Acquired Hemophilia A Patients: Clinical Experience of 12 Cases

Abstract:Objective To improve the physician' s understand of acquired hemophilia (AH) , and discuss optimal therapy schemes of AH. Methods Twelve patients with AH since 1995 to 2005 in our hospital were reviewed, including clinical manifestations, experimental data, diagnosis and treatment. Results The average age was 46 years, 6 patients (50%) were below 35 years and 5 were above 50 years older. The majority presenting symptoms included intra-muscle hematoma (6/12) and skin ecchymoses (4/12). The most common underlying conditions were autoimmune diseases (3 SLE, 3 APS, 1 RA ) and lung diseases (n=3, including chronic bronchititis, asthma and lung cancer) , only 1 patient had no reason to detect. Seven cases acquired complete remission (CR) and 4 acquired partial remission (PR) by the treatment of hemostatic and immunosuppressive agents. Only 1 patient died of intractable bleeding. The severity of the hemorrhage and the time to obtain CR or PR had no correlation with the level of aPTT, FⅧ: C and the titer of FⅧ: Ab (P>0.05). Conclusion we have reported 12 patients with acquired FVIII in a clinical hemophilia center. These patients had highly variable clinical courses and responses to steroids or immunosuppressive therapy.
