Abstract:Abstract Background Macrophages and inflammatory cytokines play important roles in bone fracture healing. However, the expression patterns of macrophages and inflammatory cytokines during fracture healing under the condition of postmenopausal osteoporosis have not been fully revealed. Methods Tibia transverse fracture was established 12 weeks after ovariectomy or sham operation in 16-week old female mice. Tibias were harvested before fracture or 1, 3, 5, 7, 14, 21, 28 days after fracture for radiological and histological examinations. M1/M2 inflammatory macrophages, osteal macrophages and gene expressions of tumor necrosis factor-α, interleukin-6, interleukin-1β and macrophage conversion related molecules in the fracture haematoma or callus were also detected. Results The processes of fracture healing, especially the phases of endochondral ossification and callus remodeling, were delayed in ovariectomized mice. The expressions of tumor necrosis factor-α and interleukin-6, but not interleukin-1β, in the fracture haematoma or callus were disturbed. Expressions of tumor necrosis factor-α were decreased at 1, 14 and 21 days post-fracture (DPF), and were increased at 3, 5 and 7 DPF. Interleukin-6 expressions at 1, 3 and 21 DPF were significantly increased. We found the decreases in M1 and M2 macrophages at 1 DPF of the initial inflammatory stage. M2 macrophages at 14 DPF of the middle stage and osteal macrophages at 14, 21 and 28 DPF of the middle and late stages of fracture healing were also reduced in ovariectomized mice. Conclusions The expressions of macrophages and inflammatory cytokines were impaired in ovariectomized mice, which might contribute partially to poor fracture healing.

Abstract:Substance P (SP) is a neuropeptide that is released from sensory nerve endings and is widely present in nerve fibres. It acts on bones and related tissues by binding to receptors, thereby regulating bone metabolism, cartilage metabolism and fracture healing. SP has attracted widespread attention as a signalling substance that can be recognized by both the immune system and the nervous system. Previous studies have shown that bone and chondrocytes can synthesize and secrete sensory neuropeptides and express their receptors, and can promote proliferation, differentiation, apoptosis, matrix synthesis and the degradation of target cells through autocrine/paracrine modes. In this paper, we review the research progress made in this field in recent years in order to provide a reference for further understanding the regulatory mechanism of bone and cartilage physiology and pathological metabolism.

Abstract:Objective To study the correlations of structural changes and localization of substance P neurofilament fibers in ovariectomied rats and to understand the basis of osteoporotic back pain Methods Sixty four 3 months old female SD rats were divided into two groups, one underwent ovariectomy(OVX) and the other shamoperation (Sham-O) Sixteen animals in earch batch were sacrificed at 1, 3, 5 , 7th month to procure the L3-4, L4-5 intervertebral disc specimens for the examinations by means of collagen VG stain, microscopy, scanning, transmission electron microscopy and the immunohistochemical PAP of disc cartilage Results Great changes were seen in annulus fibrosus and nucleus of intervertebral disc such as breaking or rupture of collagen fiberlining, particularlly in the posterior aspect of the annulus fibrosus, chondrocytes showed degenerative changes as compared with Sham-O control group And the existence of SP immunoreactive nerve fibers in disc cartilage and annulus fibrosus from the first month, which existed as neurofilament with the orlentation following collagen fibers SP positive nerve fibers were present and increasing in quantity with aging through 7th month Also SP nerve fibers were present in disc surrounding tissue and related to the local inflammation Conclusions The integration of structure and function were broken and degeneration of intervertebral disc took place early after OVX; increase of SP was possibly related with local inflammatory process as well as with lower back pain in estrogen deficiency osteoporosis
Blockade of substance P receptor attenuates osteoporotic pain, but not bone loss, in ovariectomized mice.

Abstract:Objective: The aim of this study was to investigate the effect of a substance P (SP) receptor (NK1 receptor [NK1-R]) antagonist on hyperalgesia and bone metabolism in ovariectomized mice. Methods: Thirty-six 9-week-old mice were subjected to either bilateral ovariectomy or sham surgery. Three weeks after the operation, the mice were treated with either a single-dose injection or 2-week repeated daily administration of L-703606, an NK1-R antagonist. Behavioral tests were performed for pain assessment; tibiae and the third lumbar vertebrae were dissected and assessed for microarchitectural or biomechanical properties. The expressions of SP and NK1-R in the dorsal root ganglia and spinal cord were also evaluated. Results: Both single-dose injection and 2-week repeated injections of L-703606 led to a significant increase in nociceptive threshold in ovariectomized mice. However, the antihyperalgesic effect faded at 2 hours and almost disappeared at 5 hours after a single-dose injection. With the 14-day repeated treatment of ovariectomized mice, the effect was not detectable at 24 hours after the first injection but was obvious at 24 hours after 1-week and 2-week administrations and still existed at 48 hours after the last injection. Ovariectomized mice at the hyperalgesic state had enhanced SP immunoreactivity in the dorsal root ganglia and up-regulated SP and NK1-R expressions in the spinal cord. However, no significant change in serum SP level was detected. Two-week treatment with L-703606 could down-regulate these expressions but failed to salvage the deteriorated trabecular microstructure and reduced compressive strength in ovariectomized mice. Conclusions: Estrogen deficiency-induced hyperalgesia is achieved through up-regulation of SP and NK1-R expressions. Blockade of SP receptor can alleviate pain but cannot ameliorate bone loss. NK1-R antagonist is not recommended for the treatment of estrogen deficiency osteoporosis.
Changes of Microarchitecture and Biomechanical Properties in Callus During Fracture Healing in Ovariectomized Rats

Abstract:OBJECTIVE:To study the changes of biomechanical properties and microarchitecture in callus during the healing process of osteoporotic fracture.METHODS:Ovariectomy (OVX) was performed on SD rats to establish osteoporotic rat model. Osteotomy was performed on both groups of rats with OVX and sham operations to establish bone fracture. Bone mineral density (BMD), area of type I collagen, biomechanical properties, and microarchitecture of the fractured bones during healing process were measured.RESULTS:The BMD of both groups of rats were significantly lower at the 12th week than at the 6th week after fracture (P < 0.05). Both groups of rats had significantly higher biomechanical strength and area of type I collagen at the 12th week than at the 6th week after fracture (P < 0.05). The OVX group of rats had significantly lower structure model index (SMI) values than the sham group at both 6th and 12th week after fracture (P < 0.05). Higher values of SMI appeared at the 12th week than at the 6th week in both groups of rats (P < 0.05).CONCLUSION:During the healing of fracture, the BMD of the newly-formed bone reaches its peak value at the 6th week after fracture, which is earlier than the occurrence of peak biomechanical strength, area of type I collagen, and SMI. Although the collagen and aggravation of mineral determine the biomechanical properties, BMD by itself may not be able to determine the biomechanical strength because of the change of the 3-D structure of the callus. The combination of collagen and microarchitecture may act as a factor that determines the biomechanical strength of callus independent of BMD.

Abstract:Estrogen deficiency is the main factor underlying postmenopausal osteoporosis. A large number of neuropeptides, which regulate skeletal metabolism, potentially represent a regulatory pathway for the pathogenesis of osteoporosis. The aim of this study was to explore factors involved in the regulation of bone-related neuropeptides and their association with estrogen deficiency and bone metabolism. Thirty adult female Sprague-Dawley (SD) rats were randomly divided into a control group with sham surgery (n = 15) and an ovariectomy group with bilateral oophorectomy (n = 15). After 16 weeks, serum estrogen was reduced, CTX-1 was increased and P1NP was not significantly affected in the ovariectomy group and a model of osteoporosis was established. We then investigate the gene expression and protein levels of a range of neuropeptides and their receptors, including substance P (SP) and tachykinin receptor 1 (TACR1), calcitonin gene-related peptide (CGRP) and calcitonin receptor-like (CALCRL), vasoactive intestinal polypeptide (VIP) and receptor 1 and 2 (VPAC1, 2), neuropeptide Y (NPY) and receptor Y1 and Y2, in the brain and femora. Ovariectomy reduced TACR1, CGRP, CALCRL, NPY, NPY Y2 in the brain, but increased TACR1 and decreased SP, CALCRL, VIP, VPAC2 in the bone. Collectively, our data revealed that the pathogenesis of postmenopausal osteoporosis is associated with the regulation of SP, CGRP, VIP, and NPY. These novel results are of significant importance in the development of neuropeptides as therapeutic targets.
Cytological changes in the fracture healing process of osteoporosis

Abstract:Obiective To explore the cell changes during the fracture healing process due to osteoporosis. Methods Model of osteoporosis was obtained by ovariectomy in adult female dogs. Tibia fractures had been made and fixed with screws, for observation. Results Two weeks after the fracture a large amount of oviform division cells in different sizes were found on surface of around the trabecula of fracture. Scarse and irregular collagen fibres were observed in the bone reabsorbing howship. During the modeling process after fracture, the osteoclasts were active in bone reabsorption but the osteoblasts were not that active in bone formation. Conclusions During the modeling period in the healing process of fracture, the activity of osteoblasts in bone formation should be enhanced while that of the osteoclasts in bone reabsorption be reduced.
Exogenous PTHrP Repairs the Damaged Fracture Healing of Osteoporotic Mice and Accelerates Fracture Healing of Wild Mice

Abstract:Bone fracture healing is a complicated physiological regenerative process initiated in response to injury and is similar to bone development. To demonstrate whether an exogenous supply of parathyroid hormone-related protein (PTHrP) helps in bone fracture healing, closed mid-diaphyseal femur fractures were created and stabilized with intramedullary pins in eight-week-old wild-type (WT) PTHrP+/+ and PTHrP+/- mice. After administering PTHrP for two weeks, callus tissue properties were analyzed at one, two, and four weeks post-fracture (PF) by various methods. Bone formation-related genes and protein expression levels were evaluated by real-time reverse transcriptase-polymerase chain reaction and Western blots. At two weeks PF, mineral density of callus, bony callus areas, mRNA levels of alkaline phosphatase (ALP), type I collagen, Runt-related transcription factor 2 (Runx-2), and protein levels of Runx-2 and insulin-like growth factor-1 decreased in PTHrP+/- mice compared with WT mice. At four weeks PF, total collagen-positive bony callus areas, osteoblast number, ALP-positive areas, and type I collagen-positive areas all decreased in PTHrP+/- mice. At both two and four weeks PF, tartrate-resistant acid phosphatase-positive osteoclast number and surface decreased a little in PTHrP+/- mice. The study indicates that exogenous PTHrP provided by subcutaneous injection could redress impaired bone fracture healing, leading to mutation of activated PTHrP by influencing callus areas, endochondral bone formation, osteoblastic bone formation, and bone turnover.
No pain, no gain? The effects of pain-promoting neuropeptides and neurotrophins on fracture healing

Abstract:Neuropeptides and neurotrophins are key regulators of peripheral nociceptive nerves and contribute to the induction, sensitization, and maintenance of pain. It is now known that these peptides also regulate non-neuronal tissues, including bone. Here, we review the effects of numerous neuropeptides and neurotrophins on fracture healing. The neuropeptides calcitonin-gene related peptide (CGRP), substance P (SP), vasoactive intestinal peptide (VIP), and pituitary adenylate cyclase-activating peptide (PACAP) have varying effects on osteoclastic and osteoblastic activity. Ultimately, CGRP and SP both accelerate fracture healing, while VIP and PACAP seem to negatively impact healing. Unlike the aforementioned neuropeptides, the neurotrophins nerve growth factor (NGF) and brain-derived neurotrophic factor (BDNF) have more uniform effects. Both factors upregulate osteoblastic activity, osteoclastic activity, and, in vivo, stimulate osteogenesis to promote fracture healing. Future research will need to clarify the exact mechanism by which the neuropeptides and neurotrophins influence fracture healing. Specifically, understanding the optimal expression patterns for these proteins in the fracture healing process may lead to therapies that can maximize their bone-healing capabilities and minimize their pain-promoting effects. Finally, further examination of protein-sequestering antibodies and/or small molecule agonists and antagonists may lead to new therapies that can decrease the rate of delayed union/nonunion outcomes and fracture-associated pain.
[Changes of Element Contents and Their Interrelationship in Tibia of Osteoporotic Rats]

Abstract:OBJECTIVE: To observe the changes of bone element contents in osteoporosis and their interrelationship. METHODS: Twelve female SD rats,10-month-old, were bilaterally ovariectomized (OVX group) and another ten rats were received sham-operation under anesthesia (SHAM group).The element contents in tibia, including Ca, P, Mg, Zn, Mn, Fe, Cu, Mo and Cr, were determined by atomic absorption spectrophotometer 7 month later. The data of contents of all elements were analyzed by simple regression. RESULTS: Compared with the SHAM group rats, the contents of Ca, P and Mg were decreased by 6.6 %(P<0.05), 6.3 %(P<0.05) and 14.9 %(P<0.01) respectively. The contents of Zn and Fe were reduced by 15.2 %(P<0.01) and 35.1 %(P<0.01) separately, Mo and Cr were decreased by 12.2 %(P>0.05) and 14.0 %(P>0.05), while the contents of Mn, Cu and Co were shown no change. There was a significant correlation among the contents of Mg, Mn, Zn, Ca and P. CONCLUSION: The contents of Ca, P, Mg, Zn and Fe were matkedly reduced in bone of osteoporotic rats induced by ovariectomy.
Estrogen deficiency impedes fracture healing despite eliminating the excessive absorption of the posterior callus in a semi-fixed distal tibial fracture mouse model

Abstract:Background: Treatment of distal tibial fractures is a challenge due to their specific anatomical location. However, there is no appropriate mouse model to simulate a clinical distal tibial fracture for basic research. The aim of this investigation was to evaluate the feasibility of simulating a clinical fracture of the distal tibia of mice and to investigate the effect of ovariectomy (OVX)-induced osteoporosis on fracture healing in this model. Methods: Sixty female 8-week-old C57BL/6 mice were randomly divided into two groups, either sham or OVX. A semi-fixation distal tibia fracture was established in the right tibia after 8 weeks of OVX. The right tibias were collected at 7, 14, 21, and 28 days post fracture. Results: In the semi-fixation distal tibia fracture model, the posterior callus in the sham group showed excessive bone resorption and lower bone mass phenotype compared with the anterior site; a similar trend was not found in the OVX group. At 28 days post fracture, the posterior callus was more mineralized than the anterior callus in the OVX group. Although the fracture healing of the sham group showed a special phenotype in this mode, the progress and quality of fracture healing were still better than those of the OVX group. Conclusion: A semi-fixed distal tibial closed fracture mouse model was successfully established. In this model, excess bone resorption of the posterior callus impaired normal fracture healing, but not in OVX-induced osteoporotic bone. Although the stress shielding effect was not observed in the OVX group, impaired bone healing caused by OVX was still present. Our results suggest that this fracture model may have potential for studies on distal tibial fractures and stress shielding.

The mechanism of pyrroloquinoline quinone influencing the fracture healing process of estrogen-deficient mice by inhibiting oxidative stress

Abstract:It is reported that oxidative stress plays a detrimental role in the process of bone fracture healing. And pyrroloquinoline quinone (PQQ) is used as antioxidant. However, there is no report about whether PQQ supplementation can promote fracture healing by eliminating oxidative stress. To investigate the protective effect of PQQ on fracture healing, open mid-diaphyseal femur fractures model were created in sham, ovariectomized (OVX) mice and PQQ-treated OVX mice. Our results confirmed that PQQ played a preventive and protective role in OVX-induced delay of bone fracture healing by inhibiting oxidative stress, subsequently promoting osteoblastic bone formation and inhibiting osteoclastic bone resorption. The findings of this study not only revealed the mechanism of PQQ supplementation in promoting fracture healing, but also provide experimental and theoretical basis for the clinical application of PQQ in the treatment of bone fracture.

Osteoporosis influence on fracture healing in ovariectomized rat: ultrastructural study

Abstract:Objective　To elucidate the influence of osteoporosis on the fracture healing in ovariectomized rat.　Methods　Twenty-four female 8-month-old SD rats were randomized into two groups: sham-operated (sham) (12 rats) and bilaterally ovariectomized (OVX) (12 rats). Three months later, a femoral fracture model was ostablished in both groups. We observed the healing process by transmission elcctron microscopy(TEM) at 3, 7,14, 21, 28, and 42 days after fracture.　Results　TEM findings, showed that the types of fracture healing cells, their ultrastructural changes and functional state were almost identical in both groups till 21 days after fracture. In the OVX group, the calcified cartilage was not absorbed and replaced by new woven bone. A lot of dead chondrocytes were found to be embedded in a calcified chondroid matrix in 28 days after fracture. After this period, osteoclastic bone resorption was aggravated and accompanied by osteocytic osteolysis during 28－42 days of fracture healing.　Conclusions　Osteoporosis greatly affects the fracture healing in the later period of healing. It is demonstrated that endochondral bone formation is delayed and osteoclastic bone resorption increased which was aggravated by osteocytic osteolysis during the period of bone callus remodeling.
Panax Notoginseng Saponins Regulate Angiogenic Cytokines Through the PI3K/AKT/mTOR Signaling Pathway to Promote Fracture Healing in Ovariectomized Rats

Abstract:Osteoporotic fractures seriously affect the quality of life of the elderly. Panax notoginseng saponins (PNS) have the potential function of preventing osteoporosis. The Phosphatidylinositol 3-kinase (PI3K)/protein kinase (AKT)/mammalian target of rapamycin (mTOR) pathway is involved in the regulation of osteoporosis and has been proven to be related to VEGF secretion and angiogenesis. Therefore, this study aimed to explore the effects of PNS on ovariectomized rats with osteoporotic fracture through the PI3K/AKT/mTOR pathway and angiogenesis-related factors. Female Sprague-Dawley rats were randomly divided into normal control, fracture model, ovariectomized fracture model, low-dose PNS (100 mg/kg/d), and high-dose PNS (200 mg/kg/d). The ovariectomized rat fracture model was established. In low and high dose groups, PNS was administered intraperitoneally. The vascularization of fracture ends was detected in vitro by micro-CT on the 7th, 14th, and 21st day after modeling, and the area and number of blood vessels in the unit field of vision of the callus healing plane were seen by hematoxylin-eosin staining. The expression levels of PI3K, AKT1, mTOR, hypoxia inducible factor-1; VEGF: vascular endothelial growth factor (HIF-1), VEGF, Ang-1, VEGFR2, and angiopoietin like 2 Gene (ANGPTL2) were determined using Western blotting. In the PNS treatment group, the area of cortical bone increased, the area of callus decreased, and the number and area of blood vessels increased significantly when compared with the ovariectomized fracture model group. PNS regulates the PI3K/AKT/mTOR signaling pathway and promotes the expression of vascular-related cytokines (VEGF, Ang-1, VEGFR2, and ANGPTL2) in osteoporotic fractures. PNS may regulate the expression of vascular-related factors through the PI3K/AKT/mTOR pathway and promote the healing of osteoporotic fractures in ovariectomized rats.
Endogenous Parathyroid Hormone Promotes Fracture Healing by Increasing Expression of BMPR2 through cAMP/PKA/CREB Pathway in Mice

Abstract:Background/Aims: Endogenous parathyroid hormone (PTH) plays an important role in fracture healing. This study investigated whether endogenous PTH regulates fracture healing by bone morphogenetic protein (BMP) and/or the transforming growth factor-β (TGF-β) signaling pathway. Methods: Eight-week-old wild-type (WT) and PTH-knockout (PTH KO) male mice were selected, and models of open right-femoral fracture were constructed. Fracture healing and callus characteristics of mice in the two groups were compared by X-ray, micro-computed tomography, histological, and immunohistochemical examinations. Bone marrow mesenchymal stem cells (BMMSCs) of 8-week-old WT and PTHKO male mice were obtained and induced into osteoblasts and chondrocytes. Results: We found that expression levels of Runt-related transcription factor (RUNX2), bone morphogenetic protein-receptor-type II (BMPR2), phosphorylated Smad 1/5/8, and phosphorylated cyclic adenosine monophosphate-responsive element binding protein (CREB) in the callus of PTHKO mice were significantly decreased, whereas no significant difference in expression of SOX9, TGF-βR2,or pSMAD2/3 was observed between PTHKO and WT mice. Additionally, the activity of osteoblast alkaline phosphatase was low at 7 days post-induction, and was upregulated by addition of PTH or dibutyryl cyclic adenosine monophosphate (dbcAMP) to the cell culture. Furthermore, H89 (protein kinase A inhibitor)eliminated the simulating effects of PTH and dbcAMP, and a low concentration of cyclic adenosine monophosphate (cAMP) was observed in PTHKO mouse BMMSCs. Conclusion: These results suggested that endogenous PTH enhanced BMPR2 expression by a cAMP/PKA/CREB pathway in osteoblasts, and increased RUNX2 expression through transduction of the BMP/pSMAD1/5/8 signaling pathway.

Abstract:Aim: The aims of this study are to investigate the effects of neurotransmitters NPY and CGRP on ERK signaling in fracture healing, and to identify the correlation between macrophage aggregation and fracture healing. Methods: Male Sprague-Dawley rats were used to build a fracture model. The neurotransmitter receptor inhibitors were injected intraperitoneally into the rats. Immunofluorescence staining and ELISA were employed to determine the expression of NPY and CGRP in fracture area and the peripheral blood, respectively. Micro-CT together with histological staining were utilized to assess the fracture healing conditions. Relative protein expression was determined using western blot. Immunofluorescence staining was used to detect the aggregation of macrophages in the injury area. Results: During fracture healing, the serum NPY and CGRP significantly increased. The levels of NPY and CGRP reached a peak in the 8th week and reduced significantly thereafter. NPY and CGRP inhibitors could inhibit fracture healing and down-regulate the phosphorylated ERK. Macrophages (NPY+ and CGRP+) aggregated in the injury area. Conclusion: NPY and CGRP participated in fracture healing, in which they were also shown to influence phosphorylated ERK expression. In addition, macrophages are involved in the fracture healing process.
