The Diagnostic Value of Relevant Laboratory Tests and Ultrasonographic Characteristics for Differentiating Differentiated Thyroid Carcinoma from Nodulargoiter

Abstract:Objective To explore the difference of relevant laboratory tests(TSH,TG,TGAb,TPOAb) and ultrasonographic characteristics in DTC and nodular goiter.Methods The clinical data of 354 patients (DTC or NG) were retrospectively analyzed,who accepted thyroid operation and were diagnosed by pathology,recorded the preoperative ultrasonographic characteristics and the level of TSH,TG,TGAb,TPOAb.The difference in ultrasonographic characteristics and relevant laboratory tests(TSH,TG,TGAb,TPOAb) differentiating DTC from NG was evaluated by multivariate analyses,furthermore,the ROC curve was drawn to evaluate the diagnostic significance by means of ultrasonographic characteristics and relevant laboratory tests.Results Multivariate analysis showed that there were significant difference in TSH,TG,TGAb,TPOAb,taller-than-wide shape ≥ 1,microcalcification,irregular nodule margin,irregular shape,extremely low echo,solid structure,solitary nodule in the two groups,Nodular size,heterogeneity and colour Doppler were no significant difference in DTC and NG.ROC curve was drawn.The area under ROC curve of margin,shape,aggregative indexes respectively were 0.80,0.85,0.96.Conclusions The higher level of TSH,TG,TGAb,TPOAb,taller-than-wide shape ≥ 1,microcalcification,irregular nodule margin,irregular shape,extremely low echo,solid structure,solitary nodule may be important risk factors in differentiating DTC from NG.According to the ROC curve,aggregative indexes(include the margin,shape,nodule number,TSH,taller-than-wide shape) had significant diagnostic value in differentiating DTC from NG.
