Abstract:A 51-year-old woman presented with multiple erythematous papules, nodules, and infiltrated plaques with necrotic ulcers, exudates, and crusts on her left cheek (Fig. 1a) . Histopathological examination revealed round yeast cells scattered in the dermis and within the multinucleated giant cells (Fig. 1b). Fungal culture of the biopsy specimen showed multiple, velvety, brown colonies on Sabouraud glucose agar (Fig. 1c). This microbe was further identified as Sporothrix globosa by micromorphological characteristics (Orofino-Costa et al., 2017Orofino-Costa R. Macedo P.M. The skin lesions resolved after treatment with itraconazole 400 mg/day in combination with terbinafine 250 mg/day orally for three months. The typical clinical presentation of sporotrichosis is a “sporotrichoid” pattern, which refers to the primary inoculation site plus a spread of the infection along the lymphatics in a nodular pattern (Bolognia et al., 2018Bolognia J.L. However, this patient showed multiple crusted nodules and infiltrated plaques resembling molluscum contagiosum–like lesions of cryptococcosis and Talaromyces marneffei infection. This case suggests that increased awareness is essential for fungal infections in clinical practice.
