Abstract:Objective To investigate the phylogenetic relationship among a group of drug-resistant Acinetobacter baumannii(A.baumannii).Methods From January 2010 to December 2010,20 strains of drug-resistant A.baumannii were collected from sputum samples of inpatients in ICU of a hospital in Zhejiang,China.Then,resistance genes were analyzed by PCR,including 3 kinds of housekeeping genes(carO,gyrA,parC) and 55 kinds of resistance genes acquired by horizontal transfer to beta-lactams,aminoglycosides and quinolones,as well as 12 kinds of genetic markers of conjugal plasmids,transposons,insertion sequences,and integrons.In addition,sample cluster analysis was performed.Results In the 20 strains of drug-resistant A.baumannii,3 kinds of housekeeping genes(carO,gyrA,parC),4 kinds of acquired beta-lactam-resistant genes(TEM-1,ADC-30,ADC-60,OXA-23),5 kinds of acquired aminoglycoside-resistant genes(aac(3)-I,aac(6′)-Ib,ant(3′′)-I,aph(3′)-I,armA),2 kinds of drug efflux genes(adeB,qacE△1),and 5 kinds of genetic markers of mobile genetic elements(intI1,tnpU,tnp513,IS26,ISaba1) were positive.Sample cluster analysis showed that these strains were divided into 2 clusters: cluster A(multi-drug resistant,MDR) and cluster B(Pan-drug resistant,PDR).In addition,cluster A was divided into cluster A1(ADC-60 positive) and cluster A2(ADC-30 positive),and they were all of clonal propagation.Cluster B was all of clonal propagation except Strain No.8.Conclusion Clonal propagation exists in MDR and PDR strains.So it's important to monitor and control noscomial infection in time depending on phylogenetic analysis result.
Drug resistance of Acinetobacter baumannii during 2009-2011

Abstract:OBJECTIVE To investigate the distribution and drug resistance of Acinetobacter baumannii causing infection so as to provide basis for clinical prevention and control of nosocomial infections.METHODS By means of retrospective survey,the specimens sources where the 490 strains of A.baumannii were isolated from Jan 2009 to Dec 2011 were investigated,then the distribution of infections in departments and the drug resistance were analyzed.RESULTS Totally 95 strains of A.baumannii were isolated in 2009,178 strains in 2010,and 217 strains in 2011,showing an upward trend;78.57% of the strains were isolated from the sputum and throat swabs.The drug resistance rate to the commonly used antibiotics increased year by year;in 2011,the drug resistance rate to polymyxin E was 11.27%,cefperazone-sulbactam 31.34%;the drug resistance rates to other antibiotics were more than 50.0%.The multidrug-resistant strains and pandrug-resistant strains increased year by year.A novel variant of ADC-60(Genbank JQ692087)was detected.CONCLUSIONThe A.baumannii strains are highly drug resistant and multidrug-resistant.It is of great significance to the control of nosocomial infections and delay of bacterial resistance through the continuous moniring of drug resistance and the use of antibiotics based on the result of drug susceptibility testing as well as the bacterial resistance mechanism.
Risk Factors for and Clinical Outcomes of Polymicrobial Acinetobacter Baumannii Bloodstream Infections

Abstract:Background. Although the clinical features of Acinetobacter baumannii bloodstream infection are well described, the specific clinical characteristics of polymicrobial Acinetobacter baumannii bloodstream infection have been rarely reported. The objective of this study was to examine the risk factors for and clinical outcomes of polymicrobial Acinetobacter baumannii bloodstream infection. Methods. A retrospective observational study was performed from January 2013 to December 2018 in a tertiary hospital. All patients with Acinetobacter baumannii bloodstream infection were enrolled, and the data were collected from the electronic medical records. Results. A total of 594 patients were included, 21% (126/594) of whom had polymicrobial infection. The most common copathogen was Klebsiella pneumoniae (20.81%), followed by Pseudomonas aeruginosa (16.78%) and Enterococcus faecium (12.08%). Compared with monomicrobial Acinetobacter baumannii bloodstream infection, polymicrobial Acinetobacter baumannii bloodstream infection mostly originated from the skin and soft tissue (28.6% vs. 10.5%, p<0.001). Multivariate analysis revealed that burn injury was independently associated with polymicrobial Acinetobacter baumannii bloodstream infection (adjusted odds ratio, 3.569; 95% confidence interval, 1.954-6.516). Patients with polymicrobial Acinetobacter baumannii bloodstream infection were more likely to have a longer hospital length of stay [40 (21, 68) vs. 27 (16, 45), p<0.001] and more hospitalization days after bloodstream infection than those with monomicrobial Acinetobacter baumannii bloodstream infection [22 (8, 50) vs. 13 (4, 28), p<0.001]. However, no significant difference in mortality was observed between the two groups. Conclusions. Approximately one-fifth of Acinetobacter baumannii bloodstream infections were polymicrobial in this cohort. The main sources were skin and soft tissue infections, and burn injury was the only independent risk factor. Although mortality did not differ between the groups, considering the limitations of the study, further studies are required to assess the impact of polymicrobial (vs. monomicrobial) Acinetobacter baumannii bloodstream infection on outcomes.
Interactions between host epithelial cells and Acinetobacter baumannii promote the emergence of highly antibiotic resistant and highly mucoid strains

Abstract:Acinetobacter baumannii is an important nosocomial pathogen. Upon colonizing a host, A. baumannii are subjected to selective pressure by immune defenses as they adapt to the host environment. However, the mechanism of this pathoadaptation is unknown. Here, we established an in vitro system to evolve A. baumannii driven by the continuous selective pressure exerted by epithelial cells, and we used a combination of experimental evolution, phenotypic characterization and multi-omics analysis to address the underlying mechanism. When continuously exposed to selective pressure by pulmonary epithelial cells, A. baumannii showed ptk mutation-mediated mucoid conversion (reduced adhesion and increased anti-phagocytic ability) by enhancement of capsular exopolysaccharide chain length; rsmG mutation-mediated deficiency of 7-methylguanosine modification in the 524th nucleotide of 16S rRNA, which increased ribosome translation efficiency; and rnaseI mutation-mediated changes in outer membrane permeability and efflux pump expression. Together, these mutations altered susceptibility to a variety of antimicrobial agents, including the novel antibiotic cefiderocol, by regulating siderophore and siderophore-receptor biosynthesis. In conclusion, pulmonary epithelial cells modulate A. baumannii pathoadaptation, implicating the host-microbe interaction in the survival and persistence of A. baumannii.

Acinetobacter: an old friend, but a new enemy

Abstract:Acinetobacter emerged as a significant nosocomial pathogen during the late 1970s, probably as a consequence, at least in part, of increasing use of broad-spectrum antibiotics in hospitals. Most clinically significant isolates belong to the species Acinetobacter baumannii or its close relatives, with many infections concentrated in intensive care, burns or high dependency units treating severely ill or debilitated patients. Large outbreaks can occur in such units, involving the infection or colonisation of numerous patients by specific epidemic strains of A. baumannii. Recently, a particular problem has concerned cross-infection of injured military patients repatriated from combat regions of the world (e.g. Iraq and Afghanistan). Carbapenems have previously been the treatment of choice for infected patients, but increasing reports worldwide now describe A. baumannii isolates resistant to all conventional antimicrobial regimens. Data to support therapeutic use of the limited number of new antimicrobial agents (e.g. tigecycline) with in-vitro activity against these pathogens are still very limited. Detailed advice concerning prevention and control of outbreaks caused by multidrug-resistant strains of acinetobacter is available from the UK Health Protection Agency. In addition to antibiotic prescribing policies and audit, these measures focus on reinforcing standard infection control procedures and precautions, with particular attention to thorough cleaning of patient areas to take account of the long-term survival of acinetobacter after drying and inadequate disinfection. Despite these measures, the problem continues to escalate, with many hospitals worldwide now reporting outbreaks caused by multidrug-resistant strains of acinetobacter.

The epidemiology and antibiotic resistance of A.baumannii have been extensively reviewed, but the pathogenesis and virulence remain unclear. Proteomics analysis has been applied to study the mechanism of drug resistance, biofilm, micronutrient acquisition, and the extracellular compartment. This review summarizes applications of proteomics in A. baumannii, aiming to summarize novel insights into the mechanism of A. baumannii pathogenesis and drug resistance.

Abstract:Objective To investigate the distribution and drug resistance characteristics of Acinetobacter baumannii in patients with ventilator-associated pneumonia(VAP) in intensive care unit and provide guidance for clinical treatment.Methods The endotracheal aspirates of 79 patients received mechanical ventilation for more than 48 hours were cultured with a quantitative method and their antibiotic susceptibility was detected.Results Among 214 strains of pathogens isolated from the 79 patients,A.baumannii accounted for 36.9%,which showed high rate of resistance to routine antimicrobial agents.A number of pandrug-resistant and multidrug-resistant A.baumannii were found.Its resistance rate to cefoperazone sulbactam and imipenam were 55.6% and 68.4% respectively,with a remarkable uptrend.Conclution A.baumannii is one of the most common pathogens of ventilator-associated pneumonia.The antibiotic resistance of A.baumannii is still increasing,especially that of the MDR and PDR strains of A.baumannii,to which a great attention should be paid.

Abstract:Abstract Background Acinetobacter baumannii is a common nosocomial pathogen that poses a huge threat to global health. Owing to the severity of A. baumannii infections, it became necessary to investigate the epidemiological characteristics of A. baumannii in Chinese hospitals and find the reasons for the high antibiotic resistance rate and mortality. This study aimed to investigate the epidemiologic and genetic characteristics of A. baumannii isolated from patients with hospital acquired pneumonia (HAP), bloodstream infection (BSI) and urinary tract infection (UTI) in China and uncover potential mechanisms for multi-drug resistance and virulence characteristics of A. baumannii isolates. Results All isolates were classified into two primary clades in core gene-based phylogenetic relationship. Clonal complex 208 (CC208) mainly consisted of ST195 (32 %) and ST208 (24.6 %). CC208 and non-CC208 isolates had carbapenem resistance rates of 96.2 and 9.1 %, respectively. Core genes were enriched in ‘Amino acid transport and metabolism’, ‘Translation’, ‘Energy production and conversion’, ‘Transcription’, ‘Inorganic ion transport and metabolism’ and ‘Cell wall/membrane/envelope synthesis’. Most isolates possessed virulence factors related to polysaccharide biosynthesis, capsular polysaccharide synthesis and motility. Eleven isolates belong to ST369 or ST191 (oxford scheme) all had the virulence factor cap8E and it had a higher positive rate in UTI (35.3 %) than in BSI (18.9 %) and HAP (12.9 %). ABGRI1 antibiotic resistance islands were responsible for streptomycin, tetracycline and sulfonate resistance. The bla OXA−23 gene was the most probable cause for carbapenem resistance, although the bla OXA−66 gene with nonsynonymous SNPs (F82L, I129L) was not. Conclusions A. baumannii is a genomically variable pathogen that has the potential to cause a range of infectious diseases. There is high proportion of carbapenem resistance in isolates from all three infection sites (HAP, BSI and UTI), which can be attributed to the bla OXA−23 gene. CC208 is the predominant clone in bla OXA−23 -carrying A. baumannii that should be monitored. Virulence factors involving bacteria motility and polysaccharide biosynthesis which are widespread in clinical A. baumannii strains deserve our attention.

Abstract:Being a multidrug-resistant and an invasive pathogen, Acinetobacter baumannii is one of the major causes of nosocomial infections in the current healthcare system. It has been recognized as an agent of pneumonia, septicemia, meningitis, urinary tract and wound infections, and is associated with high mortality. Pathogenesis in A. baumannii infections is an outcome of multiple virulence factors, including porins, capsules, and cell wall lipopolysaccharide, enzymes, biofilm production, motility, and iron-acquisition systems, among others. Such virulence factors help the organism to resist stressful environmental conditions and enable development of severe infections. Parallel to increased prevalence of infections caused by A. baumannii, challenging and diverse resistance mechanisms in this pathogen are well recognized, with major classes of antibiotics becoming minimally effective. Through a wide array of antibiotic-hydrolyzing enzymes, efflux pump changes, impermeability, and antibiotic target mutations, A. baumannii models a unique ability to maintain a multidrug-resistant phenotype, further complicating treatment. Understanding mechanisms behind diseases, virulence, and resistance acquisition are central to infectious disease knowledge about A. baumannii. The aims of this review are to highlight infections and disease-producing factors in A. baumannii and to touch base on mechanisms of resistance to various antibiotic classes.
Epidemiological study on hospital-acquired pneumonia of Acinetobacter baumannii

Abstract:OBJECTIVE To investigate the molecular epidemiological characteristics, antibiotic resistance and possible routes of transmission of hospital-acquired pneumonia caused by Acinetobacter baumannii in the hospital, so as to provide references for clinical treatment.METHODS Clinical data of patients were collected using the hospital medical records system between Jul.2013 and Jun.2015.Isolates were typed by pulsed-field gel electrophoresis (PFGE) and the microorganism susceptibility was tested using Kirby-Bauer (K-B) method.RESULTS The mortality of the 114 patients with hospital-acquired pneumonia due to A.baumannii was 43.9%.Totally 17 banding patterns (A-Q) were identified in 114 isolates, including 99 epidemic strains (A-E) and 15 sporadic isolates (F-Q).Most of the strains were isolated from intensive care unit (ICU).Drug sensitivity test results showed that, all epidemic strains were multi-resistant A.baumannii(MDRAB), of which 96% were the carbapenem-resistant A.baumannii (CRAB).The resistance rates to cefperazone/sulbactam and tigecycline were low.CONCLUSION Multiple epidemic clusters existed in the hospital with high drug resistance to most kinds of antibiotics.Patient-to-patient transmission and environmental transmission may be the main route.Therefore, intensive infection control measures should be taken to prevent the transmission of the pathogen due to the poor prognosis of the patients.

Abstract:Objective To study the distribution,resistance and molecular biological characters of Acinetobacter in intensive care unit(ICU) in a hospital,in order to provide the evidence for scientific prevention and control.Methods The specimens from the hospitalized patients and environment in the ICU were detected and analyzed using pathogen isolation,identification and susceptibility tests.Results The proportions of multidrug resistant Acinetobacter among the isolates from superficial specimen of patients,hands of healthcare workers and environment were 71.7%,74.4% and 62.2%,respectively.The colonization rate of multidrug resistant Acinetobacter among the 64 patients was 54.7%.The resistance rates of cephalosporin,levofloxacin and amikacin among clinical Acinetobacter isolates were probably above 70%,but minocycline possessed a high susceptibility.The antibiograms of Acinetobacter from a longtime hospitalized patient were highly similar with the bacteria form the patient's surrounding environment.ST138,which belongs to clone complex 92,was the predominate clone among the non-duplicate Acinetobacter from colonization patients.Conclusions In the ICU wards,Acinetobacter were widely disseminated,exhibited multidrug resistance profile and tended to clone transmission.
Genomic Determinants of Pathogenicity and Antimicrobial Resistance of Nosocomial Acinetobacter baumannii Clinical Isolates of Hospitalized Patients (2019-2021) from a Sentinel Hospital in Hangzhou, China

Abstract:Purpose: Acinetobacter baumannii (A. baumannii or AB) is one of the most opportunistic, nosocomial pathogens threatening public healthcare across countries. A. baumannii has become a primary growing concern due to its exceptional ability to acquire antimicrobial resistance (AMR) to multiple antimicrobial agents which is increasingly reported and more prevalent every year. Therefore, there is an urgent need to evaluate the AMR knowledge of A. baumannii for effective clinical treatment of nosocomial infections. This study aimed to investigate the clinical distribution AMR phenotypes and genotypes, and genomic characteristics of A. baumannii isolates recovered from hospitalized patients of different clinical departments of a sentinel hospital to improve clinical practices. Methods: A total of 123 clinical isolates were recovered from hospitalized patients of different clinical departments during 2019-2021 to analyze AMR patterns, and further subjected to whole-genome sequencing (WGS) investigations. Multi-locus sequence typing (MLST), as well as the presence of antimicrobial-resistant genes (ARGs), virulence factor genes (VFGs) and insertion sequences (ISs) were also investigated from WGS data. Results: The results highlighted that A. baumannii clinical isolates had shown a high AMR rate, particularly from the intensive care unit (ICU), towards routinely used antimicrobials, ie, β-lactams and fluoroquinolones. ST2 was the most prevalent ST in the clinical isolates, it was strongly associated to the resistance of cephalosporins and carbapenems, with blaOXA-23 and blaOXA-66 being the most frequent determinants; moreover, high carrier rate of VFGs was also observed such as all strains containing the ompA, adeF, pgaC, lpsB, and bfmR genes. Conclusion: Acinetobacter baumannii clinical isolates are mostly ST2 with high rates of drug resistance and carrier of virulence factors. Therefore, it requires measurements to control its transmission and infection.
Antibiotic Resistance-Related Genes of Acinetobacter Clinical Isolates

Abstract:Objective: To investigate the antibiotic resistance phenotypes and genotypes of Acinetobacter clinical isolates in Peking University First Hospital.Methods: The minimal inhibitory concentrations(MICs) of 31 Acinetobacter isolates against 13 antibiotics were determined by agar dilution method and E-test.Antibiotic resistance-related genes(aacC1,aacC2,aacA4,OXA-23,PER-1,AmpC,Int1 and Int2) were detected by PCR.Results: Eighteen isolates were multidrug resistant strains.The positive rates of PCR amplification of aacC1,aacA4,PER-1,AmpC and Int1 were 58.1%(18/31),3.2%(1/31),3.2%(1/31),54.8%(17/31) and 67.7%(21/31),respectively.However,aacC2,OXA-23 and Int2 were not found.In addition,Int1 was detected in 23.1% isolates of the sensitivity group and 100% isolates of the resistance group,respectively.Sequences of the PCR products had more than 98% similarity to their counterparts in GenBank.Conclusion: The Acinetobacter clinical isolates from this hospital display severe antibiotic resistance.The resistance to β-lactams and to aminoglycosides correlates with the existence of AmpC and aacC1,respectively;while the existence of integrase gene Int1 correlates with the development of multidrug resistance properties of the strains.

Abstract:Objective To understand the drug resistance mechanisms and epidemiology of carbapenem-resistant Acinetobacter baumannii in order to guide the rational use of antibiotics and to provide the reference for the control of nosocomial infections.Methods A total of 1 16 Acinetobacter baumannii (1 10 carbapenem-resistant isolates and 6 carbapenem-sensitive isolates)isolated from clinical specimens were classified and analyzed according to endemic areas and specimen types.The isolates were analyzed by repetitive-sequence polymerase chain reaction (REP-PCR)for genotyping.Beta-lactamase genes (OXA,IMP,VIM and soon)and efflux pump-encoding genes (adeA,adeB,adeC, adeJ,adeG)were amplified by PCR.Real-time fluorescence quantitation PCR was performed to compare the expression level of the efflux pump-encoding genes in resistant and sensitive isolates.Results The isolation rate of resistant isolates in intensive care unit (ICU )was the highest-level,and the isolates were mainly isolated from sputum.The genotype types of carbapenem-resistant and -sensitive Acinetobacter baumannii were classified into 7 categories by REP-PCR.All the carbapenem-resistant isolates were type A.OXA-23 and OXA-5 1 were positive in 1 10 carbapenem-resistant isolates.However,OXA-24,OXA-58,IMP-1,IMP-2,VIM-1 and VIM-2 could not be detected in all the isolates.The positive rates of the efflux pump-encoding genes (adeA,adeB,adeC,adeJ and adeG)were 100%.The expression level of efflux pump-encoding genes (adeA,adeB,adeC)in resistant isolates was significantly higher than that in sensitive isolates.Conclusions The main popular type of carbapenem-resistant Acinetobacter baumannii is type A. Clinical staff should pay attention to this phenomenon.The expression of OXA-23 and OXA-5 1 overexpression of adeA, adeB and adeC efflux pump-encoding gene may play an important role in carbapenem-resistant Acinetobacter baumannii.

Abstract:Acinetobacter baumannii is a common cause of healthcare-associated infections and hospital outbreaks, particularly in intensive care units. Much of the success of A. baumannii relies on its genomic plasticity, which allows rapid adaptation to adversity and stress. The capacity to acquire novel antibiotic resistance determinants and the tolerance to stresses encountered in the hospital environment promote A. baumannii spread among patients and long-term contamination of the healthcare setting. This review explores virulence factors and physiological traits contributing to A. baumannii infection and adaptation to the hospital environment. Several cell-associated and secreted virulence factors involved in A. baumannii biofilm formation, cell adhesion, invasion, and persistence in the host, as well as resistance to xeric stress imposed by the healthcare settings, are illustrated to give reasons for the success of A. baumannii as a hospital pathogen.

Abstract:Objective To investigate the clinical characteristics and homology in Acinetobacter baumannii from cilinical isolates.Methods A retrospective analysis was performed on the clinical and antimicrobial sensitivity data.Identification of the clinical Acinetobacter baumannii isolates and susceptibility testing were performed using Vitek2 Compact.Genotyping and homology of 1 1 5 strains of Acinetobacter baumannii were analyzed by enterbacterial repetitive intergenic consensus (ERIC)-polymerase chain reaction(PCR).Results The mean age of the patients was (75 ± 1 6) years old.The most frequent comorbidity were chronic obstructive pulmonary disease(COPD) and diabetes mellitus.72 (62.61%) patients were under invasive procedure.86 (74.78%)patients used more than two antibiotics. Mechanical ventilation were used in 48(41.74%) patients.The 1 1 5 strains of Acinetobacter baumannii remained sensitive to colistin(resistance rate 0%),followed by Tygecycline(resistance rate 7.83%).The 1 1 5 strains were identified as 9 epidemic clones.Conclusions The risk factors of nosocomial infection caused by Acinetobacter baumannii included advanced age,presence of comorbidity,invasive procedures, mechanical ventilation and the use of more than two antibiotics.The strains of Acinetobacter baumannii were resistant to most of the antimicrobial agents.

Abstract:Acinetobacter is ubiquitous, and it has a high species diversity and a complex evolutionary pattern. To elucidate the mechanism of its high ability to adapt to various environment, 312 genomes of Acinetobacter strains were analyzed using the phylogenomic and comparative genomics methods. It was revealed that the Acinetobacter genus has an open pan-genome and strong genome plasticity. The pan-genome consists of 47,500 genes, with 818 shared by all the genomes of Acinetobacter , while 22,291 are unique genes. Although Acinetobacter strains do not have a complete glycolytic pathway to directly utilize glucose as carbon source, most of them harbored the n -alkane-degrading genes alkB / alkM (97.1% of tested strains) and almA (96.7% of tested strains), which were responsible for medium-and long-chain n -alkane terminal oxidation reaction, respectively. Most Acinetobacter strains also have catA (93.3% of tested strains) and benAB (92.0% of tested strains) genes that can degrade the aromatic compounds catechol and benzoic acid, respectively. These abilities enable the Acinetobacter strains to easily obtain carbon and energy sources from their environment for survival. The Acinetobacter strains can manage osmotic pressure by accumulating potassium and compatible solutes, including betaine, mannitol, trehalose, glutamic acid, and proline. They respond to oxidative stress by synthesizing superoxide dismutase, catalase, disulfide isomerase, and methionine sulfoxide reductase that repair the damage caused by reactive oxygen species. In addition, most Acinetobacter strains contain many efflux pump genes and resistance genes to manage antibiotic stress and can synthesize a variety of secondary metabolites, including arylpolyene, β-lactone and siderophores among others, to adapt to their environment. These genes enable Acinetobacter strains to survive extreme stresses. The genome of each Acinetobacter strain contained different numbers of prophages (0–12) and genomic islands (GIs) (6–70), and genes related to antibiotic resistance were found in the GIs. The phylogenetic analysis revealed that the alkM and almA genes have a similar evolutionary position with the core genome, indicating that they may have been acquired by vertical gene transfer from their ancestor, while catA , benA , benB and the antibiotic resistance genes could have been acquired by horizontal gene transfer from the other organisms.

Abstract:OBJECTIVE:To investigate the molecular mechanism of multiple-drug and pan-drug resistance among Acinetobacter species.METHODS:Non-repetitive 90 carbapenem-resistant strains of Acinetobacter species were collected in Beijing, Guangzhou, and Fuzhou 1999-2004. The homology of the isolates was determined by both pulsed field gel electrophoresis and randomly amplified polymorphic DNA typing. Seven representative clones were selected from the 90 strains of Acinetobacter isolated from different hospitals to be used for further study. Analytical isoelectric focusing was used to measure the isoelectric point of the beta-lactamase. Plasmid DNA was extracted and purified Genes of different beta-lactamase, including bla(TEM--), bla(SHV-), bla(PER-), blaI(MP-), bla(VIM-), and bla(OXA-) genes, in these clone strains were amplified and sequenced. PCR was used to analyze the integrons.RESULTS:The P clone strain isolated during an outbreak of pan-drug-resistant Acinetobacter species in Peking Union Medical College Hospital 2004 was not susceptible to most common antimicrobial agents tested. The 7 representative clones produced multiple beta-lactamases: TEM-1, high-level AmpC, SHV-type, OXA-23 carbapenemase and IMP-8 and metalloenzyme respectively. One clone produced PER-1 enzyme. These 7 clone strains were resistant to most beta-lactams (including carbapenems), erythromycin, chloramphenicol, and rifampin. Two clone strains were susceptible to cefoperazone/sulbactam and amikacin while 4 clone strains susceptible to levofloxacin. All of the 7 clones were susceptible to minocycline and colistin. Five different integrons were found, harboring the genes mediating the resistance to aminoglycosides, rifampin, chloramphenicol, and carbapenems (bla(IMP-8)).CONCLUSION:The molecular bases of multiple-drug or pan-drug resistance in Acinetobacter species include production of OXA-23 carbapenemase or IMP type metalloenzyme and integrons with different resistance gene cassettes. Pan-drug-resistant Acinetobacter species are susceptible to old antimicrobials agents, such as colistin and minocycline.

Abstract:In recent years, humanity has begun to face a growing challenge posed by a rise in the prevalence of antibiotic-resistant bacteria. This has resulted in an alarming surge in fatalities and the emergence of increasingly hard-to-manage diseases. Acinetobacter baumannii can be seen as one of these resilient pathogens due to its increasing prevalence in hospitals, its resistance to treatment, and its association with elevated mortality rates. Despite its clinical significance, the scientific understanding of this pathogen in non-hospital settings remains limited. Knowledge of its virulence factors is also lacking. Therefore, in this review, we seek to shed light on the latest research regarding the ecological niches, microbiological traits, and antibiotic resistance profiles of Acinetobacter baumannii. Recent studies have revealed the presence of this bacterium in a growing range of environmental niches, including rivers, treatment plants, and soils. It has also been discovered in diverse food sources such as meat and vegetables, as well as in farm animals and household pets such as dogs and cats. This broader presence of Acinetobacter baumannii, i.e., outside of hospital environments, indicates a significant risk of environmental contamination. As a result, greater levels of awareness and new preventive measures should be promoted to address this potential threat to public health.
