Application of Vanderbilt ADHD Parent Rating Scale on clinical evaluation for children with attention-deficit/hyperactivity disorder

Abstract:[Objective] To assess the practicing value of Vanderbilt ADHD Parent Rating Scale(VADPRS)on children with attention-deficit/hyperactivity disorder(ADHD),and to provide information for assisting diagnosis of ADHD and screening comorbidity in clinics.[Methods] From March 2006 to May 2007,1 478' inattentive or hyperactivity children' considered by their parents or teachers were consecutively enrolled.All parents were asked to complete VADPRS based on DSM-IV symptoms for ADHD.At the same time,all children received DSM-IV diagnosis.The results of two methods were compared.Comorbidity and functioning impairment of ADHD children were also assessed by the results of VADPRS.[Results] Compared to DSM-IV diagnostic criteria,the sensitivity of VADPRS for ADHD was 90.2%,the specificity was 62.2%,and the agreement rate was 72.5%.For the predominantly inattentive subtype,the sensitivity,the specificity and the agreement rate of VADPRS was 64.9%,82.3% and 78.0%,respectively.For the hyperactive/impulsive subtype,the sensitivity,the specificity and the agreement rate of VADPRS was 62.2%,90.0% and 88.6%,respectively.For the combined subtype,the sensitivity,the specificity and the agreement rate of VADPRS was 62.6%,88.5% and 86.8%,respectively.165 ADHD children were screened for oppositional defiant disorder,34 for conduct disorder,and 21 for anxiety/depression.[Conclusions] Compared to DSM-IV diagnostic criteria,the sensitivity and specificity of VADPRS is good on children with ADHD.VADPRS can also provide useful information of comorbidity and functional impairment.It is easy to complete and consistent with DSM-IV thus is acceptable and feasible in clinics.In conclusion,VADPRS may be used as a useful tool to obtain parent-based information to supplement DSM-Ⅳdiagnostic criteria for ADHD and may be worthy of recommendation,especially in primary health care settings.
