Preliminary Clinical Results of Small Incision Minimally Invasive Surgery by Stages in the Treatment of Early Onset Scoliosis

Abstract:Objective To observe the preliminary clinical results and safety of small incision minimally invasive surgery in the treatment of early onset scoliosis(EOS).Method Twenty-six EOS patients including 8 males and 18 females with the age of 8.5 ± 1.2 were treated with posterior small incision minimally invasive staging surgery.The average preoperative major curve Cobb angle was(98.0 ± 15.4)° and the average preoperative kyphosis Cobb angle was(38.8 ± 14.7)°.The first stage posterior surgery was performed by 2 small incisions minimally invasive technique and corrected the deformity by 55%.Then another similar surgery was performed 6-12 months later.Final fusion and thoracoplasty surgery were performed when the patients got skeletal mature.Results All patients underwent the surgery safely and there were no severe complications.Sixteen cases were completed by only first stage surgery and 10 cases completed with second stages of the surgery.The postoperative major curve(Cobb angle) after first surgery was(42.2 ± 10.1)° and the correction rate was 56.9%,while the major curve(Cobb angle) after the second surgery was(35.8 ± 6.6)° and the correction rate was 34.1%.The overall correction rate was 65.4%.Conclusion Preliminary study on small incision minimally invasive staging surgery in the treatment of EOS has proved that the staging surgery can improve the correction rate,delay the final fusion therefore reduce the impact of early fusion on the growth and development of spine.
