Abstract:Bone is a preferential site for prostate cancer (PCa) metastasis. However, sites of synchronous distant metastases in PCa patients with bone metastases at initial diagnosis and their impacts on prognosis are still unclear, limiting our ability to better stratify and treat the patients. In this study, we examined the sites of synchronous extra-skeletal metastases in de novo PCa patients with bone metastases and their associated prognoses. In total, 16,643 de novo PCa patients with bone metastases from the SEER database were included. After stratification of metastatic sites (bone, lung, liver, and brain) and treatment modalities, overall survival (OS) and independent predictors of OS, were analyzed. Lung was the most frequent site of synchronous metastases, followed by liver, while brain metastases were relatively uncommon. Patients with bone-only metastases showed the longest mean survival time (35.87 months, p < 0.001), followed by patients with bone and lung metastases (30.74 months, p < 0.001). Patients with bone and liver metastases had the shortest mean survival time (17.39 months, p < 0.001). Age > 70 years, unmarried status, high tumor grade, prostate-specific antigen (PSA) > 50 ng/ml, and Gleason score ≥ 8 were associated with poor OS (all p < 0.01). Asian or Pacific Islander ethnic background was associated with a favorable OS (all p < 0.01). Chemotherapy improved OS in patients without brain metastases (all p < 0.05). For patients with bone-only metastases, radical prostatectomy (RP) (HR, 0.339; 95% CI 0.231–0.495; p < 0.001), brachytherapy (BT) (HR, 0.567; 95% CI 0.388–0.829; p = 0.003), and chemotherapy (HR, 0.850; 95% CI 0.781–0.924; p < 0.001) were associated with prolonged OS. Age, race, tumor grade, PSA, Gleason score, sites of synchronous extra-skeletal metastases, as well as treatment modalities affected OS in newly diagnosed PCa patients with bone metastases. Synchronous liver metastases were associated with poor OS. Chemotherapy improved OS in patients without brain metastases. RP and BT improved OS in patients with bone-only metastases. Further investigation is warranted to validate these findings.
Clinical Analysis of Prostate Cancer with Bone Metastasis: A Report of 63 Cases

Abstract:Objective: To summarize the diagnosis and treatment of 63 cases of advanced stage prostate cancer with osseous metastasis. Methods: Palliative prostatectomy or endocrine therapy was performed for the 63 cases with osseous metastasis of prostate cancer, among which 1 received radical prostatectomy after neoadjuvant therapy. Results: Fifty- two cases were followed up for 6 to 72 months (averaging 30 months). During that time, 15 patients died of metastasis of the prostate cancer within 12 to 48 months (averaging 21 months) after treatment and 1 patient died of cerebrovascular accident 6 months after treatment. The other 36 surviving cases received follow up for a minimum of 12 months (averaging 34 months), and all survived asymptomatically. The PSA response rate reached 100% 6 months after treatment. Conclusions: The early curative effect of endocrine therapy for cases with osseous metastasis of prostate cancer is obvious, and palliative prostatectomy can improve the quality of life for the patients with symptoms of obstruction.
Prognosis of Prostate Cancer and Bone Metastasis Pattern of Patients: a SEER-based Study and a Local Hospital Based Study from China

Abstract:Prostate cancer (PCa) is the leading cause of cancer-related death among men worldwide. Knowledge of the prognostic factors of PCa and the bone metastasis pattern of patients would be helpful for patients and doctors. The data of 177,255 patients with prostate cancer diagnosed between 2010 and 2013 with at least five years of follow-up were retrieved from the Surveillance, Epidemiology, and End Results (SEER) database. Multivariate Cox regression analysis was used to determine the predictive value of patients’ characteristics for survival after adjusting for other variates. Multivariate logistic regression analysis was used to evaluate the odds ratio of bone metastasis in PCa patients. The predictive value of age, race, marital status, and tumor characteristics were compared. The survival of patients with different socioeconomic statuses and bone metastasis statuses was compared by Kaplan–Meier analysis. A total of 1,335 patients with prostate cancer diagnosed between 2009 and 2015 were enrolled from the Second Affiliated Hospital of Zhejiang University School of Medicine. The survival of patients with different prostate-specific antigen (PSA) levels, Gleason scores, marital statuses and bone metastasis statuses was compared by Kaplan-Meier analysis. In SEER database, 96.74% of patients were 50 years of age or older. Multivariate Cox analysis revealed that for PCa patients, age at presentation, older age, single marital status, lower socioeconomic status, higher PSA level, T1 and N0 stage, and bone metastasis were independent risk factors for increased mortality. Multivariate logistic regression analysis revealed that patients who were married, were living in urban areas, had lower PSA levels, underwent surgery, and radiation had lower OR factors for bone metastasis. Asian or Pacific Islander, better socioeconomic status, lived in urban areas, married marital status, lower PSA levels and lower Gleason scores were better prognostic factors in PCa. Additionally, patients with single or divorced marital status, who were living in rural places had higher PSA levels, and T1 and N0 stages have a high OR for bone metastasis.

Abstract:Objective To analyze the clinical characteristics of prostate cancer with bone pain as the first clinical manifestation retrospectively.Methods During the time period from 2003 to 2010,a total of 51 patients with newly diagnosed,histologically confirmed prostate cancer,histological diagnosis was carried through the transrectal ultrasonography guided biopsy and carried on Gleason score assessment.Metastases were confirmed by positive bone scintigraphy with 555-925 MBq 99mTc-MDP using Emission Computed Tomography,some of which were identified by pathology of excision of metastatic bone tumor or bone marrow aspiration,all of those patients had been treated by androgen deprivation treatment,analyse interactions between PSA,Gleason score,ALP and bone metastasis,discuss the therapeutic effectiveness of hormonal therapy in relieving bone pain.Results The age of the patients ranged from 55 to 85 years,with the mean age of(72.5±8.5)years.The metastasis of pelvis,lumbar,cervical vertebrae and thoracic vertebra was 21.5%,29.0%,3.7% and 15.0% respectively.The initial serum PSA levels>20 ng/ml was found in 44 patients(86.2%)with bone metastasis.39 patients(76.5%)presented with digital rectal examination positive and 33 patients(64.7%)with clinical stages≥T3 and 22 patients(43.1%)with high GS(8-10).80% of patients obtained the relief of bone pain.1,3,5-year survival rate were 88.7%,67.8%,54.1% respectively.Conclusions Medium-elderly men with bone pain as initial clinical manifestations should take prostate cancer into account,ECT should be considered when patients of prostate cancer were in accordance with the following characteristics:PSA≥20 ng/ml;Gleason≥8;clinical stages≥T3;digital rectal examination positive;bone pain.Metastasis of pelvis,lumbar was much higher.It had statistical significance between PSA value and bone metastasis(P<0.05).The hormonal therapy can relieve the symptoms of bone pain and delay progression of metastasis.
Management of Prostate Cancer in China: a Multicenter Report of 6 Institutions.

Abstract:PURPOSE:In China the incidence of prostate cancer (PCa) is low and sparse data are available regarding its management. We analyzed the management of PCa at 6 Chinese urological institutions.MATERIALS AND METHODS:A retrospective analysis was performed of 431 consecutive patients treated for PCa at 6 Chinese institutions, including 5 in the divisions of Shanghai and 1 in the province of Chongqing, between January 2000 and December 2004. Tumor characteristics, therapeutic options and patient outcomes were recorded.RESULTS:At diagnosis median patient age was 72 years and median prostate specific antigen was 46.1 ng/ml. Most PCa cases were revealed by urinary symptoms (75.9%) or bone pain (12.8%). PCa was palpable on digital rectal examination in 74% of cases. At least 44 patients (10.2%) had metastases to lymph nodes at diagnosis and 112 (26%) had bone metastases. A total of 236 patients underwent bilateral orchiectomy and 100 received medical hormone therapy, which in 75% consisted of antiandrogen alone. At a median followup of 16.8 months 60% of these patients experienced biological recurrence. Radical prostatectomy was performed in 24 patients as monotherapy or in combination with bilateral orchiectomy. No patient with clinically localized PCa experienced biological recurrence after radical prostatectomy.CONCLUSIONS:The management of PCa in China differs from that in Western countries. To date surgical castration represents standard treatment. Screening detection of PCa could help detect earlier stage tumors and improve the outcome in patients.

Abstract:The incidence of prostate cancer (PCa) within Asian population used to be much lower than in the Western population; however, in recent years the incidence and mortality rate of PCa in some Asian countries have grown rapidly. This collaborative report summarized the latest epidemiology information, risk factors, and racial differences in PCa diagnosis, current status and new trends in surgery management and novel agents for castration-resistant prostate cancer. We believe such information would be helpful in clinical decision making for urologists and oncologists, health-care ministries and medical researchers.

Abstract:Prostate cancer (PCa) causes deaths worldwide, ranking second after lung cancer. Bone metastasis (BM) frequently results from advanced PCa, affecting approximately 90% of patients, and it also often results in severe skeletal-related events. Traditional diagnostic methods for bone metastases, such as tissue biopsies and imaging, have substantial drawbacks. This article summarizes the significance of biomarkers in PCa accompanied with BM, including (1) bone formation markers like osteopontin (OPN), pro-collagen type I C-terminal pro-peptide (PICP), osteoprotegerin (OPG), pro-collagen type I N-terminal pro-peptide (PINP), alkaline phosphatase (ALP), and osteocalcin (OC); (2) bone resorption markers, including C-telopeptide of type I collagen (CTx), N-telopeptide of type I collagen (NTx), bone sialoprotein (BSP), tartrate-resistant acid phosphatase (TRACP), deoxypyridinoline (D-PYD), pyridoxine (PYD), and C-terminal pyridinoline cross-linked telopeptide of type I collagen (ICTP); (3) prostate-specific antigen (PSA); (4) neuroendocrine markers, such as chromogranin A (CgA), neuron-specific enolase (NSE), and pro-gastrin releasing peptide (ProGRP); (5) liquid biopsy markers, such as circulating tumor cells (CTCs), microRNA (miRNA), circulating tumor DNA (ctDNA), and cell-free DNA (cfDNA) and exosomes. In summary, some of these markers are already in widespread clinical use, while others still require further laboratory or clinical studies to validate their value for clinical application.

Abstract:The incidence of prostate cancer (PCa) is increasing, making it one of the prevalent malignancies among men. Metastasis of PCa to the bones poses the greatest danger to patients, potentially resulting in treatment ineffectiveness and mortality. At present, the management of patients with bone metastasis focuses primarily on providing palliative care. Research has indicated that the spread of PCa to the bones occurs through the participation of numerous molecules and their respective pathways. Gaining knowledge regarding the molecular processes involved in bone metastasis may result in the development of innovative and well‑tolerated therapies, ultimately enhancing the quality of life and prognosis of patients. The present article provides the latest overview of the molecular mechanisms involved in the formation of bone metastatic tumors from PCa. Additionally, the clinical outcomes of targeted drug therapies for bone metastasis are thoroughly analyzed. Finally, the benefits and difficulties of targeted therapy for bone metastasis of PCa are discussed, aiming to offer fresh perspectives for treatment.

Abstract:To identify the clinical features and independent predictors of survival in patients with bone metastases from prostate cancer (PCa). We retrospectively analysed 115 PCa patients with bone metastases between 1997 and 2009. The overall survival rate after bone metastases was calculated using the Kaplan-Meier method. The prognostic factors were identified by univariate analysis using a log-rank test and by multivariate analysis using Cox proportional hazards regression models. The follow-up rate was 100%, the follow-up cases during 1, 3 and 5 years were 103, 79 and 55, respectively. The 1-, 3- and 5-year survival rates were 89.1%, 60.9% and 49.8%, respectively, with a median survival time of 48.5 months for patients with bone metastases from PCa. In univariate analysis, age, Gleason score, clinical stage, the number of bone lesions, alkaline phosphatase (ALP) level, invasion of neighbouring organs and non-regional lymph node metastases were correlated with prognosis. By multivariate analysis using Cox regression, ALP level, Gleason score and non-regional lymph node metastases were independent prognostic factors. These prognostic factors will help us to determine the appropriate dose and fraction of radiotherapy for these patients. Comprehensive Treatment of Prostate Cancer with Penis Metastasis: a Case Report and Literature Review

Abstract:Objective To explore the pathogenesis, clinical features, prognosis and treatment of prostate cancer with penile metastasis. Methods The clinical data of a patient with prostate cancer and penis metastasis were retrospectively analyzed. Results A 68-year-old man underwent transurethral resection of the prostate due to dysuria at our hospital, and the postoperative pathological examination revealed prostate adenocarcinoma. Fifteen months later, penis metastasis occurred. After treatment with external radiotherapy and endocrine therapy, prostate-cancer specific antigen decreased, the penis mass shrank, and the discomfort symptoms resolved. Conclusion Penile metastasis of prostate cancer is rare and has a poor prognosis. Although surgery, radiotherapy, and chemotherapy are palliative treatment, individualized treatment can delay disease progression and improve the quality of life. Abstract:Bone is the most common site for metastasis from prostate cancer, and bone metastases also is one of the main causes for the death of the patients with prostate cancer. Since the patients with bone metastases from prostate cancer still have relatively long survival periods after treatmemt, how to properly choose the treatment option for the metastatic patients is imperative in terms of improvements in both quality of life and life expectation. This paper summary pathophysiology, risk evaluation clinical assessment and the current status of the treatment options of prostate cancer with bone metastases, it also lays out the background for the individualized treatment plan that still needs to be further investigated.

Abstract:The numbers of patients diagnosed with prostate cancers is increasing due to the widespread application of prostate-specific antigen screening and subsequent prostate biopsies. The methods of systemic administration of therapeutics are not target-specific and thus cannot efficiently destroy prostate tumour cells while simultaneously sparing the surrounding normal tissues and organs. Recent advances in imaging-guided minimally invasive therapeutic techniques offer considerable potential for the effective management of prostate cancers. An objective understanding of the feasibility, effectiveness, morbidity, and deficiencies of these interventional techniques is essential for both clinical practice and scientific progress. This review presents the recent advances in imaging-guided interventional techniques for the diagnosis and treatment of prostate cancers.

Abstract:In the absence of early detection and initial treatment, prostate cancer often progresses to an advanced stage, frequently spreading to the bones and significantly impacting patients' well-being and healthcare resources. Therefore, managing patients with prostate cancer that has spread to the bones often involves using bone-targeted medications like bisphosphonates and denosumab to enhance bone structure and minimize skeletal complications. Additionally, researchers are studying the tumor microenvironment and biomarkers to understand the mechanisms and potential treatment targets for bone metastases in prostate cancer. A literature search was conducted to identify clinical studies from 2013 to 2023 that focused on pain, performance status, or quality of life as primary outcomes. The analysis included details such as patient recruitment, prior palliative therapies, baseline characteristics, follow-up, and outcome reporting. The goal was to highlight the advancements and trends in bone metastasis research in prostate cancer over the past decade, with the aim of developing strategies to prevent and treat bone metastases and improve the quality of life and survival rates for prostate cancer patients.

Abstract:Prostate cancer is a common male malignant tumor, and bone metastasis is one of the common complications in the late stage of prostate cancer. The mechanism of prostate cancer bone metastasis is a complex process involving multiple factors and steps. In recent years, with in-depth research on the mechanism of prostate cancer bone metastasis and the development of new drugs, important progress has been made in the treatment of prostate cancer bone metastasis. Based on this, this article introduces the mechanism of prostate cancer bone metastasis and the research progress of several bone-targeted drugs to provide reference and inspiration for future research.

Abstract:Prostate cancer (PCa) is a prevalent male malignancy that originates in the epithelial cells of the prostate. In terms of incidence and mortality of malignant tumors in men, PCa ranks second and fifth globally and first and third among men in Europe and the United States, respectively. These figures have gradually increased in recent years. The primary modalities used to diagnose PCa include prostate-specific antigen (PSA), multiparametric magnetic resonance imaging (mpMRI), and prostate puncture biopsy. Among these techniques, prostate puncture biopsy is considered the gold standard for the diagnosis of PCa; however, this method carries the potential for missed diagnoses. The preoperative evaluation of the patient in this study suggested advanced PCa. However, the initial prostate puncture biopsy was inconsistent with the preoperative diagnosis, and instead of waiting for a repeat puncture of the prostate primary, we performed a biopsy of the rib metastasis, which was later diagnosed as advanced PCa.

Abstract:PURPOSE:To explore treatment patterns among patients with prostate cancer and bone metastasis and to compare clinical outcomes following use of different hormone therapies including combined androgen blockade (CAB), nonsteroidal antiandrogen (NSAA) monotherapy, and castration monotherapy. METHODS:We conducted a population-based cohort study using data from the Urban Employee Basic Medical Insurance database (2011-2014) in Beijing. We identified 475 patients with newly diagnosed bone metastatic prostate cancer with at least one prescription for hormone therapy and described their treatment patterns over a median follow-up of 20.7 months. Cox proportional hazards model was used to compare time to chemotherapy initiation between patients starting on different hormone therapies. RESULTS:Hormone therapy and/or bisphosphonate therapy with zoledronic acid were the initial treatments in the majority of patients (87.8%); chemotherapy, radiotherapy, and surgery were usually given later in the treatment pathway. CAB was the most common hormone treatment (73.7%). For time to chemotherapy initiation, hazard ratios (95% confidence intervals) were 2.43 (1.08-5.44) for NSAA alone vs CAB and 1.29 (0.78-2.13) for castration alone vs CAB. CONCLUSIONS:Our findings show that while a wide range of therapies are used to treat patients with prostate cancer and bone metastasis in Beijing, hormone therapy and bisphosphonate therapy are the most commonly prescribed, and use of CAB was seen to be advantageous in delaying time to chemotherapy initiation over NSAA monotherapy. Future studies should explore longer-term treatment patterns, including use of newly approved treatments.

Abstract:Prostate cancer (PCa) is a maligmancy with high morbidity and mortality. Bone metastasis is the main cause of short survival time and difficulties in the treatment and prevention of PCa. Previous findings of our team showed 155 bone-specific genes highly expressed in bone metastatic PC3 cells, which is considered to be the key to their adaptation to the bone micro-environment, proliferation and formation of metastatic tumor, and extensively exists in cancer metastasis in multiple systems. This review summarizes the published literature on the highly expressed bone-specific genes, focusing on the roles and values of these genes in the metastasis, progression, clinical diagnosis, treatment and prognosis of PCa, offering a prospect of the direction and targets in the studies of PCa bone metastasis so as to enrich the bone metastatic theories and clinical treatment principles of this disease in the future.

Abstract:Prostate cancer with bone metastasis has a high cancer-specific mortality. Thus, it is essential to delineate the mechanism of bone metastasis. Pre-metastatic niche (PMN) is a concept in tumor metastasis, which is characterized by tumor-secreted factors, reprogramming of stromal cells, and immunosuppression by myeloid-derived suppressor cells (MDSC), which is induced by bone marrow-derived cells (BMDC) in the target organ. However, PMN does not explain the predilection of prostate cancer towards bone metastasis. In this review, we discuss the initiation of bone metastasis of prostate cancer from the perspective of PMN and tumor microenvironment in a step-wise manner. Furthermore, we present a new concept called pre-metastatic bone niche, featuring inherent BMDC, to interpret bone metastasis. Moreover, we illustrate the regulation of traditional Chinese medicine on PMN.

Prevalence and risk factors of bone metastasis and the development of bone metastatic prognostic classification system: a pan-cancer population study

Abstract:Background: The prevalence of bone metastasis (BM) varies among primary cancer patients, and it has a significant impact on prognosis. However, there is a lack of research in this area. This study aims to explore the clinical characteristics, prevalence, and risk factors, and to establish a prognostic classification system for pan-cancer patients with BM. Methods: The data obtained from the Surveillance, Epidemiology and End Results database were investigated. The prevalence and prognosis of patients with BM were analyzed. Hierarchical clustering was used to develop a prognostic classification system. Results: From 2010 to 2019, the prevalence of BM has increased by 41.43%. BM most commonly occurs in cancers that originate in the adrenal gland, lung and bronchus and overlapping lesion of digestive systems. Negative prognostic factors included older age, male sex, poorer grade, unmarried status, low income, non-metropolitan living, advanced tumor stages, previous chemotherapy, and synchronous liver, lung, and brain metastasis. Three categories with significantly different survival time were identified in the classification system. Conclusions: The clinical features, prevalence, risk factors, and prognostic factors in pan-cancer patients with BM were investigated. A prognostic classification system was developed to provide survival information and aid physicians in selecting personalized treatment plans for patients with BM.

Abstract:Objective To analyze the clinical characteristics of prostate cancer with bone pain as the first clinical manifestation retrospectively, the aim of this study was to evaluate the prognostic significance of the age, initial serum level PSA, Gleason score, bone metastases, clinical stage, ALP in the overall survival in patients with bone pain as the first manifestation of prostate cancer. Methods During the time period from 2003 to 2010, a total of 51 patients with newly diagnosed, histologically confirmed prostate cancer, histological diagnosis was carried through the transrectal ultrasonography guided biopsy and carried on Gleason score assessment. Metastases were confirmed by positive bone scintigraphy with 555-925 MBq99mTc-MDP using Emission computed Tomography, some of which were identified by pathology of excision of metastatic bone tumor or bone marrow aspiration, all of this patients had been treated by androgen deprivation treatment, probe into the impact of the age, initial serum level PSA, Gleason score, bone metastases, clinical stage, ALP on the overall survival. Results The incidence of the bone pain as the first clinical manifestation of prostate cancer was 92.2%(47/51), the metastasis of pelvis, lumbar was most common, 21.5%, 29.0% respectively, at a median follow-up of 32.1months, 20 patients had died, the median overall survival after maximal androgen blockade was 24.4 months and was significantly longer if the age was less than 71 years(29.1 months vs. 23.1 months, P=0.0169) and Gleason score(2-6, 7, 8-10) was lower(62.4 months vs. 35.1 months vs. 12.5 months, P=0.0173), bone metastases was less than 6 sites(36.1 months vs. 4.4 months, P=0.0144),only bone metastases(Compared to bone and nodes or organs metastases 23.7 months vs. 4.4 months, P=0.0304),clinical stage was less than stage T2(62.4 months vs. 22.8 months, P=0.0257), ALP was in the normal rang(37.0 months vs. 19.0 months, P=0.0151), it had not demonstrated statistically significant interactions of overall survival with the initial PSA level.Mean time±SD to PSA relapse after first-line MAB was(17.6±8.3) months, of the 44 patients(PSA relapse) treated with second-line MAB, 24(54.5%) were considered to be responders. Conclusions Gleason score, the presence of bone, lymph nodes and liver, visceral metastases, clinical stage, ALP significantly predict OS in patients with bone pain as the first clinical manifestation. Maximum androgen blockade is an effective treatment, some patients can also benefit from the second-line treatment after PSA relapse of the first-line hormonal treatment.
