Abstract:Objective To explore the changing serum trends of garnma-glutamyl transferase (GGT),clinical features and prognosis of biliary atresia (BA) patients with preoperative low GGT level.Methods From 2012 to 2014,selected patients were divided into 3 groups of L (n=61,GGT:47-150 IU/L),M (n=48,GGT:150-1000 IU/L) and H (n=85,GGT:1000-2433 IU/L).The changes of GGT and enzymes were observed pre-operation and at 1,3,6 months post-operation respectively.Persistent jaundice within one year,liver transplantation,mortality or lost to follow-up were defined as poor prognosis;jaundice subsided within 1 year and survival with native liver were defined as good prognosis.Results The mean age of BA children with low preoperative GGT (115.7 ± 26.6 IU/L) was 60.6 ± 13.4 days,accounting for 7.3％ (61/830).At Month 1 after Kasai operation,GGT increased sharply in good prognosis group (GPG) and bad prognosis group (BPG).And elevated GGT value was higher in GPG than that in BPG (P＜0.001).At Month 3,GGT began to decrease in GPG while it continued rising in BPG.In group M,the mean age was 65.3 ± 21.7 days and the mean preoperative GGT was 535.5 ± 222.6 IU/L.At Month 1 post-operation,the GGT level increased both in GPG and BPG (P＜0.05).However,no significant difference existed in elevated value.In group H,the mean age was 70.0 ± 20.0 days and the mean GGT 1496.5 ± 446.4 IU/L.At Month 1 post-operation,the level of GGT decreased significantly in both GPG and BPG (P=0.014).The preoperative levels of ALT,AST and DBIL were higher in group L than those in another two groups.Conclusions Children with low GGT can not be excluded by the diagnosis of BA independently.High levels of liver enzymes and bilirubin hint at a better prognosis if GGT increases rapidly at Month 1 post-operation and subsequently decreases gradually.Children with high preoperative GGT may have a better prognosis than those with low/medium GGT.And the prognosis of those with medium preoperative GGT level is the worst.
Progonostic Value of Serum GGT in Infants with Intrahepatic Cholestasis

Abstract:Objective Most infants with intrahepatic cholestasis recover completely, only a small proportion of them progress to cirrhosis or death. Some author found that normal or low serum GGT in infants with idiopathic neonatal hepatitis is an indicator of poor prognosis. The aim of this study is to investigate the prognostic value of serum GGT in infants with intrahepatic cholestasis. Methods A retrospective study was conducted in 38 cases of infantile hepatitis, with whom the extrahepatic biliary atresia or other biliary abnormality had been excluded. Children with death, liver transplantation or waiting for liver transplantation, persistent or recurrent jaundice over 1 year of age were regarded as poor prognosis. According to the serum GGT levels at presentation, the 38 patients can be divided into two groups: ≤50 U/L and50 U/L groups. The prognoses were compared between this two groups. Results Patients with GGT≤50 U/L at presentation have poorer prognosis than those with GGT50 U(P=0.001). Poor prognosis was observed in 8 patients. The serum GGT level was normal in 5 of them during the whole follow up period. 2 of this 5 patients died within 1 year. Jaundice persistent or recurrent with persistent pruritus was found in the other 3. 3 of 8 patients who had elevated GGT at presentation, but dropped to normal with the clinical conditions deteriorated. In 72.4% of the cases with good prognosis, AST declined with jaundice when recovered. Levels of GGT reached its peak with the process of remission then dropped to normal in 79.3% patients with good prognosis.Conclusion GGT might be a prognostic parameter in Chinese intrahepatic cholestasis infants. Low level of GGT in infants with persistent jaundice might indicated failure of liver function. Progressive familial intrahepatic cholestasis may also existed in Chinese infants.
Low Gamma-Glutamyl Transpeptidase Levels at Presentation Are Associated with Severity of Liver Illness and Poor Outcome in Biliary Atresia.

Abstract:ObjectiveTo investigate the clinical features and prognosis of biliary atresia (BA) with normal or minimally elevated gamma-glutamyl transpeptidase (GGT).MethodsThe clinical data of patients with BA in our hospital between 2012 and 2017 were retrospectively studied. The patients were divided into a low-GGT group (GGT ≤ 300 IU/L) and a high-GGT group (GGT > 300 IU/L) according to the preoperative GGT level. The perioperative clinical parameters, the postoperative jaundice clearance within 6 months, and the 2-year native liver survival were compared among the groups.ResultsA total of 1,998 children were included in this study, namely, 496 in the low-GGT group and 1,502 in the high-GGT group. The ages and weights at the surgery in the low-GGT group were significantly lower than those in the high-GGT group (64.71 ± 21.35 vs. 68.64 ± 22.42 days, P = 0.001; 4.67 ± 1.03 vs. 4.89 ± 0.98 kg, P < 0.001). The levels of serum ALP, ALT, and AST in the low-GGT group were significantly higher than those in the high-GGT group before and 2 weeks after the surgery (ALP: 647.52 ± 244.10 vs. 594.14 ± 228.33 U/L, P < 0.001; ALT: 119.62 ± 97.14 vs. 96.01 ± 66.28 U/L, P < 0.001; AST: 218.00 ± 173.82 vs. 160.71 ± 96.32 U/L; P < 0.001). The INR of the low-GGT group was higher than that of the high-GGT group (1.05 ± 0.34 vs. 0.98 ± 0.20, P < 0.001), while FIB was lower than the high-GGT group (2.54 ± 0.67 vs. 2.73 ± 1.44 g/L; P = 0.006). The decreasing amplitude of TB and DB within 2 weeks after surgery in the low-GGT group was smaller than those in the high-GGT group (TB: 51.62 ± 71.22 vs. 61.67 ± 53.99 μmol/L, P = 0.003; DB: 33.22 ± 35.57 vs. 40.20 ± 35.93 μmol/L, P < 0.001). The jaundice clearance rate in the low-GGT group was significantly lower than that in the high-GGT group at 1, 3, and 6 months after surgery (17.70 vs. 26.05%; 35.17 vs. 48.58%; 38.62 vs. 54.64%, P < 0.001). In addition, the 2-year native liver survival rate in the low-GGT group was significantly lower than that of the high-GGT group (52.5 vs. 66.3%, P < 0.001 HR 1.80, 95% CI 1.38–2.33).ConclusionCompared to patients with high GGT, patients with normal or minimally elevated pre-operative GGT in BA were found to have poorer pre-operative liver function parameters, and post-operatively had lower jaundice clearance rates and worse 2-year native liver survival. This suggests a lower GGT at presentation in biliary atresia could be a sign of more severe liver injury.
Study of Gilbert's Syndrome-Associated UGT1A1 Polymorphism in Jaundiced Neonates of ABO Incompatibility Hemolysis Disease

Abstract:OBJECTIVE:This study aimed to assess the probable relationship between icter in neonates with ABO incompatibility hemolysis and UGT1A1 gene polymorphism. STUDY DESIGN:There were 65 ABO hemolytic disease of the newborn (HDN) neonates of full term in the study group and 82 non-ABO HDN neonates of full term in the compared group. We tested the UGT1A1 gene mutation of neonates of ABO HDN and non-ABO HDN. We compared the incidence of hyperbilirubinemia between neonates with and without UGT1A1 mutations in the ABO HDN and non-ABO HDN, to determine the relationship between icter in neonates with ABO HDN and UGT1A1 gene polymorphism. SPSS 13.0 were used to analyze those two groups' data. RESULTS:There was statistically significant difference of the serum bilirubin level between the Gly71Arg homozygous and no mutation group in the ABO HDN patients (p < 0.05). When hyperbilirubinemia was defined as serum bilirubin concentration >342 μmol/L, the incidence of hyperbilirubinemia between patients of UGT1A1 and non-UGT1A1 mutations in the ABO HDN group was significantly different (p < 0.05). But in the non-ABO HDN group, no significant difference was found. CONCLUSION:Individuals with Gly71Arg homozygous contributed to their hyperbilirubinemia in ABO HDN patients.
Correlations of Plasma Somatostatin or Vasoactive Intestinal Polypeptides Level and Serum γ-Glutamyltransferase Level in Infantile Cytomegalovirus Hepatitis

Abstract:Objective To investigate the correlations between plasma somatostatin(SST) or vasoactive intestinal polypeptides(VIP) level and serum gamma glutamyltransferase(γ-GT) in infants with cytomegalovirus(CMV) hepatitis and further to evaluate the diagnostic and differential diagnostic value of plasma SST and VIP level in persistent infantile jaundice. Methods A total of 60 infants with CMV hepatitis in Nanjing Children′s Hospital Affiliated to Nanjing Medical University from Aug.2008 to Dec.2009,clinical confirmed CMV hepatitis based on the following conditions:(1)Examination of CMV-DNA level in serum or urine and serum CMV-IgM level were positive,and the examinations of hepatitis A virus antibodies,hepatitis B virus antibodies,hepatitis C virus antibodies,hepatitis delta virus antibodies and hepatitis E virus antibodies were negative.(2)Serum alanine aminotransferase(ALT) level and serum direct bilirubin level(DB) were higher than those of healthy infants.According to the level of DB,42 infants with CMV hepatitis which meet condition(1) and condition(2) while without biliary atresia(BA) excluded by imaging examination were divided into 2 groups:cholestatic hepatitis Ⅰ group containing 22 cases with serum DB ≤136.8 μmol·L-1,and cholestatic hepatitis Ⅱ group containing 20 cases with serum DB>136.8 μmol·L-1.Eighteen infants with BA totally meet condition(1) and condition(2),and with BA proved by imaging examination.Normal control group contained 14 cases of healthy infants.The plasma SST or VIP concentration of infants with CMV hepatitis,and healthy infants(healthy control group)were measured by radioimmunoassay and the serum γ-GT level was measured by biochemistry method. Results Positive correlation was found between plasma VIP level and serum γ-GT level in healthy control group,cholestatic hepatitis Ⅰ group and cholestatic hepatitis Ⅱ group(r=0.383,P<0.05),while plasma SST had no correlation with serum γ-GT level in these 3 groups(r=0.180,P>0.05);Negative correlations were found between plasma VIP or SST level and serum γ-GT level in normal control group and BA group(r=-0.743,-0.918,P_a<0.01). Conclusions Plasma VIP level can be one of sensitive index of CMV infected cholestatic hepatitis,and plasma SST or VIP level can be a better complementation to the differential diagnosis of cholestatic hepatitis and BA.

Abstract:Objectives: The aim of the study was to analyze the value of gamma-glutamyl transpeptidase (GGT) for distinguishing biliary atresia (BA) from non-BA for patients suspected of having neonatal obstructive jaundice by correlation with age. Methods: From January 2003 to March 2014, cholangiography and/or surgical exploration were undertaken in 1469 patients with suspicion of having neonatal obstructive jaundice. A total of 1338 patients were diagnosed with BA intraoperatively. Preoperative medical records were compared between BA and non-BA patients. Results: Preoperative levels of total bilirubin, direct bilirubin, and GGT were significantly higher in the BA group (P < 0.05), whereas the non-BA group had higher alkaline phosphatase levels (P = 0.0003). The area under the receiver operating characteristic curve of total bilirubin, direct bilirubin, GGT, and alkaline phosphatase was 0.584, 0.614, 0.843, and 0.398, respectively. In all age groups (i: 31–60 days; ii: 61–90 days; iii: 91–120 days; iv: ≥121 days), BA groups had higher GGT levels (in IU/L) (i: 725.3 ± 549.9; ii: 927.0 ± 679.8; iii: 1114.3 ± 823.1; iv: 767.5 ± 666.7). The level of GGT in patients with BA younger than 30 days was 834.2 ± 475.3 IU/L. GGT levels had the highest diagnostic value (0.869) in group ii (61–90 days) and the lowest diagnosis value (0.712) in group iv (≥121 days). At a cutoff of >303 IU/L in group ii (61–90 days), GGT had 82.8% sensitivity and 81.6% specificity for the discrimination of BA. Conclusions: GGT levels contribute to the diagnosis of BA before 120 days. Age must be considered if using GGT levels as a diagnostic test for BA.
Association of Serum Gamma-Glutamyl Transferase with Treatment Outcome in Chronic Hepatitis B Patients

Abstract:AIM: To investigate the association of serum gamma-glutamyl transferase (GGT) levels with chronic hepatitis B infection and hepatitis B e antigen (HBeAg) seroconversion.METHODS: A retrospective study was performed on clinical data collected from patients who had been positive for hepatitis B surface antigen for > 6 mo and who were antiviral-treatment naive (n = 215) attending the Hepatitis Clinic at Nanjing Drum Tower Hospital between August 2010 and December 2013. Healthy individuals without liver disease (n = 83) were included as controls. Patients were categorized into four groups based on disease status as recommended by the European Association for the Study of the Liver: immune tolerance (IT; n = 47), HBeAg-positive hepatitis (EPH; n = 93), HBeAg-negative hepatitis (ENH; n = 20), and inactive carrier (IC; n = 55). Prediction of complete response (CR) based on serum GGT was also examined in EPH patients (n = 33) treated for 48 wk with nucleos(t)ide analogue (NA) therapy, including lamivudine plus adefovir combination therapy (n = 20) or entecavir monotherapy (n = 13). CR was defined as a serum hepatitis B virus DNA level < 500 copies/mL and HBeAg seroconversion by 48 wk of treatment.RESULTS: Serum GGT levels were significantly increased in EPH and ENH patients relative to the IT, IC, and healthy control groups (P < 0.01 for all). However, no significant difference in serum GGT levels was found between the EPH and ENH groups. Baseline serum GGT levels were significantly higher in patients who achieved CR (7/33; 21.2%) compared to patients in the non- CR group (26/33; 78.8%; P = 0.011). In addition, the decline in serum GGT was greater in CR patients compared to non- CR patients after 24 wk and 48 wk of treatment (P = 0.012 and P = 0.008, respectively). The receiver operating characteristic curve yielded a sensitivity of 85.71% and a specificity of 61.54% at a threshold value of 0.89 times the upper limit of normal for baseline serum GGT in the prediction of CR following NA therapy.CONCLUSION: Serum GGT is significantly elevated in EPH and ENH patients and is a potential biomarker for the prediction of HBeAg seroconversion following NA therapy.

Abstract:Summary The clinical usefulness of serum γ‐glutamyl transpeptidase (γGT) assay for the diagnosis of liver disease in children was assessed retrospectively in 398 children investigated from 1981 to 1986, in whom diagnosis was ascertained according to currently accepted criteria including liver histology in each case. Serum γGT activity was within normal limits in 10 controls, in 19 children with portal vein obstruction, and in 10 of 12 children with congenital hepatic fibrosis. Serum γGT was raised in all children with biliary atresia, sclerosing cholangitis, paucity of interlobular bile ducts, and α 1 ‐antitrypsin deficiency with jaundice. Serum γGT was normal in spite of patent clinical signs of cholestasis in 3 patients with benign recurrent intrahepatic cholestasis, 1 infant with posthemolytic neonatal cholestasis, and in 22 of 28 patients with progressive idiopathic cholestasis akin to Byler disease. In the latter group, children with raised serum γGT displayed extensive portal fibrosis and bile duct proliferation on liver histology, while this was not a prominent feature in children with normal serum γGT. These results indicate (a) the value and limits of the assay for serum γGT activity in children with liver disease, (b) that raised serum γGT may be considered a fairly reliable index of bile duct damage, and (c) that serum γGT may prove a useful tool in separating two forms of progressive idiopathic cholestasis, with or without bile duct involvement.

Glutamine Supplementation in Preterm Infants Receiving Parenteral Nutrition Leads to an Early Improvement in Liver Function

Abstract:Objective: The aim of study was to confirm the protective effects of parenteral glutamine supplementation on liver injury in premature infants and determine how quickly effects became evident. Methods: We performed a double-blind, randomized, controlled clinical study to assess the effect of parenteral nutrition (PN) supplemented with glutamine in premature infants. Thirty infants from two children's centers, were randomly assigned to either a control group (Standard PN; n=15) or a glutamine-supplemented group (GlnPN; n=15). The primary endpoint was hepatic function. The secondary endpoints were total duration of PN, weight and head circumference gain, length of hospitalization, and days on a ventilator. Results: The serum level of alkaline phosphatase (AKP) after parenteral nutrition for 14 days was significantly higher (p<0.05) in the control group. But in the glutamine-supplemented group, the serum concentration of aspartate aminotransferase (AST) and gamma glutamyltransferase (GGT) significantly decreased after PN for 7 days and 14 days (p<0.05), and the level of alkaline phosphatase (AKP) showed no increase. The levels of AKP and GGT were significantly different with time by group interaction. Levels of AKP was higher in control group than glutamine-supplemented group, and GGT level was lower in glutamine-supplemented group compared with controls. There were no significant differences between the groups in terms of total duration of PN, weight gain (g/d), increase in head circumference (cm/w), length of hospitalization, and duration of mechanical ventilation. Conclusion: The longer the duration of parenteral nutrition, the more severe hepatic dysfunction became. Parenteral glutamine supplementation suggested a hepatoprotective effect.
Protective Effect of Parenteral Glutamine Supplementation on Hepatic Function in Very Low Birth Weight Infants

Abstract:Background & aims: Hepatic dysfunction is one of the most frequent complications of parenteral nutrition. Very low birth weight (VLBW) infants are more sensitive to liver injury due to physiological immaturity. Our studies in animals showed that glutamine supplementation could attenuate TPN-associated liver injury. The aim of study was to investigate whether parenteral glutamine supplementation can improve hepatic tolerance in VLBW infants.Methods: We performed a double-blind, randomized, and controlled clinical study to investigate whether parenteral glutamine supplementation can improve hepatic tolerance in VLBW infants. Thirty VLBW infants at two children's centers were randomly assigned to either a control group or a glutamine-supplemented group. The primary endpoints were hepatic function and mortality. The secondary endpoints were the time to achieve full enteral nutrition, episodes of gastric residuals, duration of parenteral nutrition, weight and head circumference gain, length of hospitalization, and days on ventilator.Results: The serum levels of aspartate aminotransferase (AST) and total bilirubin (Tbi) were decreased after PN in the glutamine-supplemented group (P < 0.05). No deaths occurred in this study. Four infants assigned to the control group and two infants in the glutamine-supplemented group were withdrawn from the study, according to intention to treat: relative risk [RR]: 1.182: 95% confidence interval [CI]: 0.937-1.490.Conclusions: Parenteral glutamine supplementation can improve hepatic tolerance in very low birth weight infant, suggesting a hepato-protective effect. Abstract:Background: This study was designed to explore the mechanisms in parenteral nutrition (PN)-associated hepatic dysfunction, and the possible effectiveness of glutathione (GSH) to alleviate this,injury. Methods: Thirty 1-week-old New Zealand rabbits were divided into 3 groups: 10 in the control group (maternal fed); 10 in the PN group (PN for 10 days); and 10 in the GSH + PN group (PN plus glutathione for 10 days). At the end of the study, blood biochemistry analysis and liver histologic examination were performed by light and electronic microscope; malondialdehyde (MDA) content of liver tissues and apoptotic hepatocytes were also measured. Results: Direct bilirubin and bile acid in the PN group were significantly higher than that in the control group and in the GSH + PN group (p <.05, for both). In the PN group, there were some cholestatic or steatotic changes. In the GSH + PN group, histologic changes were reduced compared with the PN group. The electron microscopy appearances were in agreement with the histologic findings. MDA value was higher in the PN group than in the control group and in the GSH + PN group (p <.05, respectively). Terminal deoxynucleotidyl transferase mediated nick end labeling (TUNEL) assays showed that the rate of apoptotic hepatocytes in the PN group was the highest and the control group was the lowest among 3 groups (comparison between groups, p <.0 1, individually.) Conclusions: The study showed that PN can induce hepatic dysfunction in infant rabbits. GSH can effectively reduce this injury. The study implies that oxidative stress and apoptosis contribute to PN-associated hepatic dysfunction.

Abstract:To investigate bilirubin UDP-glucuronosyltransferase (UGT1A1) gene allele in healthy Chinese neonates, their cord bilirubin level and the subsequent hyperbilirubinemia to determine relationships among them.Cord blood of 48 neonates was obtained to determine the exon 1 of UGT1A1 gene, total serum bilirubin, albumin, glutamic-pyruvic transaminase (GPT), glutamic-oxalacetic transaminase (GOT) and haemoglobin (Hb) concentration. Neonatal jaundice was assessed by measurement of transcutaneous bilirubin (TCB) and serum bilirubin. Neonates were divided into two groups according to mutant or normal allele to compare the variables.Nineteen infants had the nucleotide 211 G-->A allele, 3 had the heterozygous variation (686C-->A, 845 A-->T, 231G-->A). In the 211 A allele group, cord bilirubin was significantly higher than in the 211 G allele group (p = 0.034), but there were no differences in albumin (p = 0.678), GPT (p = 0.460), GOT (p = 0.440) and Hb (p = 0.886). The TCB (at 48, 96 h), the frequency of the hyperbilirubinemia and prolonged jaundice were also significantly higher in the 211 A allele group.The UGT1A1 gene codon G71R allele is a risk factor for neonatal hyperbilirubinemia in the Chinese population. Its effect on bilirubin metabolism may present early on, as well as late in foetal life.

Abstract:Objective To explore the clinical significance of level variation of insulin-like growth factor-1(IGF-1) and adiponectin (APN) in neonates with hypoxic-ischemic encephalopathy(HIE).Methods A total of 88 term neonates were enrolled.The cord blood at birth,peripheral venous blood at postnatal 3-5 days(acute period) and postnatal 10-14 days(recovery period) were collected to detect the serum levels of APN and IGF-1.All neonates were grouped when aged one month according to the diagnosis .And 20 healthy term neonates were taken as control group.Another 68 term neonates with HIE were divided into three subgroups including severe HIE group (n=20), moderate HIE group(n=25) and mild HIE group(n=23).The levels of APN and IGF-1 were compared among four groups.And the correlation of APN level with IGF-1 level was analyzed in different stages of neonates with HIE .Results There were significant differences in APN and IGF-1 levels among four groups(P<0.05).The levels of APN and IGF-1 were lower in the severe HIE group or moderate HIE group compared to those in the control group(P<0.05),but no significant difference was observed in the level of APN or IGF-1 between mild HIE group and control group (P<0.05).The APN and IGF-1 levels in the severe HIE group were lower than those in the moderate group,and the levels in the moderate HIE group were lower than those in the mild HIE group (P<0.05).There were significant differences in the levels of APN and IGF-1 at each time point among four groups (P<0.05).Positive correlation was observed between APN and IGF-1 levels in the neonates with HIE at birth and during the acute period (P<0.05),but no correlation between APN and IGF-1 levels was observed during the recovery period (P>0.05).Conclusion The level variation of APN and IGF-1 is closely associated with the occurrence and progression of neonatal HIE,and has suggestive effects on the clinical diagnosis and disease progression .

Abstract:Objective To explore the effect of neonatal total bilirubin (TBIL) on hepatic function, renal function and myocardial enzyme. Methods From August 2013 to July 2014, 1 303 neonates were selected from the obstetrical department. TBIL, alanineaminotransferase (ALT), aspartateaminotransferase (AST), aspartate transaminase (ALP), glutamyl transpeptidase (GGT), albumin (ALB), blood urea nitrogen (BUN), uric acid (UA), creatinine (Cr), lactate dehydrogenase (LDH), creatine kinase (CK) and hydroxybutyrate dehydrogenase (HBDH) were measured. The total of 1 303 neonate were divided into 3 groups: control group (n=606), mild jaundice group (n=428) and moderate/severe jaundice group (n=269), and the indexes of hepatic function, renal function and myocardial enzyme between groups were compared. Results Statistically insignificant difference of ALT, AST, BUN and UA between groups were detected (F=0.58, P=0.56; F=0.73, P=0.33; F=2.00, P=0.14; F=0.23, P=0.80), while the difference of ALP, GGT, ALB, Cr, CK, HBDH and LDH between groups were statistically significant (F=3.88, P=0.02; F=10.03, P<0.01; F=15.20, P<0.01; F=4.94, P<0.01; F=5.84, P<0.01; F=14.67, P<0.01; F=12.98, P<0.01). The measurements of ALP, GGT and ALB among moderate/severe jaundice group was statistically higher than those among control group (P<0.01), and the measurements of HBDH and LDH among mild and moderate/severe jaundice group were statistically higher than those among control group (P<0.01), and the measurements of GGT, HBDH and LDH among moderate/severe jaundice group were statistically higher than those among mild jaundice group (P<0.01); while the measurements of Cr and CK among mild and moderate/severe jaundice group were statistically lower than those among control group (P<0.01). Conclusions The neonatal hyperbilirubinemia has impairment effect on hepatic function, renal function and myocardium; earlier therapy and intervention should be taken in order to protect hepatic function, renal function and myocardium from impairment.
Clinical characteristics of progressive familial intrahepatic cholestasis cases with low γ-glutamyltransferase in 23 children

Abstract:Objective To analyze the clinical manifestations of various types of progressive familial intrahepatic cholestasis(PFIC) characterized by low γ-glutamyltransferase(GGT).Methods Children were clinically diagnosed as PFIC with low GGT in 2004-2007.Clinical symptoms,laboratory tests and liver histopathological changes were retrospectively analyzed.According to ATP8B1 and ABCB11 gene exon sequencing results,they were classified into three groups,PFIC-Ⅰ,PFIC-Ⅱ and unclassified groups.The differences among them were compared.Results A total of 23 cases were born with normal birth weight and the clinical manifestations of onset were jaundice,pruritus and liver function damage.The median age of onset was 40 days and the median treatment age was 6 months.Thirteen cases(56.5%) appeared white bole stool,3 cases with significant diarrhea and 6 cases with malnutrition,rickets,growth retardation.Two cases in the follow-up were found to have gallstones.Liver biopsy pathology showed non-inflammation or mild inflammation of liver cells,non-fatty degeneration of liver cells,multinucleated giant cells transformation and cholestasis of liver cells or obvious cholangiole ducts.Patients in PFIC-Ⅰ,PFIC-Ⅱ and unclassified groups were 9,7 and 7 cases respectively.Clinical symptoms of the unclassified group were the most severe among three groups.Clinical symptoms of patients in PFIC-Ⅱ group were relatively severe,with significantly elevated levels of serum ALT and total bile acid(TBA) than those in PFIC-Ⅰ group,however with lower GGT levels than those in PFIC-Ⅰ group.There was significant difference among three groups in hepatic multinucleated giant cell transformation pathologically(P<0.01).PFIC-Ⅰ group showed no hepatic multinucleated giant cell transformation.Conclusions PFIC-Ⅰ group shows no hepatic multinucleated giant cell transformation.The levels of serum ALT and TBA in PFIC-Ⅱ group elevate significantly and are higher than those in PFIC-Ⅰ patients.These results are helpful to the clinical classification and treatment of the disease.

Abstract:Background Acute kidney injury(AKI)after cardiopulmonary bypass(CPB)is a common complication especially in pedi-atric population.Plasma gelsolin(pGSN)is an anti-inflammatory factor through binding with actin and pro-inflammatory cytokines in circulation.Decrease in pGSN has been reported in some pathologic conditions.The purpose of the study was to determine the alterations of pGSN level in infants and young children after CPB and the role of pGSN as a predictor for the morbidity and severity of post-CPB AKI.Methods Sixty-seven infants and young children at age≤3 years old undergoing CPB were prospectively enrolled.PGSN levels were measured during peri-operative period with enzyme-linked immuno-sorbent assay and normalized with plasma total protein concentration.Other clinical characteristics of the patients were also recorded.Results In patients developing AKI,the normalized pGSN(pGSNN)levels significantly decreased at 6 h post-operation and remained low for 24 h post-operation as compared to the patients with non-AKI.PGSNN at 6 h post-operation combining with CPB time presents an excellent predictive value for AKI.Conclusions Decreased pGSNN identifies post-CPB AKI in the patients≤3 years old,and is associated with adverse clini-cal outcomes.The findings suggest that circulating GSN in post-CPB patients may have beneficial effects on diminishing inflammatory responses.
Detection and clinical significance of plasma somatostatin in infants with cytomegalovirus hepatitis

Abstract:Objective To determine plasma somatostatin(SST)concentration in infants with cytomegalovirus(CMV)hepatitis,and investigate the correlation between plasma SST and liver injuries,cholestasis,biliary atresia.Methods Sixty-two infants with CMV hepatitis and 15 children with breast milk jaundice admitted in Nanjing Children's Hospital from August 2008 to May 2009 were recruited,and 14 healthy infants were studied as control.The patients were clinically diagnosed with CMV hepatitis based on the following conditions:CMV-DNA level reached diagnostic value in serum or urine,serum CMV-IgM was positive,and antibodies of hepatitis A,B,C,D,and hepatitis E virus was negative.Serum alanine aminotransferase(ALT)level and serum direct bilirubin level(DB)were higher than those of healthy infants.The patients were divided into 4 groups:hepatitis group(n = 13,including 8 male and 5 female),cholestasis group(n = 15,including 8 male and 7 female),hepatitis and cholestasis group(n = 20,including 11 male and 9 female),and biliary atresia group(n = 14,including 7 male and 7 female).Plasma SST concentration was measured by radioimmunoassay.Results Plasma SST concentration was higher in hepatitis group(55.52 ± 4.32 ng/L),and lower in cholestasis group(13.29 ± 3.54 ng/L)than that in normal control group(25.40 ± 4.23 ng/L)(P < 0.01,P < 0.05,respectively).The difference between breast milk jaundice group and normal control group was not significant(24.10 ± 7.90 ng/L vs 25.40 ± 4.23 ng/L,P > 0.05).The concentration of plasma SST was significantly higher in hepatitis and cholestasis group(49.24 ± 10.41 ng/L),but lower in biliary atresia group(18.19 ± 4.03 ng/L),compared with normal control group(P < 0.01,P < 0.05,respectively).Conclusions This study suggested that elevated plasma SST level might reflect liver injury,and decreased plasma SST level may be closely correlated with increased serum DB level.
Non-ALT Biomarkers for Markedly Abnormal Liver Histology among Chinese Persistently Normal Alanine Aminotransferase-Chronic Hepatitis B Patients.

Abstract:AIM:To determine incidence and clinical biomarkers of marked necroinflammation and fibrosis characteristics among chronic hepatitis B (CHB) patients with persistently normal alanine aminotransferase (PNALT).METHODS:Liver biopsy was performed on 115 CHB patients with PNALT. Necroinflammation and fibrosis were graded by the Knodell histologic activity index and the Ishak fibrosis score, respectively. Correlations between the available clinical parameters and necroinflammation and fibrosis were analysed.RESULTS:Marked necroinflammation (Knodell activity index ≥ 7) and fibrosis (Ishak fibrosis score ≥ 3) were found in 36.5% and 15.5% of CHB patients with PNALT, respectively. Following a univariate logistic regression analysis, multiple logistic regression analysis indicated that aspartate transaminase (AST) (AUROC = 0.852, cut-off value = 22.5 U/L) serves as an independent predictor of notable liver inflammation, while platelet (PLT) count (AUROC = 0.905, cut-off value = 171.5 ×109/mL) and gamma-glutamyl transpeptidase (GGT) (AUROC = 0.909, cut-off value = 21.5 U/L) level serve as independent predictors of notable liver fibrosis.CONCLUSION:A considerable proportion of marked histological abnormalities existed in our cohort, who will benefit from optimal therapeutic strategies administered according to predictive indication by AST, PLT and GGT levels.

Abstract:Introduction: The level of amniotic fluid gamma-glutamyl transferase (AFGGT) may help identify biliary atresia (BA) in cases of non-visualisation of the fetal gallbladder (NVFGB). This study aimed to validate a serum/plasma matrix–based gamma-glutamyl transferase (GGT) assay for amniotic fluid (AF) samples, establish a local gestational age–specific AFGGT reference range, and evaluate the efficacy of AFGGT for predicting fetal BA in pregnancies with NVFGB using the constructed reference range. Methods: The analytical performance of a serum/ plasma matrix–based GGT assay on AF samples was evaluated using a Cobas c502 analyser. Amniotic fluid gamma-glutamyl transferase levels in confirmed euploid singleton pregnancies (16 +0 to 22 +6 weeks of gestation) were determined using the same analyser to establish a local gestational age–specific reference range (the 2.5th to 97.5th percentiles). This local reference range was used to determine the positive predictive value (PPV) and negative predictive value (NPV) of AFGGT level <2.5th percentile for identifying fetal BA in euploid pregnancies with NVFGB. Results: The serum/plasma matrix–based GGT assay was able to reliably and accurately determine GGT levels in AF samples. Using the constructed local gestational age–specific AFGGT reference range, the NPV and PPV of AFGGT level <2.5th percentile for predicting fetal BA in pregnancies with NVFGB were 100% and 25% (95% confidence interval=0, 53), respectively. Conclusion: In pregnancies with NVFGB, AFGGT level ≥2.5th percentile likely excludes fetal BA. Although AFGGT level <2.5th percentile is not diagnostic of fetal BA, fetuses with AFGGT below this level should be referred for early postnatal investigation.

Elevated serum gamma-glutamyltransferase predicts advanced histological liver damage in chronic hepatitis B.

Abstract:Recently, several studies have demonstrated that serum gamma-glutamyltransferase (GGT) is associated with some diseases, such as chronic hepatitis B (CHB). This study investigates the association between GGT levels and liver pathological grading in patients with CHB. 300 patients with CHB who underwent liver biopsy were enrolled. Histological assessment was based on the Scheuer scoring system. Univariate and multivariate analyses were performed to evaluate the independent predicting factors for the presence of liver pathological grade in patients with CHB. In patients with CHB, the mean GGT level was 44.14 +/- 3.69 (U/L) in low activity group and 114.87 +/- 15.75 (U/L) in the high activity group (p<0.001). Also, there was significant difference between the low and high fibrosis group with regard to GGT levels [45.32 +/- 4.64 (U/L) vs. 90.41 +/- 11.06 (U/L), p<0.001, respectively]. The variables that were significant in the univariate analysis were evaluated in multivariate logistic regression analysis, and GGT was an independent predicting factor of necroinflammation and fibrosis (OR=1.007, 95% CI: 1.001-1.014, p=0.030; OR=1.009, 95% CI: 1.003-1.014, p=0.003, respectively). Results of this study suggest that GGT is a new non-invasive marker that can be used to predict advanced histological liver damage.
