Abstract:In order to broaden contraceptive choice and improve quality of care and to make the decision of contraceptive introduction more compatible for real situation and being based on scientific evidence we systematically assessed the needs for contraceptives and reproductive health of clients in Chongqing Municipality service provision system and available contraceptive technologies in Chongqing Municipality then suggestions and recommendations were put foreword on how to improve the quality of care of Family Planning (FP) and reproductive health including the quality of care during contraceptive introduction. The methodology of Strategic Assessment of the first stage of WHOs Strategic Approach was applied and the assessment was conducted in October 2000. Of 43 countries/districts in Chongqing Municipality one district and three countries were carefully selected to provide a diverse range of situation-reflecting major-regional cultural and programmatic variations. Half of the 14 townships visited were selected by local family planning officials while half were selected at random by the team. The assessment team spent two weeks interviewing government officials administrators providers and clients as well as community leaders such as FP cadres women men and youth at the village township district and municipal levels overall 425 people were interviewed. 49 service delivery sites were observed including county/township level FP service stations and Maternal and Child Health (MCH) Hospitals township level public health hospitals village level health centers and pharmacies. The majority of people had a general acceptance of the policy: it is better that one couple one child. However especially in the less developed and predominately agricultural areas people still felt they needed a boy. The people who were younger than 30 years old or lived in urban area or had received over 9 years education or had the experiences of working outside had comparatively more knowledge of contraceptives. The knowledge and awareness of risk Reproductive Tract Infections (RTIs) and Sexually Transmitted Infections (STIs) were very limited among all segments of the population. Clients were willing to use IUD for its long life span reversibility high effectiveness and free of charge for insertion. Users and members of the general community were not familiar with the concept of informed choice. At the municipal and county levels a broad range of fertility regulation methods was available. However some township were only provided IUDs insertion and the quarterly IUD check. More than ten kinds of IUDs were available in Municipal level. However cheaper IUDs were always purchased in some townships with carelessness of IUD effectiveness. Multiple hormonal contraceptive methods were available throughout the municipality and it causes considerable confusion among providers and clients alike. As to service delivery system Chongqing Municipality has a well-established FP service network that extends from the village to township to county to municipal level with a fairly well established referral network between FP service stations at different levels as well as the public health facilities. Differences in service capacity and facilities exist among the county level service stations and particularly among the township level. Some township service stations were poorly equipped housed and did not meet the government established criteria for service stations. All level of service stations commonly used metal IUD inserters. One county level facility utilized metal IUD inserters that had not been sterilized properly and were crusted with blood. FP service facilities usually offered all available contraceptive to married women while adolescents migrating women and men did not normally access services from these facilities. Providers did not routinely check for RTIs during IUD insertion and even those who were aware of the relation had limited technical competence to diagnose and treat RTIs including STDs. The village FP special cadres usually delivered the oral contraceptives (OCs) and condoms to clients. But they were not often aware of indication and contraindication for use OCs and service delivery sites usually did not provide these service. The major emphasis of training for providers in township level was related to IUD insertion and removal of IUDs sterilization and surgical abortion. Township level service station staff had received limited or no training on RTIs including STIs. Economic considerations are one factor that has motivated service stations to expand the range of reproductive and other health services they provide and trainings for the related areas should be emphasized. It is strongly recommended that the scientific working group meetings be convened to recommend a small number of the most safe and effective contraceptives to be made available through the national programme. The suggestion is that introduction of new fertility regulation methods will be undertaken in a planned systematic effort with attention to all of the managerial and service requirements to ensure appropriate quality of care. FP technical service should be better integrated with the prevention diagnosis and treatment (referral) of RTIs. The focus on information Education and Communication (IEC) should shift from policy and administrative management of the programme to methods oriented information to improve the quality of care to support informed choice. Significant gains in quality of care could be made by implementing service training for providers. It is suggested that the contents of training should focus on the concepts of reproductive health and quality of care informed choice the provision of information and counseling and review of basic knowledge and technical service provision for all available contraceptive methods. Training should be based on national standards updated service delivery guidelines and a set training curriculum that have been developed together as a package. Standards guidelines and training curricula should be based on best practices and scientific evidence. (authors)
