Abstract:OBJECTIVE: To identify possible risk factors for cerebral palsy (CP) in children. METHODS: A Population-based survey was conducted (including 92 CP cases) in 66 townships of 15 cities of Zhejiang Province from October to November, 1998. 184 of matched controls were selected for comparison. RESULTS: Factors identified which were statistically significant for risk of subsequent childhood Cerebral Palsy included some neonatal diseases, some maternal diseases, low birth weight (<2500 g), maternal irregular menstruation, toxic, substances during pregnancy, malnutrition during pregnancy,and paternal age. CONCLUSION: Several risk factors for Cerebral Palsy were identified. Their prevention may result in redduction of the incidence of Cerebral Palsy.
The Incidence and Risk Factors of Brain Injury in Multiple Pregnancy Preterm Infants

Abstract:Background: Preterm infants have been shown to be prone to brain damage, which affect their future quality of life. Objective: The retrospective study is to evaluate the incidence and the risk factors of brain injury for multiple pregnancy preterm infants. Methods: 72 multiple pregnancy preterm infants and 99 single pregnancy preterm infants (28-35 wk of gestational age and 600-2410g of weight at birth) were studied. The incidence and risk factors of brain injury were analyzed by way of multiple regression analyses. Results: Compared with single pregnancy preterm infants, multiple pregnancy preterm infants had higher incidence rate of brain injuries(32/72 vs 27/99,P< 0.01), The more of the pregnancy number, the less of the infants weight, the less of gestational age, the higher of the brain injuries incidence rate. For infants at gestational age ≤30 weeks, the brain injury incidence rate was higher than single pregnancy preterm infants (67.9% vs 43.6%). The group of weight ≤1500g had higher incidence rate of brain injury. There's no difference between groups of multiple pregnancy preterm infants and single pregnancy preterm infants. The main risk factors are pregnancy-induced hypertension syndrome, asphyxia, fetal distress, amniotic fluid meconium III degree pollution, NRDS, frequent breath stops, hypoglycemia, neonatal hyperbilirubinemia. Conclusion: The preterm brain injuries occur more often in multiple pregnancy preterm infants, which were related to many factors.

Abstract:Aim To determine the significant risk factors of cerebral palsy (CP) in Taiwanese children and the associations between infant‐related and parent‐related factors. Method Data from 1 459 093 infants and their parents in Taiwan's national databases collected between 2009 and 2016 were used. The cohort with CP included children diagnosed with CP between birth and age 3 years; a total of 3254 children with CP were included in the final analysis. Hierarchical logistic regression models were used to estimate the odds ratio for the risk factors of CP. Results The hierarchical logistic regression models indicated that significant risk factors associated with CP are suburban location, low income, maternal and paternal diabetes mellitus, paternal substance abuse, paternal seizure disorder, male sex, birth by Cesarean section, singleton birth, low birthweight, being born extremely and very preterm, intraventricular hemorrhage, and periventricular leukomalacia, as well as tube feeding, ventilator use, and dopamine administration within 6 months of age. Interpretation In addition to common maternal and infant risk factors, we identified significant paternal risk factors associated with CP, including diabetes mellitus, seizure disorder, and substance abuse. The combination of maternal, paternal, and infant risk factors in CP holds great promise for early identification and intervention.

Exploration for pathogenic factors and diagnosis of cerebral palsy in children

Abstract:To explore the pathogenic factors and diagnosi s of cerebral palsy (CP) in children, perinatal history, clinical evaluation, im aging and electrophysiological data and parents' attitude for rehabilitation of CP were analyzed and investigated in 50 children with CP retrospectively. The re sults showed that prem ature birth, low birth weight and perinatal asphyxia were the most pathogenic fa ctors induced CP. The common CP types was spastic paralysis, in particular quadr iparesis. Abnormal CT and MRI findings were detected in 68% of CP children. It w as noticed that 44% of CP children were with speech and language disorder, 32% w i th mental retardation, 18% with hearing impairment, 16% with visual impairment a nd 16% with epilepsy. There were 86% parents to accept with physiotherapy on the ir CP children, 68% with acupuncture therapy, 46% with occupational therapy and 100% with speech therapy, respectively. It is suggested that multi-subjec t cooperation will be necessary for the follow-up study of the high- risk infants. The realization of the related-speciality knowledge a nd the enhancement of rehabilitational conception will be helpful for the early diagnosis and intervention of CP patients. The popularization of rehabilitationa l conception will be also important in the parents with CP children.
High risk factors of cerebral palsy and clinical characteristics of neurological dysfunction in children:analysis of 265 cases

Abstract:Aim: To investigate and analyze the morbidity of high risk factors in children with cerebral palsy (CP), and study the CP types and the clinical characteristics of the complicated neurological dysfunction and neurological examination. Methods: The data of 265 CP children, who were admitted in the Rehabilitation Center, Children's Hospital of Fudan University in 2003, were analyzed retrospectively. Results: The proportions of premature birth, asphyxia and abnormal delivery were highest among the high risk factors of CP, which were 43.0%, 36.6% and 30.2%, respectively. There were 60.4% of the CP children had two or more high risk factors, while 12.1% had no obvious high risk factors. Clinical typing: spastic diplegia (33.2%) and tetraplegia (37.0%) were the most common, and hemiplegia accupied 17.4%. There were 232 cases (87.5%) of the CP children were accompanied with mental retardation of different degrees, 12 cases (4.5%) of hearing disorders, 20 cases (7.5%) of epilepsy and 15 cases (5.7%) of visual dysfunction. The abnormal rates of EEG, cranial CT scan and brain-stem auditory evoked potential (BAEP) were 57.6% (34/59), 89.1% (164/184) and 40% (12/30) respectively. Conclusion: Premature birth, asphyxia, abnormal delivery, intracranial hemorrhage and twins are common in CP children. CP children have higher incidence rate of intellectual dysfunction, abnormal CT and EEG pathological changes.
Pre-pregnancy and pregnancy disorders, pre-term birth and the risk of cerebral palsy: a population-based study

Abstract:Background: Cerebral palsy (CP) is the most common cause of childhood physical disability whose aetiology remains unclear in most cases. Maternal pre-existing and pregnancy complications are recognized risk factors of CP but the extent to which their effects are mediated by pre-term birth is unknown. Methods: Population-based cohort study in Sweden including 2 055 378 singleton infants without congenital abnormalities, born between 1999 and 2019. Data on maternal and pregnancy characteristics and diagnoses of CP were obtained by individual record linkages of nationwide Swedish registries. Exposure was defined as maternal pre-pregnancy and pregnancy disorders. Inpatient and outpatient diagnoses were obtained for CP after 27 days of age. Adjusted rate ratios (aRRs) were calculated, along with 95% CIs. Results: A total of 515 771 (25%) offspring were exposed to maternal pre-existing chronic disorders and 3472 children with CP were identified for a cumulative incidence of 1.7 per 1000 live births. After adjusting for potential confounders, maternal chronic cardiovascular or metabolic disorders, other chronic diseases, mental health disorders and early-pregnancy obesity were associated with 1.89-, 1.24-, 1.26- and 1.35-times higher risk (aRRs) of CP, respectively. Most notably, offspring exposed to maternal antepartum haemorrhage had a 6-fold elevated risk of CP (aRR 5.78, 95% CI, 5.00-6.68). Mediation analysis revealed that ∼50% of the effect of these associations was mediated by pre-term delivery; however, increased risks were also observed among term infants. Conclusions: Exposure to pre-existing maternal chronic disorders and pregnancy-related complications increases the risk of CP in offspring. Although most infants with CP were born at term, pre-term delivery explained 50% of the overall effect of pre-pregnancy and pregnancy disorders on CP.

Abstract:Objective:To investigate the perinatal risk factors for preterm children and complications.Methods:Review the clinical information of preterm children in our hospital between Januery 2006 and December 2009.Analysis of perinatal risk factors for preterm children and complications in 183 cases.Results:Premature rupture of membranes,multiple pregnancy,pregnancy-induced hypertension is the main reason for preterm birth;preterm children with major complications such as pneumonia and intracranial hemorrhage.Conclusion:Enhance perinatal care,early intervention a variety of perinatal risk factors,active prevention of complications in preterm children,is the key to improve the survival rate of premature children and reduce disability.

Abstract:AimThis study aimed to identify risk factors for the onset of cerebral palsy (CP) in neonates due to placental abruption and investigate their characteristics. MethodsA retrospective case–control study was conducted using a nationwide registry from Japan. The study population included pregnant women (n = 122) who delivered an infant with CP between 2009 and 2015, where placental abruption was identified as the single cause of CP. The control group consisted of pregnant women with placental abruption, who delivered an infant without CP and were managed from 2013 to 2014. They were randomly identified from the prenatal database of the Japan Society of Obstetrics and Gynecology (JSOG‐DB; n = 1214). Risk factors were investigated using multivariate analysis. ResultsAlcohol consumption (3.38, 2.01–5.68) (odds ratio, 95% confidence interval), smoking during pregnancy (3.50, 1.32–9.25), number of deliveries (1.28, 1.05–1.56), polyhydramnios (5.60, 1.37–22.6), oral administration of ritodrine hydrochloride (2.09, 1.22–3.57) and hypertensive disorders in pregnancy (2.25, 1.27–4.07) were significant risk factors. In contrast, intravenous administration of oxytocin (odds ratio, 95% confidence interval: 0.22, 0.09–0.58) and magnesium sulfate (0.122, 0.02–0.89) attenuated risk. ConclusionAlcohol consumption, smoking during pregnancy, number of deliveries, polyhydramnios, oral administration of ritodrine hydrochloride and hypertensive disorders in pregnancy were identified as risk factors for CP following placental abruption. Regarding alcohol consumption and smoking during pregnancy, the results suggest the importance of educational activities targeting pregnant women to increase their awareness of placental abruption.

Abstract:To investigate the relationship between perinatal risk factors such as premature, low birth weight, small for gestational age and childhood cerebral palsy (CP).A cross sectional survey was carried out among 305,263 children aged 1 - 6 years old in seven cities of Jiangsu Province, China from May to July 1997. The perinatal risk factors were analysed.Four hundred and eighty-four cases of CP were found among this population. The prevalence of CP for children aged 1 - 6 years old was 1.59 per thousand. The prevalence of CP were strongly correlated to prematurity (RR = 25.16), low birth weight (RR = 19.63), and also highly correlated to small for gestational age (RR = 4.34). For smaller groups divided by small for gestational age (SGA), appropriate for gestational age (AGA), large for gestational age (LGA) and then by gestational age, prematurity was found to be at high risk in SGA (RR = 9.29), AGA (RR = 28.34) and LGA (RR = 21.41) groups. For groups divided by gestational age and then by SGA, AGA and LGA, SGA was found to have significantly high risk in premature (RR = 1.45), mature (RR = 4.41) and postmature (RR = 3.19) groups. Nine groups were divided by the gestational age along with SGA, AGA and LGA, rates of CP were found to be significantly higher in most groups than in the term AGA group. Compared with the rate of CP in the term AGA group, the RR were calculated and showed as followings (from higher to lower), premature SGA (RR = 40.99), premature AGA (RR = 28.34), premature LGA (RR = 21.08), postmature SGA (RR = 8.39), mature SGA (RR = 4.41) and postmature AGA (RR = 2.63).Prematurity and small for gestational age are both independent risk factors for cerebral palsy. Postmaturity and large for gestational age are not risk factors.
Clinical Characteristics of 100 Children with Cerebral Palsy

Abstract:ObjectiveTo analysis the clinical characteristics and the high risk factors of children with cerebral palsy.MethodsClinical data of 100 children with cerebral palsy were retrospectively analyzed.Results51% of children were born with asphyxia, 42% were born prematurely, 39% were low birth body weight. 85.7% of children involved had mental retardation, 12% had hearing loss and 7% had visual impairment. 88% of children involved had abnormal cranial CT results and 46.1% had abnormal EEG. 50.0% of the children had abnormal brain stem auditory evoked potentials.ConclusionAsphyxia, prematurely birth and low birth body weight are high risk factors of cerebral palsy. Most of the children with cerebral palsy in this group are mental retarded.
Analysis of misssed diagnosis and misdiagnosis of 1212 cases with placental abruption

Abstract:Objective To investigate the risk factors and clinical manifestations of placental abruption, and to analyze the causes of missed diagnosis and misdiagnosis. Methods A retrospective analysis was conducted in 135584 women who delivered in Women′s Hospital, School of Medicine, Zhejiang University from January 2005 to December 2015. The diagnosis of placental abruption was made in 1212 cases. According to the consistency of prenatal and postnatal diagnosis, they were divided into 3 groups.(1) The diagnosis was consistent prenatally and postnatally in 715 cases(58.99%,715/1212) as the diagnosis group.(2)In 312 cases (25.74%,312/1212), the diagnosis was made after birth as the missed diagnosis group.(3)In 185 cases (15.26%,185/1212), the diagnosis was made prenatally but excluded after birth as the misdiagnosis group. The disease classification was made, and the risk factors, clinical manifestations, lab results, the time of termination and perinatal outcomes were recorded in the 3 groups. The reasons of missed diagnosis and misdiagnosis were analyzed. Results (1) In the 1212 cases, the diagnosis of placental abruption was confirmed in 1027 cases, with the incidence of 0.76%(1027/135584). The rate of missed diagnosis was 30.38%(312/1027), and the rate of misdiagnosis was 0.14%(185/134557). (2) There were significant differences in the degree of placental abruption among the 3 groups (P＜0.05). (3)Significant differences were found among the 3 groups regarding the ratio of hypertensive disorders, trauma, induced labor and advanced maternal age (all P＜0.05). (4) There were statistically significant differences among the 3 groups regarding the incidence of vaginal bleeding, persistent abdominal pain and uterine tenderness, bloody amniotic fluid, increased uterine tension and stillbirth (all P＜0.05). (5) There was no significant difference in the rate of abnormal fetal heart rate mornitoring among the 3 groups (P=0.22). The differences were statistically significant among the 3 groups when regarding the incidence of abnormal ultrasound finding and abnormal blood coagulation (P＜0.01), with the highest incidence of abnormal ultrasound in the diagnosis group (68.1%) and the highest incidence of abnormal coagulation in the misdiagnosis group (24.9%). (6)There was statistically significant difference among the 3 groups when comparing the ratio of termination of pregnancy within 24 hours (P=0.01). (7) There were statistically significant differences among the 3 groups when the ratios of postpartum hemorrhage, DIC, neonatal asphyxia and perinatal death were compared (all P＜0.05). The highest incidence of postpartum hemorrhage was in the diagnosis group (17.9%) and the lowest was in the misdiagnosis group (5.4%). The highest incidence of DIC was in the diagnosis group (3.9%) and the lowest was in the misdiagnosis group (0). The highest incidence of neonatal asphyxia was in the diagnosis group (30.6%) and the lowest was in the misdiagnosis group (7.6%). And for perinatal death, the highest incidence was in the diagnosis group (12.6%), the lowest was in the misdiagnosis group (2.2%). Conclusions Placental abruption could be misdiagnosed when depending on risk factors, such as trauma. And it could be missed diagnosis during the induction of labor. Uterine contraction, abnormal fetal heart rate mornitoring, abnormal ultrasound and abnormal coagulation function are important in the diagnosis of placental abruption.
Analysis on Perinatal Factors of 30 Neonates with Cerebral Infarction

Abstract:Objective:To explore the epidemic factors and the incidence of neonatal cerebral infarction during perinatal period,in order to prevent and treat early,reduce the incidence of the disease. Methods:The clinical features and imaging appearances of 30 neonates with confirmed cerebral infarction were summarized,the perinatal factors and other probable causes were analyzed. Results:Within one year,6 neonates with cerebral infarction were found in the hospital,accounting for 0.6‰ of hospitalized neonates.83.3% of the pregnant women had high risk factors before and during delivery,the proportion of neonatal birth asphyxia was 30.0%.36.7% of the neonates with cerebral infarction combined with left to right shunt congenital heart disease,and 26.7% of the neonates with cerebral infarction combined with patent foramen ovale.Paroxysmal seizures occurred in 50.0% of the neonates in preliminary clinical stage,and 80.0% of them occurred within 72 hours after birth;63.3% of the neonates lacked positioning signs and with non-specific nervous system manifestations including consciousness change at different levels,dystonia and(or) abnormal primary reflex.Positive findings were found through cerebral CT scan or MRI,75.0% of the neonates occurred at left middle cerebral artery,and the positive rate at left middle cerebral artery(73.9%) was higher than that at right middle cerebral artery(26.1%). Conclusion:Neonatal cerebral infarction is not rare,which is one of the early causes of neonatal cerebral infarction,and related to high risk factors of pregnant women during perinatal period,neonatal asphyxia and congenital heart disease closely,timely and targeted cerebral CT scan or MRI are the main methods of early diagnosis of neonatal cerebral infarction,preventing and treating all kinds of high risk factors during perinatal period is very important for decrease of neonatal cerebral infarction incidence.
Study on the Relationship Between Cerebal White Matter Injury in Preterm Infants and Maternal Preeclampsia

Abstract:【Objective】 To recognize the relationship between cerebal white matter injury in preterm infants and maternal preeclampsia. 【Methods】 Through image examination,the development of cerebal white matter injury in 72 preterm infants borned to maternal preeclampsia were observed.The effect of the severety of preeclampsia on the severety of cerebal white matter injury was analyzed,for the more,the factors of the severe preeclampsia that related to the cerebal white matter injury,including the types and with complications were studyed. 【Results】 72 infants showed different white matter injury: 27 infants had mild cerebal white matter injury(transient periventricular echodensity).45 infants had severe cerebal white matter injury(5 infants had periventrcular leucomalacia;40 infants had ventricular dilation).The incidence of severe cerebal white matter injury in the severe preeclampsia was 71.1%,higher than that in the mild preeclamosia(44.4%)(P0.05).The incidence of severe cerebal white matter injury in the early severe preeclampsia was 76.5%,higher than that in the late severe preeclampsia(60%)(P0.01).The incidence of severe cerebal white matter injury in the severe preeclampsia with complications was 84.4%,higher than that in the non-with complication(41.7%)(P0.01). 【Conclusion】 These results suggest that severe preeclampsia,especially,the early severe preeclampsia and the severe preeclampsia with complications have an important effect on the severe cerebal white matter injury.

Abstract:SUMMARY Objective: To evaluate risk factors for childhood cerebral palsy(CP) in China. Methods: We conducted a population based case control study among children in six provinces in China. They were 1-6 years old children with CP Two controls were selected for each child; controls were selected from children of same sex, same age, and living in the same township. Information on pregnancy, labor, delivery, and the neonatal period was obtained. Results: 1 047 327 children aged 1-6 years old were enrolled and 2 009 children of CP were found.Cases were much more likely than controls to experience hypoxic ischemic encephalopathy ( OR = 26.4, 95%CI = 4.6, 152.2 ), hyperbilirubinemia(OR=14.2,95%CI=5.3,38.2), intracranial hemorrhage(OR=133.1,95%CI= 25.4,697.7) , and neonatal meningitis (OR=267.6, 95%CI=21.2,3 372) in the neonatal period. Prenatal or intradelivery factors such as family history of mental retardation(OR=5.4, 95%CI=3.1, 9.4), poor maternal nutrition status(OR=1.9, 95%CI=1.6,2.4), maternal fever(OR=1.9, 95%CI=1.4,2.5) and drug exposure(OR=7.8, 95%CI=2.6,23.2), intrauterine growth retardation(OR=6.6, 95%CI=2.5,16.7), premature labor(OR=7.6, 95%CI=4.2,13.7), and low birth weight(OR=5.3, 95%CI=3.2,8.9); and demographic factors such as poor socioeconomic status(OR= 1.8, 95%CI=1.4,2.5) and exposure to cigarette smoke(OR=1.5, 95%CI=1.1,1.9) were with lower odds ratio. Conclusion: In this study, adverse events occurring during the neonatal period were more strongly associated with development of CP than were prenatal or demographic factors; however, the influence of prenatal or demographic factors on neonatal events needs further study.

Abstract:Preeclampsia (PE) is a leading condition threatening pregnant women and their offspring. The offspring of PE pregnancies have a high risk of poor neurodevelopmental outcomes and neuropsychological diseases later in life. However, the pathophysiology and pathogenesis of poor neurodevelopment remain undetermined. Abnormal placental functions are at the core of most PE cases, and recent research evidence supports that the placenta plays an important role in fetal brain development. Here, we summarize the relationship between abnormal fetal brain development and placental dysfunction in PE conditions, which include the dysfunction of nutrient and gas-waste exchange, impaired angiogenesis stimulation, abnormal neurotransmitter regulation, disrupted special protectors, and immune disorders. All these factors could lead to poor neurodevelopmental outcomes.

Abstract:Cerebral palsy (CP) describes a group of disorders of the development of movement and posture,causing activity limitation attributed to disturbances,which occurred in the fetal or infant brain.Early identification and intervention of CP has always been a difficult topic in the research of neuroscience.The intervention should be focused on infants showing early signs of CP.Such signs may be efficiently detected by a combination of neuroimaging and the General Movements Assessment.Besides movements,enriched environments,active participation,parental coaching have benefits to early intervention.In this investigation,the early identification and intervention in cerebral palsy were focused.

Abstract:Objective: Because of improvement in obstetrical and neonated care, the perinatal mortality is decreased, yet infants at high risk cerebral palsy (CP) are increased. The aim of this study was to investigate and evaluate the method of early intervention in high risk infants, in order to reduce the sequelae of brain damages. Method: Ninety infants (61 boys, 29 girls) ranging from 12 days to 12 months (average age: 3.83±2.56 months) underwent an early intervention program. Sixty-eight of them were suspected as high risk infants of CP, and the other 22 infants (average age: 4.74±2.77 months) were diagnosed CP already. The Gesell Scale of Infant Development and neurologic examination were used for evaluation. Result: After 2 years early rehabilitation and follow-up, 68.2% infants with CP were cured, 18.2% were at borderline and only 13.6% were still abnormal. Sixty-eight infants with suspected CP developed normal except 3 of them were lost to follow-up. The prevalence of CP among the infants with premature, low birth weight, apnea and hypoxic ischemic encephalopathies were reduced significantly. Conclusion: The sequelae of brain damages could be reduced by early intervention of high risk infants of CP, and the cured effect could be achieved by early rehabilitation in infants with CP.

Abstract:Objective To explore the incidence and risk factors of brain injury in preterm infants and provide basic data for prevention of brain injury and improvement of outcome in preterm infants.Methods Data were collected retrospectively among neonates with gestational age of< 37 weeks,who were admitted to NICU in St.Louis Children's Hospital in Washington University School of Medicine from Jan.2005 to Dec.2006.Intraventricular haemorrhage(IVH)and periventricular leukomalacia(PVL)rates were calculated by gestational age.Binary Logistic regression was adopted to identify risk factors associated with brain injury in preterm infants.Results The incidence of IVH and PVL in total preterm population was 17.7% and 4.9%,respectively,while 14.4% and 4.5% in survival preterm infants,48.1% and 14.8% in group at gestational age of 23 to less 25 weeks,35.2% and 11.2% at 25 to less 28 weeks,13.8% and 3.1% at 28 to less 33 weeks,2.8% and 1.7% at 33 to less 37 weeks.Neonatal necrotizing enterocolitis,patent ductus arterious(PDA),and mechanical ventilation(MV)were risk factors in IVH development,while larger gestational age and higher 5-min Apgar scores were protective factors.IVH,MV and maternal perinatal infection were risk factors in PVL,birth weight and female were protective factors.IVH was associated with increased mortality in preterm infants.Conclusions The incidence of brain injury is negatively related to gestational age.Perinatal infection,hypoxia,and the factors influencing cerebral blood dynamics such as PDA and mechanical ventilation are all associated with increased risk of brain injury in premature infants.

Abstract:Abstract Background The aim of the present study was to clarify fetal heart rate (FHR) evolution patterns in infants with cerebral palsy (CP) according to different types of umbilical cord complications. Methods This case–control study included children born: with a birth weight ≥2000 g, at gestational age ≥33 weeks, with disability due to CP, and between 2009 and 2014. Obstetric characteristics and FHR patterns were compared among patients with CP associated with (126 cases) and without (594 controls) umbilical cord complications. Results There were 32 umbilical cord prolapse cases and 94 cases with coexistent antenatal umbilical cord complications. Compared with the control group, the persistent non-reassuring pattern was more frequent in cases with coexistent antenatal umbilical cord complications ( p = 0.012). A reassuring FHR pattern was observed on admission, but resulted in prolonged deceleration, especially during the first stage of labor, and was significantly identified in 69% of cases with umbilical cord prolapse and 35% of cases with antenatal cord complications, compared to 17% of control cases ( p < 0.001). Conclusion Hypercoiled cord and abnormal placental umbilical cord insertion, may be associated with CP due to acute hypoxic-ischemic injury as well as sub-acute or chronic adverse events during pregnancy, while umbilical cord prolapse may be characterized by acute hypoxic-ischemic injury during delivery.

Abstract:Cerebral palsy is a disorder characterized by abnormal tone, posture, and movement. In clinical practice, it is often useful to approach cerebral palsy based on the predominant motor system findings - spastic hemiplegia, spastic diplegia, spastic quadriplegia, extrapyramidal or dyskinetic, and ataxic. The prevalence of cerebral palsy is between 1.5 and 3 per 1,000 live births with higher percentage of cases in low to middle income countries and geographic regions. Pre-term birth and low birthweight are recognized as the most frequent risk factors for cerebral palsy; other risk factors include hypoxic-ischemic encephalopathy, maternal infections, and multiple gestation. In most cases of cerebral palsy, the initial injury to the brain occurs during early fetal brain development. Intracerebral hemorrhage and periventricular leukomalacia are the main pathologic findings found in preterm infants who develop spastic cerebral palsy. The diagnosis of cerebral palsy is primarily based on clinical findings. Early recognition of infants at risk for cerebral palsy as well as those with cerebral palsy is possible based on a combination of clinical history, use of standardized neuromotor assessment and findings on magnetic resonance imaging; however, in clinical practice, cerebral palsy is more reliably diagnosed by 2 years of age. Magnetic resonance imaging scan is indicated to delineate the extent of brain lesions and to identify congenital brain malformations. Genetic testing and tests for inborn errors of metabolism are indicated to identify specific disorders, especially treatable disorders. Because cerebral palsy is associated with multiple associated and secondary medical conditions, its management requires a sustained and consistent collaboration among multiple disciplines and specialties. With appropriate support, most children with cerebral palsy grow up to be adults with good functional abilities.
