Clinicopathological analysis of 48 cases of gastric submucosal eminence lesions treated by endoscopic therapy

Abstract:Objective To evaluate the feasibility, safety and clinical effect of endoscopic therapy in the treatment of gastric submucosal eminence lesions (GSEL). Methods The clinical data of 48 patients of GSEL confirmed by endoscopy and endoscopic ultrasonography (EUS) were analyzed retrospectively. Results 11 cases of GSEL (0.2~0.5 cm in diameter) were removed completely without complications by high-frequency electrosurgery. 27 cases of GSEL (0.6~2.0 cm) were cut off completely by endoscopic submucosal dissection (ESD), endoscopic mucosal resection (EMR) and endoscopic submucosal excavation (ESE). 10 cases of GSEL (≥2.0 cm) were completely removed by ESD, EMR and ESE, with 1 case (2.2 cm) perforated (0.5 cm) and 1 case (2.0 cm) bleeding (50 ml). The perforated case was failed to be closed by ligation of nylon rope and was converted to open surgery. The bleeding case was hemostasised successfully by hemostatic forceps. No recurrence was found in these patients. Pathological examination showed negative incision margins and the diagnostic results were basically in accordance with that of EUS, including 26 gastrointestinal stromal tumors, 8 leimyomas, 4 lipomas and 10 ectopic pancreases. Conclusion Endoscopic therapy of gastric submucosal eminence lesions (GSEL) is safe, feasible and minimally invasive, especially suitable for the GSELs less than 2 cm in diameter. The treatment efficacy for the GSELs larger than 2.0 cm is not satisfactory.
