kg, P=0.001). The adjusted CsA trough blood concentration was significantly higher in haplotype T-T-T and haplotype T-T-C carriers than in non-carriers, regardless of diltiazem co-administered. MDR1 variants influence the adjusted CsA trough concentration in Chinese patients with renal transplant, and the influence more prominent when diltiazem is co-administered.

Abstract:FK506, a first-line immunosuppressant, is routinely administered orally and intravenously following heart transplantation. However, frequent administration can result in a substantial psychological burden to patients, resulting in non-adherence to medication. The purpose of our study is to overcome the disadvantages of systemic drug administration by developing a polymer-based delivery system that is tunable and biodegradable and that can release highly hydrophobic FK506 over extended periods to treat or prevent acute cardiac allograft rejection. Using an electrospinning method, long-acting microfibers were prepared, and FK506 appeared to be continuously released for up to 14 days based on the in vitro release profiles. After implanting the microfiber subcutaneously into the abdominals of transplanted rats, it was found that the infiltration of T cells and macrophages and the secretion of interleukin-2 (IL-2) and IL-1β were significantly reduced compared with those of the free FK506 groups. More importantly, the mean survival time (MST) of the PCL-FK506 group was significantly extended in comparison with that of untreated control recipients and free FK506 (MST of untreated control recipients, free FK506, and PCL-FK506 was 8, 26.1, and 37, respectively). In conclusion, we propose that this drug delivery approach would be suitable for developing long-lasting immunomodulatory agents that prolong cardiac graft survival safely and effectively.
Individualization of Tacrolimus Dosage Basing on Cytochrome P450 3A5 Polymorphism – a Prospective, Randomized, Controlled Study

Abstract:We investigated how cytochrome P450 (CYP) 3A5 polymorphism affects pharmacokinetics of tacrolimus and its interaction with diltiazem in Chinese kidney transplant recipients. Sixty-two CYP3A5 expressers and 58 non-expressers were, respectively, randomized to receive diltiazem supplement or not. Their pharmacokinetic profiles were acquired on 14th day, sixth month, and 18th month post-transplant and compared among groups. A dosing equation was fit based on above data with CYP3A5 genotype and diltiazem co-administration as variables. Then, necessary initial doses with or without diltiazem were calculated and used in 11 CYP3A5 expressers, respectively, when another 11 expressers received routine doses as control. Trough concentration was measured on the third-day post-transplant and patients failed to reach target range were presented in percentage. These two parameters were compared among three groups. Patients were followed up until June 2010, kidney function, biopsy-proved acute rejection, and other adverse events were monitored. Results showed that CYP3A5 expressers needed more tacrolimus to reach therapeutic concentration window and were more susceptible to diltiazem-induced concentration increase than CYP3A5 non-expressers. CYP3A5 polymorphism-guided dosing equation helped to determine appropriate initial doses of tacrolimus in individuals. In conclusion, CYP3A5 polymorphism profoundly influences pharmacokinetics of tacrolimus and helps to individualize tacrolimus dose.
Rapid and simultaneous determination of tacrolimus (FK506) and diltiazem in human whole blood by liquid chromatography–tandem mass spectrometry: Application to a clinical drug–drug interaction study

Abstract:Tacrolimus (FK506) is a potent immunosuppressant widely used for organ transplantation patients while diltiazem (DTZ), a calcium-channel inhibitor, is often used in renal transplantation patients to prevent post-transplant hypertension. However, DTZ has a significant pharmacokinetic interaction with FK506. In this study, a rapid and sensitive ammonium-adduct based liquid chromatography–tandem mass spectrometry (LC/MS/MS) method has been developed and validated for the simultaneous determination of FK506 and DTZ in human whole blood using ascomycin as the internal standard (IS). After extraction of the whole blood samples by ethyl acetate, FK506, DTZ and the IS were subjected to LC/MS/MS analysis using electro-spray positive-ion mode ionization (ESI+). Chromatographic separation was performed on a Hypersil BDS C18 column (50mm×2.1mm, i.d., 3μm). The MS/MS detection was conducted by monitoring the fragmentation of 821.7→768.9 (m/z) for FK506, 415.5→310.3 (m/z) for DTZ and 809.8→757.0 (m/z) for IS. The method had a chromatographic running time of approximately 2min and linear calibration curves over the concentrations of 0.5–200ng/mL for FK506 and 2–250ng/mL for DTZ. The recoveries of liquid–liquid extraction method were 58.3–62.6% for FK506 and 50.4–58.8% for DTZ. The lower limit of quantification (LLOQ) of the analytical method was 0.5ng/mL for FK506 and 2ng/mL for DTZ. The intra- and inter-day precision was less than 15% for all quality control samples at concentrations of 2, 10, and 50ng/mL for FK506 and 5, 25, and 100ng/mL for DTZ. The validated LC/MS/MS method has been successfully used to analyze the concentrations of FK506 and DTZ in whole blood samples from pharmacokinetic studies in renal transplanted patients.
Application of Tacrolimus in 862 Renal Transplantation Recipients

Abstract:Objective To explore the long-term clinical effectiveness and safety of tacrolimus (FK506) used in kidney transplant recipients. Methods A total of 862 kidney transplant recipients were divided into pediatric recipients (PR), adult recipients (ADR) and old recipients (ER) according to the age, and into diabetic recipients (DR) and non-diabetic recipients (NDR) according to the complications with or without diabetic mellitus (DM). Survival of patients and grafts, incidence of rejection and side-effects were compared. To those patients with DM, the influence of FK506 and CsA on glycometabolism was compared also. Results The incidence of chronic rejection in 3 and 5 years were 5.25 % and 12.5%, respectively. The incidence of abnormal liver function, nephrotoxicity, infection, nervous system disturbance and alopecia was 7.54 %, 5.33 %, 12.41 %, 15.89 % and 4.76 % respectively. To those recipients without DM, the incidence of glycometabolism disorder was 16. 7 %. To those patients with DM, the incidence of insulin therapy in FK506 and CsA group was 68. 3 % and 61.9 % in a month, 38. 4 % and 31. 7 % in a year respectively (P0. 05). The dose of insulin was 32.72 IU/d and 29.08 IU/d in a month, 13.46 IU/d and 12.29 IU/d (P0. 05) in a year, respectively. Conclusions FK506 was an effective and high safe basic immunosuppressant in long-term application in renal transplant recipients, especially suitable for children and old recipients. To those patients with DM, FK506 had almost the same influence on the glucose metabolism as the CsA.
Effect of CYP3A4*1G/3A5*3 gene polymorphism on concentration/dose ratios of diltiazem in kidney transplant patients

Abstract:Objective To investigate the impact of the CYP3A4*1G and CYP3 A5*3 polymorphisms on the concentration/dose ratios of dilti-azem in kidney transplant patients.Methods Forty -seven cases of kidney transplant recipients were recruited.CYP3A4 and CYP3A5 geno-types were determined by polymerase chain reaction -restriction fragment length polymorphism.Plasma trough concentration of diltiazem was meas-ured by HPLC.The effects of CYP3A4*1G and CYP3A5*3 gene poly-morphisms on the concentration/dose ratios of diltiazem were evaluated and compared.Results There were fourteen cases of CYP3A4*1*1 (29.79%) homozygous genotype , twenty six cases of CYP3A4*1*1G (55.32%) heterozygous genotype , and seven cases of CYP3A4*1G*1G (14.89%) homozygous genotype.Allele frequencies of CYP3A4*1 and *1G were 57.45%and 42.55%, respectively.There were six ca-ses with CYP3A5*1*1 (12.77%), twenty one cases with CYP3A5*1*3(44.68%), and twenty cases with CYP3A5*3*3(42.55%).Allele frequencies of CYP3A5*1 and *3 were 35.11%and 64.89%, respectively.CYP3A4*1G gene polymorphisms were significantly related to the concentration /dose of diltiazem in kidney transplant patients.The diltiazem blood trough concentration/dose in CYP3A4*1*1 recipients was 1.52 times higher than that in patients with CYP 3A4*1G allelic genes.There was significant difference in diltiazem blood trough concen-tration/dose ratios between recipients with CYP3A4*1*1 genotypes and CYP3A4*1G carriers(P<0.05).CYP3A5*3 gene polymorphisms had slight impacts on diltiazem blood trough concentration /dose ratios but no difference was found among the CYP3A5 genotypes.Conclusion The CYP3A4*1G gene polymorphism is closely related to the blood con-centration/dose ratios of diltiazem in kidney transplant patients.
Interaction between tacrolimus and calcium channel blockers based on CYP3A5 genotype in Chinese renal transplant recipients

Abstract:Objective: To investigate the effect of calcium channel blockers (CCBs) on tacrolimus blood concentrations in renal transplant recipients with different CYP3A5 genotypes. Methods: This retrospective cohort study included renal transplant recipients receiving tacrolimus-based immunosuppressive therapy with or without CCBs in combination. Patients were divided into combination and control groups based on whether or not they were combined with CCBs, and then further analyzed according to the type of CCBs (nifedipine/amlodipine/felodipine). Propensity score matching was conducted for the combination and the control groups using SPSS 22.0 software to reduce the impact of confounding factors. The effect of different CCBs on tacrolimus blood concentrations was evaluated, and subgroup analysis was performed according to the patients' CYP3A5 genotypes to explore the role of CYP3A5 genotypes in drug-drug interactions between tacrolimus and CCBs. Results: A total of 164 patients combined with CCBs were included in the combination groups. After propensity score matching, 83 patients with nifedipine were matched 1:1 with the control group, 63 patients with felodipine were matched 1:2 with 126 controls, and 18 patients with amlodipine were matched 1:3 with 54 controls. Compared with the controls, the three CCBs increased the dose-adjusted trough concentration (C0/D) levels of tacrolimus by 41.61%-45.57% (P < 0.001). For both CYP3A5 expressers (CYP3A5*1*1 or CYP3A5*1*3) and non-expressers (CYP3A5*3*3), there were significant differences in tacrolimus C0/D between patients using felodipine/nifedipine and those without CCBs (P < 0.001). However, among CYP3A5 non-expressers, C0/D values of tacrolimus were significantly higher in patients combined with amlodipine compared to the controls (P = 0.001), while for CYP3A5 expressers, the difference in tacrolimus C0/D values between patients with amlodipine and without was not statistically significant (P = 0.065). Conclusion: CCBs (felodipine/nifedipine/amlodipine) can affect tacrolimus blood concentration levels by inhibiting its metabolism. The CYP3A5 genotype may play a role in the drug interaction between tacrolimus and amlodipine. Therefore, genetic testing for tacrolimus and therapeutic drug monitoring are needed when renal transplant recipients are concurrently using CCBs.
The effects of diltiaze in renal transplantation patients treated with cyclosporine A

Abstract:Objective To investigate the clinical effects of diltiaze in renal transplantation patients treated with cyclosporine A (CsA). Methods 1529 renal transplant cases were randomly ~divi -ded into experimental group 1 receiving CsA, Aza, Pred and Diltiaze, experimental group 2 receiving CsA, MMF, Pred and diltiaze, and control group receiving CsA, Aza and Pred without diltiaze. The dosage and blood concentrations of CsA, the outcome of renal transplant, the incidence of acute rejection, and the hepatic and renal toxicity were observed in the experimental groups and control group.Results The dosage of CsA in experimental group 1 was less, while the blood concentrations of CsA was higher than in control group (P<~0.05 ,P<~0.01 ), The CsA dosage in experimental group was obviously lower than in control group (P<~0.01 ), but blood concentrations was close to those in control group (P>~0.05 ). The recovery time of the graft function was cut down to 4.7 days (experimental group 1) and 3.9 days (experimental group 2) respectively with the difference being significant between the experimental groups and control group (P<~0.05 ). The accidence of acute rejection in the two experimental groups was significantly lower than in control group (P<~0.05 ,P<~0.01 ). Conclusion The combined use of diltiaze with CsA in renal transplantation patients could obviously increase the blood concentrations of CsA, reduce the dosage of CsA. Simultaneously, it could promote the ~recovery of graft function and improve the effectiveness of transplantation.
Clinical Study on the Use of FK506 after Renal Transplantation

Abstract:Objective To investigate the therapeutic efficacy and side-effects of tacrolimus(FK506) used for renal transplantation.Methods sixty-two cases of renal transplantation recipients were randomly divided into two groups:FK506 group(n=24),receiving FK506,mycophenolat-mofetil(MMF) and prednisone(Pred) ;CsA group(n=38),receiving CsA,MMF and Pred.Results Up to date,24 months follow-up showed that acute transplantation renal rejection was found in 2 cases of the FK506 group and 9 cases of the CsA group respectively(P0.05).The differences in renal function(serum Cr) in the two groups the difference was not significant(P0.05),hepatic dysfunction occurred in 3 cases in the FK506 group and 14 cases in the CsA group(P0.05).Hypertension was found in 13 cases the FK506 group and 22 cases in the CsA group(P0.05).Hyperglycemia occurred in 4 cases in the FK506 group and 16 cases in the CsA group(P0.05).Conclusion FK506 is a potent immunosuppressive agent,it is effective and safe to substitute CsA by FK506 in renal transplantation recipients with acute transplantation renal rejection and hepatic dysfunction.That the dosage of FK506 should be adjusted according to its wholeblood trough levels is helpful for reducing the side effects.
Drug-drug interaction between diltiazem and tacrolimus in relation to CYP3A5 genotype status in Chinese pediatric patients with nephrotic range proteinuria: a retrospective study

Abstract:Background: Tacrolimus is widely used to treat pediatric nephrotic range proteinuria (NRP). Diltiazem, a CYP3A4/5 inhibitor, is often administered with tacrolimus, affecting its pharmacokinetic profile. The impact of this combination on tacrolimus exposure, particularly in CYP3A5*3 genetic polymorphism, remains unclear in pediatric NRP patients. This study aimed to evaluate the effects of diltiazem on tacrolimus pharmacokinetics, focusing on the CYP3A5*3 polymorphism. Methods: We conducted a retrospective clinical study involving pediatric NRP patients, divided into two groups: those receiving tacrolimus with diltiazem and those receiving tacrolimus alone. Propensity score matching (PSM) was used to balance the baseline characteristics between the groups. We compared daily dose-adjusted trough concentrations (C0/D) of tacrolimus in both the original and PSM cohorts. The influence of diltiazem on tacrolimus C0/D, stratified by CYP3A5*3 genetic polymorphism, was assessed in a self-controlled case series study. Results: Before PSM, the tacrolimus C0/D in patients taking diltiazem was significantly higher compared to those with tacrolimus alone (75.84 vs. 56.86 ng/mL per mg/kg, P = 0.034). This finding persisted after PSM (75.84 vs. 46.93 ng/mL per mg/kg, P= 0.028). In the self-controlled case study, tacrolimus C0/D elevated about twofold (75.84 vs. 34.76 ng/mL per mg/kg, P < 0.001) after diltiazem administration. CYP3A5 expressers (CYP3A5*1/*1 and *1/*3) and CYP3A5 non-expressers (CYP3A5*3/*3) experienced a 1.8-fold and 1.3-fold increase in tacrolimus C0/D when combined with diltiazem, respectively. Conclusion: Diltiazem significantly increased tacrolimus C0/D, with CYP3A5*3 expressers showing higher elevations than non-expressers among pediatric NRP patients. These findings highlight the importance of personalized tacrolimus therapy based on CYP3A5*3 genotypes in pediatric patients taking diltiazem.

Abstract:Objective Objective To investigate the effects of diltiazem on renal function in patients who underwent cardiopulmonary bypass. Methods Forty cases of ASA grade Ⅱ~Ⅲ undergoing replacement of valve under cardiopulmonary bypass were divided into two groups:Treated group(n=20)receiving a continuous intravenous infusion of diltiazem at 2 μg/(kg·min) from the beginning of CPB and continue for 1.5 hours.Control group(n=20)received no diltiazem.Cr clear ratio(CCR),urine-α_1-microglobulin,urine-β_2-microglobulin were measured before anesthesia,the duration of CPB,4 hours after CPB and 12 hours after CPB.Results ①Before anesthesia there is no remarkable difference between two groups in CCR,u-α_1-MG and u-β_2-MG(P>0.05).②In the end of CPB,4 hours after CPB,the level of CCR obviously decreased in the two groups respectively(P<0.05),and it was obviously lower in Control group than Treatment group(P<(0.05)),but it came back to the base line until 12 hours after CPB in two groups(P>0.05).③In the duration of CPB,4 hours after CPB,12 hours after CPB,the levels of u-α_1-MG and u-β_2-MG obviously increased in the two groups respectively(P<0.05),and it was obviously lower in Treatment group than Control group(P<0.05).Conclusion Cardiopulmonary bypass have a negative effect on renal function.Diltiazem has protection on renal function in patients who undergoing cardiopulmonary bypass.
