Abstract:Objective To investigate the current status of aspirin use for control of cardiovascular diseases in urban residents of Beijing,and the relative factors influencing compliance of long term use of aspirin.Methods Totally 932 patients,who were on the treatment of aspirin for prevention and control of cardiovascular diseases,were randomly chosen from local communities in Beijing.They were divided into two groups: aspirin-continuance and aspirin-discontinuance.Results Among the 932 patients,64.3% of them took aspirin for primary prevention and 35.7% for second prevention.Some 4.3% of the patients had visceral bleeding,of which the incidences of epistaxis,black stool,gingival bleeding,hematuria,and hemoptysis were 1.7%,1.0%,1.0%,0.5%,and 0.1% respectively.The incidence of epigastric pain or discomfort was 9.0%.Some 13.7% of the aspirin users discontinued the therapy.A 10-year risk assessment for cardiovascular diseases showed that the risk was higher in discontinuance group than in continuance group(10% vs.7%,P=0.023).In discontinuance group the incidences of epigastric pain or discomfort,visceral bleeding and subcutaneous hemorrhage were higher,there were more patients who believed that it was not necessary to take aspirin and worried about adverse drug reaction the dose of aspirin was lower and the duration was shorter,as compared with continuance groups.Multivariate analysis indicated that the major reasons for aspirin discontinuation were visceral bleeding,self-reported low necessity for aspirin taking,subcutaneous hemorrhage,and worry about adverse drug reaction,epigastric pain or discomfort.And the longer the duration of a patient′s taking aspirin was,the better the patient′s continuance was.Conclusion The aspirin use by urban residents in Beijing for controlling cardiovascular diseases remain to be standardized.The aspirin discontinuance is mainly associated with the adverse drug reaction of aspirin,self-reported low necessity and worry about adverse drug reaction for aspirin taking.
