Abstract:We estimated seroincidence of HIV, hepatitis B virus (HBV) and hepatitis C virus (HCV), and the prevalence of risk behaviors among injection drug users (IDUs) who accepted inpatient detoxification by 14-day methadone tapering treatment in the Shanghai Drug Abuse Treatment Center. We also evaluated the effect of an HIV/AIDS prevention education intervention on those IDUs. Data including demographic characteristics, HIV, HBV and HCV seroincidence, sexual and injection-related risk behaviors were collected from 101 IDUs. All subjects received HIV/AIDS prevention education during inpatient detoxification treatment. An HIV-knowledge questionnaire was used to evaluate the effects of this intervention. We found that risk behaviors, including unsafe sex and unclean injection practices, were common among the subjects. The seroincidence of HBV and HCV infection rates was 56.4% and 46.5%, respectively, but no HIV-infected case was found among the subjects. After participating in the HIV/AIDS prevention intervention, subjects’ scores (M±SD) on the HIV-knowledge questionnaire were significantly improved from baseline (97.29±7.42 vs. 31.1±12.1). Our study confirmed that IDUs in Shanghai are a high-risk population for blood borne diseases such as hepatitis B and hepatitis C and HIV. HIV/AIDS prevention education increased HIV knowledge, improved understanding of HIV prevention methods and changed attitudes toward HIV/AIDS. Therefore, HIV/AIDS prevention education should to be an important component of drug treatment.

Abstract:To analyze the risk factors, particularly sexual behaviors, associated with HIV infection, and to describe the risks for HIV transmission among male injecting drug users (IDUs) in China.A cross-sectional study of 314 IDUs in Yunnan Province was conducted. Information on demographics, HIV serostatus, and sexual and drug-using behaviors was collected.HIV prevalence among the study subjects was 59.9%. HIV infection was associated with older age (> or = 27 years), early drug initiation (at < or = 20 years of age), and frequent injection (> or = once a day). Thirty-seven percent reported multiple sexual partners. Consistent condom use rates were lowest with regular partners (23.8%), followed by 42.5% with casual partners, and 57.3% with female sex workers. Ninety-eight percent of subjects received high 'HIV knowledge' scores. Few of the subjects who needed medical care sought it out.Despite awareness of HIV, needle sharing and unprotected sex persist in the population, and the HIV prevalence is high. Further interventions should not only seek to educate but also to reduce high-risk behaviors.

Abstract:Objective To study HIV transmission risk factors among in jection drug users (IDUs) in Lianshan of Sichuan province. Methods Community-based survey was conducted to investiga te high risk behavior factors of HIV transmission and to test HIV antibody among IDUs.Results The HIV prevalence was 11.3%(43/379) among IDUs.I n multivariate Logistic regression model( P 0.05),the OR of sharing times of injection equipment in the past was 2.034.In another multivariate Logistic regre ssion model ( P 0.10),sharing times of injection equipment in the past and ha ving new sex partners in the last six months were risk factors of HIV transmissi on,respectively. Conclusion Health education and behavioral intervention t argeted toward both injection drug use and high risk sexual behavior were urgent ly needed to reduce HIV transmission.

Abstract:Objective To investigate high-risk behaviors among HIV patients in Pudong,Shanghai,analyze the features of high-risk behaviors and provide suggestions for improving measures in controlling the spread of HIV among the populations.Methods A cross-sectional survey was conducted to collect information on social and demographic characteristics,sex partners,high-risk behaviors,etc.All newly identified HIV patients were investigated face to face and one by one from January 2011 to June 2012.Results Among 249 subjects investigated,212 were males and 37 were females,with a sex ratio of 5.7:1.Of them 22.1%,22.1%,22.9% and 7.6% admitted having sex with spouses,regular sex partners,casual sex partners and commercial sex workers,respectively and the rate of consistently using condoms was 12.7%,16.4%,5.2% and 0,respectively in the recent three months.And 41.4%,36.1%,59.4% and 28.5% of the subjects admitted having sex with spouses,regular sex partners,casual sex partners and commercial sex workers respectively before and in the last three months and the rate of consistently using condoms was 5.8%,6.7%,1.3% and 4.2% respectively.Of the spouses and regular sex partners of the subjects,32.1%(25/78) were tested HIV positive.And 4.4%,3.2% and 5.6% of the subjects admitted having intravenous drug use,surgical operations and a history of blood donation.Conclusion Newly found HIV patients in Pudong District have various types of sex partners,the condom use rate is very low and HIV infection rate among their spouses and regular sex partners is high.More targeted measures should be taken in controlling the spread of HIV in these populations.

Abstract:Background Currently, human immunodeficiency virus (HIV) sentinel surveillance among men who have sex with men (MSM) in China conducted in large and medium-sized cities, and no HIV sentinel surveillance conducted in rural areas. HIV testing and intervention is predominantly conducted in urban areas, there have been a limited number of studies in rural areas MSM, it is necessary to conduct the investigation of HIV risk sexual behavior, HIV testing among rural and urban MSM. Method Between December 2013 and August 2015, a cross-sectional study was conducted in rural and urban areas in Zhejiang Province using respondent-driven sampling (RDS). Participants completed face-to-face interviewer-administered questionnaire surveys and were tested for HIV. Results A total of 710 MSM participants were recruited, of whom 36.1% were from rural areas. The overall HIV prevalence was 16.6%, and was considerably lower among MSM living in rural areas (3.9%) than those living in urban areas (24.2%). 61.1% participants had not condom use with male sexual behavior in the past 6 months (86.7% in rural areas and 46.7% in urban areas). The social demographic and behavioral characteristics had significance difference among rural and urban MSM. Multivariate logistic regression revealed that, compared to men living in urban areas, MSM living in rural areas MSM were more likely to use dating apps to find sexual partners, were more likely to engage in bisexual behavior, and had lower condom use. 43.0% participants had been tested for HIV in the past year (41.8% in rural areas and 43.6% in urban areas). Multivariate logistic regression also revealed that among participants living in rural areas, having rural health insurance and not accepting HIV intervention were associated with lower HIV testing rates, while a higher monthly income and through use of internet to find sexual partner were associated with higher rates of HIV testing. Conclusion High risk behavior was prevalent, and HIV testing rates were low among MSM living in rural areas compare to urban areas in Zhejiang Province, therefore, preventative intervention measures should be immediately among rural MSM urgently to reduce HIV transmission and to promote HIV testing.

Abstract:Goal. The goal of this study was to investigate the prevalence and different transmission risk factors of HIV among intravenous drug users (IDUs) in the Liangshan Yizu Autonomous Region of Sichuan province, China. Methods: From November 8, 2002, to November 29, 2002, a community-based survey was conducted to investigate demographic characteristics, sharing patterns of injection equipment, frequencies of injection equipment, and sexual behaviors in IDUs. Blood samples were also collected to test for HIV and syphilis infection. A total of 379 subjects were recruited for the study through community outreach and peer recruiting methods. Results: Of 379 IDUs, HIV prevalence was 11.3% (43 of 379). Ethnicity, total times of shared injection equipment in the past 3 months, shared cotton in the past 3 months, and syphilis infection were associated with HIV infection after univariate analysis using the chisquared test. Multivariate logistic regression analysis showed total times of sharing injection equipment in the past 3 months (odds ratio [OR], 2.28; 95% confidence interval [CI], 1.18-4.43) and syphilis infection (OR, 3.10; 95% CI, 1.48-6.48) in the final model were independently associated with HIV prevalence. Conclusion: The results of this study suggested total times of sharing injection equipment in the past 3 months and syphilis infection were significantly associated with HIV infection. Further seroepidemiologic prospective cohort studies should be conducted to clarify the impact of syphilis and high-risk sexual behaviors on HIV transmission through unprotected sexual intercourse.
Factors Associated with Recent Risky Drug Use and Sexual Behaviors among Drug Users in Southwestern China.

Abstract:A cross-sectional survey was conducted in 2007 among 504 drug users who were recruited mainly from detoxification centers in southwest China. About one-third (34.3%) of participants reported recent risky drug use behavior, which was defined as injecting drugs in the past 3 months, and more than one-fifth (21.6%) reported recent risky sexual behavior, or had multiple sexual partners in the past 30 days. Male sex (odds ratio [OR], 1.9; 95% confidence interval [CI], 1.2-3.2) and more episodes of detoxification (OR, 3.7; 95% CI, 2.3-6.0) were associated with higher odds of risky drug use behavior, while unmarried status (OR, 1.7; 95% CI, 1.0-2.9), higher personal annual income (OR, 1.8; 95% CI, 1.1-2.8) and history of sexually transmitted infections (OR, 3.7; 95% CI, 2.1-6.6) were associated with higher odds of having risky sexual behavior. Subgroup analyses showed 15% participants who used drugs in the past 3 months also shared needles, and 77% participants who had multiple sexual partners in the past 30 days did not use condoms during sex with non-primary sexual partners. The study findings are useful for developing HIV risk reduction intervention programs among drug users.
Risk Behaviors of AIDS among Female CSW and Also IDU

Abstract:Objective To compare the differences of HIV-related risk behaviors and demographic characteristics among sex workers who were injecting drug users,female drug users who never engaged in sex exchange and low-grade sex workers,so as to provide a scientific basis for the development of effective interventions.Method Sichuan Province integrated AIDS monitoring data from 2003 to 2005 were collected and studied,and sex workers who injected drug were the main research objects.CSW or IDU were surveyed by questionnaire.Result By comparing,female drug users who exchanged sex for drugs or money were more dangerous on injecting drug use and sex behavior.From 2003-2005,37.0%-47.1% of femal IDU had commercial sex behavior and their condom use rate was 15%-20% lower than that of low-grade CSW,and there were significant differences(P0.05).Conclusion HIV high-behaviors in the females who have a double identity on injecting drug and exchanging sex are more universal.Effective behavior intervention for these people will become an important mission on the areas where drug abuse are serious.

Abstract:Objectives: To investigate, among male injecting drug users (IDU), the relationships between higher risk IDU-related and sex-related behaviors. Goal: To highlight the clustering effects of the 2 forms of risk behaviors. Study Design: Behavioral surveillance surveys conducted in Sichuan were analyzed (n = 3105). Results: Of all respondents, 35.7% had injected with others' used syringes in the last 6 months, which was associated with female sex workers (FSW) and nonregular sex partners (NRP) partnership (OR = 1.81 and 1.59 respectively), and having had unprotected sex with FSW, NRP, and regular sex partners (OR = 1.96, 1.57, and 1.38 respectively). Higher drug use frequency was associated with having unprotected sex with FSW (among client of FSW, OR = 1.7) and with NRP (among those with NRP, OR = 1.61). Giving used syringes to others for injection was also associated with unprotected sex with NRP (OR = 1.97). Conclusions: Interventions are urgently required to reduce both IDU-related and sex-related risk behaviors among male IDU.

Abstract:Objective:To assess the syphilis prevalence and its risk factors among injection drug users (IDUs) in Beijing. Methods:An investigation on the demographic characteristics,drug use and sexual behavior,knowledge of HIV/AIDS and related behavior of 232 IDUs who were recruited from Beijing compulsory detoxification centers and communities was conducted with an investigator administered anonymous questionnaire from May,2001 to June,2002. Blood samples were collected and tested for leptospira syphilis after the questionnaires were filled. Results:The syphilis prevalence among IDUs in Beijing was 5.17% and female was the risk factor. Conclusion: The syphilis infection rate among IDUs in Beijing is higher than that of normal population,and its risk factor is female,which indicates that high-risk sexual behaviors should be taken into account in the development of intervention targeting this population,especially for females.

Abstract:BACKGROUND:China is currently facing a rapid and widespread increase in human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS). The activities of female sex workers (FSWs) have contributed to the mounting epidemic of HIV/AIDS and other sexually transmitted diseases (STDs). Therefore, this study aimed to assess the HIV/AIDS-related knowledge, attitude and risk behaviors among FSWs operating in Shanghai China.METHODS:A cross-sectional study was conducted in five districts of Shanghai, including three suburbs and two downtown locales. We adopted a cluster randomized sampling method to obtain ten geographic sites which consisted of one or more communities/villages proximal to a location where FSWs were accessible. A total of 324 FSWs from 109 Xitou Fang, massage parlors and hair salons who explicitly provided sexual services were enrolled in the study. Each participant completed a questionnaire survey and interview aimed to collect information on the individual's knowledge, attitude, and behaviors associated with risk for HIV/AIDs.RESULTS:The overall correct answer rate of HIV/AIDS-related knowledge was 60.8%, and the knowledge of FSWs from downtown areas was significantly higher than those from suburban areas (P < 0.05). The percentage of FSWs who reported having experiences in commercial sexual services without the use of condoms was 33.6%. Condom slippage or breakage was reported as having occurred at least once by 51.2% of the FSWs. FSWs from suburban areas were found to more often engage in high-risk behaviors, including oral and anal sex, than those from downtown areas (P < 0.001). Many of the FSWs (65.7%) reported having non-client sexual partners (most were identified as boyfriends or husbands); however, condom usage with these partners were lower (34.3%).CONCLUSIONS:Based on the findings from our survey, we advise that promotion of HIV/AIDS-related knowledge be targeted towards FSWs in Shanghai, especially those operating in the suburbs. HIV prevention efforts, such as urging constant condom usage with both clients and steady partners, should be sustained and reinforced among the female sex workers population.
AIDS High Risk Sexual Behavior and its Sociological Factors among MSM in Pudong New Area of Shanghai

Abstract:[Objective] To explore AIDS high risk behavior and its associated sociological factors among men who sex with men(MSM),and to provide suggestion for the intervention against AIDS.[Methods] A cross-sectional study was conducted with snowball and internet sampling in Pudong New Area.A total of 158 MSM were interviewed face to face,whose demographic characteristics,sexual orientations,sexual behaviors and sexual histories were collected after informed consent was obtained.[Results] There were 130(28.3%) gays and 28(17.7%) bisexuals in 158 MSM.In the past 6 months,97(61.4%) of them had fixed male sexual partners,80(50.6%) had multiple sexual partners and 82(51.9%) had occasional sexual partners.In addition,46(48.4%) who had fixed male partners,31(38.8%) who had multiple sexual partners,and 33(40.2%) who had occasional sexual partners had AIDS high risk sexual behaviors in the past 6 months.Single and low-income MSM were more inclined to have high risk sexual behaviors than others.[Conclusion] The rate of AIDS high risk sexual behaviors is high and keeping multiple sexual partners is common in MSM.It is urgent to conduct intervention programs among them.
Sexual Risk Behaviors among Club Drug Users in Shanghai, China: Prevalence and Correlates

Abstract:We surveyed 276 club drug users in Shanghai, China. Overall, 43.8 % reported ≥2 sex partners in the past 30 days, and 48.9 % reported having sex with non-regular partners, 67.4 % of whom had unprotected sex. Having ≥2 recent sex partners was associated with being 35 years or older, male, living with friends or others, introduced to club drug use by non-regular sex partners, using methamphetamine recently, self-identified as gay/lesbian or bisexual, had sexual debut before 20 years old, and recently had sex under the influence of drugs. Having unprotected sex with non-regular partners in the past 30 days was associated with lower education levels, having sex to obtain drugs, and lower levels of HIV/AIDS knowledge. Club drug users should be targeted for intervention programs. Future research needs to identify other protective and risk factors for sexual risk behaviors and design interventions to reduce club drug use and associated sexual risk behaviors.
Retrospect Analysis of 344 HIV Infection in Drug-Abuse Patients in Shanghai

Abstract:AIM To investigate the characteristics and current status of HIV infection population in Shanghai and to provide evidence for drug abuse surveillance. METHODS An analysis was conducted on 344 drug abuse patients with HIV infection, including their socio-demographics, behavioral characteristics of drug use, injection equipments sharing and so on. All the data were recorded in the "Drug Abuse Surveillance Regulation List" issued by National Drug Abuse Surveillance Center; through medical staffs and inspectors. RESULTS Total 344 patients with HIV infection were from 14 bureau compulsory detoxification, among 286 patients ( 83.1% ) were from Shanghai Municipal Public Security Bureau Compulsory Detoxification. The 67 patients (19.5%) were registered permanent residence in Shanghai, the 277 patients (80.5%) were for other parts of the country. The males (312 patients) were apparently more than the females (32 patients). There were 285 patients (82.5% ) distributed 20 to 40 years in youth and prime. Heroin was the main drug abuse ( 89.5% ) including 314 patients (88.4%) of venous injection, with 138 patients (45.4%) sharing the injector or syringe needle. CONCLUSION We should take appropriate measure with strict surveillance and management to control the spread of HIV.
A Mixed Methods Approach To Identifying Factors Related To Voluntary Hiv Testing Among Injection Drug Users In Shanghai, China

Abstract:OBJECTIVES:Injection drug use is a major route of HIV transmission in China, yet relatively little is known about why so few injection drug users utilize free HIV testing services. This study aimed to examine barriers to HIV testing and voluntary counseling and testing (VCT) service utilization among injection drug users in Shanghai, China. METHODS:Utilizing mixed methods, we analyzed data from a survey of 540 compulsory drug abuse treatment patients and data from focus groups with 70 service providers and patients. RESULTS:Only 24.4% of patients expressed willingness to be tested for HIV. Willingness to be tested was associated with younger age and more positive attitudes towards condom use. Patients reported several barriers to utilization of voluntary HIV testing services, including lack of information about these services, perceptions of no risk or low-risk for HIV infection, fear of positive results, and the stigma or discrimination that may be experienced by the patient or their family. Having limited skills related to HIV counseling was reported by service providers as the primary barrier to encouraging patients to utilize HIV testing/VCT services. CONCLUSIONS:Special intervention programs targeting injection drug users, their family members, and service providers may increase HIV testing in China.
Association Between Recreational Drug Use and Sexual Practices among People Who Inject Drugs in Southwest China: a Cross-Sectional Study

Abstract:Objective To describe the differences in sexual practices among individuals with various drug administration patterns. Setting A detoxification centre in Southwest China, a part of the Chinese national sentential surveillance network for hepatitis C virus (HCV), HIV and syphilis infections, was recruited. Participants A total of 610 newly enrolled injection drug users (IDUs) from detoxification centre were included during 2015. Primary and secondary outcome measures Self reported sexual activities, drug-related practices and laboratory-confirmed HCV, HIV and syphilis infection status were collected. Results Of the 610 IDU, 295 (48.4%) used heroin only, 277 (45.4%) poly-drug users reported the mixed use of synthetic drugs (SDs) with heroin and 38 (6.2%) used SDs only. The average daily drug injection frequency for poly-drug users (3.3 +/- 1.2 times) was the highest, followed by heroin-only (2.2 +/- 0.8 times) and SD-only users (1.2 +/- 0.4time). SD-only drug users reported the highest proportion (86.8%) of engaging in sexual activities in the previous month, with more than half (54.5%) reporting any condomless sex. A higher frequency of daily injecting in heroin-only users was significantly correlated with the less likelihood of sex, condomless sex in the past month, having sex with fixed partners, condomless commercial sex in the previous 12 months (all p<0.01). In poly-drug users, who injected drugs two times per day was associated with the highest proportion of people who engaged in sex and commercial sex (p<0.05). For SD-only users, increased drug use was riot associated with reducing sexual risk (p>0.05). Different patterns of HCV, HIV and syphilis infections prevalence rates were shown among the IDU depending on the roles and length of exposure. Conclusions The daily drug injecting frequency of heroin only and poly-drug users was negatively associated with sexual activities, hut SD-only users kept a high frequent engagement in sex. The interventions for relevant diseases should adapt to characteristics of IDU.
Peer-based Behavioral Health Program for Drug Users in China: a Pilot Study

Abstract:BACKGROUND:Many injection drug users (IDUs) in China have high risk sexual behaviors that contribute to the spread of HIV infection. Although many IDUs in China move through drug rehabilitation centers, this opportunity for sexual health education has largely been overlooked.METHODS:A convenience sample of 667 drug users from two rehabilitation centers in South China was recruited in the study. Two hundred and forty seven drug users from a single Guangdong Province rehabilitation center received the peer-based education intervention, while 420 drug users from another rehabilitation center received routine HIV/STI education and was used as the control. One hundred and eighty nine (22.1%) individuals refused to participate in the study. HIV/STI behavioral and knowledge domains were assessed at 3 months in rehabilitation centers after the intervention (first follow-up) and at 2-23 months in the community after release (second follow-up).RESULTS:Drug users who completed the intervention reported more frequent condom use with casual sex partners (60.0% vs. 12.5% condom use every time, p = 0.011) and less frequent injection (56.7% vs. 26.4% no injection per day, p = 0.008) at the second follow-up compared to those in the routine education group. Loss to follow up was substantial in both control and intervention groups, and was associated with living far from the detention center and having poor HIV knowledge at baseline.CONCLUSIONS:This study shows that rehabilitation centers may be a useful location for providing behavioral HIV/STI prevention services and referral of individuals to community-based programs upon release. More research is needed on behalf of detained drug users in China who have complex social, medical, and legal needs.
Survey of HIV High Risk Behavior and Gender Difference among Heroin Dependents in Shanghai

Abstract:To understand the characteristics and gender difference of human immunodeficiency virus(HIV) related risk behavior among heroin dependents in Shanghai.Methods Four hundred and sixty six heroin dependents recruited from three Compulsory Treatment Centers and a Voluntary Drug Abuse Treatment Center were interviewed about the HIV related risk behavior,HIV and attitude toward prevention.Results HIV high risk behaviors were common in this population including sharing syringes with others(26.3%),multiple sex partners(≥ 3)(24.0%).Willingness to take HIV testing was low,only 6.7% of the samples were very likely to do HIV testing in next year.Male dependents had higher proportion of HIV high risk behaviors than females including share syringes with others,total number of sex partners in a life,total number of sex partners in a year,total number of sex partners in 3 months, multiple sex partners(≥ 3), number of occasional sex partner among multiple sex partners(≥ 2),oral sex,anal sex(P0.05);the proportion of having HIV test and thinking HIV education participation is meaningful among the female heroin dependents were higher than males(P0.05).Conclusion The gender difference is existing among heroin dependents in terms of HIV high risk behaviors,and effective HIV prevention and intervention strategies and treatment formula for different genders of heroin dependents should be developed.

Abstract:In China, men who have sex with men (MSM) are at high risk for substance use, HIV, and other sexually transmitted infections (STIs). However, there has been little research into their drug use and the sexual behaviors they engage in after drug and alcohol use. Using respondent-driven sampling methodology, this cross-sectional study of 404 MSM described drug use and examined sexual behaviors after drug and alcohol use among general MSM and money boys (those who engage in same-sex commercial sex) in Shanghai, China. Money boys reported using more drugs in the last week, 3 months and ever, and having sex after drinking and sex after using drugs more frequently than general MSM. It is vital to continue research and begin science-based interventions in these high-risk and underserved populations.
