Diagnosis and Treatment of Sigmoid Sinus Thrombosis Following Closed Craniocerebral Injury

Abstract:Objective To discuss the diagnostic method and principle of treatment of sigmoid sinus thrombosis following the closed craniocerebral injury. Methods Seven articles related to sigmoid sinus thrombosis following closed craniocerebral injury, which were published between 1995 to January 2010, were found by science citation index expended (SCIE) data retrieved system. Fifteen patients with posttraumatic sigmoid sinus thrombosis were reported in these 7 articles. One patients with posttraumatic sigmoid thrombosis was conservatively treated in our department. The clinical data of these 16 patients including the patients' conditions, diagnosis, treatment and prognosis were analyzed. Results The GCS of the 16 patients with posttraumatic sigmoid sinus thrombosis, whose mean age was 20.9 years, ranged from 13 to 15 points. The symptoms mainly included terrible headache and vomiting produced by high intracranial pressure in all the patients. Of 16 patients with posttraumatic sigmoid sinus thrombosis, 12 had right sigmoid thrombosis and 4 left. Twelve patients among the patients suffered from posttraumatic sigmoid sinus thrombosis accompanied with the thrombosis of transvers sinuses or jugular veins. Of 16 patients, 13 (5 were treated by anticoagulation) were conservatively treated and 3 by surgery. The prognoses wrere good in all the patients. Conclusions The sigmoid sinus thrombosis should be highly suspected when the intracranial hypertension was unable to explained by the known conditions in the patients with closed craniocerebral injury. The diagnosis of the sigmoid sinus thrombosis can definitely made by computerized tomography angiography and MR venography in most of the patients, and by digital subtraction angiography must be performed to make definite diagnosis of the sigmoid sinus thrombosis in some patients. Most of the patients can be cured without anticoagulation and surgery, but some patients should be treated by anticoagulation under monitoring coagulation condition.
