Abstract:With the MTT tetrazolium WEHI 164 clone 13 cell cytotoxicity assay, we measured TNF alpha (tumor necrosis factor) activity in synovial fluids of TMJDS (Temporomandibular joint dysfunction syndrome). We found no detected TNF alpha level from 5 patients with muscle dysfunction, raised TNF alpha levels from 5 of 11 patients with internal derangement and from 9 of 11 patients with organic destruction (osteoarthritis). The findings of biologically active TNF alpha in synovial fluids of TMJDS suggest that TNF alpha may play a role in the pathogenesis of TMJDS.
Interleukin-6 in Synovial Fluid and HLA-DR Expression in Synovium from Patients with Temporomandibular Disorders.

Abstract:Interleukin-6-dependent mouse hybridoma cell line KD83 was used to test the biologic activity of interleukin-6 in synovial fluid from 37 patients with temporomandibular disorders. The results showed that the interleukin-6 level was greater than 100 U/mL in 13 of 18 patients with degenerative joint disease and in five of 12 patients with temporomandibular disc displacement. However, the interleukin-6 level was less than 100 U/mL (range, 20 to 75 U/mL) in all patients with masticatory muscle disorder. It has been found that degenerative joint disease tends to have acute and chronic stages, and interleukin-6 activity was probably related to the acute stage in the patients. Histologic studies of the synovium from seven patients with degenerative joint disease showed a variable degree of hyperplasia of the synovial lining cells and chronic inflammation in five of eight specimens. Immunostaining studies clearly showed the presence of significantly more HLA-DR-expressing cells (human leukocyte antigen-D-related) in synovium. Although it is unlikely that immune responses play an important primary role in initiating synovial inflammation and cartilage destruction, immune reactions may be one important factor in the maintenance and severity of some patients with temporomandibular disorders.
Osteocalcin and Estradiol in Synovial Fluid of the Patients with Temporomandibular Disorders

Abstract:Objective To determine whether osteocalcin and estrogen existed in synovial fluid and to study the relationship between the synovial osteocalcin and estradiol levels and temporomandibular disorders.Methods Synovial fluid was obtained from 31 temporomandibular joints (TMJs) of the patients with TMD for osteocalcin,and 42 TMJs for estradiol detection respectively.All patients were diagnosed as internal derangement (ID) or osteoarthrosis by raiographs and clinical examination.7 synovial fluid samples of healthy volunteers served as normal control.The concentration of osteocalcin and estradiol in synovial fluid was determined by radioimmunoassay (RIA).The association of synovial osteocallcin and estradiol levels with TMD and their clinical significances were also discussed.Results Synovial fluid osteocalcin was found in all patients and controls.There was no significant difference in synovial osteocalcin level between ID and normal control (P＞0.05),but the concentration of synovial osteocalcin was significant higher in OA than that of the normal control and ID (P＜0.05).Synovial fluid E2 was found in all patients and controls.The concentration of synovial E2 was significant higher in ID and OA than that of normal control,but there was no significant difference in synovial E2 level between ID and OA.Synovial E2 level was much higher in TMJs with osteosclerotic change and no difference between male and female patients with TMD.Conclusion These results suggest that estrogen could be involved in the pathogenesis and development of TMD.High level of synovial estradiol may contribute to osteoselerotic change of TMJs.Meanwhile,TMJOA may relate to local increase of bone turnover and abnormalities of bone metabolism.Quantification of osteocalcin in synovial fluid holds promise as a useful means of assesing the alteration of bone at the molecular level in the patients with TMD.
Effect of TLR4/MyD88 Signaling Pathway on Expression of IL-1β and TNF-α in Synovial Fibroblasts from Temporomandibular Joint Exposed to Lipopolysaccharide

Abstract:Accumulating evidence from previous studies suggested that interleukin-1 (IL-1β) and tumor necrosis factor-α (TNF-α) play an important role in pathogenesis of temporomandibular disorders (TMD). However, the cell surface receptors and the intracellular signal pathways leading to these cytokines expression are not fully understood. In the current study, we investigated the roles of Toll-like receptor 4 (TLR4) and its adaptor myeloid differentiation factor 88 (MyD88) in the expression of IL-1β and TNF-α in synovial fibroblasts (SFs) separated from rat temporomandibular joint (TMJ) with lipopolysaccharide (LPS) stimulation. The results showed that treatment with LPS could increase TLR4, MyD88, IL-1β, and TNF-α expression at both mRNA and protein levels. In addition, increased expression of IL-1β and TNF-α could be blocked by treatment with TAK-242, a blocker of TLR4 signaling, and also by MyD88 inhibitory peptide (MIP). These findings suggested that maybe TLR4/MyD88 signal transduction pathway participates in enhanced expression of IL-1 and TNF-α in patients with TMD. The activation of TLR4/MyD88 signal transduction pathway which results in production of proinflammatory factors may play a role in the pathogenesis of TMD.

Interleukin-6 family of cytokines as biochemical markers of osseous changes in the temporomandibular joint disorders

Abstract:The aim of this study was to find out whether there was a correlationship between the concentrations of interleukin-6 (IL-6), IL-11, and IL-17 in synovial fluid and osseous changes in the condyle. The synovial fluid was obtained from 61 patients with temporomandibular joint disorders (TMD) and seven healthy volunteers (controls). The concentrations of IL-6, IL-11, and IL-17 were measured by enzyme-linked immunosorbent assay. IL-6 was detected in 43 of 59 (73%), IL-11 in 23 of 52 (44%) and IL-17 in 14 of 51 (27%) samples of synovial fluid. The concentrations of IL-6 and IL-11 in the joints with osseous changes in the condyle were significantly higher than in the joints without osseous changes (P < 0.05) and also higher than in the joints of the control group (P < 0.05). In addition, there was a correlation of concentrations between IL-6 and IL-11 (P < 0.05). These results suggest that IL-6 and IL-11 may participate in the pathogenesis of TMD and induce osseous changes in the condyle.
[Substance P in Synovial Fluid in Patients with Temporomandibular Joint Dysfunction Syndrome].

Abstract:Radioimmunoassay technique was used to measure substance P levels in synovial fluid samples from thirty patients with temporomandibular joint dysfunction syndrome (TMJDS). Twenty-four samples (80%) were found to have substance P. Concentrations of substance P in the synovial fluid from patients with painful TMJDS were higher than those patients without pain (P < 0.05). The study supported the evidence for the secondary synovitis in painful TMJDS, and suggested that the nervous system play a role in the pathophysiology of TMJDS.

Abstract:Introduction: Temporomandibular joint (TMJ) osteoarthritis (OA) is a common TMJ degenerative disease with an unclear mechanism. Synovial fluid (SF), an important component of TMJ, contains various proteins and metabolites that may directly contribute to OA. The present study aimed to investigate the influence of SF in TMJOA at the metabolite level. Methods: Untargeted and widely targeted metabolic profiling were employed to identify metabolic changes in SF of 90 patients with different TMJOA grades according to TMJ magnetic resonance imaging. Results: A total 1498 metabolites were detected. Most of the metabolites were amino acids and associated metabolites, benzene and substituted derivatives, and lipids. Among patients with mild, moderate and severe TMJOA, 164 gradually increasing and 176 gradually decreasing metabolites were identified, indicating that biosynthesis of cofactors, choline metabolism, mineral absorption and selenocompound metabolism are closely related to TMJOA grade. Combined metabolomics and clinical examination revealed 37 upregulated metabolites and 16 downregulated metabolites in patients with pain, of which 19 and 26 metabolites were positively and negatively correlated, respectively, with maximum interincisal opening. A model was constructed to diagnose TMJOA grade and nine biomarkers were identified. The identified metabolites are key to exploring the mechanism of TMJOA. Discussion: In the present study, a metabolic profile was constructed and assessed using a much larger number of human SF samples from patients with TMJOA, and a model was established to contribute to the diagnosis of TMJOA grade. The findings expand our knowledge of metabolites in human SF of TMJOA patients, and provide an important basis for further research on the pathogenesis and treatment of TMJOA.

[Interleukin-6 Activity in Synovial Fluids of Temporomandibular Joint Distrubance Syndrome].

Abstract:Using the IL-6-dependent mouse hybridoma cell line KD 83, IL-6 biological activity in synovial fluids of the patients with Temporomandibular Joint Disturbance Syndrome (TMJDS) was tested. The results show that IL-6 level is high in 13 cases of 18 patients with TMJ degenerative changes and 5 cases of 12 patients with TMJ Disc Displacement. IL-6 level is less than 100 U/ml in all patients with Masticatory Muscle Disorder (59.4 +/- 29.4 U/ml). In addition, it has been found that TMJDS tends to have acute and chronic stages and IL-6 is probably related to acute stage of the patients.

Abstract:Objective: The aim of this study was to evaluate the levels of interleukin (IL)-1beta, IL-6, IL-8, IL-11, and tumor necrosis factor (TNF)-alpha in temporomandibular joint (TMJ) synovial fluid of the patients with internal derangement before and 2 weeks after arthrocentesis. Study design: Forty TMJs of 35 patients (29 females and 6 males, mean age 22.9 years) were included to the study. TMJs were divided into 2 groups: disc displacement with reduction (Group 1, n = 24) and disc displacement without reduction (Group 2, n = 16). Synovial fluid samples were obtained before and 2 weeks after arthrocentesis. IL-1beta, IL-6, IL-8, IL-11, and TNF-alpha concentrations were measured by using specific kits. Results: Two weeks after the arthrocentesis procedures, all cytokines were found to be significantly decreased (P < .05) both in Group 1 and Group 2. The difference between 2 groups was insignificant (P > .05). Conclusion: Arthrocentesis is an effective technique for eliminating the studied cytokines from the TMJ synovial fluid.

Abstract:Tumor necrosis factor alpha (TNF-α) is a homotrimeric protein encoded within the major histocompatibility complex (MHC). TNF-α can bind its receptors in the body to play a role in immune regulation and to participate in the various pathophysiology processes including fever, inflammation, infection, wound healing and tumors necrosis. TNF-α, an important component of inflammatory pathways, is up-regulated in the synovial tissue, synovial fluid and serum of rheumatoid arthritis (RA) patients. In RA patients, TNF-α along with many kinds of tissue factors and matrix proteins together promotes the inflammatory response, abnormal apoptosis of synovial cells, pannus formation and cartilage and bone destruction, which maintain the sustainability of the progress of RA. Thus, TNF-α may be a clinical indicator of RA activity as well as an effective target for RA.
Pathologic Study on Synovial Fluid after Intra Articular Corticosteroid Therapy in Temporomandibular Joint Disorders

Abstract:Objective To study what happens after intra articular injection of corticosteroid. Methods Synovial fluid after intra articular corticosteroid therapy in temporomandibular joint disorders (TMD) was studied by hematoxylin eosin, PAS, and Atcian blue staining. Results The results were that broken synovial membrane and articular cartilage , lots of synovial lining cells, macrophagocytes, inflammatory cells and free cartilage matrix could be found in the lavage fluid. Conclusion The results suggest that drug synovectomy may be one mechanism about intra articular corticosteroid therapy in TMD, and this therapy could lead to progressive degradation and destruction of the articular cartilage. Arthrocentesis and lavage are needed three to five days after this therapy.
Structural and Ultrastructural Studies of the Synovial Membrane in Temporomandibular Joint Disturbance Syndrome.

Abstract:To gain further knowledge on temporomandibular joint disturbance syndrome (TMJDS), the synovial membranes of 7 patients and 3 cadavers were examined with light microscope and transmission electron microscope. The results showed that the synoviocytes degenerated or disappeared in TMJDS. The cytoplasm contained numerous fibrils, while the organelles diminished in number. The surface of the synovial membrane was covered with a fibrinoid material. The surface of the synovial membrane was covered with a fibrinoid material. The authors consider that these pathological changes reduce or eliminate the normal functions of the synovial membrane, which may lead to damage to the articular cartilage. The articular adhesion found sometimes in TMJDS may be related to the changes in the synovial membrane.

Abstract:HMGB1 usually serves as a damage-associated molecular pattern (DAMP) molecule (also known as alarmin) that regulates the inflammatory and immune responses via different receptors or direct uptake. Numerous studies have reported the association between HMGB1 and inflammatory diseases; however, its role in temporomandibular joint (TMJ) osteoarthritis (OA) has not been elucidated. In this retrospective study, we aimed to investigate HMGB1 levels in the synovial fluid (SF) in patients with TMJOA and TMID, their correlation with TMJOA and TMID severity, and the therapeutic effect of sodium hyaluronate (hyaluronic acid, HA) on TMJOA.

Macrophage Activation in Synovitis and Osteoarthritis of Temporomandibular Joint and Its Relationship with the Progression of Synovitis and Bone Remodeling

Abstract:This study investigates the regulatory mechanisms of synovial macrophages and their polarization in the progression of temporomandibular joint osteoarthritis (TMJOA). Macrophage depletion models were established by intra-articular injection of clodronate liposomes and unloaded liposomes. TMJOA was induced by intra-articular injection of 50 μL Complete Freund's Adjuvant and the surgery of disc perforation. The contralateral joint was used as the control group. The expression of F4/80, CD86, and CD206 in the synovium was detected by immunofluorescence staining analysis. Hematoxylin and eosin staining and TMJOA synovial score were detected to show the synovial changes in rat joints after TMJOA induction and macrophage depletion. Changes in rat cartilage after TMJOA induction and macrophage depletion were shown by safranin fast green staining. The bone-related parameters of rats' joints were evaluated by micro-computed tomography analysis. The TMJOA model induced by Complete Freund's Adjuvant injection and disc perforation aggravated synovial hyperplasia and showed a significant up-regulation of expression of F4/80-, CD86-, and CD206-positive cells. F4/80, CD86, and CD206 staining levels were significantly decreased in macrophage depletion rats, whereas the synovitis score further increased and cartilage and subchondral bone destruction was slightly aggravated. Macrophages were crucially involved in the progression of TMJOA, and macrophage depletion in TMJOA synoviocytes promoted synovitis and cartilage destruction.
Regulating Fibrocartilage Stem Cells via TNF-α/Nf-κB in TMJ Osteoarthritis

Abstract:In this study, we investigate harnessing fibrocartilage stem cell (FCSC) capacities by regulating tumor necrosis factor α (TNF-α) signaling for cartilage repair in temporomandibular joint osteoarthritis (TMJOA). Stem cell specifics for FCSCs were characterized in the presence of TNF-α. Etanercept as a TNF-α inhibitor and BAY 11-7082 as an Nf-κB inhibitor were used to study TNF-α regulation of FCSCs. Lineage tracing was performed in Gli1-CreERT + ;Tm fl/fl mice when etanercept (1 mg/kg, every 3 d) or isometric vehicle was subcutaneously injected to trace specific changes in FCSCs. Surgically induced TMJOA Sprague-Dawley rats were generated with BAY 11-7082 (5 mg/kg, every 3 d) or vehicle subcutaneous injection to investigate the functional role of TNF-α/Nf-κB in TMJOA. Anterior disc displacement (ADD) rabbits were used to analyze the therapeutic effect of etanercept as a TMJOA intra-articular treatment with etanercept (0.02 mg in 100 μL, every 2 wk) or isometric vehicle. In vitro, TNF-α inhibited proliferation of FCSCs and increased FCSC apoptosis. TNF-α activation interfered with osteogenic and chondrogenic differentiation of FCSCs, while etanercept could partially recover FCSC specificity from TNF-α. FCSC lineage tracing in Gli1-CreERT + ;Tm fl/fl mice showed that the chondrogenic capacity of Gli1 + cell lineage was markedly suppressed in osteoarthritis cartilage, the phenotype of which could be significantly rescued by etanercept. Specifically blocking the Nf-κB pathway could significantly weaken the regulatory effect of TNF-α on FCSC specificity in vitro and in TMJOA rats in vivo. Finally, intra-articular etanercept treatment efficiently rescued TMJ cartilage degeneration and growth retardation in ADD rabbits. Inhibition of TNF-α signaling reduced Nf-κB transcripts and recovered FCSC specificities. In vivo, etanercept treatment effectively rescued the osteoarthritis phenotype in TMJOA mice and ADD rabbits. These data suggest a novel therapeutic mechanism whereby TNF-α/Nf-κB inhibition promotes FCSC chondrogenic capacity for cartilage transformation in TMJOA.

Abstract:Objective. It has been proposed that mechanisms in the central nervous system contribute to the development and maintenance of pain in temporomandibular disorders. In this study, we tested whether spontaneous brain activity and functional connectivity (FC) were altered in patients with temporomandibular joint synovitis pain. Study Design. A prospective, cross-sectional design was adopted. Each of 8 patients and 10 healthy controls (HCs) underwent 2 sessions of functional magnetic resonance imaging: mouth closed and mouth open (painful for patients). Regional homogeneity (ReHo) was used to measure spontaneous brain activity in each participant. Brain areas with altered ReHo in patients conpared with HCs were identified, and their FCs with the rest of the brain was examined and compared. Results. Compared with I ICs, patients showed decreased pain-related Rel lo in the right anterior insula (rAI). The rAl showed a weaker positive FC with the left middle cingulate cortex (MCC) and a weaker negative FC with the right precuneus in patients compared with HCs. Furthermore, the rAl-MCC FC showed a negative correlation with pain intensity in patients. Conclusions. These results provide evidence supporting altered pain-related spontaneous brain activity and functional connectivity in the central nervous system in patients with temporomandibular joint synovitis pain.
Profiling of inflammatory mediators in the synovial fluid related to pain in knee osteoarthritis

Abstract:Abstract Background Inflammatory mediators in the synovial fluid (SF) play critical roles in the initiation and development of pain in knee osteoarthritis (KOA). However, data for inflammatory marker expression are conflicting, and the role of SF inflammatory mediators in neuropathic pain is not clear. Therefore, the aim of this study was to identify SF inflammatory mediators associated with nociceptive and neuropathic pain in KOA. Methods Levels of IL-1β, IL-6, TNF-α, macrophage colony-stimulating factor, MMP-3, MMP-13, metalloproteinase with thrombospondin motifs 5, calcitonin gene-related peptide, neuropeptide Y, substance P and bradykinin were measured using enzyme-linked immunosorbent assays in 86 patients. Nociceptive pain was assessed using the numeric rating scale (NRS), visual analog scale (VAS) and Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) pain score. Neuropathic pain was determined using the PainDETECT questionnaire. Moreover, knee function was evaluated by the WOMAC score and range of motion (ROM) assessments. Radiological grade was defined using the Kellgren-Lawrence (K-L) grading scale. Results Pain scores measured using different methods correlated highly with each other. A worse K-L grade and knee function were associated with worse pain. Expression of IL-1β and IL-6 was increased in the early stage compared with the late stage. The NRS score correlated positively with age, K-L grade, and the WOMAC score and negatively with ROM and TNF-α expression. The VAS correlated positively with age, K-L grade, and the WOMAC score but negatively with ROM and levels of IL-1β, IL-6 and TNF-α. The WOMAC pain score did not correlate with any of the inflammatory mediators measured; it correlated only with ROM. The PainDETECT score correlated only with the WOMAC score. Expression of other inflammatory mediators did not correlate with any of the pain scores. Conclusions IL-1β, IL-6 and TNF-α play critical roles in pain in the early stage of KOA and correlate with pain. The catabolic enzymes and neuropeptides measured do not correlate with nociceptive and neuropathic pain. New biomarkers related to pain in the late stage need to be further investigated.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Objective</h3><p>Osteoarthritis (OA) is a joint disease characterized by articular cartilage loss and afflicts many people worldwide. However, diagnostic methods and treatment options remain limited and are often characterized by low sensitivity and low efficacy. The focus of the present study was to identify proteomic biomarkers in synovial fluid to improve diagnosis and therapy of OA patients. Methods: Antibody array technology was utilized for protein expression profiling of synovial fluid from 24 OA patients and 24 healthy persons. Results: Compared with healthy persons, twenty proteins showed lower expression levels in OA patients, while thirty proteins had higher levels. Among these differential proteins, GITRL, CEACAM-1, FSH, EG-VEGF, FGF-4, PIGF, Cystatin EM and NT-4 were found for the first time to be differentially expressed in OA. Bioinformatics analysis showed that most of these differential proteins were involved leukocytes events, and some differentially expressed proteins including IL-18, CXCL1, CTLA4, MIP-3b, CD40, MMP-1, THBS1, CCL11, PAI-1, BAFF, aggrecan, angiogenin and follistatin were located in central positions of the protein–protein interaction (PPI) network. Conclusion: We speculate that leukocyte proliferation and migration to the joint may be an important pathogenesis of OA, which needs a further validation. The central proteins of the PPI network may play a more pivotal role in OA. The newly identified differentially expressed proteins may be novel biomarkers for OA diagnosis and targets for OA therapy.</p>

Synovial osteoclastogenesis mediated by chondrocyte‐secreted TNFα promotes TMJ condylar resorption

Abstract:Background Insufficient occlusal support (IOS) frequently causes subchondral bone absorption in temporomandibular joint osteoarthritis, and the underlying mechanism requires further investigation. Methods An IOS model was established by abrading rat molars. Micro‐computed tomography was used to evaluate subchondral bone changes. Osteoclastogenesis of synovium‐derived macrophages (SDMs) was confirmed by TRAP staining. Cartilage‐specific TNFα depletion was achieved by intra‐articular injection of adeno‐associated virus carrying shRNA against murine TNFα under control of collagen type II. In vitro, chondrocytes were mechanically compressed and conditioned medium (CM) was collected to detect its ability to induce osteoclastogenesis of SDMs. Results Synovial osteoclastogenesis and condyle resorption were observed following IOS. TNFα level was elevated in hypertrophic chondrocytes after IOS. Synovial Wnt5a level increased, but Wnt3a level decreased after IOS. Depletion of TNFα in chondrocytes alleviated the synovial osteoclastogenesis and condyle bone resorption. In vitro compression of chondrocytes potentiated TNFα expression and secretion. The CM promoted osteoclastogenesis of SDMs, which were partially prohibited by TNFα neutralizing antibody. Furthermore, inhibition of Wnt3a facilitated osteoclastogenesis, whereas inhibition of Wnt5a partially suppressed osteoclastogenesis, of SDMs cultured in CM. Conclusion Chondrocyte‐secreted TNFα induced by IOS is a critical regulator of synovial osteoclastogenesis and subsequent condylar resorption, partially through non‐canonical Wnt5a pathway.
