Association of single nucleotide polymorphisms in APOE and BMI with clinical outcome of target therapy in advanced renal cell carcinoma

Abstract:Objective To investigate the effect of body mass index(BMI) and polymorphisms in apolipoprotein E(APOE) gene on treatment outcome in patients treated with targeted therapy for metastatic RCC(mRCC).Methods Eighty patients treated with sunitinib or sorafenib for mRCC were enrolled in this study.BMI was recorded on the first day of treatment and genotype analyses were done before treatment.All the patients were followed up for progression free survival(PFS),overall survival(OS) and objective response rate(RR).Results The rs8106822 GA genotype was associated with a superior objective response rate(94.7%) when compared with 71.8% of the GG+AA genotypes(P=0.01).With a median follow-up time of 20 months(range: 1.5-36.2 months),no significant differences was observed on the median PFS for the rs8106822 GA,GG and AA genotype subgroups(P=0.96).No differences were found between rs405509 genotype subgroups on either RR or survivals.According to the WHO criteria,patients were stratified into three groups:normal weight group(BMI<25 kg/m2),overweight group(BMI 25-30 kg/m2),and obesity group(BMI>30 kg/m2),and no significant differences were detected in median PFS(P=0.75).When redefining BMI of 24 kg/m2 and 26 kg/m2 as the cut-off values,patients with BMI≥26 kg/m2 had longer PFS(14 months) compared with patients with BMI of 24-25.9 kg/m2(8months),and there was significant difference(P=0.04).Conclusion This study showed that polymorphisms in apolipoprotein E(APOE) gene is probably associated with clinical outcome of mRCC patients treated with targeted agents.Patients with higher BMI have longer OS than the low or normal BMI patients.Further survival data needs long-term follow-up.
