Endometrial Stromal Tumors: a Clinicopathologic Analysis of 35 Cases

Abstract:Purpose To investigate the clinicopathologic features and differential diagnosis of endometrial stromal tumors (ESTs) and to improve the diagnosis and treatment level of ESTs. the history records and the clinicopathologic data were retrospectively evaluated in 35 patients with ESTs, some of which were also studied with immunohistochemical staining (EnVision method). Results The average age of the patients was 45 years. The major clinical manifestations were uterine masses and abnormal vaginal bleeding. Among 35 cases of ESTs, there were 4 cases of endometrial stromal nodule (ESN), 26 cases of low-grade endometrial stromal sarcoma (ESS), and 5 cases of undifferentiated endometrial sarcoma (UES). Both of ESN and ESS were typically composed of cells analogous to the endometrium stromal cells of the proliferative phase accompanied with spiral arterioles. UES showed evident cytological atypia and lacked spiral arterioles. Among 18 cases stained with immunohistochemical method, 17 cases were positive for CD10, 7 were focally positive for SMA, 4 were focally positive for desmin. Conclusions ESTs are the uterine mesenchymal tumors that may pose a diagnostic challenge. Morphologic features as well as immunohistochemical markers may help in the classification, and the diagnosis should be distinguished from smooth muscle tumors and PEComa.
