Clinicopathologic features of extraventricular neurocytoma of the spinal cord

Abstract:Purpose To study clinicopathologic features,diagnosis,treatment and prognosis of extraventricular neurocytoma (ENC),especially spinal ENC. Methods A case of spinal neurocytoma was studied by light microscopy,electron microscopy and immunohistochemical staining. Based on the present case and 9 reported cases in English literature,clinicopathologic features,treatment and prognosis of spinal ENC were analyzed. Results We reported a case of spinal ENC in a 27-year-old man. The tumor occurred in cervical spinal cord. Microscopic examination showed a typical ENC,composed of oligodendroglia-like cells (OLC). Ganglion-like cells were prominent. Neuropil-island was not present. Necrosis,vascular endothelial proliferation and mitosis were not seen. Immunohistochemical examination showed that Syn,Neu-N and NF were positive. Besides,a few tumor cells were reactive to GFAP. MIB-1 labeling index was less than 1%. Electron microscopic investigation showed that the cell cytoplasm and processes was populated by rough endoplasmic reticulum and free ribosomes. Distinct nucleoli were seen in some cells. Neurosecretory granules were occasional. In addition,a few cells contained glial filament at the same time. In the 10 cases of spinal neurocytoma,the male-to-female ratio was 9 :1. The age of the patients ranged form 8 to 67 years. Most occurred in the cervical or cervico-thoracic region. 7 cases showed typical histological features,while 3 cases were atypical based upon the presence of necrosis,vascular endothelial proliferation,increased mitotic activity (≥2/10 HPF) or high proliferation rates (>2%). Follow-up showed that 4 patients with typical histological features were alive after total resection and 2 cases of atypical spinal ENC recurred after subtotal resection. Conclusions ENC is a new entity in 2007 WHO Classification of Tumours (WHO grade Ⅱ). Compared with CNC,ENC exhibits a wider morphologic spectrum. High degree of ganglionic differentiation is noted in ENC,but neuropil-island is less common than CNC. ENC is often misdiagnosed as other tumors composed of OLC. Immunohistochemical examination and electron microscopy are important for diagnosis. Gross total resection is the treatment of choice. In view of the benign histological appearances,we think that postoperative adjuvant radiotherapy should be avoided after total resection. However,radiation therapy is best reserved for subtotal resection,atypical,high proliferation rates (>2%),progressive and recurrent spinal ENC.
