Abstract:Objectives: To examine the handling system for patient complaints and to identify existing barriers that are associated with effective management of patient complaints in China.Setting: Key stakeholders of the handling system for patient complaints at the national, Shanghai municipal and hospital levels in China.Participants: 35 key informants including policymakers, hospital managers, healthcare providers, users and other stakeholders in Shanghai.Primary and secondary outcome measures: Semistructured interviews were conducted to understand the process of handling patient complaints and factors affecting the process and outcomes of patient complaint management.Results: The Chinese handling system for patient complaints was established in the past decade. Hospitals shoulder the most responsibility of patient complaint handling. Barriers to effective management of patient complaints included service users' low awareness of the systems in the initial stage of the process; poor capacity and skills of healthcare providers, incompetence and powerlessness of complaint handlers and non-transparent exchange of information during the process of complaint handling; conflicts between relevant actors and regulations and unjustifiable complaints by patients during solution settlements; and weak enforcement of regulations, deficient information for managing patient complaints and unwillingness of the hospitals to effectively handle complaints in the postcomplaint stage.Conclusions: Barriers to the effective management of patient complaints vary at the different stages of complaint handling and perspectives on these barriers differ between the service users and providers. Information, procedure design, human resources, system arrangement, unified legal system and regulations and factors shaping the social context all play important roles in effective patient complaint management.

Abstract:Based on the experiences of community care development in Britain,this paper aims to analyze the meanings,elements,barriers of home-care system of elderly in China.There are many differences in the characteristics of the meanings,in particular in the field of elderly social service.Researcher conducted qualitative date by using comparative analysis of literature and programs to answer the proposed aims of this research.There are valuable lessons that we can learn from the British experiences for developing community home-care system of elders in China,in particular its series of laws,reliable source of funding and professionals involved.Finally,based on the above discussions,the author reflects on the community development difficulties faced by China and offers some suggestions for the future.

Abstract:Primary healthcare institutions primarily refer to level-one hospitals and lower-level medical facilities. Due to limited medical resources, various medical equipment, and service quality cannot be compared with large hospitals. [ 1 ] However, medical complaints are inevitable in any healthcare institution. Complaints refer to the behavior of patients and their families reporting dissatisfaction with the services provided by the hospital or its staff during the process of receiving medical services to relevant departments. This is a way for patients to vent dissatisfaction and protect their rights. It is also a valuable, low-cost means of obtaining various expectations and needs of patients and continuously improving service quality, serving as important avenues and information resources. Abstract:Health service quality is a component of system performance, as Australian perspective. Implementing clinical governance, Australian managers have accountability of quality and safety. Government plays important roles on quality improvement as establishing administrative and research institutions, implementing audit, quality standards and evaluation projects. Hospitals have set up strategic management plans ensuring quality and safety improvement. Patients and community have actively involved in this process. In this article, authors focused on health service quality concept, management experience and regulatory practice in Australia, followed by a discussion on how those lessons meaningful for China practice.

Abstract:At present, the increasing trend of medical disputes has become a serious problem in the work of medical institutions, and directly affects the social stability and orderly development of the institutions. We searched the literature on medical disputes and crisis management in China and Australia within PubMed (2010-2019), China Knowledge Network (CNKI, 2010-2019), and Wanfang Data Knowledge Service Platform (2010-2019). There are several drawbacks in the management of hospital medical disputes in China: 1) the knowledge of crisis is unilateral and not systematic; 2) there are too much stereotype thoughts in crisis management; 3) the crisis attribution is too simple; the crisis impact assessment is insufficient. It is worth learning from Australia’s system, including the legal system, relevant non-governmental organizations, and doctor-patient communication. In view of the malpractice existing in China's medical dispute management, Australia's legal system for handling disputes, doctor-patient communication and other aspects are worthy of our reference. In particular, the construction of third-party supervision and mediation institutions and the prevention of doctor-patient disputes should be optimized.

Analysis on cognition and experience of obstetrical staff to hospital complaint management

Abstract:Objective: To understand the cognition and experience of obstetrical staff to hospital complaint management in Shanghai city. Methods: A questionnaire survey was performed in partial obstetricians and obstetrical nurses from twenty- nine maternity hospitals in Shanghai city in 2012. Results: Among the respondents,45. 6% of them knew Measures for the Administration of Patient Complaints in Hospitals,and the awareness rates varied in different occupations,positions,working years,and categories of hospitals; 89. 4% of them thought hospital complaint management was important,81. 7% of them expressed their willing to cooperate with patient's complaint handling, 48. 9% of them personally participated in the handling process of patients' complaint,the respondents who were complained by patients accounted for 36. 5%. Conclusion: It is necessary to enhance related education and training about Measures for the Administration of Patient Complaints in Hospitals among the obstetric staff,establish a learning process from complaints and improve health service.

Abstract:OBJECTIVE:Patient experience and satisfaction survey has been regarded as an important method of collecting patients' opinions on medical services and improving the quality of management. From 2005 to 2009, Australia Victorian patient satisfaction monitoring, VPSM, was introduced, localized and adopted by over 50 Chinese hospitals. To compare and analyze each localized applications of VPSM and submit suggestions for further applications.METHODS:The paper adopted the way of document research. The researchers collected all of the published and unpublished investigation plans and monitoring reports during these five years, compared the operations and the results of investigations and indicated the advantages and weaknesses.RESULTS:In respect of methods, they were similar but showed some small differences in details such as the questionnaire structure, the reminding letter and the organization of data acquisition and analysis. The localized VPSM was gradually moving towards standardization. The monitoring was able to find the changing of medical services and provided the referential information for management.CONCLUSION:The localization of VPSM is still on its way and it needs improving by means of extending application.

Abstract:Objective To objectively study the current progress of China Healthcare Improvement Initiative, and its effectiveness and shortcomings for further improvement. Methods Questionnaires were customized by expert consultation and pre-investigation, and distributed by the National Health Commission in April 2018 to the hospitals.Data of 5 469 hospitals were recovered and analyzed with a statistics software for descriptive analysis.Results Implementation progress of the five working systems varied with regions, and rooms of improvement were found in such aspects as outpatient appointment, clinical pathway management and social work system.In 2017, the proportion of outpatient appointments of 5 469 hospitals averaged 19.1%.Progress of the ten major service models also varied, as defects were found in such service models as continuous medical service, intelligent service, and interconnection service.In 2017, only 6.7% of the 5 469 hospitals had put in place hierarchical integrated clinical pathways within their medical alliances.Conclusions Improvement of the medical services calls for not only the efforts of medical institutions themselves, but also top-level design by the local health authorities. In particular, an online information platform should be established for the whole region, to unify the information standards and processes, and corresponding mechanisms and system support are needed.

Abstract:Aims/Background: In an era where patient-centred care is paramount, effectively managing and analyzing hospital complaints is crucial for improving service quality and patient satisfaction. This study examines hospital complaints to enhance management practices by differentiating between surgery-related and non-surgery-related grievances. By identifying patterns in complaint types and outcomes, we aim to inform targeted quality improvement strategies that address specific patient concerns and boost operational efficiency. Methods: The study utilized data from an internal complaint management system over one year. Complaints were categorized as either surgery-related or non-surgery-related. Descriptive statistics and cross-tabulation analysis were employed to examine the data. The sample comprised 132 complaints, with 67 being surgery-related and 65 non-surgery-related. Results: The analysis revealed that surgery-related complaints frequently involved issues with 'Patient Communication' and 'Surgical Error', while non-surgery-related complaints were primarily about the 'Medical Treatment Process'. The Surgery Department received the highest number of complaints, indicating a critical area for intervention. Additionally, the correlation between complaint types and outcomes provided insights into potential areas for improvement. Conclusion: The findings highlight the need for targeted communication training and procedural enhancements in surgical departments. Non-surgical departments should focus on improving treatment protocols and transparency. These strategies can reduce complaints and improve patient satisfaction. Future research should develop and test interventions based on these insights to further enhance healthcare quality.

Methods Literature review and case studies. Results Most violence happened in emergency department and mental/psychiatric services in Australia. Based on report of Australian Incident Monitoring System,causes of violence could be classified as patient,staff and system reasons.Violence in hospital has been recognised as part of nature of medical professional practice,and it is unacceptable as violence both negatively affect clinicians and health consumers. Staff training is found for improving knowledge and skill of preventing violence and protecting medical staff. A risk screening and management tool is used in hospital,which roles as first prevention strategy for safety of staff and patients. Conclusion Violence in Chinese hospital is increasing along with recent health policy implementation for health sector reform and development. Attribution of violence was likely pointed to social and political aspects,such as lack of trust,unsatisfied social harmony,increased social inequality and weakness of social security. Contribution of patients and their families has been under estimated,especially when ‘patient-centred health care’ encouraged.In addition,as lack of information on violence reporting and analysis and weak of mental health service in China,patient/family’s attack under psychiatric reasons are unknown.It is recommended that an information system for incidence monitoring is crucial for evidence-based planning of violence prevention and protection. The nature of violence then could be further discovered.Furthermore,Chinese hospital managers could develop their tool of risk screening and management.

Abstract:Objective To provide relevant evidence for improving quality of general practice through collecting and analysing patients' experiences and opinions.Methods A self-administered questionnaire investigation that made through localised Australian RACGP patients satisfaction questionnaire was carried out at a community health service centre and its stations in Beijing.Results There were 10.7% and 25.7% respondents suggested that facilities for children and aged patients in community health facilities as or excellent.Half of respondents argued necessary of updating community doctor's knowledge and skills,especially,80% of them thought doctor's ability to deal with child patient need further improvement.More than 50% of patients made other complaints such as long waiting time,unfriendly attitude of the information desk staff.Inter-personal quality of care should be improved,especially,privacy protection and doctor's concerning on patient's illness.There were only 30.7% and 22.7% of patients rated follow up service and referral service as good,very good or excellent,respectively.It indicates that quality of pre and post general practice service need to be improved for better accessibility of both community service and specialist services.Conclusion From views of patients,accessibility and effectiveness of community health services are facing challenges.It is necessary of examining structure,process and outcome quality of community health care along with phases of before,during and after general practice services.To link with quality improvement,the monitoring should be allied with planning and implementation of community health quality initiatives.
The Healthcare Needs of International Clients in China: A Qualitative Study

Abstract:Background: Due to globalization, an increasing number of international visitors comes to China. The needs of their medical care are understudied, which can cause low patient satisfaction and lead to poor clinical outcomes for the clients. To meet those international clients' medical care needs, hospitals in China are seeking strategies to improve services. Purpose: The aim of this study was to explore international clients' medical care experiences in China, and their perceptions of the quality of these international healthcare services. Patients and methods: In May 2020, focus group interviews with 24 clients and four healthcare professionals were conducted in the international clinic at Sir Run Run Shaw Hospital (SRRSH). In the client group, 24 representatives of international clients from nine countries were invited and divided into three groups to discuss healthcare needs of international clients who seek healthcare in China. Four healthcare providers, including two nurses and two physicians who usually serve in the international clinic, were also interviewed. Data were analyzed using hybrid inductive/deductive thematic analysis. Results: Six major healthcare needs of international clients were identified, namely: needs for privacy and confidentiality; effective communication; multicultural sensitive care; pleasant environments; qualified care and procedures; and respect. International healthcare is a complex process for both international clients and healthcare professionals. Conclusion: The government and institutional administrators around the world should construct the policies and protocols and integrate cultural competence, communication skills, and privacy and confidentiality protection into health professionals training program to ensure the quality services in the international clinics.

Abstract:Background: Improving health care in cities with a diverse, international population is crucial for ensuring health equity, particularly for foreigners facing challenges due to cultural and language barriers. This situation is especially relevant in China, a major destination for expatriates and travelers, where optimizing health care services and incorporating international standards in the public sector are vital. Achieving this involves understanding the operational details, cultural and linguistic nuances, and advancing medical digitalization. A strategic approach focusing on cultural competence and awareness of health care systems is essential for effectively navigating health care for foreigners and expatriates in China. Objective: The aim of this study was to perform an in-depth analysis of the subjective and objective experiences of local and international patients in public hospitals in China to provide a basis for enhancing the medical experience of all patients. Methods: A structured questionnaire was provided to patients at an international outpatient service of a top-tier university hospital in China. Qualitative analysis of the survey responses was performed to methodically categorize and analyze medical treatment, focusing on patient demand and satisfaction across four main category elements ("high demand, high satisfaction"; "high demand, low satisfaction"; "low demand, high satisfaction"; and "low demand, low satisfaction"), enabling a detailed cross-sectional analysis to identify areas for improvement. Results: Elements falling under "high demand, high satisfaction" for both Chinese and international patients were primarily in the realms of medical quality and treatment processes. In contrast, elements identified as "high demand, low satisfaction" were significantly different between the two patient groups. Conclusions: The findings highlight the importance of systematic, objective research in advancing the quality of international health care services within China's leading academic medical centers. Key to this improvement is rigorous quality control involving both patients and providers. This study highlights the necessity of certifying such centers and emphasizes the role of digital platforms in disseminating information about medical services. This strategy is expected to cater to diverse patient needs, enhancing the overall patient experience. Furthermore, by developing comprehensive diagnosis and treatment services and highlighting the superior quality and costs associated with international health care, these efforts aim to foster a sense of belonging among international patients and increase the attractiveness of China's medical services for this demographic.

Abstract:Objective To analyse the causes of medical complaints ,seek methods to prevent medical disputes .Methods All medical complaints were collected and analyzed retrospectively in 2011 .Results In 278 medical complaints ,there were 161 (57 .91% ) complaints by telephone ,101 (36 .33% ) complaints by hospital electronic touch screen ,16 (5 .76% ) complaints by E-mail;the top three complaint departments were vasculocardiology department 33(11 .87% ) ,department of gynaecology and obstet-rics 29(10 .43% ) ,urology department 26(9 .35% ) ,respectively ;the top three complaint reason were poor doctor-patient communi-cation 101(36 .33% ) ,poor service level 69(24 .82% ) and ward disadvantaged 45(16 .19% ) ,respectively .Conclusion Prevention of medical complaints of the most important measures is to improve the skill of doctor-patient communication and technical level of medical staff .Furthermore ,hospital should strengthen the construction of infrastructure .

Abstract:Objective To compare and analyze the continuous and comprehensive characters of hospital service and community health service.Methods A questionnaire on patients who seek for medical care on community health service (CHS) institutions and hospitals from 3 provinces (or municipalities) was carried out. Health education, health instruction, continuity of care and situation of referral were compared.Results The ratios of patients who got health instruction were 76.60% and 87.34% of those who went to hospitals and CHS institutions respectively; the ratios of patients' family members who got health instruction in the two types were 26.10% and 70.19% respectively. There were 38.25% of the patients who went to hospitals and saw the same doctor frequently, whereas 71.95% of the patients who went to CHSI and saw the same doctor; and 62.65% of patients who went to CHS institutions and got the instruction of seeking further medical service.Conclusion Health education has been widely carried on, however the ways and methods of it should be expanded. The continuous medical care is the foundation of CHS. Family-based continuous CHS is the key to improve the quality of CHS, and the direction of developing CHS. The instruction of further health care fro the patients should be enhanced, and the system of dual-referral should be founded.
