Abstract:Objective to detect serum TNF-α,IL-18,ADPN in Patients with type 2 diabetic nephropathy.Methods 50 patients with T2DM were randomly selected,based on urinary protein excretion rate into diabetes without nephropathy(DN group) 25 cases;diabetic nephropathy(NDN group) 25 cases.And select the healthy control group(CTL group) 25 cases.Unites the immunity adsorption law determination with the enzyme the trying blood serum TNF-α,IL-18,the ADPN density.Results(1)in diabetic group,serum TNF-α,IL-18 concentrations higher than the CTL group,DN group was significantly higher than the concentration of serum ADPN CTL group,NDN Serum ADPN concentrations lower than CTL group.(2)serum TNF-α,IL-18,ADPN levels were significantly higher than in the DN group NDN group,the difference was statistically significant.(3) linear correlation analysis showed that TNF-α and FBG,HbAIC,UAER was positively correlated;IL-18 and FBG,HbAIC,UAER was significantly positively correlated;ADPN and UAER was significantly positively correlated,ADPN and BMI,FBG,HbA1C was negatively related.Conclusion The blood serum TNF-α,IL-18,the ADPN height and diabetes nephrosis's pathological change stage is closely related.Serum TNF-α,IL-18,ADPN levels can determine the type 2 diabetic nephropathy as the extent of the damage and the prognosis.
Clinical significance of plasma CD146 and P-selectin in patients with type 2 diabetic nephropathy.

Abstract:To investigate the levels of plasma CD146 and P-selectin in patients with type 2 diabetic nephropathy at different stages.A total of 80 patients with type 2 diabetes mellitus were enrolled in the present study. According to 24h urinary albumin excretion ratio and renal function, they were further divided into group of diabetes without microalbuminuria (DN0, n=20), microalbuminuria group (DN1, n=20), macroalbuminuria group (DN2, n=20) and renal insufficiency group (DN3, n=20). Another 20 healthy subjects were enrolled as control group (non-DM). Plasma CD146 and P-selectin were measured by ELISA.Plasma CD146 and P-selectin were significantly increased in patients with type 2 diabetes with microalbuminuria (DN1) compared with health control (CD146: 415.3±29.0 vs. 243.5±14.7 ng/ml, P<0.05; P-selectin: 66.8±3.4 vs. 45.3±2.7 ng/ml, P<0.001). With the development of diabetic nephropathy, both plasma CD146 and P-selectin level progressively rise, with the highest levels in patients with significant renal insufficiency (DN3: 515.9±36.9 and 81.5±5.1 ng/ml respectively, P<0.001). Moreover, the increase in CD146 is positively co-related to the rise of P-selectin in patients with type 2 diabetes.Expression of CD146 and P-selectin in patients with type 2 diabetes is elevated, and they are positively correlated with severity of diabetic nephropathy.

Abstract:Objective To investigate the clinical significance of CD146 and P-selectin in peripheral blood in patients with type 2 diabetic nephrophathy. Methods A total of 80 patients with type 2 diabetes nephrophathy were randomized into the diabetes group (DN0 group), micro-albuminuria group (DN1 group), Macro-albuminuria uria group (DN2 group) and renal insufficiency group (DN3 group), with 20 cases in each group. Twenty healthy individuals were enrolled as the control group (Non-DM group). ELISA was used to detect the expression of CD146 and P-selectin. Results The levels of CD146 and P-selectin in DN0 group was significantly higher than those in Non-DM group [(319.6±15.9) ng/ml and (57.9±3.1) ng/ml vs (243.5±14.7) ng/ml and (45.3±2.7) ng/ml, respectively], P<0.05. In DN1 group, DN2 group and DN3 group, the levels of CD146 and P-selectin was significantly higher than those in DN0 group (P<0.05). The levels of CD146 in the three groups were (415.3±29.0) ng/ml, (486.6±34.3) ng/ml and (515.9±36.9) ng/ml, respectively, and the levels of P-selectin were (66.8±3.4) ng/ml, (79.6±4.6) ng/ml and (81.5 ± 5.1) ng/ml, respectively. Conclusion Expression of CD146 and P-selectin in patients with type 2 diabetic nephrophathy is elevated with the progression of diabetic nephropathy.
The effect of P-seletin on the pathogenesis of type 1 diabetic nephropathy in rats

Abstract:Objective To investigate the effect of P-selectin on the pathogenesis of diabetic nephropathy (DN) and the renoprotective action of fragmin,a low molecular weight haperin. Methods The murine model of type 1 diabetes was established by intraperitoneal injections of streptozotocin. Rats were divided into normal control group (N group),group with diebetic nephropathy (DN group),high-dose fragmin group (T1 group),and low-dose fragmin group (T2 group). The body weight,24-hour urinary protein and albumin excretion,and serum creatinine (Scr) were measured before and 4,8,and 12 weeks after the use of fragmin. Levels of P-selectin expression were detected by ELISA,immunohistochemistry,and real-time PCR. Results As compared with N group,the body weight,kidney / body weight (KW / BW) ratio,24-hour urinary protein and albumin excretion,and Scr were markedly increased (P < 0.05),and both P-selectin mRNA and protein expression in the renal tissue were significantly enhanced P< 0.05,respectively) in DN group. The KW / BW ratio was declined in T1 group,and 24-hour urinary protein and albumin excretion,Scr,and P-selectin mRNA expression were evidently decreased in both T1 and T2 group (P< 0.01 for all comparisons),as compared to DN group. Conclusions P-selectin may be involved in the pathogenesis of diabetic nephropathy. Fragmin may play a renoprotective role partially through down-regulating the expression of P-selectin.
Expression of P-selectin in peripheral blood of patients with systemic lupus erythematosus and its clinical significance

Abstract:Objective To investigate the expression of peripheral blood P-selectin in patients with systemic lupus erythematosus(SLE)and its clinical significance.Methods Forty-three SLE patients and20controls were enrolled.Flow-cytometric assay was employed for detection of P-selectin.Correlations between P-selectin and SLE disease activity and renal damage were analyzed.Results Significant higher P-selectin levels were detected in patients of SLE than that in control group(P0.05).There was no difference between the expression of P-selectin in active and stable SLE patients(P0.05).The levels of P-selectin in patients with renal damage were markedly higher(P0.05)than those without renal damage.No correlations were obˉserved between P-selectin levels and ESR,ANA titers,serum C3level and SLE-disease activity index(SLEDAI)in SLE patients(P0.05).However,positive correlation was found between P-selectin levels and24hr urinary protein excretion(r=0.411,P0.05).Conclusion P-selectin might be involved in the pathogenesis of SLE,and the level of P-selectin might be used as an indicator of renal damage.
Significance of Serum Adiponectin in Type 2 Diabetic Patients with Nephropathy

Abstract:Objective:To measure the serum level of adiponectin(ADPN) and soluble vascular cell adhesion molecule(sVCAM-1),an endothelial cell injury marker,and to evaluate the relationship of ADPN and sVCAM-1 in type 2 diabetic patients with nephropathy.Methodology:A total of fifty-three type 2 diabetic patients with nrphropathy and 10 control(NC) subjects were enrolled in this study.They were matched for body mass index(BMI),age and sex.According to the progression of nephropathy,these patients were divided into three groups:DM(microalbuminuria 30 mg/d,serum creatinine147.6 μmol/L);DN1(albuminuria 30 mg/d,serum creatinine 147.6 μmol/L);DN2(macroalbuminuria,albuminuria 300 mg/d,serum creatinine 147.6 μmol/L).The renal function was estimated with the MDRD formula.The levels of serum ADPN and sVCAM-1 were measured by enzyme-linked immunosorbent assay(ELISA).Results:The levels of serum ADPN were relatively lower in group DM than that in group NC,and significantly higher in the advanced stages of diabetic nephropathy [NC(6.58±3.10) mg/L,DM(4.97±2.89) mg/L,DN1(10.0±9.0) mg/L,DN2(15.82±6.53) mg/L,P 0.01].The levels of serum sVCAM-1 were steadily increased with the progression of DN,and that in the group DN2 was significantly increased compared with that in the group NC(P 0.05).Serum ADPN positively correlated with serum creatinine(r=0.296,P0.05),uric acid(r=0.313,P0.05) and negatively correlated with eGFR(r=-0.320,P0.05).In stepwise multiple regression analysis adjusted for established risk factors,serum creatinine was selected to be an independent determinants of serum ADPN levels(β=3.130,P0.01).Interestingly,there was a positive association between serum ADPN and sVCAM-1 level in all tested samples(r=0.331,P0.01) and in diabetic patients(r=0.316,P0.05).Conclusion:The high level of serum ADPN in DN may be associated with renal insufficiency,which might be involved in the endothelial dysfunction.
Clinical Significance of Determination of Serum Cystatin C,lipoprotein (α) and High Sensitivity C-reactive Protein in Type 2 Diabetic Patients with Early Renal Impairment.

Abstract:Objective To study the clinical significance of measurement of serum cystatin C(Cysc),lipoprotein α(LPα) and high sensitivity C-reactive protein(hs-CRP) in type 2 diabetic mellitus(T2DM)patients with early renal impairment.Methods One hundred and twenty six patients were divided into three groups: 40 cases in simple diabetic group(DM),44 cases in early diabetic nephropathy group(EDN) and 42 cases in clinical diabetic nephropathy group(DN),and 70 healthy persons were chosen as normal control group.Serum levels of Cysc,LP(α) and hs-CRP were detected by BECKMAN UniCel Dxc600 automatic biochemistry analyzer.Results Serum levels of Cysc,LP(α)and hs-CRP in T2DM patients were higher than those of normal control group(P0.01),they were also significantly higher in DN group than in DM group and EDN group(P0.01).The serum levels of Cysc,LP(α)and hs-CRP were positively correlated with urinary mALB in T2DM patients.Conclusion Serum levels of Cysc,LP(α)and hs-CRP are sensitive indicators for detection of early renal impairment in patients with type 2 diabetes mellitus.

Abstract:Objective To explore the significance of CD146 in patients with type 2 diabetes mellitus.Methods The plasma levels of CD146 were detected by enzyme linked immunosorbent assay(ELISA) in 25 type 2 diabetes mellitus patients and 10 normal controls.Results The plasma levels of CD146 in diabetes mellitus patients(464.86±134.55ng/ml) were significantly higher than that of control group(268.85±70.90ng/ml)(P0.01).Conclusion The type 2 diabetes mellitus patients had highly-expressed CD146 in blood.
Expression of P-selectin in cerebrovascular endothelial cells of rats with diabetes

Abstract:Objective To explore the mechanisms of cerebral microvascular disorders in rats with diabetes.Methods Diabetes was induced by injecting 1% streptozocin (STZ). Twenty-four Sprague-Dawley rats were randomly assigned into three groups: control group normal rats, DM8 group with an 8-weeks course of diabetes and DM24 group with 24-weeks course of diabetes (n=8 each). The expression of P-selectin (Ps) in cerebral microvascular and plasm C-reactive protein (CRP) were detected.Results Ps was not expressed in the control group. Ps positive endothelial cells were mostly expressed in microvessels with a diameter of 7.5-30 μm in rats with diabetes. The expression of Ps was significant of higher in the two DM groups than that in the control group (P<0.01). There were significant differences in the Ps expression between the DM24 and DM8 groups (P<0.05). Plasm CRP concentration increased in the DM8 and DM24 groups compared with that in the control group (P<0.01). A significant difference was also observed in the CRP concentration between the DM8 and DM24 groups (P<0.005).Conclusions The increased expression of Ps in cerebral microvascular may be related to chronic low grade inflamemation, which may be one of the mechanisms of cerebral microvascular disorders in diabetes rats.
Effects of Fragmin on Expression of P-selectin in Serum and Renal of Diabetic Nephropathy Rats

Abstract:Objective:To investigate the role of P-selectin in the progress of diabetic nephropathy(DN),and to explore the renal protection mechanism of Fragmin(low molecular weight haperin) against DN.Methods:The SD rats by intraperitoneal injection of streptozotocin(STZ) were divided into normal control group(N group),diebetic nephropathy group(DN group) and treatment group(T group).At week 4,8,and the week 12 with Fragmin,body weight,24-hour urinary protein and albuminuria,serum creatinine(Scr) were measured,and renal tissue were also observed.Additionally,Lipid level and function of blood coagulation were measured before final dissection of the rats at the 12th week.The expression level of P-selectin was examined by ELISA,immunohistochemistry and real-time PCR.Results:Body weight,kidney/body weight rario (KW/BW),24-hour albuminuria and urinary protein,Scr of DN group were higher than those of the controlled (P<0.05).P-selectin level and protein expression in serum and kidney tissue of DN and T group were all significantly higher than those of N group (P<0.05).P-selectin mRNA expression was also obviously higher than the controlled (P<0.01).Body weights,KW/BW,albuminuria,urinary protein of T group rats were lower than those of DN group.P-selectin mRNA expression of T group was also lower than those of DN group (P<0.01).Conclusion:P-selectin may play an important role in the progression of diabetic nephropathy.Fragmin has renal protective effect in diabetic rats, partially through down-regulating the expression of P-selectin.
Changes and Significance of NF-KB and MCP-1 Express on Peripheral Blood Mononuclear Cells in Patients with Diabetic Nephropathy

Abstract:Objective To approach the significance of nuclear factor-kappaB(NF-κB) and monocyte chemoattractant protein-1(MCP-1) by comparing the levels of NF-κB and MCP-1 protein expressions on peripheral blood mononuclear cells in patinets with diabetic nephropathy.MethodTotal 20 normal healthy examinees(control group) and 52 type 2 diabetics were recruited into the study.The latter were divided into 3 groups according to the urinary albumin excretion rate(UAER):normoalbuminuria group(UAER20 μg/min,groupA=27), microalbuminuria group(20 μg/min≤UAER 200 μg/min, groupB=15), macroalbuminuria group(UAER≥200μg/min, groupC=10).Results The expressions of NF-κB and MCP-1 on peripheral blood mononuclear cells in T2DM were significantly higher than those in controls(P 0.01), which in the group B were obviously higher than those in the group A(P 0.05) and increased further in the groupC(P 0.05).Likewise, the group C was higher than the group B(P 0.05).The relative analysis showed that the NF-κB and MCP-1 were positively related to UAER, serum creatinine and course of disease(r =0.765,r =0.778,r =0.461;r =0.736, r =0.680, r =0.434.P all0.01).Conclusion Increased expressions of NF-κB and MCP-1 might take an important part in the progression of diabetic nephropathy.
Association between Lipoprotein(a) and diabetic nephropathy in patients with type 2 diabetes

Abstract:Background: Diabetic nephropathy (DN) is one of the most prevalent and severe microvascular complications of type 2 diabetes (T2DM). However, little is currently known about the pathogenesis and its associated risk factors in DN. The present study aims to investigate the potential risk factors of DN in patients with T2DM. Methods: A total of 6,993 T2DM patients, including 5,089 participants with DN and 1,904 without DN, were included in this cross-sectional study. Comparisons between the two groups (DN vs. non-DN) were carried out using Student's t-test, Mann-Whitney U-test, or Pearson's Chi-squared test. Spearman's correlation analyses were performed to assess the correlations of serum lipids and indicators of renal impairment. Logistic regression models were applied to assess the relationship between blood lipid indices and the presence of DN. Results: T2DM patients with DN were older, and had a longer duration of diagnosed diabetes compared to those without DN. Of note, the DN patients also more likely develop metabolic disorders. Among all serum lipids, Lipoprotein(a) [Lp(a)] was the most significantly correlated indicators of renal impairment. Moreover, univariate logistic regression showed that elevated Lp(a) level was associated with an increased risk of DN. After adjusted for confounding factors, including age, gender, duration of T2DM, BMI, SBP, DBP and lipid-lowering drugs usage, Lp(a) level was independently positively associated with the risk of DN [odds ratio (OR):1.115, 95% confidence interval (CI): 1.079-1.151, P =6.06×10 -11 ]. Conclusions: Overall, we demonstrated that serum Lp(a) level was significantly positively associated with an increased risk of DN, indicating that Lp(a) may have the potential as a promising target for the diagnosis and treatment of diabetic nephropathy.

Abstract:Abstract Background and Aims Diabetic nephropathy (DN) is the leading cause of end-stage renal disease in the developed world. Podocyte injury has been shown to be a critical cellular event in the progression of DN. Our previous studies demonstrated that platelet-derived microparticles (PMPs) mediated endothelial injury in diabetic rats. Therefore, this study aimed to investigate whether the PMPs deposit in podocytes and its potential effects on podocyte injury in DN. Method Deposition of PMPs in podocytes was checked by immunofluorescent staining and electron microscopy. Renal pathological change and ultra-microstructure were respectively checked by periodic acid-schiff staining and electron microscopy. The protein expression of inflammatory cytokines and extracellular matrix was measured by immunohistochemical staining and Western blot. Results The PMPs were found to be widely deposited in podocytes of glomeruli in diabetic patients and animal models and closely associated with progression of DN. Interestingly, aspirin treatment significantly inhibited accumulation of PMPs in glomeruli of diabetic rats, accompanied with alleviated mesangial matrix expansion, fusion of foot processes, decreased protein expression of inflammatory cytokines and extracellular matrix secretion. The in vitro study further confirmed the deposition of PMPs in podocytes. Moreover, PMPs stimulation induced phenotype transition of podocytes through decreased protein expression of podocin and increased protein expression of α-SMA and fibronectin, which was correlated with increased production of inflammatory cytokines. Conclusion Our findings for the first time demonstrated that deposition of PMPs in podocytes contributed to the development of DN.

The Association of Single Nucleotide P-selectin Gene Polymorphism with IgA Nephropathy

Abstract:OBJECTIVE:IgA nephropathy is one of the most common form of primary glomerulonephritis throughout the world and a main renal disease which causes renal failure. P-selectin plays an important role in the pathogenesis and development of IgA nephropathy. The purpose of this study is to find a possible relationship between P-selectin gene polymorphism and IgA nephropathy.METHODS:In this study, a comprehensive P-selectin gene survey, including entire coding region, part of regulatory region and exon-intron connection region, was performed with PCR-direct sequencing and 16 single nucleotide polymorphism (SNPs) were detected. Then 210 renal-biopsy proven sporadic IgA nephropathy cases of Chinese Han nationality and 103 normal volunteers were recruited. 9 candidate SNPs (frequency > 5%) in P-selectin gene of both the case and control groups were genotyped with direct sequencing and a case-control association study was carried out.RESULTS:It was revealed that the SNP of -825A/G in promoter region of P-selectin gene was significantly associated with IgA nephropathy. Both the frequency of AG/GG genotype (21.9% vs 34.9%, P < 0.05) and G allele (11.4% vs 18.4%, P < 0.05) were less in the IgA nephropathy cases than in the healthy controls. Those carrying the low frequency G allele (AG/GG genotype) had a protective effect on the morbidity of IgA nephropathy and the odds ratio for the IgA nephropathy patients versus controls was 0.522 with AG/GG genotype versus AA genotype. Within the IgA nephropathy group, patients with AA genotype tended to present higher level of serum creatinine and soluble P-selectin than those carrying at least one G allele (AG/GG genotype) (P < 0.05).CONCLUSION:The SNP of -825A/G in the promoter region of P-selectin gene was significantly associated with IgA nephropathy susceptibility and patient's renal function, thus suggesting the genetic effect of P-selectin gene in the pathogenesis of IgA nephropathy.
Diagnostic Significance of Serum Levels of Nerve Growth Factor and Brain Derived Neurotrophic Factor in Diabetic Peripheral Neuropathy.

Abstract:Background: Our study aimed to explore the levels of nerve growth factor (NGF) and brain derived neurotrophic factor (BDNF) in healthy participants, type 2 diabetes mellitus (T2DM) patients, and diabetic peripheral neuropathy (DPN) patients in order to find their effects on DPN. Material/Methods: The clinical data of 110 healthy participants (age: 57.3 +/- 8.2 year, height: 165.4 +/- 5.5 cm, weight: 64.1 +/- 7.5 kg), 83 T2DM patients (age: 56.5 +/- 7.9 year, height: 164.8 +/- 6.2 cm, and weight: 63.6 +/- 6.6 kg), and 65 DPN patients (age: 58.2 +/- 7.3 year, height: 166.7 +/- 6.7 cm, weight: 63.1 +/- 5.8 kg) were observed. ELISA was applied to detect serum NGF and BDNF levels. Receiver operating characteristic (ROC) curve analysis was performed to evaluate diagnostic value of serum NGF and BDNF levels in DPN. Logistic regression analysis was performed to analyze risk factors for DPN. Results: Serum NGF and BDNF levels decreased most in DPN patients. Subsequently, we determined that serum NGF and BDNF levels were correlated with: the course of disease for patients, fasting C-peptide (FCP), 2-hour postprandial C-peptide level (2-h PCP), glycosylated hemoglobin level (HbAlc), and 24-hour urinary microalbumin excretion (24-h UME). ROC curve analysis identified high sensitivity, specificity, and accuracy of NGF and BDNF levels on DPN. Serum levels of NGF and BDNF, course of disease, 2-h PCP level, and postprandial blood glucose level were determined to be risk factors for DPN. Conclusions: Our study highlights that serum levels of NGF and BDNF might be associated with the occurrence and development of DPN.

Abstract:Objective To study the relationship between diabetic nephropathy(DN) and impaired lipid metabolism.Methods Eighty-six patients with type 1 diabetes mellitus(DM) were divided into two groups according to the Mogensen criterion of DN : groupA,65 DN patients who were divided into 3 subgroups according to levels of urinary album excretion rate(UAER) and creatinine,and group B,21 nonDN DM patients.Plasma total lipid,apolipoprotein and GHbA_(1c) were tested in all of them,and compared with control group.Results Levels of plasma triglycerides(TG) and low density lipoprotein cholesterol(LDL-C) in group B were high compared with control group(P< 0.05 or P< 0.01).Differences of level of plasma lipid and apolipoprotein among the 3 subgroups in group A were statistically significant different(P< 0.01),while levels of plasma TG,LDL and apolipoprotein B were positively correlated with UAER and plasma glucose(P< 0.01).Conclusion Impaired lipid metabolism is obvious in DN patients,which is correlated with bad controlled plasma glucose and degree of impaired kidney function.Hyperlipidemia accelerates the occurrence and development of DN,while hyperglycaemia aggravates impaired lipid metabolism,and a vicious circle is established.
Association of serum unsaturated fatty acid patterns with the risk of diabetic nephropathy

Abstract:Introduction: Unsaturated fatty acids play an essential role in the progression of diabetic nephropathy (DN). Previous studies were mainly focused on the role of individual unsaturated fatty acid. The serum unsaturated fatty acid patterns in patients with DN remains to be determined. Methods: A total of 135 patients with DN (DN group) and 322 patients with type II diabetes without nephropathy (non-DN group) were included in this study. Clinical data, serum levels of unsaturated fatty acids and other laboratory indicators were collected. Multivariate logistic regression was applied to identify risk factors for serum unsaturated fatty acids level in both groups. Serum unsaturated fatty acids were subjected to factor analysis to identify distinct fatty acid patterns (FAPs). Multivariable logistic regression was employed to assess the risk of DN associated with different serum FAPs. The role of FAPs major components was further validated at the cellular level. Results: After adjusting for confounders, three types of unsaturated fatty acid including C20:5 (eicosapentaenoic acid, EPA), C22:6 (docosahexaenoic, DHA) and C22:5 n-3 (docosapentaenoic acid, DPA n-3) were significantly associated with DN in the population. The odds ratios (ORs) (95% confidence interval [CI]) of DN were 0.583 (0.374, 0.908), 0.826 (0.716, 0.954) and 0.513 (0.298, 0.883), respectively. Factor analysis revealed five major FAPs, among of which, only FAP2 (enriched with EPA and DHA) exhibited a significant inverse association with DN. In the multivariate-adjusted model, the OR (95% CI) was 0.678 (0.493, 0.933). At the cellular level, DHA and EPA enriched in FAP2 reduced and a combination of which further decreased extracellular matrix production induced by transforming growth factor beta 1 (TGFβ1) in podocytes and tubular cells. Conclusions: Our findings suggest that FAP2, enriched with DHA and EPA, is associated with a reduced risk of DN. This highlights the potential of targeting FAP2 for the patients with DN.

Abstract:Purpose To discuss the expression and significance of CD62p in patients with type 2 diabetes mellitus combined with arteriosclerosis obliterans of lower extremity. Methods Seventy-six patients were assigned into two groups by with or without type 2 diabetes mellitus.CD62P,CD63,CD61,CD41a and CD42b in two groups were detected and the results were compared. Results CD62p,CD63p in type 2 diabetes mellitus(22.92±12.08,45.59±16.48)were higher than those in normal group(10.90±3.92,27.38±11.05,P0.01),other indexes had no difference. Conclusions The detection of CD62p and CD63p can be used to evaluate the function state in type 2 diabetes mellitus combined with arteriosclerosis obliterans of lower extremity.
Investigation of Serum Leptin and Adiponectin Levels in Type 2 Diabetic Patients With Nephropathy

Abstract:Objective To investigate the serum leptin and adiponectin levels in type 2 diabetic patients with nephropathy(DN), and to provide the basis for understanding the pathogenesis process of DN. Methods This study included 87 patients with type 2 diabetes mellitus and 30 healthy subjects, and the diabetic patients were further divided into 3 subgroups according to their urinary album in excretion rate(UAER): 32 patients with normal UAER(DM group), 28 patients with micro-albuminuia (Pre-DN group) and 26 patients with clinical proteinuria (clinical DN group). Serum leptin and adiponectin levels were also measured by RIA. Results All of the 4 groups were matched in age, sex and BMI (P>0.05). Among these 4 groups, the levels of serum leptin and adiponectin were the highest in clinical DN group (15.29±4.03μg/L and 11.32±2.13 mg/L, P<0.001); the levels of serum leptin and adiponectin in DM group, Pre-DN group and healthy subjects group were 7.12±3.01μg/L and 5.81±1.03 mg/L, 11.23±3.31μg/L and 6.31±1.99 mg/L, 6.12±2.31μg/L and 4.25±1.62 mg/L respectively; and the levels of serum leptin and adiponectin in the Pre-DN group were significantly higher than those of DM group and healthy subjects group. Correlative analyses revealed a positive correlation between serum leptin and adiponectin levels and UAER in type 2 diabetic patients with nephropathy (r=0.515 and r=0.453, P<0.05). Conclusion The diabetic nephropathy patients have higher serum leptin and adiponectin levels, which correlate with the degree of diabetic nephropathy.
