Prognostic influencing factors in stage ⅠB non-small cell lung cancer

Abstract:Objective:To investigate the prognostic influencing factors in stage ⅠB non-small cell lung cancer(NSCLC).Methods:Sixty-seven NSCLC patients with pathological T2N0 disease who underwent pulmonary lobectomy from Jun.1,2004 to Dec.31,2004 in our center were examined.These patients accepted either limited mediastinal lymphnode sampling or systematic lymphadenectomy.Three hundred and inity-two lymph nodes in all were resected containing 201 mediastinal nodal stations(103 superior mediastinal nodal stations,98 inferior mediastinal nodal stations). The sizes of primary tumors were measured.The status of visceral pleura involved by tumor and other relevant factors were examined.All of the patients were followed up,the last visit date was Jan. 1,2007. The data was analysed with the software SPSS10.0. Kaplan-Meier was used in survival analysis and Cox regression was adopted to study the relationship between some factors and survival time.Results:Eight patients were dead,2-year survival rate was 88.4%.The analysis showed that the patients with 3 or more than 3 mediastinal nodal stations resected had better prognosis than those with less than 3 mediastinal nodal stations resected(P=0.041)and the patients with size of primary tumor less than 4cm in diameter also had better prognosis than those with size of primary tumor bigger than 4cm(P=0.016).The study showed that there was no significant relationship among number of lymph nodes resected,status of visceral pleura involved by tumor,post-operative chemotherapy,location of primary tumor and survival time.Conclusion:The number of negative mediastinal nodal station in pulmonary lobectomy maybe correlate with survival time in the stage ⅠB NSCLC.The patients with size of primary tumor bigger than 4cm are apt to recurrence and have poor prognosis probably,so further treatment would be beneficial to these patients.
