Clinical efficacy of combined endovascular and microsurgical management of complex intracranial vascular diseases

Abstract:Objective:To investigate the clinical efficacy of combined endovascular and microsurgical treatment of complex intracranial vascular diseases.Methods:A retrospective analysis was conducted on the clinical data of 76 patients with complex intracranial vascular diseases treated by combined endovascular and microsurgical operations at the Department of Neurosurgery, Nanjing Drum Tower Hospital, Nanjing University School of Medicine from September 2018 to December 2019. Among them, 22 patients had intracranial aneurysms, 21 had intracranial arteriovenous malformations (AVM), 25 had carotid artery stenosis/occlusion, 6 had spinal dural arteriovenous fistula, 1 had intracranial dural arteriovenous fistula and 1 case had cavernous malformation (CM) of the cavernous sinus. After surgery, one patient with CM was re-examined with head MRI, and the remaining 75 cases were re-examined with digital subtraction angiography (DSA) or CT angiography (CTA) to evaluate the efficacy of the operation. Clinical follow-up was performed for all patients 3 months after surgery and included re-examination of head DSA, CTA or MRI to assess the progress of the disease after surgery. In addition, the outcomes were assessed using Glasgow outcome scale (GOS), the point of ≥ 4 was defined as good outcome, and that of 1-3 was defined as poor outcome.Results:The operations of 76 patients were successful. Postoperative re-examination of head DSA, CTA or MRI showed that there was no residual lesion in those patients. Intracranial infections occurred in 14 patients(18.4%) after operation, including 9 cases of intracranial aneurysms and 5 case of cerebral AVM. After active treatment, 12 cases were cured and the other 2 (both with posterior circulation aneurysms) died. Seventy-four patients were followed up for 3.4±0.8 months (2-6 months). At 3 months post operation, the head DSA, CTA or MRI were re-examined, and there was no recurrence or residual lesion. The assessment using GOS showed that 73 patients(98.6%) had a good outcome and the other 1(1.4%) had a poor outcome.Conclusion:Combined endovascular and microsurgical treatment of complex intracranial vascular diseases is associated with a low incidence of postoperative complications, a low recurrence rate, and a good outcome in most cases.
