Abstract:For healthy term or preterm neonates who do not require resuscitation after birth, delayed cord clamping (DCC) for 1-3 min has been shown to have a beneficial effect on neonatal and infant health. DCC is recommended as a standard procedure by the World Health Organization. Umbilical cord milking (UCM) can be considered as an alternative when DCC is unfeasible. However, the safety of UCM in preterm infants less than 28 weeks still needs more evidence.

Abstract:In recent years,recommendations for delayed cord clamping at birth have been made by the World Health Organization and academic organizations in several countries.A consensus is yet to be reached regarding the timing of delayed cord clamping.Previous studies indicated that delayed cord clamping could decrease the risk of anemia,intraventricular hemorrhage,necrotizing enterocolitis,and hypotension,and improve neurodevelopment for infants,with no increased risk of postpartum hemorrhage.This article reviews the effect of delayed cord clamping on maternal and neonatal outcomes worldwide over the past five years.

Epidemiological Investigation on the Current Practice of Umbilical Cord Clamping in China.

Abstract:Objective Despite known benefits, the timing of and method used for umbilical cord clamping (UCC) in neonates remain controversial in China, as well as internationally. The objective of this study was to assess knowledge, attitudes, and practice of UCC amongst health care providers in China, as recommended by medical professional organizations. Study Design A web-based questionnaire on cord clamping practices was administered to midwives, obstetricians, and neonatologists in 126 hospitals from 16 provinces. The provinces were selected from seven different regions of China. Results A total of 5,005 (60.5% of eligible respondents) health care providers returned completed questionnaires. The awareness rates for immediate cord clamping (ICC) and delayed cord clamping (DCC) were over 85%, but the implementation rate for DCC was relatively low (ICC 58.3% vs. DCC 41.6%). Most neonates were placed below the introitus (92.8%) during cord clamping and this correlated with the route of delivery. The choice of UCC was impelled by different factors. Benefits for neonates influenced the choice of ICC (50%) and promoting a larger blood volume to stabilize systemic circulation influenced the choice of DCC (92.3%). Majority (91.5%) of respondents acquiesced that it was necessary to develop national clinical guidelines for UCC. Conclusion The majority of obstetricians, neonatologists, and midwives who participated in this study had a positive perception of DCC. However, this did not translate to daily practice. The practice of UCC is variable and there are no standard guidelines.

Abstract:Background: Evidence supporting the benefits of delayed cord clamping is increasing; however, there is no clear recommendation on cord management during newborn resuscitation. This study aimed to investigate the effects of resuscitation initiated with an intact umbilical cord, hypothesizing it is a safe stabilization procedure that improves neonatal outcomes. Methods: Systematic search was conducted in MEDLINE, Embase, CENTRAL, and Web of Science from inception to March 1, 2024. Eligible articles compared neonatal outcomes in newborns receiving initial stabilization steps before and after cord clamping. Results: Twelve studies met our inclusion criteria, with six RCTs included in the quantitative analysis. No statistically significant differences were found in delivery room parameters, in-hospital mortality, or neonatal outcomes between the examined groups. However, intact cord resuscitation group showed higher SpO2 at 5 min after birth compared to cord clamping prior to resuscitation group (MD 6.67%, 95% CI [-1.16%, 14.50%]). There were no significant differences in early complications of prematurity (NEC ≥ stage 2: RR 2.05, 95% CI [0.34, 12.30], IVH: RR 1.25, 95% CI [0.77, 2.00]). Conclusion: Intact cord management during resuscitation appears to be a safe intervention; its effect on early complications of prematurity remains unclear. Further high-quality RCTs with larger patient numbers are urgently needed. Impact: Initiating resuscitation with an intact umbilical cord appears to be a safe intervention for newborns. No statistically significant differences were found in delivery room parameters, in-hospital mortality, and neonatal outcomes between the examined groups. The utilization of specialized resuscitation trolleys appears to be promising to reduce the risk of intraventricular hemorrhage in preterm infants. Further high-quality RCTs with larger sample sizes are urgently needed to refine recommendations.

Abstract:Background: The timing of cord clamping has become a focal point for neonatal caregivers due to the promising outcomes associated with delayed cord clamping, which is a simple and cost-effective method to enhance the survival and well-being of preterm infants. While initially the rationale behind delaying clamping was to facilitate increased placental transfusion, research has unveiled additional hemodynamic benefits. Summary: Experimental studies have demonstrated improved circulatory transition when clamping is postponed until the lungs are adequately aerated. This suggests that infants requiring assistance during the transition phase may benefit from stabilization while still attached to the cord. The Aeration, Breathing, and then Clamping (ABC) project aimed to translate these experimental findings into clinical practice. Key message: In this review, we will discuss the insights gained and lessons learned from the project's implementation.

Abstract:Objective To determine the effects of delayed umbilical cord clamping on the postpartum hemorrhage,instant and long-term newborn anemia,newborn jaundice.Methods In total,303 infants were selected during October 2016 to June 2017 in three hospitals in Beijing.They were randomly allocated into two groups receiving instant clamping of umbilical cord (less than 60s after delivery,n=158) and delayed clamping of umbilical cord(after cord pulsation ceased,n=145).Relevant indicators of maternal and neonatal outcomes are compared.Results There were significant differences between two groups in instant hemoglobin concentration and in 5～7 days (P＜0.05).There were no differences between two groups in transcutaneous bilirubin,the risk of anemia in three months,the risk of jaundice in 5～7 days and the need of blue-light therapy (P＞0.05).There were no differences between two groups of women in postpartum hemorrhage,the length of third stage of labor and the rate of breast feeding (P＞0.05).Conclusion Clamping the umbilical cord when cord pulsation has ceased does not have negative effects on delivery process and postpartum hemorrhage,but it increases the instant hemoglobin concentration and hemoglobin concentration after delivery in 5～7 days.Still it is unclear whether it will affect the risk of jaundice.
Variations in umbilical cord clamping practices in the United States: a national survey of neonatologists

Abstract:<span> Since the first publication of the American College of Obstetricians and Gynecologists committee opinion in 2012, and following the update in 2017, multiple institutions in the United States (US) adopted the practice of delayed cord clamping (DCC) and/or umbilical cord milking (UCM) in preterm and term infants. However, there have been variations reported in practices with regard to method of placental transfusion, timing of cord clamping and gestational age thresholds. Furthermore, the optimal cord clamping practice in situations of depressed infants needing resuscitation or in higher-risk delivery situations, such as placental abruption, intrauterine growth restriction, multiple gestation, chorioamnionitis, maternal human immunodeficiency virus syndrome/hepatitis or maternal general anesthesia is often debated. An evaluation of these variations and exploration of associated factors was needed to optimally target opportunities for improvement and streamline research activities. The objective of this survey, specifically aimed at neonatologists working in the US was to identify and describe current cord clamping practices and evaluate factors associated with variations. The survey was distributed electronically to the US neonatologists in August 2019 with a reminder email sent in October 2019. Clinicians were primarily identified from Perinatal Section of AAP, with reminders also sent through various organizations including California Association of Neonatologists, Pediatrix and Envision national groups. Descriptive variables of interest included years of experience practicing neonatology, affiliation with a teaching institution, level of the neonatal intensive care unit (NICU) and practicing region of the US. Questions on variations in cord management practices included information about center specific guideline/protocol, cord clamping practices, gestational age threshold of placental transfusion, performance of UCM and practice in higher-risk delivery situations. The response rate was 14.8%. Among 517 neonatologists whom responded, majority (85.5%) of the practices had a guideline and performed (81.7%) DCC in all gestational ages. The cord clamping practice was predominantly DCC and it was categorized as reporting clamping times <60 s in 46.6% and ≥60 s in 48.7% of responses. A significant association was detected between time of delay in cord clamping and region of practice. The Northeast region was more likely to clamp the cord in <60 s than other regions in the US. More than half of the providers responded not performing any UCM (57.3%) in their practice. Significant associations were detected between performance of UCM and all queried demographic variables independently. Clinicians with >20 years of experience were more likely from institutions performing UCM compared to the providers with fewer years of experience. However, teaching hospitals were less likely to perform UCM compared to non-teaching hospitals. Similarly, practices with level IV NICUs were less likely to perform UCM compared to practices with level III units. Hospitals in the Midwest region of US were less likely to perform UCM compared to hospitals in the Western region. Significant variations were also noticed for not providing placental transfusion in higher-risk deliveries. Demographic and professional factors were noted to be associated with these differences. Although the majority of practices have a guideline/protocol and are performing DCC in all gestational ages, there are variations noted with regard to timing, method, and performance in higher-risk deliveries. Demographic and professional factors play an important role in these variations. Future research needs to focus on the modifiable factors to optimize the procedure and impact of DCC.</span>
Effects of Delayed Cord Clamping on Infants after Neonatal Period: A Systematic Review and Meta-Analysis

Abstract:Background: The majority of current evidences simply showed the short-term benefits of delayed cord clamping, mainly focusing on the first week after birth. Without follow-up data, we can hardly come to the conclusion that delayed cord clamping may do more harm than good. Objective: To evaluate the long-term effects of delayed cord clamping compared with early cord clamping on infants after neonatal period. Design: Systematic review and meta-analysis of randomized controlled trials (RCTs). Data sources: PubMed, EMBASE, and the Cochrane Library were systematically searched from inception date to June 22,2018 for randomized clinical trials comparing early cord clamping with delayed cord clamping in infants beyond 1 month of age. Review methods: Two reviewers independently assessed trial eligibility and quality and extracted all infants' follow-up data after one month of age, which were divided into two groups for analysis, with follow-up periods of less than 6 months (<6 months) and beyond 6 months (>6 months) respectively. Results: A total of twenty RCTs were identified and included in this study. All data of the twenty studies were pooled for final meta-analysis (3733 infants). Among preterm deliveries, delayed cord clamping slightly increased hematocrit (6-10 weeks) and serum ferritin (6-10 weeks). For term infants, delayed cord clamping reduced the incidence of anemia after six months of age (>6 months), iron deficiency (< 6 months, >6 months) and iron deficiency anemia (4-12 months), while increased mean corpuscular volume before six months of age (< 6 months), hemoglobin after six months of age (>6 months), serum iron (2-4 months), total body iron (4-6 months), serum ferritin (< 6 months, >6 months) and transferrin saturation (2-12 months). There were no significant differences between early versus late cord clamping groups for other variables. Conclusion: Delayed cord clamping modestly improved hematological and iron status of both preterm and term infants after neonatal period. This affords cogent evidence on the practice of delayed cord clamping for medical staff, especially for countries and regions suffering from relatively higher prevalence rate of iron deficiency during infancy and childhood. Abstract:The yolk sac and the allantois give rise to and are preserved in the umbilical cord. By the fifth week of development, it has formed and is the embryo's new source of nutrition, taking the place of the yolk sac. The cord joins the placenta, which transports substances to and from the mother's blood without allowing them to directly mix, rather than being directly attached to the mother's circulatory system. The umbilical cord can be clamped at various times, although postponing it until at least one minute after delivery improves results, provided that the minor risk of jaundice can be treated if it does occur. Clamping is followed by a painless cord cutting procedure because there are no nerves present. Out of those 20 studies, only 5 studies discuss the risk associated with delayed cord clamping, 2 studies discuss the benefit associated with good nutritional status, 2 studies discuss the benefit associated with good neurodevelopment, and the remaining studies discuss the increase in hemoglobin due to delayed cord clamping.

Abstract:Objective To systematically evaluate the effects of delayed cord clamping on anemia and jaundice in term neonates .Method We search The Cochrane Library、JBI、RANO、PubMed、EMbase、Medline 、CBM、Wanfang Data、CNKI and VIP data ,to find the literature which report the effects of the method of delayed cord clamping on anemia and jaundice in term neonates .The retrieved studies were screened using inclusion and exclusion criteria in‐dependently ,and after evaluating the quality of selected studies by 2 reviewers ,we extract the data from literatures , RevMan 5 .1 was used to carry out meta‐analysis .Result Seven studies indicated that delayed cord clamping can im‐prove the hematocrit within 3 days after birth .Three studies demonstrated that delayed cord clamping can improve the hemoglobin concentration within 2 days after birth .Three studies proved that delayed cord clamping will not in‐crease the concentration of bilirubin detected through surface significantly .Three RCTs show that delayed cord clamping will not increase the risk of neonatal polycythemia .Conclusion Delayed cord clamping can effectively prevent neonatal anemia ,and do not increase the risk of neonatal polycythemia and jaundice .It is a safe and effective meth‐od .

Abstract:Background This study aimed to evaluate the effect of ICC (cord clamping within the first 15 s), DCC (delayed cord clamping at 60 s), and cut-UCM (cut-umbilical cord milking, cord clamping within the first 15 s) groups on oxygen saturation (SpO2), heart rate (HR), and perfusion index (PI) up to 10 min after birth in newborn infants. Methods We conducted this randomized clinical trial in the delivery unit of a University Hospital with 189 infants born between 35 and 42 weeks of gestation. Participants were randomly assigned to one of three groups: ICC, DCC, and cut-UCM. The primary outcomes measured were SpO2, HR, and PI at the 1st, 3rd, 5th, and 10th minutes after birth. We utilized ANOVA and Bayesian calculations in this study. Results There was no difference between the ICC, DCC, and cut-UCM groups in SpO2, HR, and PI values at the 1st, 3rd, 5th, and 10th minutes of life, which did not significantly alter between the three groups in one-way ANOVA. Bayesian repeated-measure ANOVA calculations showed that SpO2 and heart rate results at the 1st, 3rd, 5th, and 10th minutes did not differ between ICC, DCC, and cut-UCM techniques with strong evidence. At the 3rd minute, PI was slightly higher in the DCC and cut-UCM groups compared to the ICC group, with anecdotal evidence. We found no difference between DCC and cut-UCM regarding the 3rd-minute PI, with moderate evidence. Conclusion Umbilical clamping procedures (ICC, DCC, and cut-UCM) did not affect SpO2 and HR in the first ten minutes of life, but 3rd-minute PI values were slightly higher in DCC and cut-UCM compared with ICC among late preterm and term neonates.

Abstract:Fetal omphalocele is a surgical disease with severe defect of fetal abdominal wall which requires emergency treatment after birth. The incidence rate is 2.1/10000, and the mortality rate is 52% to 67%. The improper handling of omphalocele can cause capsular rupture and intestinal exposure, leading to neonatal fluid loss, hypothermia, intestinal necrosis and other complications. It is the major cause of death in newborns. In this paper, we reviewed the characteristics, delivery mode, delivery time, umbilical cord care, and transferring of omphalocele newborns to provide reference for clinical nursing.

Abstract:Objective: To anlysis the efficacy and safety of cut-umbilical cord milking (C-UCM) compared with immediate cord clamping in preventing anemia and iron deficiency among term cesarean-delivered newborns. Methods: A total of 485 pregnant women planning to deliver by cesarean section were recruited in this randomized controlled trial in Hunan Maternal and Child Health Hospital and Liuyang Maternal and Child Health Care Hospital from July 2016 to April 2019. A block randomization was conducted to evenly allocate them to the controlled group and the C-UCM group. In the controlled group, the cord was clamped within 30 seconds as routine. In the C-UCM group, the cord was first clamped at 25 cm from the newborn's navel, and then the blood in the cord was gently squeezed into the newborn's body until the cord became white and shriveled. The cord was clamped twice at 2-3 cm from the newborn's navel subsequently. Neonatal jaundice, hyperbilirubinemia and polycythemia were monitored before discharge. After the newborns discharged, their hemoglobin, red blood cell count, hematocrit (at the age of 1, 6 and 12 months) and serum ferritin (at the age of 6 and 12 months) were followed up; body length and weight were measured; and information about their feeding and iron supplementation were collected (at the age of 1, 6, 12 and 18 months). The two groups were compared by t test, Mann-Whitney U test, χ² test, or Fisher exact probability method. The hospital was set as a random item, and the mixed effects regression model was used to evaluate the effect of C-UCM on relevant indicators of cesarean-delivered newborns. Results: There were 244 women in the C-UCM group with an average age of (31.9±4.4) years, and 241 in the control group with an average age of (31.8±4.2) years (P>0.05). There was no statistically significant difference between the C-UCM group and the control group at 1, 6 and 12 months of age in hemoglobin [(123.6±14.5) vs (122.2±14.5) g/L, (115.3±9.4) vs (114.1±8.5) g/L, (115.6±9.6) vs (116.1±12.6) g/L] or anemia incidence rate [15.2% (17/112) vs 18.4% (19/103), 22.7% (34/150) vs 26.8% (44/164), 22.3% (25/112) vs 19.5% (22/113)] (all P>0.05). There was no statistically significant difference between the two groups at 6 and 12 months of age in serum ferritin [M (Q1, Q3), 39.9 (24.9, 61.8) vs 43.6 (25.2, 100.9) μg/L, 40.3 (25.4, 259.2) vs 40.3 (26.4, 167.6) μg/L)] or iron deficiency incidence rate [6.1% (5/82) vs 4.2% (3/72), 6.7% (5/75) vs 3.8% (3/80)] (all P>0.05). There were also no significant difference between the two groups in other indicators, such as the Z-score of weight-for-length, the incidence of neonatal jaundice, and the incidence of neonatal hyperbilirubinemia (all P>0.05). After adjusting for the relevant covariates, there were still no significant effects of C-UCM on these outcomes above. Conclusions: Compared to immediate cord clamping, the intervention of gently squeezing 25 cm of the cord does not significantly reduce the risk of anemia or iron deficiency in term cesarean-delivered newborns, nor does it have a significant impact on infant growth and development. Yet this intervention does not increase the risk of jaundice or hyperbilirubinemia in newborns as well.
System review:the effects of different time of delayed cord clamping on anemia prevention in term infants

Abstract:【Objective】To study the effects of different time of delayed cord clamping on anemia prevention in term infants. 【Method】PubMed,EMBASE,VIP and CNKI were searched till January 31 2009,using terms(delayed cord clamping and anemia and RCT).According to inclusion and exclusion criteria,articles were evaluated by 2 reviewers.Randomized controlled trials were assessed according to the Juni assessment.Software RevMan 5.0.18 was used to carry out statistical analysis. 【Results】7 articles were eligible randomized controlled trials.There was no significantly difference between the early cord clamping group(within 30 s after birth) and the delayed cord clamping group 1(exceed 30 s and within 2 min) in the onset risk of anemia(OR=0.43,95%CI 0.15～1.23),polycythemia in the first day after birth(OR=1.71,95%CI 0.52～5.65) and hyperbilirubinemia(OR=1.15,95%CI 0.66～2.01).The onset risk of anemia in the delayed cord clamping group 2(exceed 2 min or after cord pulsation ceased or descent of placenta) was lower than the early cord clamping group(OR=0.22,95%CI 0.11～0.43).Although the morbidity of polycythemia in the first day after birth was higher in the delayed cord clamping group 2(OR=2.08,95%CI 1.01～4.28),the morbidity of hyperbilirubinemia was not increased(OR=0.69,95%CI 0.30～1.55). 【Conclusions】Delayed cord clamping after 2 min will be effective in preventing term infant anemia,though which might also increase the morbidity of polycythemia in the first day after birth.The multi-center large sample size RCT is still need to access the safety of the technique.

Abstract:Objective To explore the right way to initiate delivery in cases of umbilical cord entanglement and assess its effect on perinatal infants.Methods Retrospective analysis of 3801 clinical records between 2004 and 2005.Comparison between cases of non-cord entanglement without gestation syndrome and those of umbilical winding.Results The rates of vagina operations on pregnant women whose babies have necks wound and the cesarean section are higher than those of non-cord entanglement.Conclusion Umbilical cord entanglement has bad effect on perinatal infants.Therefore close watch should be given during the whole process of delivery.In cases of two or more cycles of cord entanglement cesarean section should be considered under appropriate circumstances.
