Abstract:Objective:To investigate the imaging features of various types of facial nerve tumors in order to improve the accuracy of diagnosis.Mathods:The imaging findings of facial nerve tumors proved by surgery and pathology of 21 cases were analyzed retrospectively.CT was performed in all 21 patients,among them 2 cases had multiplanar reconstruction(MPR).Plain and enhanced MRI were performed in 17 cases.Results:Of the twenty-one facial nerve tumors,there were 15 patients with schwannoma,which involved internal auditory canal(n=1),labyrinth segment(n=2),anterior genu(n=6),tympanic segment(n=11),mastoid segment(n=7),intra-parotid segment(n=5),external auditory canal(n=3),jugular fossa(n=2),posterior cranial fossa(n=2) and middle cranial fossa(n=1);3 patients with hemangioma involved anterior genu(1 patient),anterior genu and tympanic segment(2 patients);3 patients with neurofibroma involved labyrinth,tympanic and mastoid segments extensively(2 patients);tympanic and mastoid segment were involved in the other patient.The CT findings included enlargement and/or destruction of facial nerve canal(17 patients),intra-tympanic cavity soft tissue mass(12 patients),destruction and/or lateral displacement of auditory ossicles(5 patients),destruction of the posterior wall of external autitory canal(3 patients) and destruction of lateral wall of jugular fossa(2 patients).1 patient with hemangioma showed characteristic honey-combed bony changes within the tumor located at anterior genu.The MRI findings of 16 patients were nodular of irregular soft tissue mass(16 patients) and enlargement of facial nerve(1 patient).Compared with the signal intensities of brain,the tumor appeared isointense on T1WI and isointense or hyperintense,homogeneous or heterogeneous on T2WI.After contrast administration,heterogeneously moderate to marked enhancement could be assessed.Conclusion:Multiple segments of the facial nerve could be involved by one facial nerve tumor.CT is superior in demonstrating the bony changes of facial nerve canal,MRI can accurately display the location,shape,extension and intra-tumoral structure of facial nerve tumors.CT in combination with MRI are helpful in the localization and differential diagnosis as well as provide significant information for surgical planning and approach.
