Changes of Postoperative Serum Lactate Levels and Lactate Clearance Predict Outcome in Neonates and Infants with Complex Congenital Heart Defects

Abstract:Cbjecfive To evaluate the relationship between postoperative serum lactate levels,lactate clearance and outcome in neonates and infants undergoing open heart surgery,Methods There wre 118 neonates and infants underwent repair of complex congenital heart disease under cardiopulmonary bypass in cardiac care unit,Children's Hospital of Fudan Lhiversity from Aug 2006 to 2007.Serial serum lactate levels were measured after open heart surgery.Results For survivors (n=112) and nonsurvivors (n=6),the 12-hour lactate levels wre a little increased (3.54±1.62 mmol/L vs 4.33±0.71 mmol /L,P0.05).Then the lactate levels of survivors significantly decreased than those of nonsurvivors (1.57±0.54 mmol/L vs 7.56±1.72 mmol /L,P0.05).The lactate clearance of survivors was significantly high than that of nonsurvivors (5.85%±29.75% vs-32.33% +25.04%,P0.001).Conclusions Changes of postoperative serum lactate levels after pediatric open heart surgery may be predictive of outcome.Lactate levels are higher in patients who go on to develop worse condition.Elevated postoperative lactate levels and decreased lactate clearance may reflect poor postoperative outcome.
